Aceitabilidade e conhecimento sobre remoção seletiva de tecido cariado de graduandos em odontologia e cirurgiões-dentistas
Aim: The aim of this study was to evaluate the acceptability and knowledge of undergraduate dental students and dentists on selective caries tissue removal (SCTR). Materials and Methods: Third- and fourth-year Dentistry undergraduates (Group 1) and graduates working in a Hospital Dentistry Center (Group 2) were included in the study. Participants anonymously and confidentially answered a validated questionnaire containing eleven questions on the diagnosis and management of deep caries lesions. Pearson’s Chi-square test and multivariate logistic regression compared the answers (p<0.05). Results: Total sample comprised 146 participants. Of these, 81.5% were female; 52.05% were graduates and the most prevalent age group was 18-29 years old (85.62%). Sixty-nine participants chose stepwise caries removal (p=0.027). The logistic regression analysis showed statistically significant differences. The participants who considered pulp wall moist have approximately eight times more likelihood to choose an invasive treatment (p=0.028). Those who chose endodontic treatment as an option for two-year survival have three times more likelihood to choose an invasive treatment (p=0.032). Those who affirmed that the carious dentin close to the pulp should not be removed had almost three times more likelihood to choose minimally invasive treatments (p=0.031). Discussion: Studies with questionnaires can be useful tools to detect whether the students and dentists are following the most current evidences to treat deep carious lesions. Conclusion: The participants had certain level of knowledge on SCTR, but the technique acceptability lacked consensus.Objetivo: Avaliar a aceitabilidade e o conhecimento de estudantes de graduação em Odontologia e cirurgiões-dentistas sobre a remoção seletiva de tecido cariado (RSTC). Materiais e Métodos: Participaram do estudo graduandos do terceiro e quarto ano de Odontologia (Grupo 1) e egressos de um Centro Hospitalar de Odontologia (Grupo 2). Os participantes responderam de forma anônima e confidencial um questionário validado contendo onze questões sobre diagnóstico e manejo de lesões cariosas profundas. Teste qui-quadrado de Pearson e regressão logÃstica multivariada foram aplicados (p<0.05). Resultados: A amostra total foi composta por 146 participantes. Destes, 81.5% eram do sexo feminino; 52.05% eram graduados e a faixa etária mais prevalente foi de 18 a 29 anos (85.62%). Sessenta e nove participantes escolheram o tratamento expectante (p=0.027). A análise de regressão logÃstica mostrou diferenças estatisticamente significativas. Os participantes que consideraram a parede pulpar úmida têm aproximadamente oito vezes mais chances de escolha pelo tratamento invasivo (p=0.028). Aqueles que escolheram o tratamento endodôntico como opção de sobrevida em dois anos têm três vezes mais chances de optar pelo tratamento invasivo (p=0.032). Aqueles que afirmaram que a dentina cariada próxima à polpa não deveria ser removida tiveram quase três vezes mais chances de optar por tratamentos minimamente invasivos (p=0.031). Discussão: Estudos com questionários podem ser ferramentas úteis para detectar se estudantes e cirurgiões-dentistas estão seguindo as evidências mais atuais para o tratamento de lesões cariosas profundas. Conclusão: Os participantes tinham certo nÃvel de conhecimento sobre RSTC, mas a aceitabilidade da técnica carecia de consenso
Rat subcutaneous tissue response to MTA Fillapex® and Portland cement
