    Case History — Settlement Mitigation for Mat Foundation Using Lean Concrete Columns

    This paper presents a case history of Lean Concrete Column (LCC) design, prediction, installation, and monitoring for a 34-story high-rise condominium tower over a five-level underground parking substructure supported on a mat foundation in San Diego, California. The site constraints and the building configuration imposed unusual design and construction challenges, which resulted in high foundation pressures and eccentric loading for the planned building mat foundation. A geotechnical investigation consisting of deep test borings, Cone Penetrometer Tests (CPT), and laboratory tests indicated that formational soils underlying the site did not provide the necessary bearing capacity to directly support the structure on a conventional mat foundation system within acceptable settlement and structural limitations. Design constraints, economics, and constructability issues dictated solutions requiring an integral waterproofed substructural system supported on a mat foundation. A foundation system incorporating conventional piles structurally tied into the mat was not feasible because of waterproofing constraints. A determined practical solution was to incorporate a ground improvement technique that would be separate from the mat foundation, provide improved mat support, and reduce differential settlement to within tolerable limits. LCCs were found to be a viable method of ground improvement for reducing differential settlement of the mat to acceptable limits

    The Impact of Fall Efficacy on Occupational Performance in Community-Dwelling Older Adults

    Falls are a major health concern in the older adult (OA) population. While there is research on falls and their prevention, research on how low fall efficacy (FE) impacts the occupational engagement of the OA population is limited. FE is defined as the confidence a person has in his/her ability to complete a task without falling (Tinetti & Powell, 1993). A qualitative study was conducted using a phenomenological approach to explore the lived experiences of OAs with low FE and the impact on occupational performance. Participants who scored ≤ 6 on the Modified Fall Efficacy Scale (MFES) engaged in a semi-structured interview, that explored the relationship between low FE and participation in occupations. Researchers asked open-ended questions to explore the activities impacted by participants low FE. A constant comparison method was utilized to analyze the interviews. The findings suggested that participants discontinued certain occupations due to a poor fit between the environment and the occupational challenges. However, those who experienced a good fit between the environment and the occupational challenges continued to participate in the activity using environmental modifications when needed. The occupations that had the lowest average scores on the MFES were occupations that mandated a narrow base of support (BOS) and the shifting of one’s weight.     The effectiveness of dietary approaches to stop hypertension (DASH) counselling on estimated 10-year cardiovascular risk among patients with newly diagnosed grade 1 hypertension : a randomised clinical trial

    The Dietary Approaches to Stop Hypertension (DASH) has been shown to lower blood pressure in the West. However, the real-life impact of DASH on reducing cardiovascular (CV) risk in routine clinical setting has not been studied. Methods A parallel-group, open-labelled, physician-blinded, randomised controlled trial was conducted in January–June 2013 and followed up for 6- and 12-months in primary care settings in Hong Kong. Patients newly diagnosed with grade 1 hypertension (aged 40–70 years) who had no concomitant medical conditions requiring dietary modifications were consecutively recruited. Subjects were randomised to standard education (usual care) (n = 275), or usual care plus dietitian-delivered DASH-based dietary counselling in a single one-to-one session (intervention) (n = 281). Primary outcomes were the changes in estimated 10-year CV risk. Results Outcome data were available for 504 (90.6%) and 485 (87.2%) patients at 6 and 12 months, respectively. There was no difference in the reduction of 10-year CV risk between the two groups at 6 months (−0.13%, 95% confidence interval [95% CI] −0.50% to 0.23%, p = 0.477) and 12 months (−0.08%, 95% CI −0.33% to 0.18%, p = 0.568). Multivariate regression analyses showed that male subjects, younger patients, current smokers, subjects with lower educational level, and those who dined out for main meals for ≥4 times in a typical week were significantly associated with no improvements in CV risk. Conclusions The findings may not support automatic referral of newly diagnosed grade 1 hypertensive patients for further one-to-one dietitian counselling on top of primary care physician's usual care. Patients with those risk factors identified should receive more clinical attention to reduce their CV risk

    Transplantation of canine olfactory ensheathing cells producing chondroitinase ABC promotes chondroitin sulphate proteoglycan digestion and axonal sprouting following spinal cord injury

