    Youth Extremist Attitudes Driven By Destabilization of Traditional Culture

    The paper considers a relevant and burning issue of youth extremism. The authors analyze the reasons for radical and extreme attitudes of the youth and state that extremism among youth is caused by destabilization of the traditional culture. The core of the traditional culture is purposefully destabilized by creating some pseudo-scientific problems and assigning heroic features to minor people in history. Many young people are conscious in their choice of traditionalism with different archaic violence practices which try to openly challenge the modern civility standardized under liberal capitalism and a consumerist society. Mass culture extensively contributes into the development and expansion of the youth extremism patterns. The paper underlines that overall social psychological attitude and confidence of young people in their future, a developed self-consciousness and tradition succession play a huge role in combatting extremism among the youth. Traditional behavioral aspects could oppose extremist manifestations which are rejected at a deep ethnical level of culture, because they contradict its nature and search for development in the today’s world.     Keywords: youth; extremism; traditions; destabilization; culture

    Antiremodelling Efficacy and Clinical Safety of Zofenopril in Patients with Grade 1and 2 Hypertension

    Objective: to estimate the antihypertensive, antiremodelling efficacy and clinical tolerability of the monotherapy with Zofenopril in patients with Grade 1 and 2 hypertension (HT 1 and 2) Materials and Methods: The study included 30 patients aged from 30 to 60 years with HT 1 and 2 (ESH/ESC, 2013) without severe comorbidities and cardiovascular complications. Zofenopril was prescribed as monotherapy to HT patients who had never been treated before or patients after one week of lavage from previous antihypertensive therapy, who did not reach target levels of BP. Before and during treatment all patients were checked on office BP using Korotkov’s method and ambulatory blood pressure monitoring (ABPM). Echocardiography and Doppler sonography were carried out by standard methods using the recommendations of the American Society of Echocardiography. Intima-media thickness (IMT) of the carotid artery and brachial artery was measured by a 7.5MHz high-resolution ultrasound. Assessment of flow-mediated dilation (FMD) of the brachial artery was used as a method of determining endothelial function. Results: A 12-week monotherapy with Zofenopril in average daily dose of 36.0±19.54 mg showed a high antihypertensive efficacy and a good safety profile without side effects. We noted a reliable decrease in systolic BP (SBP), diastolic BP (DBP), mean BP, and pulse pressure (PP) by -19.53±5.93%, -18.64±7.18%, BP -19.05±6.14%, and -20.65±12.07%, respectively. Target SBP, DBP, and SBP+DBP were reached in 90%, 86.6%, and 83.3% of patients, respectively. We found a significant regression of LVH, significant improvement in volume indicators of LV echogeometry and parameters of FMD of the brachial artery, as well as a decrease in IMT of carotid and brachial arteries. Monotherapy with Zofenopril showed metabolic neutrality regarding the lipid and carbohydrate metabolism, a good safety profile without the side effects and undesired events

    Plant protection measures in flax cultivation should provide sufficiently clean from weeds, healthy sowings, forming the crop of necessary quality level, corresponding to economic and ecological criteria of modern agrotechnologies. The work aims to find promising herbicidal and protectivestimulating agents for providing cleanness from weeds and the health of flax crops when treating them with new pesticides, contributing to increasing the yield and quality of flax products. According to the classical methodology of scientific agronomy, the main experiments were carried out in the fields of FSBSI Federal Scientific Centre for Bast Crop, Flax Research Institute in the Tver region. As a result of experiments in 2018-2020, the herbicide Shanti and its mixture with Shanstrel 300 showed relatively high biological efficacy in protecting flax against dicotyledonous weeds. Furthermore, the addition of graminicides Haloshans or Cletoshans, growth regulator Artafit or fungicide Zimoshans to anti-wort herbicides did not reduce the effectiveness of the drug mixtures on dicotyledonous weeds and eliminated cereal weeds in crops. Additionally, Artafit and Zimoshans effectively protected flax from pasmo and other diseases, naturally increasing the yield of flax products. Its maximum values in the experiment were obtained in the variant of Artafit application together with herbicides. The use of Artafit had a positive effect on the quality of flax straw, increased it by 1-2 grades (from 2.00 to 2.50).Меры защиты растений при возделывании льна должны обеспечивать в достаточной степени чистые от сорняков, здоровые посевы, формирующие урожай культуры необходимого уровня качества, соответствующего экономическим и экологическим критериям современных агротехнологий. Цель работы – поиск перспективных гербицидных и защитно-стимулирующих средств для обеспечения чистоты от сорняков и здоровья посевов льна при обработке их новыми пестицидами, способствующими повышению урожайности и качества льнопродукции. Основные эксперименты по классической методике научной агрономии выполнены на полях ФГБНУ ФНЦ ЛК ОП НИИ льна в Тверской области. В результате проведения опытов в 2018–2020 гг. гербицид Шансти и его смесь с препаратом Шанстрел 300 продемонстрировали относительно высокую биологическую эффективность защиты льна от двудольных сорняков. Добавка к противодвудольным гербицидам граминицидов Галошанс или Клетошанс, регулятора роста Артафит или фунгицида Зимошанс не снизила эффективности действия смесей препаратов на двудольные сорняки и уничтожила в посевах злаковые сорняки. Дополнительно Артафит и Зимошанс эффективно защищали лен от пасмо и других болезней, закономерно повышая урожайность льнопродукции. Ее максимальные в опыте показатели получены в варианте применения Артафита совместно с гербицидами. Применение препарата Артафит оказало положительное влияние на качество льносоломы – повысило его на 1–2 сортономера (с 2,00 до 2,50)

