Family-building desires among adopted adolescents with lesbian, gay, and heterosexual parents
Objective: This study qualitatively examined family-building desires of diverse adopted adolescents. Background: Research on parenting aspirations has rarely included youth with LGBTQ+ parents and/or from adoptive families. Understanding diverse adopted adolescents\u27 feelings about parenthood may yield insights regarding identity and ideas about family. Methods: We conducted a thematic analysis of interview data from 48 adopted adolescents (27 were LGBTQ+) in the United States, aged 13 to 18, from lesbian, gay, and heterosexual two-parent families. Results: Most adolescents desired future parenthood, after achieving other normative milestones, and they typically did not feel familial or societal pressure to become parents. LGBTQ+ participants showed a preference for adoption while transracially adopted adolescents preferred biological parenthood. Conclusion: Guided by developmental approaches about identity and adoption, as well as queer family theory, we found that teenagers adopted by lesbian, gay, and heterosexual couples generally envisioned parenthood for themselves. Plans to do so varied by minoritized gender, sexual, and racial/ethnic identities. Thus, an intersectional perspective is imperative to understand youths\u27 thoughts about family building. Implications: Our findings reveal insights into adopted adolescents\u27 constructed future identities. Practitioners\u27 understanding of adopted adolescents\u27 development and future planning may be enhanced when adolescents\u27 perspectives are considered
Family Conflict Observations and Outcomes among Adopted School-Age Children with Lesbian, Gay, and Heterosexual Parents
Children’s externalizing problems are associated with family conflict among children and their biological parents, yet these linkages have remained unexamined among adoptive or lesbian and gay parent families. Investigating family processes facilitative of adjustment among adoptees, who face unique developmental challenges, is warranted. This multimethod study of 96 (26 lesbian, 29 gay, 41 heterosexual parent) adoptive families examined observations of adoptive family conflict and associations with child adjustment and feelings about adoption (children’s Mage = 8 years). The sample was recruited from 5 private, domestic infant adoption agencies across the United States. Parents and children reported about children’s externalizing problems and feelings about adoption, respectively. Observations of family conflict interaction were rated from videotaped family discussions. Family interactions were associated with children’s behavioral and adoption-specific adjustment, yet analysis of variance and hierarchical linear modeling analyses revealed no differences by parental sexual orientation in family dynamics (i.e., negativity/conflict, positive affect, cohesiveness) or child outcomes. Parents generally reported children to have few externalizing behaviors. Children reported positive feelings, moderate preoccupation, and low negativity about their adoption. These findings extend the family systems literature about conflict and child development among diverse families with sexual minority parents and adopted children. Practitioners who work with adoptive and sexual minority parent families can encourage positive and cohesive family interactions in supporting children’s adjustment. Coparenting among lesbian, gay, and heterosexual couples: Associations with adopted children’s outcomes
Coparenting is associated with child behavior in families with heterosexual parents, but less is known about coparenting among lesbian-and gay-parent families. Associations were studied among self-reported divisions of labor, coparenting observations, and child adjustment (M age = 3 years) among 104 adoptive families headed by lesbian, gay, or heterosexual couples. Lesbian and gay couples reported sharing child care, whereas heterosexual couples reported specialization (i.e., mothers did more child care than fathers). Observations confirmed this pattern-lesbian and gay parents participated more equally than heterosexual parents during family interaction. Lesbian couples showed the most supportive and least undermining behavior, whereas gay couples showed the least supportive behavior, and heterosexual couples the most undermining behavior. Overall, supportive coparenting was associated with better child adjustment

Theory in highly cited studies of sexual minority parent families: variations and implications
This paper includes a systematic review and citation analysis of the literature regarding sexual minority parent families, particularly attending to what theories have been used, and how. We consider the importance of theoretical frameworks for future research and implications for policy, practice, and law related to sexual minority parent families. Our review targets 30 highly cited studies located through Google Scholar (as an interdisciplinary search engine) and published within a specific time frame (2005-2010). We highlight the dominant theoretical models employed across disciplines studying sexual minority parent families. While the majority of studies reviewed referred to theoretical models or perspectives, explicit theoretical grounding was frequently lacking. Instead, the empirical work reviewed appeared to have a predominantly applied focus in addressing public debates on sexual minority parent families. We provide recommendations for how theory might be more fully integrated into the social science literature on sexual minority parents and their children
Mapping and modelling the geographical distribution and environmental limits of podoconiosis in Ethiopia
BACKGROUND
Ethiopia is assumed to have the highest burden of podoconiosis globally, but the geographical distribution and environmental limits and correlates are yet to be fully investigated. In this paper we use data from a nationwide survey to address these issues.
METHODOLOGY
Our analyses are based on data arising from the integrated mapping of podoconiosis and lymphatic filariasis (LF) conducted in 2013, supplemented by data from an earlier mapping of LF in western Ethiopia in 2008-2010. The integrated mapping used woreda (district) health offices' reports of podoconiosis and LF to guide selection of survey sites. A suite of environmental and climatic data and boosted regression tree (BRT) modelling was used to investigate environmental limits and predict the probability of podoconiosis occurrence.
