    Transmission of Schistosoma mansoni in Tikur Wuha area, Southern Ethiopia

    Background: Although the epidemiology of schistosomiasis is well established and the disease distribution has also been mapped in Ethiopia, discovery of new foci has continuously been reported.Objectives: The objective of this study was to assess the establishment of transmission of schistosomiasis mansoni in Tikur Wuha area, southern Ethiopia.Methods: A cross-sectional epidemiological study involving 375 school children in Tikur Wuha Elementary School was conducted in December 2007 and January 2008. Stool specimens were collected and microscopically examined using Kato-Katz method. Snail survey was also conducted using scoop in Tikur Wuha River and littoral zone of Lake Awassa on the side of Tikur Wuha Kebele (administrative unit). The snails collected were checked for trematode infection by shedding. Laboratory-bred mice were exposed to schistosome cercariae and definite identification of the schistosome was made using eggs and adult worm morphology.Results: The prevalence and intensity of schistosomiasis mansoni was 12% and 69 eggs per gram (epg) of stool, respectively. Biomphalaria sudanica collected in Tikur Wuha River shed schistosome cercariae. Adult S. mansoni worms were harvested from laboratory-bred mice after 6 weeks of laboratory maintenance.Conclusion: The prevalence and intensity of schistosomiasis mansoni among school children was low and the area represents low-risk community. The finding of S. mansoni infected young children, the collection of B. sudanica infected with schistosome cercariae, and the establishment of infection in lab-bred mice all confirmed the transmissionof schistosomiasis mansoni in Tikur Wuha area. Appropriate intervention measures need to be in place to reduce morbidity and transmission of intestinal schistosomiasis in the area

    Mechanical Property of Plastic Lumber Produced from Recycled High Density Polyethylene (HDPE)

    Recycling is a strategy to combat environmental pollution due to plastic wastes. Of the many plastic wastes, researchers confirmed the possibility to produce plastic lumber from used HDPE plastics. The aim of this study is to experimentally determine the mechanical property of the plastic lumber produced from pure HDPE and its composite with wood to proof usability and applicability for different purpose. The experiment considered different conditions. Pure plastic lumber and wood plastic composite lumber is initially produced and examined under room temperature and below room temperature. Under each condition, bending, tensile and impact strength were examined. For each test eight specimens were prepared based on international standards and the experiments were conducted using standard testing machines. The result shows that pure plastic lumber has demonstrated superior load carrying capability, tensile load resistance capability and impact load resistance at room temperature. Moreover, the pure plastic lumber has an elastic nature and the wood composite has a brittle nature. In both cases the mechanical property of the plastic lumber permits to use for different applications. This has been also tested and resulted in similar phenomena at 20C below room temperature

    Level of compassionate health care service provision and its associated factors among health professionals working in public hospitals of Addis Ababa: health professionals' perspective

    Background: Compassionate health care service is important for good clinical outcomes and patient satisfaction. However, complaints of non-compassionate care became very familiar and popular grievance of the community in the health care system. The aim of this study was to assess the level of compassionate health care service provision and its associated factors among health professionals working in public hospitals of Addis Ababa, Ethiopia. Methods: A facility based cross sectional study was conducted among six public hospitals of Addis Ababa, Ethiopia. Simple random sampling and systematic sampling methods were applied to select hospitals and each study participants respectively. Data were collected from 400 participants using a self-administered structured questionnaire. The mean of compassionate health care was taken as the cut point to label respondent as good and poor compassionate health care service provider. Bivariate and multivariable logistic regressions were done to determine the associated factors for compassionate care provision. Variables with p values < 0.05 at the multivariable analysis were considered as significantly associated with compassionate care provision. Results: Only 48.0% of health care professionals provided good compassionate care. Learned to be compassionate (AOR ¼ 5.083; CI 95% 2.69, 9.58); patient and their family realistic expectation (AOR ¼ 2.24; CI 95% 1.07, 4.66) were found to be significantly associated with good compassionate health care provision. Conclusions: and recommendations: Good compassionate health care service provision in Addis Ababa hospitals was very low. Teaching health science and medical students to be compassionate and ensuring health care clients to have realistic expectation may be important to farther enhance compassionate health care service provision.Mulugeta Abate, Nigussie Tadesse, Kindie Mitik

    Integrated human-animal sero-surveillance of Brucellosis in the pastoral Afar and Somali regions of Ethiopia

