    Dual frequency laser with two continuously and widely tunable frequencies for optical referencing of GHz to THz beatnotes

    International audienceA dual-frequency 1.55 ÎŒm laser for CW low noise microwave, millimeter and sub millimeter wave synthesis is demonstrated, where frequency stabilization is possible on each wavelength independently. The solid state Er:Yb laser output power is 7 mW. The amplitude noise is −150 dBc/Hz at 1 MHz offset frequency. In free running regime, the frequency noise is 3.10^5/f Hz/sqrt(Hz) (800 Hz on a 1ÎŒs timescale), better than commercial fibered or semi-conductor sources at this wavelength

    Predictors for prolonged hospital stay solely to complete intravenous antifungal treatment in patients with candidemia: Results from the ECMM candida III multinational European observational cohort study

    Background To date, azoles represent the only viable option for oral treatment of invasive Candida infections, while rates of azole resistance among non-albicans Candida spp. continue to increase. The objective of this sub-analysis of the European multicenter observational cohort study Candida III was to describe demographical and clinical characteristics of the cohort requiring prolonged hospitalization solely to complete intravenous (iv) antifungal treatment (AF Tx). Methods Each participating hospital (number of eligible hospitals per country determined by population size) included the first ~ 10 blood culture proven adult candidemia cases occurring consecutively after July 1st, 2018, and treating physicians answered the question on whether hospital stay was prolonged only for completion of intravenous antifungal therapy. Descriptive analyses as well as binary logistic regression was used to assess for predictors of prolonged hospitalization solely to complete iv AF Tx. Findings Hospital stay was prolonged solely for the completion of iv AF Tx in 16% (100/621) of candidemia cases by a median of 16 days (IQR 8 – 28). In the multivariable model, initial echinocandin treatment was a positive predictor for prolonged hospitalization to complete iv AF Tx (aOR 2.87, 95% CI 1.55 – 5.32, p < 0.001), while (i) neutropenia, (ii) intensive care unit admission, (iii) catheter related candidemia, (iv) total parenteral nutrition, and (v) C. parapsilosis as causative pathogen were found to be negative predictors (aOR 0.22 – 0.45; p < 0.03). Interpretation Hospital stays were prolonged due to need of iv AF Tx in 16% of patients with candidemia. Those patients were more likely to receive echinocandins as initial treatment and were less severely ill and less likely infected with C. parapsilosis

    An operational HF system for single site localization

    International audienceThis paper presents an operational HF (3-30MHz) system designed for single site localization (SSL). An original version of the MUSIC algorithm jointly estimates the angles of arrival (azimuth and elevation) of incident waves and their polarization. The corresponding measurements are the input data of a ray tracing software based on the PRIME model of the ionosphere which computes the estimated position of the transmitter. Numerous radio links have been tested for long distances up to 2000 km. A very good agreement is observed between the actual and the estimated positions of the transmitters with a standard localization error of less than 10% of the distance to the receiving system

    Disease Entities in Mucormycosis

    Mucormycosis is an emerging life-threatening fungal infection caused by Mucorales. This infection occurs mainly in immunocompromised patients, especially with hematological malignancy, transplantation, or diabetes mellitus. Rhino-orbito-cerebral and pulmonary mucormycosis are the predominant forms. Interestingly, location is associated with the underlying disease as pulmonary mucormycosis is more frequent in hematological malignancy patients whereas rhino-orbito-cerebral mucormycosis is associated with diabetes. Cutaneous mucormycosis results from direct inoculation, mainly after trauma or surgery. Gastro-intestinal mucormycosis occurs after ingestion of contaminated food or with contaminated device and involves the stomach or colon. Disseminated disease is the most severe form and is associated with profound immunosuppression. Uncommon presentations with endocarditis, osteoarticluar or isolated cerebral infections are also described. Finally, health-care associated mucormycosis is a matter of concern in premature newborns and burn units. Clinical symptoms and CT scan findings are not specific, only the early reversed halo sign is associated with pulmonary mucormycosis. Circulating Mucorales DNA detection is a recent promising diagnostic tool that may lead to improving the diagnosis and prompting therapeutic initiation that should include antifungal treatment, correction of the underlying disease and surgery when feasible

    International audienceWe report experimental evidence of the coupling between the pump diode current and the output differential phase in a two-axis two-polarization dual-frequency solid-state laser. The effect has the same transfer function shape as that for a pump current to output power one, suggesting a pump AM/laser PM coupling in the laser medium. We report that the pump amplitude noise is negligible in the laser differential phase noise assuming a linear coupling. (C) 2016 Optical Society of Americ

    Background: In June 2020, a large randomised controlled clinical trial in the UK found that dexamethasone was effective in reducing the number of deaths in patients with severe coronavirus disease 2019 (COVID-19). Case description: We describe a patient with rapid worsening of COVID-19 pneumonia and its dramatic improvement under corticosteroids. Discussion: Corticosteroids could be useful in patients with an inflammatory profile, considering that acute respiratory distress syndrome may be the consequence of cytokine storm syndrome