The aim of this study was to evaluate the response of rat subcutaneous tissue to MTA Fillapex® (Angelus), an experimental root canal filling material based on Portland cement and propylene glycol (PCPG), and a zinc oxide, eugenol and iodoform (ZOEI) paste. These materials were placed in polyethylene tubes and implanted into the dorsal connective tissue of Wistar rats for 7 and 15 days. The specimens were stained with hematoxylin and eosin, and evaluated regarding inflammatory reaction parameters by optical microscopy. The intensity of inflammatory response against the sealers was analyzed by two blinded and previously calibrated examiners for all experimental periods (kappa=0.96). The histological evaluation showed that all materials caused a moderate inflammatory reaction at 7 days, which subsided with time. A greater inflammatory reaction was observed at 7 days in the tubes filled with ZOEI paste. Tubes filled with MTA Fillapex presented some giant cells, macrophages and lymphocytes after 7 days. At 15 days, the presence of fibroblasts and collagen fibers was observed indicating normal tissue healing. The tubes filled with PCPG showed similar results to those observed in MTA Fillapex. At 15 days, the inflammatory reaction was almost absent at the tissue, with several collagen fibers indicating normal tissue healing. Data were analyzed by the nonparametric Kruskal-Wallis test (?=0.05). Statistically significant difference (p<0.05) was found only between PCPG at 15 days and ZOEI at 7 days groups. No significant differences were observed among the other groups/periods (p>0.05). MTA Fillapex and Portland cement added with propylene glycol had greater tissue compatibility than the PCPG paste.O objetivo deste estudo foi avaliar a resposta do tecido subcutâneo de rato ao MTA Fillapex® (Angelus), a um cimento endodôntico experimental à base de cimento Portland e propilenoglicol, e à pasta de óxido de zinco e eugenol com iodofórmio. Estes materiais foram colocados em tubos de polietileno e implantados no tecido conjuntivo do dorso de ratos Wistar, por 7 e 15 dias. Os espécimes foram corados com hematoxilina e eosina e os parâmetros de reação inflamatória foram avaliados em microscópio óptico. A intensidade da resposta inflamatória provocada pelos cimentos foi analisada em todos os perÃodos por dois observadores previamente calibrados (kappa 0,96) e sem conhecimento dos grupos experimentais. O exame histológico mostrou que todos os materiais provocaram reação inflamatória moderada aos 7 dias que regrediu com o tempo. A maior resposta inflamatória do tecido foi observada aos 7 dias, nos tubos preenchidos com pasta de Óxido de Zinco e Eugenol com Iodofórmio. Os tubos com MTA Fillapex apresentaram algumas células gigantes, macrófagos e linfócitos após 7 dias. Aos 15 dias, a presença de fibroblastos e fibras de colágenas foi observada, indicando processo de cicatrização do tecido. Os tubos com o cimento Portland mostraram resultados semelhantes aos observados no grupo MTA Fillapex. Aos 15 dias, a reação inflamatória apresentada foi praticamente ausente, com muitas fibras colágenas, indicando cicatrização normal do tecido. A análise estatÃstica mostrou diferença estatisticamente significante entre o grupo de cimento Portland (15 dias) e óxido de zinco eugenol com Iodofórmio (7 dias) (p<0,05). Nos outros grupos não houve diferença estatÃstica significante. MTA Fillapex e cimento Portland são mais biocompatÃveis do que os outros cimentos testados
A randomized clinical trial of cavity liners after selective caries removal: one-year follow-up
Alternatives for the treatment of caries disease, such as minimally invasive approaches, have been developed in recent years. Objective: To carry out clinical and radiographic evaluations of three cavity liners after selective caries removal. Methodology: Thirty-six primary molars with deep occlusal caries lesions without pulp involvement (from children of both genders, aged between 5 and 8 years) were randomly divided into the following groups: calcium hydroxide cement (CHC) group; mineral trioxide aggregate (MTA) group and Portland cement with added zirconium oxide (PCZ) group. The following-up period was 6- and 12-month. The clinical and radiographic success rates were evaluated through chi-square test. The radiographic measurements were compared by ANOVA followed by Tukey's test (p<0.05). Results: Thirty-six patients were included, but thirty-four returned for 12-month follow-up. The overall success rate of the therapy for the three groups was 94.11% and no statistically significant differences occurred in the comparison among groups (p>0.05). Nineteen radiographs were selected to measure the dentin barrier thickness. The intragroup comparison presented a statistically significant increase of the dentin barrier for all groups, at 12-month follow-up. However, the MTA group showed increase of the dentin barrier, over time, 6- to 12-month follow-up. The intergroup comparison revealed no statistically significant differences (p>0.05). Conclusion: The clinical and radiographic data showed that all cavity liners provided effective treatment of primary teeth after selective caries removal