PRINCIPAL FINDINGS
Data were available for 141,238 individuals from 1,442 communities in 775 districts from all nine regional states and two city administrations of Ethiopia. In 41.9% of surveyed districts no cases of podoconiosis were identified, with all districts in Affar, Dire Dawa, Somali and Gambella regional states lacking the disease. The disease was most common, with lymphoedema positivity rate exceeding 5%, in the central highlands of Ethiopia, in Amhara, Oromia and Southern Nations, Nationalities and Peoples regional states. BRT modelling indicated that the probability of podoconiosis occurrence increased with increasing altitude, precipitation and silt fraction of soil and decreased with population density and clay content. Based on the BRT model, we estimate that in 2010, 34.9 (95% confidence interval [CI]: 20.2-51.7) million people (i.e. 43.8%; 95% CI: 25.3-64.8% of Ethiopia's national population) lived in areas environmentally suitable for the occurrence of podoconiosis.
CONCLUSIONS
Podoconiosis is more widespread in Ethiopia than previously estimated, but occurs in distinct geographical regions that are tied to identifiable environmental factors. The resultant maps can be used to guide programme planning and implementation and estimate disease burden in Ethiopia. This work provides a framework with which the geographical limits of podoconiosis could be delineated at a continental scale
Mapping the geographical distribution of podoconiosis in Cameroon using parasitological, serological, and clinical evidence to exclude other causes of lymphedema
Background
Podoconiosis is a non-filarial elephantiasis, which causes massive swelling of the lower legs. It was identified as a neglected tropical disease by WHO in 2011. Understanding of the geographical distribution of the disease is incomplete. As part of a global mapping of podoconiosis, this study was conducted in Cameroon to map the distribution of the disease. This mapping work will help to generate data on the geographical distribution of podoconiosis in Cameroon and contribute to the global atlas of podoconiosis.
Methods
We used a multi‐stage sampling design with stratification of the country by environmental risk of podoconiosis. We sampled 76 villages from 40 health districts from the ten Regions of Cameroon. All individuals of 15-years old or older in the village were surveyed house-to-house and screened for lymphedema. A clinical algorithm was used to reliably diagnose podoconiosis, excluding filarial-associated lymphedema. Individuals with lymphoedema were tested for circulating Wuchereria bancrofti antigen and specific IgG4 in the field using the Alere Filariasis Test Strips (FTS) test and the Standard Diagnostics (SD) BIOLINE lymphatic filariasis IgG4 test (Wb123) respectively, in addition to thick blood films. Presence of DNA specific to W.bancrofti was checked on night blood using a qPCR technique.
Principal Findings
Overall, 10,178 individuals from 4,603 households participated in the study. In total, 83 individuals with lymphedema were identified. Of the 83 individuals with lymphedema, two were found to be FTS positive and all were negative using the Wb123 test. No microfilaria of W. bancrofti were found in the night blood of any individual with clinical lymphedema. None were found to be positive for W. bancrofti using qPCR. Of the two FTS positive cases, one was positive for Mansonella perstans DNA, while the other harbored Loa loa microfilaria. Overall, 52 people with podoconiosis were identified after applying the clinical algorithm. The overall prevalence of podoconiosis was found to be 0.5% (95% [confidence interval] CI; 0.4-0.7). At least one case of podoconiosis was found in every region of Cameroon except the two surveyed villages in Adamawa. Of the 40 health districts surveyed, 17 districts had no cases of podoconiosis; in 15 districts, mean prevalence was between 0.2% and 1.0%; and in the remaining eight, mean prevalence was between 1.2% and 2.7%.
Conclusions
Our investigation has demonstrated low prevalence but almost nationwide distribution of podoconiosis in Cameroon. Designing a podoconiosis control program is a vital next step. A health system response to the burden of podoconiosis is important, through case surveillance and morbidity management services
Plasma lipid profiles discriminate bacterial from viral infection in febrile children
Fever is the most common reason that children present to Emergency Departments. Clinical signs and symptoms suggestive of bacterial infection are often non-specific, and there is no definitive test for the accurate diagnosis of infection. The 'omics' approaches to identifying biomarkers from the host-response to bacterial infection are promising. In this study, lipidomic analysis was carried out with plasma samples obtained from febrile children with confirmed bacterial infection (n = 20) and confirmed viral infection (n = 20). We show for the first time that bacterial and viral infection produces distinct profile in the host lipidome. Some species of glycerophosphoinositol, sphingomyelin, lysophosphatidylcholine and cholesterol sulfate were higher in the confirmed virus infected group, while some species of fatty acids, glycerophosphocholine, glycerophosphoserine, lactosylceramide and bilirubin were lower in the confirmed virus infected group when compared with confirmed bacterial infected group. A combination of three lipids achieved an area under the receiver operating characteristic (ROC) curve of 0.911 (95% CI 0.81 to 0.98). This pilot study demonstrates the potential of metabolic biomarkers to assist clinicians in distinguishing bacterial from viral infection in febrile children, to facilitate effective clinical management and to the limit inappropriate use of antibiotics