    BACKGROUND: Brucellosis is widespread in Ethiopia with variable reported prevalence depending on the geographical area, husbandry practices and animal species. However, there is limited information on the disease prevalence amongst pastoral communities, whose life is intricately linked with their livestock. METHODOLOGY: We conducted an integrated human-animal brucellosis sero-surveillance study in two adjacent pastoral regions, Afar and Somali region (SRS). This cross-sectional study included 13 woredas (districts) and 650 households. Blood samples were collected from people and livestock species (cattle, camel, goats and sheep). Sera were analyzed with C-ELISA for camels and shoats (sheep and goats), with I-ELISA for cattle and IgG ELISA for humans. Descriptive and inferential statistics analyses were performed. RESULTS: A total of 5469 sera were tested by ELISA. Prevalence of livestock was 9.0% in Afar and 8.6% in SRS (ranging from 0.6 to 20.2% at woreda level). In humans, prevalence was 48.3% in Afar and 34.9% in SRS (ranging from 0.0 to 74.5% at woreda level). 68.4% of all households in Afar and 57.5% of households in SRS had at least one animal reactor. Overall, 4.1% of animals had a history of abortion. The proportion of animals with abortion history was higher in seropositive animals than in seronegative animals. Risk factor analysis showed that female animals were significantly at higher risk of being reactors (p = 0.013). Among the species, cattle had the least risk of being reactors (p = 0.014). In humans, there was a clear regional association of disease prevalence (p = 0.002). The older the people, the highest the odds of being seropositive. CONCLUSION: Brucellosis is widespread in humans and animals in pastoral communities of Afar and SRS with the existence of geographical hotspots. No clear association was seen between human and particular livestock species prevalence, hence there was no indication as whether B. abortus or B. melitensis are circulating in these areas, which warrants further molecular research prior to embarking on a national control programs. Such programs will need to be tailored to the pastoral context

    Factors associated with prelacteal feeding practices in Debre Berhan district, North Shoa, Central Ethiopia: a cross-sectional, community-based study

    Abstract Background Prelacteal feeding is one of the major harmful newborn feeding practices and is top on the list of global public health concerns. The practice deprives newborns of valuable nutrients and protection of colostrum and exposes them to preventable morbidity and mortality. Studying the prevalence and factors influencing the prelacteal feeding practice of mothers will help program managers and implementers to properly address broad major public health problems. Therefore, this study aims to investigate the prevalence of prelacteal feeding practices and its associated factors among mother-infant dyads in the Debre Berhan district of North Shoa administrative zone, central Ethiopia. Methods A community-based cross-sectional study design was conducted from January through to April 2014 among 634 mother-infant dyads. The data were entered into EPI Info version 3.5.1. (CDC, Atlanta, Georgia). All statistical analysis was conducted using Statistical Package for Social Sciences (SPSS) research IBM version 20.0. The prevalence of prelacteal feeding was determined using the ‘recall since birth’ method. Multi-variable logistic regression analysis was employed to control confounders in determining the association between prelacteal feeding practices and selected independent variables. Adjusted Odds Ratio (AOR), with 95% Confidence Interval (CI) and P < 0.05 was used to claim statistical significance. Results The prevalence of prelacteal feeding practice was 14.2% (95% CI: 11.00–17.00%). Slightly greater than half, 48 (53.3%) of prelacteal fed newborns were given butter. Home delivery was a major risk factor for practicing prelacteal feeding. Mothers who delivered their indexed infant at home practiced prelacteal feeding over four folds more than mothers who delivered in a health institution (Adjusted Odds Ratio (AOR) 4.70; 95% CI: 2.56–8.60, p-value = 0.001). Mothers who did not initiate breastfeeding within an hour were six times more likely to practice prelacteal feeding (AOR 5.58; 3.21–9.46, p-value = 0.001). Similarly, with regards to the occupation of mothers, farmers practiced prelacteal feedings (AOR 4.33; 95% CI: 1.73–10.81, p-value = 0.002) up to four folds more than their counterpart housewives. Mothers who can read and write are 54% less likely to practice prelacteal feeding than their counterpart, illiterate mothers, with (AOR 0.46; 95% CI: 0.22–0.98, p-value = 0.044). Conclusions In the Debre Berhan town of North Shoa administrative zone, central Ethiopia, almost one-sixth of mothers practiced prelacteal feeding. Therefore, improving access to information about appropriate newborn feeding practices, encouraging mothers to deliver their babies in health institutions and inspiring them to initiate breastfeeding within an hour of birth is recommended

    De-identification of primary care electronic medical records free-text data in Ontario, Canada