Comparison between the rotary (Hyflex EDM®) and manual (k-file) technique for instrumentation of primary molars: a 12-month randomized clinical follow-up study
Rotational instrumentation is an alternative for the clinical practice of pediatric dentists. However, there are few records in the literature on the clinical and radiographic aspects of treated teeth over time. Objectives: Compare instrumentation time and filling quality between manual (k-file) and rotary (Hyflex EDM®) files, and clinically and radiographically follow-up the treated teeth for 12 months. Moreover, the characteristics of glass ionomer restorations and their interference in the treatment prognosis over time were evaluated. Methodology: In total, 40 children with pulp involvement in primary molars received treatment with Hyflex EDM® or manual rotary files, performed by an operator. Clinical and radiographic aspects were observed at different times to determine the effectiveness of each technique. Results: The rotary system reduced instrumentation time when compared to the use of manual files (p≤0.05), but there was no difference in filling quality between the groups (p≥0.05). Moreover, both types of instrumentation were effective for 12 months (p≥0.05), and restoration retention influenced the emergence of periapical lesions (p≤0.05). Conclusion: Although rotary files reduce clinical time, the clinical and radiographic aspects of both techniques were similar over 12 months. Moreover, restoration retention has been shown to be related to treatment prognosis
Pulpotomies with portland cement in human primary molars
Two clinical cases in which Portland cement (PC) was applied as a medicament after pulpotomy of mandibular primary molars in children are presented. Pulpotomy using PC was carried out in two mandibular first molars and one mandibular second molar, which were further followed-up. At the 3, 6 and 12-month follow-up appointments, clinical and radiographic examinations of the pulpotomized teeth and their periradicular area revealed that the treatments were successful in maintaining the teeth asymptomatic and preserving pulpal vitality. Additionally, the formation of a dentin bridge immediately below the PC could be observed in the three molars treated. PC may be considered as an effective alternative for primary molar pulpotomies, at least in a short-term period. Randomized clinical trials with human teeth are required in order to determine the suitability of PC before unlimited clinical use can be recommended
Pulp therapy in deciduous teeth: therapeutic possibilities based on evidences
Introduction: Pulpotomy of primary teeth has been the treatment of choice in cases of inflammation of the coronal pulp caused by caries or trauma with no involvement of the radicular pulp, thus avoiding the premature loss of the teeth. Not only is the diagnosis of the inflamed dental pulp required for this therapy to succeed, but also the selection of an effective and biocompatible medicament. However, none of the drugs available and recommended for pulpotomy therapy present all the requirements of an ideal pulp capping agent. Objective: Through a systematic literature review, between the years of 2000 and 2011, focusing in randomized clinical trials, systematic reviews and meta-analysis, aiming to analyze and discuss based on alternatives for the treatment of pulpotomy in human deciduous teeth. Conclusion: Scientific evidences showed that the medicaments and techniques influence were few. Thus, further and well-designed studies are suggested for clarification. The information generated in such experiments may contribute to a better understanding of the mechanisms of pulp therapy to new therapeutic approaches aimed at improving pulp therapy in deciduous teeth.Introdução: A pulpotomia em dentes decÃduos é uma técnica conservadora de terapia pulpar amplamente utilizada \ud