    То select the optimal method of contraception for women with uterine scar after Cesarean section with consideration of the state of endometrium and of uterine scar, the method of intrauterine contraceptive was used in 29 patients. Hormonal contraception is offered for 47 women: 19 breast feeding women - progestin oral contraceptive and 28 patients who stopped breastfeeding - monophasic combined OK. During the study a high contraceptive efficacy and acceptability of intrauterine and hormonal contraception were revealed in women with a uterine scar during a year of use.С целью выбора оптимального метода контрацепции женщинам с рубцом на матке после кесарева сечения с учетом состояния эндометрия и рубца на матке использовали внутриматочный метод контрацепции 29 пациенткам. 47 женщинам предложена гормональная контрацепция: из них 19 кормящим грудью - оральный гестагенный контрацептив и 28 пациенткам, прекратившим грудное вскармливание -монофазный комбинированный ОК. В ходе исследования была выявлена высокая контрацептивная эффективность и приемлемость внутриматочной и гормональной контрацепции у женщин с рубцом на матке в течение года использования

    Immunoglobulin, glucocorticoid, or combination therapy for multisystem inflammatory syndrome in children: a propensity-weighted cohort study

    Background: Multisystem inflammatory syndrome in children (MIS-C), a hyperinflammatory condition associated with SARS-CoV-2 infection, has emerged as a serious illness in children worldwide. Immunoglobulin or glucocorticoids, or both, are currently recommended treatments. Methods: The Best Available Treatment Study evaluated immunomodulatory treatments for MIS-C in an international observational cohort. Analysis of the first 614 patients was previously reported. In this propensity-weighted cohort study, clinical and outcome data from children with suspected or proven MIS-C were collected onto a web-based Research Electronic Data Capture database. After excluding neonates and incomplete or duplicate records, inverse probability weighting was used to compare primary treatments with intravenous immunoglobulin, intravenous immunoglobulin plus glucocorticoids, or glucocorticoids alone, using intravenous immunoglobulin as the reference treatment. Primary outcomes were a composite of inotropic or ventilator support from the second day after treatment initiation, or death, and time to improvement on an ordinal clinical severity scale. Secondary outcomes included treatment escalation, clinical deterioration, fever, and coronary artery aneurysm occurrence and resolution. This study is registered with the ISRCTN registry, ISRCTN69546370. Findings: We enrolled 2101 children (aged 0 months to 19 years) with clinically diagnosed MIS-C from 39 countries between June 14, 2020, and April 25, 2022, and, following exclusions, 2009 patients were included for analysis (median age 8·0 years [IQR 4·2–11·4], 1191 [59·3%] male and 818 [40·7%] female, and 825 [41·1%] White). 680 (33·8%) patients received primary treatment with intravenous immunoglobulin, 698 (34·7%) with intravenous immunoglobulin plus glucocorticoids, 487 (24·2%) with glucocorticoids alone; 59 (2·9%) patients received other combinations, including biologicals, and 85 (4·2%) patients received no immunomodulators. There were no significant differences between treatments for primary outcomes for the 1586 patients with complete baseline and outcome data that were considered for primary analysis. Adjusted odds ratios for ventilation, inotropic support, or death were 1·09 (95% CI 0·75–1·58; corrected p value=1·00) for intravenous immunoglobulin plus glucocorticoids and 0·93 (0·58–1·47; corrected p value=1·00) for glucocorticoids alone, versus intravenous immunoglobulin alone. Adjusted average hazard ratios for time to improvement were 1·04 (95% CI 0·91–1·20; corrected p value=1·00) for intravenous immunoglobulin plus glucocorticoids, and 0·84 (0·70–1·00; corrected p value=0·22) for glucocorticoids alone, versus intravenous immunoglobulin alone. Treatment escalation was less frequent for intravenous immunoglobulin plus glucocorticoids (OR 0·15 [95% CI 0·11–0·20]; p<0·0001) and glucocorticoids alone (0·68 [0·50–0·93]; p=0·014) versus intravenous immunoglobulin alone. Persistent fever (from day 2 onward) was less common with intravenous immunoglobulin plus glucocorticoids compared with either intravenous immunoglobulin alone (OR 0·50 [95% CI 0·38–0·67]; p<0·0001) or glucocorticoids alone (0·63 [0·45–0·88]; p=0·0058). Coronary artery aneurysm occurrence and resolution did not differ significantly between treatment groups. Interpretation: Recovery rates, including occurrence and resolution of coronary artery aneurysms, were similar for primary treatment with intravenous immunoglobulin when compared to glucocorticoids or intravenous immunoglobulin plus glucocorticoids. Initial treatment with glucocorticoids appears to be a safe alternative to immunoglobulin or combined therapy, and might be advantageous in view of the cost and limited availability of intravenous immunoglobulin in many countries.