    <p>Abstract</p> <p>Background</p> <p>Electronic medical records (EMRs) represent a potentially rich source of health information for research but the free-text in EMRs often contains identifying information. While de-identification tools have been developed for free-text, none have been developed or tested for the full range of primary care EMR data</p> <p>Methods</p> <p>We used <it>deid </it>open source de-identification software and modified it for an Ontario context for use on primary care EMR data. We developed the modified program on a training set of 1000 free-text records from one group practice and then tested it on two validation sets from a random sample of 700 free-text EMR records from 17 different physicians from 7 different practices in 5 different cities and 500 free-text records from a group practice that was in a different city than the group practice that was used for the training set. We measured the sensitivity/recall, precision, specificity, accuracy and F-measure of the modified tool against manually tagged free-text records to remove patient and physician names, locations, addresses, medical record, health card and telephone numbers.</p> <p>Results</p> <p>We found that the modified training program performed with a sensitivity of 88.3%, specificity of 91.4%, precision of 91.3%, accuracy of 89.9% and F-measure of 0.90. The validations sets had sensitivities of 86.7% and 80.2%, specificities of 91.4% and 87.7%, precisions of 91.1% and 87.4%, accuracies of 89.0% and 83.8% and F-measures of 0.89 and 0.84 for the first and second validation sets respectively.</p> <p>Conclusion</p> <p>The <it>deid </it>program can be modified to reasonably accurately de-identify free-text primary care EMR records while preserving clinical content.</p

    Adoption Studies on Improved Chickpea Varieties in Ethiopia

    Chickpea (Cicer arietinum L.) is one of the most important food legumes in Ethiopia contributing to about 17% of the countries’ total pulse production. Ethiopia is the largest chickpea growing country in Africa, with a share of about 37% in area and 48% in production. During 2003/2004, Ethiopia produced 135,930 m t of chickpea from an area of 168,089 ha. There has been an increase of 12% in area and 34% in production since 1981/1982. Most of the chickpea production goes for domestic consumption. However, there has been substantial export of chickpea during the past five years, with maximum of 48,549 t (valued at US$14.7 million) during 2002 (FAOSTAT 2005). Chickpea is an important source of dietary protein and minerals for many Ethiopians who cannot afford animal products. It is used in various forms, e.g., green seeds, dried seeds, dehulled-splits and flour. Chickpea straw is highly valued as animal feed. The farmers recognize the importance of legumes in improving soil fertility and thus grow chickpea and other legumes in rotation with cereals. The Debre Zeit Agricultural Research Center (DZARC) has been the premier institute for chickpea research in Ethiopia. It has collaborated with the International Crops Research Institute for the Semi-Arid Tropics (ICRISAT), Patancheru, India, and the International Center for Agricultural Research in the Dry Areas (ICARDA), Aleppo, Syria, in chickpea improvement and released 10 chickpea varieties in Ethiopia. Of these, three (DZ-10-4, DZ-10-11 and Dubie) were developed from its own breeding materials, five (Mariye, Worku, Akaki, Shasho and Chefe) from the breeding materials supplied by ICRISAT, and two (Arerti and Habru) from the breeding materials supplied by ICARDA.........

    Long-term impacts of conservation pasture management in manuresheds on system-level microbiome and antibiotic resistance genes

    Animal manure improves soil fertility and organic carbon, but long-term deposition may contribute to antibiotic resistance genes (ARGs) entering the soil-water environment. Additionally, long-term impacts of applying animal manure to soil on the soil-water microbiome, a crucial factor in soil health and fertility, are not well understood. The aim of this study is to assess: (1) impacts of long-term conservation practices on the distribution of ARGs and microbial dynamics in soil, and runoff; and (2) associations between bacterial taxa, heavy metals, soil health indicators, and ARGs in manures, soils, and surface runoff in a study following 15 years of continuous management. This management strategy consists of two conventional and three conservation systems, all receiving annual poultry litter. High throughput sequencing of the 16S ribosomal RNA was carried out on samples of cattle manure, poultry litter, soil, and runoff collected from each manureshed. In addition, four representative ARGs (intl1, sul1, ermB, and blactx-m-32) were quantified from manures, soil, and runoff using quantitative PCR. Results revealed that conventional practice increased soil ARGs, and microbial diversity compared to conservation systems. Further, ARGs were strongly correlated with each other in cattle manure and soil, but not in runoff. After 15-years of conservation practices, relationships existed between heavy metals and ARGs. In the soil, Cu, Fe and Mn were positively linked to intl1, sul1, and ermB, but trends varied in runoff. These findings were further supported by network analyses that indicated complex co-occurrence patterns between bacteria taxa, ARGs, and physicochemical parameters. Overall, this study provides system-level linkages of microbial communities, ARGs, and physicochemical conditions based on long-term conservation practices at the soil-water-animal nexus
