Developmental-psychopathological considerations on the deficient integration of aspects of the theory of attachment, of pedagogics, and of family law
Bindung, Bildung und Entwicklungspsychopathologie sind bislang weitgehend unintegrierte Konzepte mit parallel verlaufenden Diskursen, die im Rahmen der kindlichen Entwicklung anhaltend eine wichtige Rolle spielen. Hinzu kommen in bestimmten Fällen (wie Trennung und Scheidung sowie gefährdendem Elternverhalten) familienrechtliche Aspekte, die ebenfalls separat diskutiert und nicht systematisch integriert werden. Der Beitrag versucht, die unterschiedlichen Diskurse und die Spannungsfelder, die sich dadurch auftun, aus Sicht eines Kinder- und Jugendpsychiaters zu beleuchten, Bezüge herzustellen und Ansatzpunkte für eine gelingende Zusammenführung und Integration herauszuarbeiten. To these are added, in some cases (such as separation or divorce as well as endangering parental behavior), aspects of family law which, in turn, are discussed separately and are not systematically integrated. The authors look at the different resulting discourses and fields of tension from the perspective of a child and adolescent psychiatrist and point out references and possible approaches for a successful conflation and integration. Risikoeinschätzung bei Vernachlässigung und Kindeswohlgefährdung im Säuglings- und Kleinkindalter - Chancen früher beziehungsorientierter Diagnostik
In Deutschland fehlt eine interdisziplinäre Verständigung auf standardisierte und wissenschaftlich geprüfte Risikoscreening- und Diagnoseverfahren sowie auf Vorgehensweisen und Angebote bei Vernachlässigung und Kindeswohlgefährdung. Für die Risikoeinschätzung haben sich Vorgehensweisen bewährt, die auf empirisch geprüfte Verfahren zurückgreifen und auf Grundlage aller Einzelergebnisse eine zusammenfassende "klinische Synopse" vornehmen. Bei Säuglingen und Kleinkindern mit besonders hoher Gefährdung hat es sich bewährt, elterliche Erziehungs- und Beziehungskompetenzen als relevante Faktoren bei der Risikoeinschätzung, aber auch als Ansatzpunkt für Prävention und Intervention zu berücksichtigen. Hierzu gehören bindungsorientierte und videogestützte Interaktionsdiagnostik beziehungsweise der Einbezug elterlicher Bindungsrepräsentationen. Ebenso haben sich für Intervention und Versorgung bindungstheoretisch konzeptualisierte Angebote zur Förderung elterlicher Feinfühligkeit bzw. zur Förderung sicherer Bindung als wirksam erwiesen. What Protects Youth Residential Caregivers from Burning Out? A Longitudinal Analysis of Individual Resilience
Background: Professional caregivers are exposed to multiple stressors and have high burnout rates; however, not all individuals are equally susceptible. We investigated the association between resilience and burnout in a Swiss population of professional caregivers working in youth residential care. Methods: Using a prospective longitudinal study design, participants ( n = 159; 57.9% women) reported on burnout symptoms and sense of coherence (SOC), self-efficacy and self-care at four annual sampling points. The associations of individual resilience measures and sociodemographic variables, work-related and personal stressors, and burnout symptoms were assessed. Cox proportional hazards regressions were calculated to compute hazard ratios over the course of three years. Results: Higher SOC, self-efficacy and self-care were related to lower burnout symptoms in work-related and personal domains. Higher SOC and self-efficacy were reported by older caregivers and by those with children. All three resilience measures were highly correlated. A combined model analysis weakened the protective effect of self-efficacy, leaving only SOC and self-care negatively associated with burnout. Conclusion: This longitudinal analysis suggests that SOC and self-caring behaviour in particular protect against burnout. Our findings could have implications for promoting self-care practices, as well as cultivating a meaningful, comprehensible and manageable professional climate in all facets of institutional care

Framework and effects of the life story work based group intervention ANKOMMEN for adolescents in residential care in Germany. A qualitative analysis of interviews with participants
Adolescents in out-of-home care are a high-risk population due to their accumulation of adverse childhood experiences. Furthermore, out-of-home placement itself is a critical life event. Life story work (LSW) is a method for coping with critical life events. However, the need for a high amount of resources makes its implementation in care settings difficult. To improve the accessibility of LSW in residential care, the ANKOMMEN (English “Arriving”) intervention was developed as a manualized and cost-efficient group intervention. This study aims to explore the experiences of adolescents who took part in the intervention in order to examine how participation benefited them and what contributed to these beneficial effects. For this reason, we analyzed n = 48 individual interviews with adolescents after completing the intervention. The focused interview analysis method was used to transcribe and code the interviews. The qualitative analyses revealed that the framework of the intervention helped the participants to open up and to process parts of their history within the group. Moreover, the group setting was a major contributory factor to the positive effects of the intervention by encouraging the participants to exchange their experiences, through mutual peer support and the normalization of adverse thoughts and feelings. Furthermore, participation in the intervention increased self-esteem and self-efficacy in some participants. It contributed to the improvement of positive relationships, helped clarify placement associated events, thoughts and feelings, and improved the acceptance of the placement. Further research is needed to validate these findings and thus widen the range of application of LSW
Daring to process the trauma: using a web-based training to reduce psychotherapists’ fears and reservations around implementing trauma-focused therapy

European Journal of Psychotraumatology. 2019;10(1): 1696590.Although trauma-focused interventions are the first-line therapies for patients with posttraumatic stress disorder (PTSD), they are not frequently used in clinical practice. Factors preventing therapists from applying trauma-focused methods include a lack of training and negative attitudes towards trauma-focused therapy. The aim of the present study was to investigate which factors predict willingness to carry out trauma-focused therapy and to examine whether a web-based training is able to reduce negative attitudes and reservations about these interventions. In a wait-list controlled evaluation study, therapists (N = 499) were randomized into an intervention or a wait-list control group. Results show that trauma-treatment specific competencies and overcoming pre-existing concerns towards trauma-focused therapy significantly predict therapists’ willingness to utilize trauma-focused interventions. Thus, the content alignment of the web-based course is appropriate for improving therapists’ willingness to conduct trauma-focused therapy. A retrospective examination of therapists after the training and a comparison of fears and reservations before and after the training demonstrate a significant reduction of fears and reservations. In terms of perceived contraindications, no effects of the web-based training were found. The present study provides compelling evidence that web-based training in evidence-based PTSD therapy is able to reduce reservations that may prevent therapists from applying evidence-based trauma-focused interventions