In vivo evaluation of tissue response to new endodontic sealers
Introduction: The sealers can be in direct contact with the periapical tissues. Thus, these materials must have appropriate physical and biological properties, providing conditions for repair to occur. Objective: The aim of this study was to evaluate the response of rat subcutaneous tissue to endodontics sealers. Material and methods: Three materials comprised the groups: group I – Zinc Oxide, Eugenol and Iodoform paste, group II – Portland cement with propylene glycol, and group III – MTA Fillapex® (Angelus). These materials were placed in polyethylene tubes and implanted into dorsal connective tissue of Wistar rats for seven and 15 days. The specimens were stained with hematoxylin and eosin and evaluated regarding to inflammatory reaction parameters through a light microscope. The data were compared using Kruskal-Wallis test with significance level of 5%. The intensity of inflammatory response against the sealers was analyzed by two blinded and previously calibrated observers for all experimental periods. Results: The histological evaluation showed that all the materials caused a moderated inflammatory reaction at seven days which decreased with time. A greater inflammatory reaction was observed at seven days in group I. The other specimens had significantly less inflammatory cells when compared to this group. Tubes with MTA Fillapex® presented some giant cells, macrophages and lymphocytes after seven days. At 15 days, the presence of fibroblasts and collagen fibers was observed indicating normal tissue healing. The group II showed similar results to those observed in MTA Fillapex® already at seven days. Clinical, radiographic, histological, and immunohistochemistry study on pulpal response after the use of different pulp capping materials in pulpotomies of human primary teeth
O objetivo deste estudo foi avaliar: 1) a resposta pulpar de dentes decÃduos humanos após pulpotomia com diferentes materiais capeadores por meio de análise clÃnica, radiográfica e histológica; e 2) a expressão da ProteÃna 1 da Matriz Dentinária (DMP-1) na resposta pulpar. Vinte e cinco molares decÃduos inferiores de 22 crianças com idade entre 6 e 9 anos foram criteriosamente selecionados. Os dentes foram randomicamente divididos nos grupos Formocresol de Buckley diluÃdo a 1/5 (Grupo I - FC), Hidróxido de Cálcio (Grupo II HC), Agregado Trióxido Mineral (Grupo III MTA), Otosporin® + Hidróxido de Cálcio (Grupo IV OTO+HC) e Cimento Portland (Grupo V PC), e tratados pela técnica convencional de pulpotomia em sessão única. As avaliações clÃnicas e radiográficas foram realizadas até o perÃodo normal de esfoliação. Os dentes foram extraÃdos e processados para análise histológica e imuno-histoquÃmica. Os resultados clÃnicos e radiográficos não mostraram diferença estatisticamente significativa nos grupos estudados (p>0,05). A análise estatÃstica da avaliação histológica revelou diferença estatisticamente significativa para presença de barreira dentinária e camada odontoblástica entre o Grupo I(FC) e os Grupos II(HC), III(MTA), IV(OTO+HC) e V(PC) visto que o Grupo I(FC) não apresentou barreira dentinária e camada odontoblástica em nenhuma das lâminas observadas. Não houve diferença estatisticamente significativa entre os grupos estudados quando da avaliação da calcificação pulpar. Para o item reabsorção interna observou-se diferença estatisticamente significativa entre o Grupo I(FC), II(HC) e os Grupos III(MTA), IV(OTO+HC) e V(PC) onde não se observaram reabsorções internas nas análises histológicas. A análise da expressão da DMP-1 demonstrou para o Grupo I(FC) imunomarcação localizada na região de calcificações pulpares localizadas nas raÃzes dos dentes avaliados, não havendo camada odontoblástica nem formação de barreira dentinária. Os Grupos II(HC), III(MTA), IV(OTO+HC) e V(PC) apresentaram imunomarcação na região de barreira dentinária e da camada odontoblástica, evidenciando a biocompatibilidade destes materiais, bem como a capacidade indutora para secreção de dentina. A análise conjunta dos resultados obtidos neste trabalho, com o uso de