    Olfactory ensheathing cell (OEC) transplantation is a promising strategy for treating spinal cord injury (SCI), as has been demonstrated in experimental SCI models and naturally occurring SCI in dogs. However, the presence of chondroitin sulphate proteoglycans within the extracellular matrix of the glial scar can inhibit efficient axonal repair and limit the therapeutic potential of OECs. Here we have used lentiviral vectors to genetically modify canine OECs to continuously deliver mammalian chondroitinase ABC at the lesion site in order to degrade the inhibitory chondroitin sulphate proteoglycans in a rodent model of spinal cord injury. We demonstrate that these chondroitinase producing canine OECs survived at 4 weeks following transplantation into the spinal cord lesion and effectively digested chondroitin sulphate proteoglycans at the site of injury. There was evidence of sprouting within the corticospinal tract rostral to the lesion and an increase in the number of corticospinal axons caudal to the lesion, suggestive of axonal regeneration. Our results indicate that delivery of the chondroitinase enzyme can be achieved with the genetically modified OECs to increase axon growth following SCI. The combination of these two promising approaches is a potential strategy for promoting neural regeneration following SCI in veterinary practice and human patients

    Why Naive Quark Model Can Yield a Good Account of the Baryon Magnetic Moments

    The chiral quark model suggests that the baryon quark-sea is negatively polarized. This modifies the spin structure as given by the naive quark model and agrees with experimental data. However, for the magnetic moments, there is significant cancellation between the contritutions from this sea spin-polarization and the orbital angular momentum so that effectively the moments are given by the valence constituent quarks alone, as in the NQM

    The Angio-Fibrotic Switch of VEGF and CTGF in Proliferative Diabetic Retinopathy

    BACKGROUND: In proliferative diabetic retinopathy (PDR), vascular endothelial growth factor (VEGF) and connective tissue growth factor (CTGF) cause blindness by neovascularization and subsequent fibrosis, but their relative contribution to both processes is unknown. We hypothesize that the balance between levels of pro-angiogenic VEGF and pro-fibrotic CTGF regulates angiogenesis, the angio-fibrotic switch, and the resulting fibrosis and scarring. METHODS/PRINCIPAL FINDINGS: VEGF and CTGF were measured by ELISA in 68 vitreous samples of patients with proliferative DR (PDR, N = 32), macular hole (N = 13) or macular pucker (N = 23) and were related to clinical data, including degree of intra-ocular neovascularization and fibrosis. In addition, clinical cases of PDR (n = 4) were studied before and after pan-retinal photocoagulation and intra-vitreal injections with bevacizumab, an antibody against VEGF. Neovascularization and fibrosis in various degrees occurred almost exclusively in PDR patients. In PDR patients, vitreous CTGF levels were significantly associated with degree of fibrosis and with VEGF levels, but not with neovascularization, whereas VEGF levels were associated only with neovascularization. The ratio of CTGF and VEGF was the strongest predictor of degree of fibrosis. As predicted by these findings, patients with PDR demonstrated a temporary increase in intra-ocular fibrosis after anti-VEGF treatment or laser treatment. CONCLUSIONS/SIGNIFICANCE: CTGF is primarily a pro-fibrotic factor in the eye, and a shift in the balance between CTGF and VEGF is associated with the switch from angiogenesis to fibrosis in proliferative retinopathy

    Spin Studies of Nucleons in a Statistical Model

    We decompose various quark-gluon Fock states of a nucleon in a set of states in which each of the three-quark core and the rest of the stuff, termed as a sea, appears with definite spin and color quantum number, their weight being determined, statistically, from their multiplicities. The expansion coefficients in the quark-gluon Fock states have been taken from a recently proposed statistical model. We have also considered two modifications of this model with a view to reduce the contributions of the sea components with higher multiplicities. With certain approximations, we have calculated the quark contributions to the spin of the nucleon,the ratio of the magnetic moments of nucleons, their weak decay constant, and the ratio of SU(3) reduced matrix elements for the axial current. A reasonably close agreement with the corresponding experimental values have been obtained in all the three cases.Comment: 19 page
