    α-Synuclein attenuates maneb neurotoxicity through the modulation of redox-sensitive transcription factors

    The accumulation and aggregation of α-synuclein is a pathognomonic sign of Parkinson's disease (PD). Maneb (MB) exposure has also been reported as one environmental triggering factor of this multifactorial neurodegenerative disease. In our laboratory, we have previously reported that mild overexpression of α-synuclein (200% increase with respect to endogenous neuronal levels) can confer neuroprotection against several insults. Here, we tested the hypothesis that α-synuclein can modulate the neuronal response against MB-induced neurotoxicity. When exposed to MB, cells with endogenous α-synuclein expression displayed increased reactive oxygen species (ROS) associated with diminished glutamate-cysteine ligase catalytic subunit (GCLc) and hemeoxygenase-1 (HO-1) mRNA expressions and upregulation of the nuclear factor erythroid 2-related factor 2 (NRF2) repressor, BTB domain and CNC homolog 1 (BACH1). We found that α-synuclein overexpression (wt α-syn cells) attenuated MB-induced neuronal damage by reducing oxidative stress. Decreased ROS found in MB-treated wt α-syn cells was associated with unaltered GCLc and HO-1 mRNA expressions and decreased BACH1 expression. In addition, the increased SOD2 expression and catalase activity were associated with forkhead box O 3a (FOXO3a) nuclear compartmentalization. Cytoprotective effects observed in wt α-syn cells were also associated with the upregulation of silent information regulator 1 (SIRT1). In control cells, MB-treatment downregulated glutathione peroxidase 4 mRNA levels, which was coincident with increased ROS content, lipid peroxidation, and mitochondrial alterations. These deleterious effects were prevented by ferrostatin-1, an inhibitor of ferroptosis, under conditions of endogenous α-synuclein expression. The overexpression of α-synuclein attenuated MB toxicity by the activation of the same mechanisms as ferrostatin-1. Overall, our findings suggest that mild overexpression of α-synuclein attenuates MB-induced neurotoxicity through the modulation of NRF2 and FOXO3a transcription factors and prevents cell death probably by intervening in mechanisms associated with ferroptosis.     COVID-19 epidemiological surveillance through pool testing focused on asymptomatic healthcare workers in the city of BahĂ­a Blanca

    INTRODUCCIÓN: Uno de los desafĂ­os mĂĄs relevantes al comienzo de la pandemia consistiĂł en implementar estrategias dirigidas a prevenir la transmisiĂłn del virus SARS-CoV-2 y mitigar el impacto de la COVID-19. El propĂłsito de este estudio fue contribuir a reducir la transmisiĂłn comunitaria a travĂ©s de una iniciativa interinstitucional, cuyos objetivos fueron validar un mĂ©todo de detecciĂłn de ARN del SARS-CoV-2 e implementar y evaluar la vigilancia en trabajadores de salud asintomĂĄticos de instituciones de salud pĂșblica de BahĂ­a Blanca. MÉTODOS: Se validĂł una prueba de detecciĂłn del gen E del coronavirus mediante RT-PCR a partir de ARN aislado de hisopados nasofarĂ­ngeos. Para aumentar la capacidad de testeo se validĂł la detecciĂłn del ARN viral en muestras agrupadas (pooles). Se realizĂł un estudio de cohorte prospectiva entre el 15/09/20 y el 15/09/21. RESULTADOS: La sensibilidad y especificidad de la prueba en muestras individuales fue del 95% (IC 95%: 85%-100%). La sensibilidad de la detecciĂłn en pooles fue del 73% (IC 95%: 46%-99%) y la especificidad, del 100%. A lo largo de la vigilancia se incluyeron 855 trabajadores y 1764 hisopados, con una incidencia acumulada anual de 2,3% (IC 95%: 1,2%-3,4%). Se detectaron 20 casos asintomĂĄticos positivos. DISCUSIÓN: El tamizaje de trabajadores de salud asintomĂĄticos en la pandemia contribuyĂł a reducir el riesgo de brotes hospitalarios. Asimismo, se generĂł un marco de trabajo interdisciplinario aplicable a otros problemas de saludINTRODUCTION: One of the most important challenges at the beginning of the pandemic was to implement strategies to prevent SARS-CoV-2 virus transmission and reduce the impact of COVID-19. The purpose of this study was to contribute to the reduction of community transmission through an interinstitutional initiative, aimed at validating a SARS-CoV-2 RNA detection method, and at implementing and assessing the surveillance of asymptomatic infected healthcare workers (HCWs) at public health institutions in the city of BahĂ­a Blanca. METHODS: To validate a coronavirus RNA detection method, RNA was extracted from nasopharyngeal swabs and identification of the viral E gene was done by RT-PCR. Validation of sample pooling was performed to increase the testing capacity. A prospective cohort study was conducted from 15 September 2020 to 15 September 2021. RESULTS: The sensitivity and specificity of the test in individual samples was 95% (CI 95%: 85%-100%). The sensitivity of the pooling strategy was 73% (CI 95%: 46%-99%) and the specificity was 100%. A total of the 855 HCWs were included in the surveillance and 1764 swabs were performed, with an annual cumulative incidence of 2.3% (CI 95%: 1,2%-3,4%), and 20 positive asymptomatic cases were detected. DISCUSSION: The screening of asymptomatic HCWs during the pandemic contributed to reduce the risk of outbreaks in hospital settings.     Treatment variability and its relationships to outcomes among patients with Wernicke's encephalopathy: A multicenter retrospective study

