    ASYMMETRIC LEAVES1 regulates abscission zone placement in Arabidopsis flowers

    BACKGROUND: The sepals, petals and stamens of Arabidopsis flowers detach via abscission zones formed at their boundaries with the underlying receptacle. The ASYMMETRIC LEAVES1 (AS1) MYB transcription factor plays a critical role in setting boundaries between newly formed leaf primordia and the shoot meristem. By repressing expression of a set of KNOTTED1-LIKE HOMEODOMAIN (KNOX) genes from developing leaf primordia, AS1 and its partner ASYMMETRIC LEAVES2 allow the patterning and differentiation of leaves to proceed. Here we show a unique role for AS1 in establishing the positions of the sepal and petal abscission zones in Arabidopsis flowers. RESULTS: In as1 mutant flowers, the sepal abscission zones are displaced into inverted V-shaped positions, leaving behind triangular stubs of tissue when the organs abscise. Movement of the petal abscission zones is also apparent. Abscission of the medial sepals is delayed in as1 flowers; loss of chlorophyll in the senescing sepals contrasts with proximal zones that remain green. AS1 has previously been shown to restrict expression of the KNOX gene, BREVIPEDICELLUS (BP), from the sepals. We show here that loss of BP activity in as1 flowers is sufficient to restore the positions of the sepal and petal abscission zones, the sepal-receptacle boundary of the medial sepals and the timing of their abscission. CONCLUSIONS: Our results indicate that AS1 activity is critical for the proper placement of the floral organ abscission zones, and influences the timing of organ shedding

    Langerhans cell histiocytosis (LCH) may present as unifocal disease of the suprasellar region, with symptoms and signs of hypopituitarism, arginine vasopressin deficiency (AVP-D), and weight gain. Transcranial biopsy is necessary to define diagnosis and guide treatment decisions, but it is associated with significant morbidity. We describe a patient with Hashimoto thyroiditis and a single hypothalamic mass in whom LCH diagnosis was made by thyroid fine-needle aspiration cytology (FNAC) performed despite nonspecific findings in thyroid imaging, on the basis of a slightly elevated [18F]-fluorodeoxyglucose (FDG) avidity on PET/CT and volume increase during follow-up

    A digital self-management intervention for adults with type 2 diabetes: Combining theory, data and participatory design to develop HeLP-Diabetes

    Background Digital health interventions have potential to contribute to better health outcomes, better healthcare and lower costs. However, evidence for their effectiveness is variable. The development and content of digital health interventions are often not described in enough detail to enable others to replicate the research or improve on previous interventions. This has led to a call for transparent reporting of intervention content and development. Purpose To describe the development process and content of a digital self-management intervention for people with type 2 diabetes (HeLP-Diabetes) that has been found to achieve its target clinical outcome, the reduction of HbA1c, a measure of glycaemic control. Method We synthesised theory, data from existing research evidence and international guidelines, and new qualitative data from target users to identify the determinants of self-management and the content to be included in HeLP-Diabetes. Using an ongoing iterative participatory design approach the content of the intervention was written, produced, reviewed and changed. Conclusion It is possible to develop and transparently report self-management programmes for long-term conditions, which reflect current best evidence, theoretical underpinning and user involvement. We intend that reporting the development process and content will inform future digital intervention development

