Update on Extended Treatment for Venous Thromboembolism
The importance of assessing the probability of venous thromboembolism recurrence, a condition that includes deep vein thrombosis and pulmonary embolism, lies in the fact that it is the most important factor in deciding the duration of anticoagulant treatment. Risk of recurrence depends mostly on the presence of a risk factor for developing venous thromboembolism, with patients with unprovoked events being at the higher risk of recurrence. The risk of recurrence needs to be balanced with the risk of bleeding and the potential severity of these thrombotic and hemorrhagic events. In patients with an unprovoked venous thromboembolism who complete treatment for the acute (first 10 days) and post-acute phase of the disease (from day 10 to 3-6 months), decision has to be made regarding prolonged antithrombotic therapy to prevent recurrences. The main goal of extended treatment is preventing recurrences with a safe profile in terms of bleeding risk. Many therapeutic options are now available for these patients, including antiplatelet therapy with aspirin or direct oral anticoagulants. Moreover, apixaban and rivaroxaban at prophylactic doses have demonstrated efficacy in preventing recurrences with a low risk of bleeding
Marginal/peripheral populations of forest tree species and their conservation status: report for Mediterranean region
The Mediterranean region includes 13 countries among Europe, Near Orient, and Africa. This area is a huge “hot spot” of cultures, religions, socio-economical situations, and of habitats and biodiversity. The report illustrates the geographical and ecological features of the region. Forest ecosystems and vegetation traits, with particular focus on forest species growing at the edge of their distribution range, are here compiled. The accuracy of reports, shows the interest and attention that the Mediterranean countries have for the different and complex situations of marginality that characterizes the presence of many forest species in this region. In this area the occurrence of 166 marginal and peripheral (MaP) populations of different species has been detected. Most of populations are characterised by vulnerability and fragility. Many MaP survive in environmental refugia and /or in isolated stands. However, most of the MaP populations identified by FP1202 experts are located in protected areas and also sometimes registered as seed sources, although Mediterranean region appears heterogeneous with respect to protection measures
Diffusion Pattern and Hotspot Detection of Dengue in Belo Horizonte, Minas Gerais, Brazil
This study considers the dengue occurrence in the city of Belo Horizonte over the last fifteen years. Approximately 186,000 cases registered from 1996 to 2011 were analyzed. The home address of individuals whose dengue case was notified was used as a proxy for exposure location. For determining possible outbreaks of disease and the specific patterns of dengue cases, spatial statistics used included Kernel's estimation. The occurrence of waves of dengue outbreaks was correlated with climatic and vector presence data. Outbreaks had different durations and intensities: case clustering, thinned out both spatially and temporally. These findings may be useful for public health professionals responsible for fighting the disease providing some tools for improving evaluation of interventions such as vector control and patient care, minimizing the collective and individual burden of the disease
A Hidden Eruption: The 21 May 2023 Paroxysm of the Etna Volcano (Italy)
On 21 May 2023, a hidden eruption occurred at the Southeast Crater (SEC) of Etna (Italy); indeed, bad weather prevented its direct and remote observation. Tephra fell toward the southwest, and two lava flows propagated along the SEC’s southern and eastern flanks. The monitoring system of the Istituto Nazionale di Geofisica e Vulcanologia testified to its occurrence. We analyzed the seismic and infrasound signals to constrain the temporal evolution of the fountain, which lasted about 5 h. We finally reached Etna’s summit two weeks later and found an unexpected pyroclastic density current (PDC) deposit covering the southern lava flow at its middle portion. We performed unoccupied aerial system and field surveys to reconstruct in 3D the SEC, lava flows, and PDC deposits and to collect some samples. The data allowed for detailed mapping, quantification, and characterization of the products. The resulting lava flows and PDC deposit volumes were (1.54 ± 0.47) × 106 m3 and (1.30 ± 0.26) × 105 m3, respectively. We also analyzed ground-radar and satellite data to evaluate that the plume height ranges between 10 and 15 km. This work is a comprehensive analysis of the fieldwork, UAS, volcanic tremor, infrasound, radar, and satellite data. Our results increase awareness of the volcanic activity and potential dangers for visitors to Etna’s summit area
Impacts of air pollution on human and ecosystem health, and implications for the National Emission Ceilings Directive. Insights from Italy
Across the 28 EU member states there were nearly half a million premature deaths in 2015 as a result of exposure to PM2.5, O3 and NO2. To set the target for air quality levels and avoid negative impacts for human and ecosystems health, the National Emission Ceilings Directive (NECD, 2016/2284/EU) sets objectives for emission reduction for SO2, NOx, NMVOCs, NH3 and PM2.5 for each Member State as percentages of reduction to be reached in 2020 and 2030 compared to the emission levels into 2005. One of the innovations of NECD is Article 9, that mentions the issue of “monitoring air pollution impacts” on ecosystems. We provide a clear picture of what is available in term of monitoring network for air pollution impacts on Italian ecosystems, summarizing what has been done to control air pollution and its effects on different ecosystems in Italy. We provide an overview of the impacts of air pollution on health of the Italian population and evaluate opportunities and implementation of Article 9 in the Italian context, as a case study beneficial for all Member States. The results showed that SO42− deposition strongly decreased in all monitoring sites in Italy over the period 1999–2017, while NO3− and NH4+ decreased more slightly. As a consequence, most of the acid-sensitive sites which underwent acidification in the 1980s partially recovered. The O3 concentration at forest sites showed a decreasing trend. Consequently, AOT40 (the metric identified to protect vegetation from ozone pollution) showed a decrease, even if values were still above the limit for forest protection (5000 ppb h−1), while PODy (flux-based metric under discussion as new European legislative standard for forest protection) showed an increase. National scale studies pointed out that PM10 and NO2 induced about 58,000 premature deaths (year 2005), due to cardiovascular and respiratory diseases. The network identified for Italy contains a good number of monitoring sites (6 for terrestrial ecosystem monitoring, 4 for water bodies monitoring and 11 for ozone impact monitoring) distributed over the territory and will produce a high number of monitored parameters for the implementation of the NECD
The Influence of Volcano Topographic Changes on Infrasound Amplitude: Lava Fountains at Mt. Etna in 2021
Infrasound signals are used to investigate and monitor active volcanoes during eruptive and degassing activity. Infrasound amplitude information has been used to estimate eruptive parameters such as plume height, magma discharge rate, and lava fountain height. Active volcanoes are characterized by pronounced topography and, during eruptive activity, the topography can change rapidly, affecting the observed infrasound amplitudes. While the interaction of infrasonic signals with topography has been widely investigated over the past decade, there has been limited work on the impact of changing topography on the infrasonic amplitudes. In this work, the infrasonic signals accompanying 57 lava fountain paroxysms at Mt. Etna (Italy) during 2021 were analyzed. In particular, the temporal and spatial variations of the infrasound amplitudes were investigated. During 2021, significant changes in the topography around the most active crater (the South East Crater) took place and were reconstructed in detail using high resolution imagery from unoccupied aerial system surveys. Through analysis of the observed infrasound signals and numerical simulations of the acoustic wavefield, we demonstrate that the observed spatial and temporal variation in the infrasound signal amplitudes can largely be explained by the combined effects of changes in the location of the acoustic source and changes in the near-vent topography, together with source acoustic amplitude variations. This work demonstrates the importance of accurate source locations and high-resolution topographic information, particularly in the near-vent region where the topography is most likely to change rapidly and illustrates that changing topography should be considered when interpreting local infrasound observations over long time scales
