On Rigorous Derivation of the Enskog Kinetic Equation

We develop a rigorous formalism for the description of the kinetic evolution of infinitely many hard spheres. On the basis of the kinetic cluster expansions of cumulants of groups of operators of finitely many hard spheres the nonlinear kinetic Enskog equation and its generalizations are justified. It is established that for initial states which are specified in terms of one-particle distribution functions the description of the evolution by the Cauchy problem of the BBGKY hierarchy and by the Cauchy problem of the generalized Enskog kinetic equation together with a sequence of explicitly defined functionals of a solution of stated kinetic equation is an equivalent. For the initial-value problem of the generalized Enskog equation the existence theorem is proved in the space of integrable functions.Comment: 28 page

On the spectrum of a bent chain graph

We study Schr\"odinger operators on an infinite quantum graph of a chain form which consists of identical rings connected at the touching points by δ\delta-couplings with a parameter αR\alpha\in\R. If the graph is "straight", i.e. periodic with respect to ring shifts, its Hamiltonian has a band spectrum with all the gaps open whenever α0\alpha\ne 0. We consider a "bending" deformation of the chain consisting of changing one position at a single ring and show that it gives rise to eigenvalues in the open spectral gaps. We analyze dependence of these eigenvalues on the coupling α\alpha and the "bending angle" as well as resonances of the system coming from the bending. We also discuss the behaviour of the eigenvalues and resonances at the edges of the spectral bands.Comment: LaTeX, 23 pages with 7 figures; minor changes, references added; to appear in J. Phys. Aim. To study the frequency of prescribing antithrombotic agents in patients with non-valvular atrial fibrillation (AF) in real clinical practice, to evaluate changes of prescriptions from 2012 till 2020.Material and methods. The medical records of inpatients (Form 003/y) with the diagnosis AF, hospitalized in the cardiological department were analyzed. According to the inclusion criteria, the patients were over 18 years of age, established diagnosis of non-valvular AF. There were two exclusion criteria: congenital and acquired valvular heart disease and prosthetic heart valves. In retrospective analysis we have included 263 case histories in 2012, 502 ones in 2016 and 524 in 2020. CHA2DS2-VASc score was used for individual stroke risk assessment in AF. The rational use of the antithrombotic therapy was evaluated according with current clinical practice guidelines at analyzing moment.Results. During period of observation the frequency of antiplatelet therapy significantly decreased from 25,5% to 5,5% (р<0.001), decreased the frequency of administration of warfarin from 71,9% to 18,3% (р<0.001). The frequency of use of direct oral anticoagulants increased in 2020 compared to 2016 (р<0.001). For patients with a high risk of stroke anticoagulant therapy was administered in 71.8% of cases in 2012, 88.5% in 2016 and 92.5% in 2020. Before discharge from hospital majority of patients (72%) achieved a desired minimum international normalized ratio (INR) from 2.0 to 3.0 in 2012. In 2016 and 2020 an only 33% and 40.6% of patients achieved INR (2.0-3.0).Conclusion. Doctors have become more committed to following clinical guidelines during the period of the investigation. In 2020 antithrombotic therapy for atrial fibrillation was suitable according to current clinical guidelines

The Generalized Star Product and the Factorization of Scattering Matrices on Graphs

In this article we continue our analysis of Schr\"odinger operators on arbitrary graphs given as certain Laplace operators. In the present paper we give the proof of the composition rule for the scattering matrices. This composition rule gives the scattering matrix of a graph as a generalized star product of the scattering matrices corresponding to its subgraphs. We perform a detailed analysis of the generalized star product for arbitrary unitary matrices. The relation to the theory of transfer matrices is also discussed

Methods and materials: In June 2014 mobile military hospital was set up for providing qualified surgical help to warriors in the region of eastern Ukraine. 1460operations (62 laparoscopic) were carried out during first 9 months. Results: We use data of 23 patients suffered under abdomen and pelvis injuries: 18 had missile and gunshot wounds, 5 had closed traumas. Acute diseases of the abdomen cavity were diagnosed 39 patients. The penetrative character of shoot wound was excluded by 6 patients using the diagnostic laparoscopy. Out of 5 patients with closed traumas of abdomen, injuries of internal organs were identified in 2 cases, operations were finished laparoscopically. Suffering from urgent diseases of the abdomen cavity organs, 10 patients underwent the laparoscopic appendectomy for acute appendicitis; 2 persons - the laparoscopic diverticulectomy for Meckel’s diverticulitis; in 3 cases the ovarian apoplexy and haemoperitoneum were detected during the laparoscopy, the laparoscopic resection of ovarium was performed; 1 patient underwent laparoscopy for pancreonecrosis, 20 laparoscopic cholecystectomy for acute calculous cholecystitis; one laparoscopic cholecystectomy for cancer of the pancreas, obstructive jaundice; 2 patients had laparoscopic suturing of perforative ulcers in duodenum. Conclusion: Well-founded approaches of treatment and diagnosis of shoot wounds of abdomen and pelvis by using the video laparoscopic equipment in the field conditions (first time in the Ukrainian history) were performed. Application of the endovideo surgical technics allowed avoiding 20 useless laparotomies

Antibiotic Dosing in Chronic Kidney Disease

    Get PDF
Infectious process is an important cause of morbidity and mortality among patients with chronic kidney disease. Prescription of antibacterial drugs should take into account the pharmacokinetic parameters of the medicine and the individual characteristics of the patient. Adequate antibiotic dosing is crucial for positive treatment outcome and minimisation of side effects. The aim of the study was to analyse scientific literature on factors affecting the dosing of antibacterials in patients with chronic kidney disease. Since most antibacterial medicines are eliminated by the kidneys, a decrease in glomerular filtration rate or kidney function should be followed by the dose adjustment in order to prevent the medicine accumulation and reduce the risk of side effects. Antibiotic dosing in such patients should be accompanied by kidney function assessment and be adjusted to ensure effective and safe treatment, as well as prevention of bacterial resistance. The review provides data on the dosing of some antibiotic groups (beta-lactams, aminoglycosides, fluoroquinolones) at different creatinine clearance rates. Extrarenal excretion of medicines does not usually require the dose adjustment in patients with chronic kidney disease