diferentes técnicas, permitiu concluir que todos os materiais empregados apresentaram Ãndices de sucesso clÃnico e radiográfico aceitáveis para indicação clÃnica. Na análise histológica os materiais HC, MTA e PC apresentaram melhores resultados no reparo pulpar. A expressão da DMP-1 na resposta pulpar permitiu visualizar a capacidade bioindutora dos materiais HC, MTA e PC no processo de reparo pulpar e formação de barreira. A comparação entre os grupos estudados permitiu constatar que o MTA e o PC se mostraram mais eficientes, proporcionando formação de barreira dentinária e respostas do complexo dentinho-pulpar mais satisfatórias.The aim of this study was to evaluate: 1) the pulpal response of human primary teeth after pulpotomies with different pulp capping materials through clinical, radiographic and histological analysis; and 2) the expression of Dentin Matrix Protein 1 (DMP-1) in the pulpal response. Twenty-five primary molars of 22 children aged 6-9 years-old were criteriously selected. The teeth were randomly divided into the following groups: Buckleys Formocresol (diluted 1:5) (Group I - FC), Calcium Hydroxide - (Group II CH), Mineral Trioxide Aggregate (Group III MTA), Otosporin® + Calcium Hydroxide (Group IV OTO+CH) and Portland cement (Group V PC); and treated by the conventional technique of pulpotomy at a single appointment. The clinical and radiographic evaluations were performed until the teeth underwent normal exfoliation. The teeth were extracted and processed for histological and immunohistochemical analyses. The clinical and radiographic studies did not show statistically significant difference among the groups studied (p>0.05). The statistical analysis of the histological evaluation revealed statistically significant difference relating to the presence of the dentine barrier and odontoblastic layer between Group I(FC) and Groups II(CH), III(MTA), IV(OTO+CH) and V(PC) since the Group I(FC) did not exhibited dentine barrier and odontoblastic layer in any of the cuts observed. There was no statistically significant difference among the groups studied regarding to the evaluation of pulpal calcification. Concerning to internal resorption, there was a statistically significant difference among Group I(FC), II(CH) and Groups III(MTA), IV(OTO+CH) and V(PC) where internal resorption was not seen in the histological analyses. The analysis of the expression DMP-1 demonstrated for Group I(FC) immunostaining located in the area of pulpal calcifications within the roots of the teeth evaluated; however, neither odontoblastic layer nor dentine barrier were seen. The Groups II(CH), III(MTA), IV(OTO+CH) and V(PC) showed immunostaining in the area of dentine barrier and odontoblastic layer, evidencing the biocompatibility of these materials, as well as, the capacity for inducing dentine secretion. The comprehensive analyses of the results obtained in this study with the use of different techniques enabled to conclude that all materials employed showed clinical and radiographic success rates acceptable for the clinical indication. In the histological analysis, CH, MTA and PC materials exhibited the best results for pulpal repair. The expression of DMP-1 in the pulpal response allowed visualizing the biological inductive capacity of CH, MTA and PC materials on the process of pulpal repair and barrier formation. The comparison among the groups studied enable to find that MTA and PC were more efficient, providing the dentine barrier formation and more satisfactory responses of the dentino-pulpal complex
Clinical and radiographic study on 1:5 dilution of Buckleys formocresol, calcium hydroxide and calcium hydroxide preceded by an anti-inflammatory medicine used for pulpotomias of human primary teeth
