    Objectives: To quantify livestock-associated MRSA (LA-MRSA) exposure to workers in pig slaughterhouses and assess associated risk factors for carriage in slaughterhouse workers. Methods: A cross-sectional study in three Dutch pig slaughterhouses was undertaken. Nasal swabs of 341 participants, surface wipes, air, and glove samples were screened for presence of MRSA. Достоевском по преимуществу гностического мыслителя, а также эксплицируются теоретические предпосылки тенденциозного подхода к творчеству русского писателя. Акцентируется внимание на связи романов Достоевского с православной традицией, а также значимости этой позиции для становления русского самосознания и истории России.У статті на основі тексту роману «Брати Карамазови» спростовується позиція І. Євлампієва, який вбачає у Ф.М. Достоєвському переважно гностичного мислителя, а також експлікуються теоретичні передумови тенденційного підходу до творчості російського письменника. Робиться акцент на зв’язку романів Достоєвського з православною традицією, а також на значимості цієї позиції для становлення російської самосвідомості та історії Росії.In the article the position of I. Evlampiev, who considers F.M. Dostoyevskiy, for the most part, as a gnostic thinker, is being disproved on the basis of the text of the novel “The Karamazov brothers”. Also, theoretical preconditions of tendentious understanding of the creative work of the Russian writer are being explicated. Besides, the relationship between Dostoyevskiy’s novels and orthodox tradition and the importance of this position for establishing Russian self-consciousness and the history of Russia are being indicated

    Healthy worker survivor analysis in an occupational cohort study of Dutch agricultural workers

    OBJECTIVES: High microbial exposures in farmers and agricultural workers are associated with less atopy. Although it has been speculated that healthy worker survival could be an explanation, this has not been studied so far. Therefore, we investigated the presence of healthy worker survival in a five-year follow-up study of an occupational cohort of Dutch farmers and agricultural industry (company) workers. METHODS: We compared baseline demographic characteristics, respiratory health, atopy and endotoxin exposure of 259 workers followed up with 124 workers lost to follow-up. Additionally, baseline health status of 31 participants who had changed to lower exposure jobs at follow-up was compared to those with similar or higher exposure jobs at follow-up. RESULTS: In general, no major healthy worker survival effect was found. Nonetheless, small differences were observed between subjects included in follow-up and those lost to follow-up. Those lost to follow-up were older, had a lower peak expiratory flow, and were less often raised on a farm. Company workers lost to follow-up with a farm childhood had more often self-reported allergy, but this was not observed for subjects with atopic sensitization or other respiratory symptoms. No differences were found for any of the studied characteristics in participants with lower exposure at follow-up compared to participants with similar or higher exposure at follow-up. CONCLUSIONS: No major healthy worker survival is present in this organic dust exposed cohort. Differences between participants lost to follow-up and participants included in follow-up with regard to health characteristics are small and unlikely to explain the previously reported inverse associations between endotoxin exposure and atopy

    Respiratory symptoms and occupation: a cross-sectional study of the general population

    BACKGROUND: This study focused on respiratory symptoms due to occupational exposures in a contemporary general population cohort. Subjects were from the Dutch Monitoring Project on Risk Factors for Chronic Diseases (MORGEN). The composition of this population enabled estimation of respiratory risks due to occupation from the recent past for both men and women. METHODS: The study subjects (aged 20–59) were all inhabitants of Doetinchem, a small industrial town, and came from a survey of a random sample of 1104 persons conducted in 1993. A total of 274 cases with respiratory symptoms (subdivided in asthma and bronchitis symptoms) and 274 controls without symptoms were matched for age and sex. Relations between industry and occupation and respiratory symptoms were explored and adjusted for smoking habits and social economic status. RESULTS: Employment in the 'construction' (OR = 3.38; 95%CI 1.02 – 11.27), 'metal' (OR = 3.17; 95%CI 0. 98 – 10.28), 'rubber, plastics and synthetics' (OR = 6.52; 95%CI 1.26 – 53.80), and 'printing' industry (OR = 3.96; 95%CI 0.85 – 18.48) were positively associated with chronic bronchitis symptoms. In addition, the 'metal' industry was found to be weakly associated with asthma symptoms (OR = 2.59; 95%CI 0.87 – 7.69). Duration of employment within these industries was also positively associated with respiratory symptoms. CONCLUSION: Respiratory symptoms in the general population are traceable to employment in particular industries even in a contemporary cohort with relatively young individuals

    Airborne Endotoxin Concentrations in Homes Burning Biomass Fuel

    BACKGROUND: About half of the world's population is exposed to smoke from burning biomass fuels at home. The high airborne particulate levels in these homes and the health burden of exposure to this smoke are well described. Burning unprocessed biological material such as wood and dried animal dung may also produce high indoor endotoxin concentrations.OBJECTIVE: In this study we measured airborne endotoxin levels in homes burning different biomass fuels. Methods: Air sampling was carried out in homes burning wood or dried animal dung in Nepal (n = 31) and wood, charcoal, or crop residues in Malawi (n = 38). Filters were analyzed for endotoxin content expressed as airborne endotoxin concentration and endotoxin per mass of airborne particulate.RESULTS: Airborne endotoxin concentrations were high. Averaged over 24 hr in Malawian homes, median concentrations of total inhalable endotoxin were 24 endotoxin units (EU)/m(3) in charcoal-burning homes and 40 EU/m(3) in wood-burning homes. Short cooking-time samples collected in Nepal produced median values of 43 EU/m(3) in wood-burning homes and 365 EU/m(3) in dung-burning homes, suggesting increasing endotoxin levels with decreasing energy levels in unprocessed solid fuels.CONCLUSIONS: Airborne endotoxin concentrations in homes burning biomass fuels are orders of magnitude higher than those found in homes in developed countries where endotoxin exposure has been linked to respiratory illness in children. There is a need for work to identify the determinants of these high concentrations, interventions to reduce exposure, and health studies to examine the effects of these sustained, near-occupational levels of exposure experienced from early life