    Sífilis Congênita: desafios da assistência pré-natal e suas consequências

    RESUMO: A sífilis congênita é causa de grande morbidade na vida intrauterina e leva a desfechos negativos em grande parte dos casos. É transmitida verticalmente através do patógeno Treponema pallidum, quando há falha no tratamento da gestante e pode acontecer em qualquer fase da doença. Objetivou-se analisar os dados da literatura, visando relacionar os dados crescentes da sífilis com suas possíveis causas e consequências, atentando para a atenção primária de saúde no Brasil. Esta revisão se deu a partir da leitura de 20 artigos científicos, sendo que 13 são da base de dados Scielo, 3 do Pubmed e 4 do LILACS e buscou dados epidemiológicos, fatores agravantes, relação com o pré-natal, a escolaridade, raça, classe social e as dificuldades no tratamento da sífilis congênita. Foram encontrados índices de prevalência da sífilis em gestantes de 1,4% a 2,8%. É fato que cerca de 12 milhões de pessoas adultas são acometidos anualmente, sendo que 90% destas estão em países em desenvolvimento. O pré-natal é de suma importância para o diagnóstico precoce e tratamento correto das gestantes; a menor frequência de atendimento pré-natal foi associada aos desfechos de óbito fetal ou infantil, bem como a alta proporção de mortalidade fetal entre as mães que foram testadas no momento do parto. Foram também encontrados dados quanto às dificuldades na terapêutica, que são principalmente relacionadas à falta da penicilina e ao tratamento inadequado do parceiro. Após averiguar tais dados, foi percebida a correlação entre a situação da sífilis e a qualidade da assistência pré-natal.Palavras-chave:Sífilis Congênita. Transmissão vertical de doença infecciosa.     THE HEALTH SCHOOL PROGRAMME: A HEALTH PROMOTION STRATEGY IN PRIMARY CARE IN BRAZIL

    Introduction: the Health School Programme (HSP) should be understood as a permanent development process. In this context, the actions of a policy aimed at children and adolescentsare paramount in the HSP. Objective: to identify and describe the actions developed by the Family Health Group in the HSP, from the National Programme for ImprovingAccess and Quality of Primary Care (PIPCAQ). Methods: this cross-sectional research used secondary data collected fromthe 17,202 groups who joined PIPCAQ in 2012. Results: all regions showed significant results concerning the execution of school activities. the Northern region was the one that performed most school actions (80.5%), followed by the Northeast, Midwest, South and Southeast, respectively. However, some items, such as professional training in education and health work need to be streng the ned. Conclusion: HSP in Brazil has mobilised significant actions, even though it has not happened in homogeneously in all Brazilian regions

    Serotonergic mechanisms of trigeminal meningeal nociception: Implications for migraine pain

    Serotonergic mechanisms play a central role in migraine pathology. However, the region-specific effects of serotonin (5-HT) mediated via multiple types of receptors in the nociceptive system are poorly understood. Using extracellular and patch-clamp recordings, we studied the action of 5-HT on the excitability of peripheral and central terminals of trigeminal afferents. 5-HT evoked long-lasting TTX-sensitive firing in the peripheral terminals of meningeal afferents, the origin site of migraine pain. Cluster analysis revealed that in majority of nociceptive fibers 5-HT induced either transient or persistent spiking activity with prevailing delta and theta rhythms. The 5-HT3-receptor antagonist MDL-72222 or 5-HT1B/D-receptor antagonist GR127935 largely reduced, but their combination completely prevented the excitatory pro-nociceptive action of 5-HT. The 5-HT3 agonist mCPBG activated spikes in MDL-72222-dependent manner but the 5HT-1 receptor agonist sumatriptan did not affect the nociceptive firing. 5-HT also triggered peripheral CGRP release in meninges, which was blocked by MDL-72222.5-HT evoked fast membrane currents and Ca2+ transients in a fraction of trigeminal neurons. Immunohistochemistry showed expression of 5-HT3A receptors in fibers innervating meninges. Endogenous release of 5-HT from degranulated mast cells increased nociceptive firing. Low pH but not histamine strongly activated firing. 5-HT reduced monosynaptic inputs from trigeminal Aδ- and C-afferents to the upper cervical lamina I neurons and this effect was blocked by MDL-72222. Consistent with central inhibitory effect, 5-HT reduced CGRP release in the brainstem slices. In conclusion, 5-HT evokes powerful pro-nociceptive peripheral and anti-nociceptive central effects in trigeminal system transmitting migraine pain.The authors declare that the research was conducted in the absence of any commercial or financial relationships that could be construed as a potential conflict of interest.
