    Stent pour implantation percutanée d'une prothÚse textile de valve cardiaque

    L'implantation percutanée de prothÚses valvulaires cardiaques est un défi aujourd'hui relevé par la médecine. Pour y répondre, nous développons un dispositif capable d'assurer le fonctionnement d'une prothÚse textile de valve cardiaque sur son site d'implantation tout en étant compressible pour passer par les voies naturelles. L'expérimentation met en avant l'influence du stent sur la régurgitation statique et confirme le bon fonctionnement dynamique de la valve ainsi que le maintien en place géométrique de l'endoprothÚse. De part sa fabrication à la maniÚre d'un tressage, la compressibilité du stent est bonne et ce malgré l'usage de fil d'acier. L'implantation nécessite quant à elle le développement d'un cathéter spécifique afin de positionner l'endoprothÚse sur le lieu du déploiement. Les résultats actuels sont encourageants et les performances in-vitro proches de celles observées pour les prothÚses mécaniques et biologiques. La réponse du stent vis à vis de sollicitations répétées sera étudiée à court term

    Nellix Device Failure Mechanisms Analysis on Explanted Grafts

    Objective: To understand possible reasons for poor durability of the Nellix (Endologix Inc., Irvine, USA) endovascular aneurysm sealing (EVAS) device. Materials and Methods: 21 Nellix endoprostheses explanted for endoleaks and migration underwent visual examinations of stent structures and instrumental examinations of the polymer endobags on 4 devices. We harvested 2.0-gram polymer slices out of each of them and tested the samples in an in vitro implantation replication that included wet and dry exposures. During the wet phase, we placed samples in a beaker with saline, mimicking the filling of the endobags during implantation. An exposure to a 37°C environment with 60% humidity during the dry phase replicated the postimplantation conditions inside the aneurysmal sac. Results: Iatrogenic defects affected 16 (76%) metal stents and 20 (95%) endobags. The polymer was disintegrated owing to degradation in 15 (71%) cases. The polymer could lose more than 70% of its initial weight when partially dehydrated and regain 80% when placed in saline. We observed volume decrease and polymer fragmentation during these study phases. Conclusions: The polymer can lose weight and volume while it dehydrates. This structural degradation of the polymer could lead to the development of endoleaks and/or migration of the device. Clinical Impact: Based on the results of previous investigations, due to possible endovascular device degradation, patients with endografts should be offered life-long surveillance, and the Nellix device is no exception. Herein we suggest polymer degradation as one of the possible reasons for the device failure. Although Nellix has been withdrawn from the market, there are numerous patients with this type of endograft. Due to its unpredictable performance in the medium and long term, these patients should be recommended enhanced life-long surveillance every 6 months. Any suspicious conditions during the follow-up must be taken seriously and explantation should be considered.Peer reviewe

    Développement d'une prothÚse de valve cardiaque en matériau textile

    Cette étude a pour but de développer un concept de prothÚse de valve cardiaque obtenue à partir de matériau souple textile, qui de part sa structure discontinue allie des caractéristiques de trÚs bonne résistance mécanique et de souplesse. Différents principes de fabrication de prototypes ainsi que les contraintes qui leurs sont associées sont présentés. Les performances de ces prototypes, en terme de régurgitation statique, de régurgitation dynamique et de pertes de charge sont ensuite évaluées par des essais in vitro. Les résultats obtenus permettent de mettre en évidence les différents paramÚtres de fabrication de la prothÚse, tant au niveau de l'étoffe (armure, indice de saturation, diamÚtre de filament ... ) qu'au niveau du process de réalisation, qui ont une influence significative sur le comportement de celle-ci. Une évaluation qualitative de l'influence de la rugosité de la surface textile sur 1 'écoulement est également présentée.The goal of this work is to develop a textile heartvalve prosthesis concept. Textile material, thanks to its discrete structure, associùtes low bending stiffness with high membrane résistance. Different prototype manufacturing techniques and their constraints are presented. Performances of the obtained prototypes are evaluated in vitro, in terms of static and dynamic régurgitation as well as pressure drop accross the valve. The results bring to the fore which are the textile parameters (weawing, saturation index, filament diameter ... ) and the manufacturing process that do influence the prosthesis behaviour signlf'icantly.     Mechanical Performance Assessment of Physician Modified Aortic Stent Graft

