    Effects of steroid hormones on uterine tissue remodeling of mouse menopause model

    Since the uterine is a sensitive tissue to steroid hormones, the aim of the present study was to investigate the effects of 17ÎČ-estradiol (E2) and progesterone (P4) alone or in combination on morphological and morphometrically parameters of ovariectomized mouse uterus.Adult virgin female mice (8-10 weeks old)were ovariectomized and treated with E2, P4, E2 followed by P4 and the oil vehicle alone for 5-days period. Uterine tissue was removed, and processed for histology assessment. The total uterine diameter were significantly higher (P < 0.05) following E2 treatment and Maximum diameter of uterine lumen, myometrium and endometrium were recorded after this treatment regimen. Sequential treatment with oestradiol and then progesterone caused both mitotic activity and cell degeneration. P4 treatment induced signs of active secretion in the endometrium glands and symptoms of degeneration and cell death. Estradiol treatment induced growth of uterine tissue. Subsequent treatment with progesterone stimulated uterine tissues to reach maximum size and maturity which is necessary to modify the uterus in preparation for pregnancy.

    Introduction: Chronic pain and the experience of living with it are unique. Individual perception of pain is affected by physical, psychological, and social variables. This study aimed to determine the effectiveness of transcranial direct current stimulation (tDCS) on anxiety and depression and physical symptoms of patients with chronic pain.Method: This study was a semi-experimental study with pre-test, post-test, and follow-up with the control group. The statistical population of the study included all outpatients of men and women referred to all medical centers in Tehran in 2019 who had received a definitive diagnosis of chronic pain by neurologists, rheumatologists and psychiatrists, and other relevant specialists. The sampling method in this study had two stages: first, through available sampling, selected from several women with a definite diagnosis of chronic pain, and then, based on inclusion criteria and conducting a pre-test session with interviews. A clinical trial conducted by a researcher under the supervision of a psychiatrist based on DSM-5 diagnostic criteria, 30 women (15 for each group) of women who had the highest scores (as baseline) after completion The questionnaires were selected by simple random sampling method and after random allocation, they were replaced in two groups of intervention and control. Data were obtained using the Beck depression questionnaire and the Beck anxiety questionnaire. Repeated variance analysis was used to analyze the data. The above analysis was carried out using SPSS.22 software.Results: The results showed that the effectiveness of transcranial direct current stimulation (tDCS) was effective on anxiety and depression and physical symptoms of people with chronic pain.Conclusion: It can be concluded that transcranial direct current stimulation (tDCS) is effective in reducing anxiety, depression, and physical symptoms of people with chronic pain and can be used to improve psychological problems in people with chronic pain

    Development and Validation of an Authorial Identity Model and Questionnaire: A Factor Analytic Approach

    From when the black box of authorial identity has been unpacked, the paucity of authorial identity model on the basis of a comprehensive theoretical framework addressed the need to establish a robust one (Cheung, Stupple, & Elander, 2015). The current study was comprised of three main phases including hypothesizing a model of authorial identity, developing and validating a questionnaire based on the model and finally testing the model based on the questionnaire data. The participants, including M.A. and PhD students, were 30 for initial piloting, 60 for reliability estimation, 140 for exploratory factor analysis, and 175 for confirmatory factor analysis. At first, drawing on Ivanič’s (1998) model of writer identity and Prior’s (2001) ways of classifying voice, reviewing the related literature, and consulting with a cadre of experts, a model of authorial identity was proposed. Secondly, a questionnaire was developed and validated based on the hypothesized model. The reliability of the questionnaire, estimated through Cronbach’s alpha, was 0.73. Following that, exploratory factor analysis identified four components, namely authorial voice and identity, authorial persona, authorial background, and authorial style. Ultimately, SEM was run using AMOS in the confirmatory factor analysis phase to test the model. The results of this multi-phase research are presented and discussed for underlining the key role of authorial identity in academic writing for both novice and professional academicians

    Synthesis and antibacterial activity of N-(5-benzylthio-1,3,4-thiadiazol-2-yl) and N-(5-benzylsulfonyl-1,3,4-thiadiazol-2-yl)piperazinyl quinolone derivatives