O objetivo deste estudo foi comparar a eficácia clÃnica e radiográfica do formocresol de Buckley diluÃdo a 1/5, do Hidróxido de cálcio e do Hidróxido de cálcio precedido de um medicamento antiinflamatório o Otosporin® como agentes capeadores pulpares em dentes decÃduos humanos acometidos por cárie extensa. Quarenta e cinco molares decÃduos inferiores de 35 crianças com idades entre 5 e 9 anos foram criteriosamente selecionados. Os dentes foram aleatoriamente divididos nos grupos formocresol, hidróxido de cálcio ou hidróxido de cálcio precedido de antiinflamatório e tratados pela técnica convencional de pulpotomia em uma única sessão. Após a remoção da polpa coronária e hemostasia, no grupo do formocresol, uma bolinha de algodão embebida na solução foi colocada por 5 minutos sobre o remanescente pulpar e então a câmara pulpar preenchida com óxido de zinco e eugenol. Para o grupo hidróxido de cálcio, o pó foi acomodado sobre o remanescente pulpar com o auxÃlio de um porta amálgama estéril. No terceiro grupo uma bolinha de algodão embebida em Otosporin® foi colocada por 5 minutos sobre o remanescente pulpar e em seguida o pó de hidróxido de cálcio foi acomodado sobre o remanescente pulpar com o auxÃlio de um porta amálgama estéril. Todos os dentes foram restaurados com uma base de óxido de zinco e eugenol reforçado (IRM) e cimento de ionômero de vidro modificado por resina (Vitremer®). As avaliações clÃnicas e radiográficas foram realizadas nos perÃodos de 3, 6 e 12 meses do pós-operatório. Dos 45 dentes apenas 43 foram avaliados nos perÃodos de 3 e 6 meses, devido a desistência de 2 crianças, e aos 6 meses 2 dentes do grupo hidróxido de cálcio necessitaram ser extraÃdos devido falha clÃnica. Aos 12 meses, 7 dentes não puderam ser avaliados (3 dentes esfoliaram naturalmente e 4 dentes necessitaram ser extraÃdos devido falha clÃnica). Nenhum sinal de falha clÃnica ou radiográfica foi observado no grupo formocresol em todos os perÃodos avaliados. Os demais grupos estudados apresentaram reabsorções internas já aos 3 meses de controle pós-operatório, assim como a presença de barreira dentinária também foi observada já nesse perÃodo, e o grupo hidróxido de cálcio foi o único a apresentar falhas clÃnicas. O uso de um antiinflamatório parece auxiliar para um maior Ãndice de sucessos clÃnicos quando do uso de hidróxido de cálcio em pulpotomias de dentes decÃduos, pois esta se mostrou uma técnica extremamente sensÃvel.The aim of this study was to compare the clinical and radiographic effectiveness of formocresol, calcium hydroxide and calcium hydroxide preceded by an anti-inflammatory medicine: Otosporin® as pulp dressing agents in carious primary teeth. Forty-five primary mandibular molars of 35 children between 5 and 9 years old were carefully selected. The teeth were randomly assigned to formocresol, calcium hydroxide or calcium hydroxide proceeded of anti-inflammatory medicine groups, and treated by a conventional pulpotomy technique in a single session. After coronal pulp removal and hemostasia, in formocresol group, a cotton pellet, soaked in the solution, was placed for 5 minutes over the pulp stumps and the chamber was filled with zinc oxide-eugenol paste. For the group of calcium hydroxide, the powder was accommodated over the pulp stumps with the aid of a sterile door amalgamation. In the third group a cotton pellet, soaked in Otosporin® was placed for 5 minutes over the pulp stumps and then the calcium hydroxide powder was accommodated over the pulp stumps with the aid of a sterile door amalgamation. All the teeth were restored with reinforced zinc oxide-eugenol base and resin modified glass ionomer cement (Vitremer®). Clinical and radiographic evaluations were recorded at 3, 6 and 12-month follow-up. Only 43 of 45 teeth were available for 3 and 6 month follow-up due to withdrawal of 2 children. But at 6 month follow-up 2 teeth of the calcium hydroxide group need to be extracted because clinical failure. At 12 month follow-up, 7 teeth could not be assessed (3 teeth naturally exfoliate and 4 teeth need to be extracted because clinical failure). No sign of clinical or radiographic failure was observed in the formocresol group in all periods. The other groups of the study revealed internal resorption already at 3 month follow-up, as well as the presence of dentin barrier was also observed that time already, and calcium hydroxide group was the only one to show clinical failure. The use of an anti-inflammatory medicine seems to help a larger index of clinical success when the use of calcium hydroxide for pulpotomies in primary teeth, because this seems to be a extremely sensitive technique