    International audienceObjective: This study aimed to compare various fenestration configurations of physician modified aortic stent grafts in order to identify which design parameters have a significant influence on the mechanical behaviour of the fenestration. Methods: the fenestration configurations were considered according to different manufacturing parameters: cutting technique, fenestration reinforcement, suture material, reinforcement loop design, and number of suture points. The performance of the graft/bridging stent assembly was assessed at various levels: (1) branch pull out force; (2) fenestration enlargement and rupture strength; (3) balloon angioplasty resistance; and (4) behaviour under cyclic fatigue. Results: Sixty manual fenestrations were created. The tests performed on the fenestrations had several main findings. First, reinforcement increased the radial force on the branch, which increased the pull out force; this may limit migration of the bridging stent in vivo. The phenomenon was amplified with a snare reinforced fenestration, which seemed to be the most efficient. Moreover, increasing the number of suture passes also appeared to increase the branch extraction force securing the assembly. The enlargement tests showed that non-reinforced fenestrations had the weakest radial strength. This was confirmed with the balloon angioplasty test, which showed that these latter specimens undergo the most significant textile degradation. After fatigue tests, all fenestrations were larger, showing that elastic recoil was incomplete in all samples. The largest recoil was observed in the non-reinforced ophthalmological cautery (OC) fenestrations (40%). Regarding the behaviour of the samples up to rupture, all samples behaved in a similar way; however, the double loop fenestration strength level was the highest. Conclusion: This study demonstrated that the snare double loop reinforcement has an advantage regarding durability of the graft branch assembly. Moreover, non-reinforced fenestrations show signs of weakness and lack of stability, which questions the in situ or laser fenestration procedures

    Apprentissage profond pour l’aide au diagnostic du mĂ©lanome Ă  partir d’exemple

    One study reveals that 15404 new cases of cutaneous melanoma have been estimated in France in 2017. The 5-year survival rate of a person with advanced melanoma is much lower than 20%, which raises the need for diagnose it at an early stage. The purpose of this work is to build a supervised computer-aided diagnosis system for melanoma. The database used for the implementation includes 1356 images divided into 9 classes. Two approaches have been implemented : classical approach and deep learning approach. The classical approach combinestwo support vector machine classifiers (SVM) trained on features extracted from three extractors. This approach yielded an area under the receptor curve (AUC) of 0.88, a sensitivity (SE) of 89% and a specificity (SPEC) of 77%. The deep learning approach uses features extracted from two pre-trained models VGG16 and resnet50 to train two linear SVM. The scores from these two classifiers are combined using a logistic regression algorithm to obtain the classification. This approach yielded an AED-CCR of 0.88, SE of 78% and SPEC of 83%

    Comprehensive Review of Physician Modified Aortic Stent Grafts: Technical and Clinical Outcomes

    International audienceObjective: Physician modified stent grafts (PMSGs) present satisfactory results in selected cases of complex aortic pathologies. However, the technique lacks standardisation and depends on the surgeon and aortic segment. The aim of this article is to review comprehensively the technical details and clinical results of PMSGs related to patients with pathology in all aortic locations. Methods: A MEDLINE search (last search 20 April 2020) identified 20 relevant papers in the English language published over the last 20 years evaluating clinical outcomes after a PMSG and specifying the technical details to design it. Results: Seven hundred and eleven patients were included in the analyses, with 59% being operated on as an emergency. Ninety-two per cent of abdominal aortic segment PMSGs (A-PMSGs) were performed either as an emergency or before 2012. The main indications were available in 670 cases; 435 were degenerative aneurysms (64.9%) and 171 were aortic dissections (25.5%). Most of the endografts used were composed of polyethylene terephthalate, except for the Ankura (expanded polytetrafluoroethylene [Lifetech Scientific, Shenzhen, China]; n = 50, 7.5%). The Valiant (Medtronic, Minneapolis, MN, USA) represented 65% (n = 169) of aortic arch PMSGs (aa-PMSGs) and the Zenith platform (Cook Medical, Bloomington, IN, USA) 51% (n = 139) of A-PMSGs. A snare was used to reinforce the fenestration in 458 PMSGs (66%) and a cautery device cut the fenestration in 484 (75%) PMSGs. No bridging stent was used in 47 (7.0%) PMSGs (these aa-PMSGs had large fenestrations). Technical success ranged from 87.5% to 100% and 30 day mortality from 0% to 8%. Primary branch patency ranged from 96.3% to 100% at 12 month follow up. Zero to 14% of patients experienced type 3 or type 1 endoleak at 14.8 month follow up. Conclusion: PMSG is a useful technique, particularly when validated treatments are not available. However, it is a non-standardised technique and the long term consequences of modifications remain unknown