    A series of N-(5-benzylthio-1,3,4-thiadiazol-2-yl) and N-(5-benzylsulfonyl-1,3,4-thiadiazol-2-yl) derivatives of piperazinyl quinolones was synthesized and evaluated for antibacterial activity against Gram-positive and Gram-negative microorganisms. Some of these derivatives exhibit high activity against Gram-positive bacteria; Staphylococcus aureus and Staphylococcus epidermidis, comparable or more potent than their parent N-piperazinyl quinolones norfloxacin and ciprofloxacin as reference drugs. The SAR of this series indicates that both the structure of the benzyl unit and the S or SO2 linker dramatically impact antibacterial activity

    Global overview of the management of acute cholecystitis during the COVID-19 pandemic (CHOLECOVID study)

    Background: This study provides a global overview of the management of patients with acute cholecystitis during the initial phase of the COVID-19 pandemic. Methods: CHOLECOVID is an international, multicentre, observational comparative study of patients admitted to hospital with acute cholecystitis during the COVID-19 pandemic. Data on management were collected for a 2-month study interval coincident with the WHO declaration of the SARS-CoV-2 pandemic and compared with an equivalent pre-pandemic time interval. Mediation analysis examined the influence of SARS-COV-2 infection on 30-day mortality. Results: This study collected data on 9783 patients with acute cholecystitis admitted to 247 hospitals across the world. The pandemic was associated with reduced availability of surgical workforce and operating facilities globally, a significant shift to worse severity of disease, and increased use of conservative management. There was a reduction (both absolute and proportionate) in the number of patients undergoing cholecystectomy from 3095 patients (56.2 per cent) pre-pandemic to 1998 patients (46.2 per cent) during the pandemic but there was no difference in 30-day all-cause mortality after cholecystectomy comparing the pre-pandemic interval with the pandemic (13 patients (0.4 per cent) pre-pandemic to 13 patients (0.6 per cent) pandemic; P = 0.355). In mediation analysis, an admission with acute cholecystitis during the pandemic was associated with a non-significant increased risk of death (OR 1.29, 95 per cent c.i. 0.93 to 1.79, P = 0.121). Conclusion: CHOLECOVID provides a unique overview of the treatment of patients with cholecystitis across the globe during the first months of the SARS-CoV-2 pandemic. The study highlights the need for system resilience in retention of elective surgical activity. Cholecystectomy was associated with a low risk of mortality and deferral of treatment results in an increase in avoidable morbidity that represents the non-COVID cost of this pandemic

    Global, regional, and national incidence of six major immune-mediated inflammatory diseases: findings from the global burden of disease study 2019

    Background The causes for immune-mediated inflammatory diseases (IMIDs) are diverse and the incidence trends of IMIDs from specific causes are rarely studied. The study aims to investigate the pattern and trend of IMIDs from 1990 to 2019. Methods We collected detailed information on six major causes of IMIDs, including asthma, inflammatory bowel disease, multiple sclerosis, rheumatoid arthritis, psoriasis, and atopic dermatitis, between 1990 and 2019, derived from the Global Burden of Disease study in 2019. The average annual percent change (AAPC) in number of incidents and age standardized incidence rate (ASR) on IMIDs, by sex, age, region, and causes, were calculated to quantify the temporal trends. Findings In 2019, rheumatoid arthritis, atopic dermatitis, asthma, multiple sclerosis, psoriasis, inflammatory bowel disease accounted 1.59%, 36.17%, 54.71%, 0.09%, 6.84%, 0.60% of overall new IMIDs cases, respectively. The ASR of IMIDs showed substantial regional and global variation with the highest in High SDI region, High-income North America, and United States of America. Throughout human lifespan, the age distribution of incident cases from six IMIDs was quite different. Globally, incident cases of IMIDs increased with an AAPC of 0.68 and the ASR decreased with an AAPC of −0.34 from 1990 to 2019. The incident cases increased across six IMIDs, the ASR of rheumatoid arthritis increased (0.21, 95% CI 0.18, 0.25), while the ASR of asthma (AAPC = −0.41), inflammatory bowel disease (AAPC = −0.72), multiple sclerosis (AAPC = −0.26), psoriasis (AAPC = −0.77), and atopic dermatitis (AAPC = −0.15) decreased. The ASR of overall and six individual IMID increased with SDI at regional and global level. Countries with higher ASR in 1990 experienced a more rapid decrease in ASR. Interpretation The incidence patterns of IMIDs varied considerably across the world. Innovative prevention and integrative management strategy are urgently needed to mitigate the increasing ASR of rheumatoid arthritis and upsurging new cases of other five IMIDs, respectively.