    Background: Despite guidelines and recommendations, Wernicke's encephalopathy (WE) treatment lacks evidence, leading to clinical practice variability.Aims: Given the overall lack of information on thiamine use for WE treatment, we analyzed data from a large, well-characterized multicenter sample of patients with WE, examining thiamine dosages; factors associated with the use of different doses, frequencies, and routes; and the influence of differences in thiamine treatment on the outcome.Methods: This retrospective study was conducted with data from 443 patients from 21 centers obtained from a nationwide registry of the Spanish Society of Internal Medicine (from 2000 to 2012). Discharge codes and Caine criteria were applied for WE diagnosis, and treatment-related (thiamine dosage, frequency, and route of administration) demographic, clinical, and outcome variables were analyzed.Results: We found marked variability in WE treatment and a low rate of high-dose intravenous thiamine administration. Seventy-eight patients out of 373 (20.9%) received > 300 mg/day of thiamine as initial dose. Patients fulfilling the Caine criteria or presenting with the classic WE triad more frequently received parenteral treatment. Delayed diagnosis (after 24 h hospitalization), the fulfillment of more than two Caine criteria at diagnosis, mental status alterations, and folic acid deficiency were associated significantly with the lack of complete recovery. Malnutrition, reduced consciousness, folic acid deficiency, and the lack of timely thiamine treatment were risk factors for mortality.Conclusions: Our results clearly show extreme variability in thiamine dosages and routes used in the management of WE. Measures should be implemented to ensure adherence to current guidelines and to correct potential nutritional deficits in patients with alcohol use disorders or other risk factors for WE

    Contribution of subunits to Caenorhabditis elegans levamisole-sensitive nicotinic receptor function

    Caenorhabditis elegans muscle contains seven different nicotinic receptor (AChR) subunits, five of which have been shown to be components of adult levamisole-sensitive AChRs (L-AChRs). To elucidate the reason for such subunit diversity, we explore their functional roles in larva 1 (L1) muscle cells. Singlechannel and macroscopic current recordings reveal that the α-type LEV-8 subunit is a component of native L1 L-AChRs but behaves as a nonessential subunit. It plays a key role in maintaining a low rate and extent of desensitization of L-AChRs. In the absence of the α-type ACR-8 subunit, L-AChR channel properties are not modified, thus indicating that ACR-8 is not a component of L1 L-AChRs. Together with our previous findings, this study reveals that L1 muscle cells express a main L-AChR type composed of five different subunits: UNC-38, UNC-63, UNC-29, LEV-1, and LEV-8. Analysis of a double lev-8; acr-8-null mutant, which shows an uncoordinated and levamisole-resistant phenotype, reveals that ACR-8 can replace LEV-8 in its absence, thus attributing a functional role to this subunit. Docking into homology modeled L-AChRs proposes that ACh forms the typical cation-π interaction, suggests why levamisole is less efficacious than ACh, and shows that ACR-8 can form activatable binding-sites, thus opening doors for elucidating subunit arrangement and anthelmintic selectivity.Fil: Hernando, Guillermina Silvana.     Neurotransmitter GABA Activates Muscle but not a7 Nicotinic Receptors

    Cys-loop receptors are neurotransmitter-activated ion channels involved in synaptic and extrasynaptic transmission in the brain and are also present in non-neuronal cells. As GABAA and nicotinic receptors (nAChR) belong to this family, we explored by macroscopic and single-channel recordings whether the inhibitory neurotransmitter GABA has the ability to activate excitatory nAChRs. GABA differentially activates nAChR subtypes. It activates muscle nAChRs, with maximal peak currents of about 10% of those elicited by acetylcholine (ACh) and 15- fold higher EC50 with respect to ACh. At the single-channel level, the weak agonism is revealed by the requirement of 20-fold higher concentration of GABA for detectable channel openings, a major population of brief openings, and absence of clusters of openings when compared with ACh. Mutations at key residues of the principal binding-site face of muscle nAChRs (aY190 and aG153) affect GABA activation similarly as ACh activation, whereas a mutation at the complementary face (EG57) shows a selective effect for GABA. Studies with subunit-lacking receptors show that GABA can activate muscle nAChRs through the a/d interface. Interestingly, single-channel activity elicited by GABA is similar to that elicited by ACh in gain-of-function nAChR mutants associated to congenital myasthenic syndromes, which could be important in the progression of the disorders due to steady exposure to serum GABA. In contrast, GABA cannot elicit single-channel or macroscopic currents of a7 or the chimeric a7-serotonin-type 3 receptor, a feature important for preserving an adequate excitatory/inhibitory balance in the brain as well as for avoiding activation of nonneuronal receptors by serum GABA.Fil: Dionisio, Leonardo Raul.     Implementation of an epidemiological surveillance strategy for COVID-19 focused on groups at high risk of SARS-CoV-2 infection

    The COVID-19 pandemic has been a challenge to implement strategies to mitigate the transmission of the SARS-CoV-2 virus. Asymptomatic transmission among healthcare workers (HCWs) at the front line of care is particularly concerning due to the potential emergence of outbreaks at healthcare centers. With the final goal of early identification and isolation of asymptomatic infected HCWs, we validated a pooled-sample screening and assessed implementation outcomes and results of an epidemiological surveillance strategy carried out during a 12-month period at public health institutions in BahĂ­a Blanca. To develop and validate a coronavirus detection method, RNA was extracted from nasopharyngeal swabs and iden- tification of the viral E gene was done by an “in-house” RT-qPCR using Taqman probes and the human gene RNAseP as a control. Validation against a commercial kit demonstrated high sensibility and specificity of our test (95%, IC 95%: [85%-100%]). To increase our testing capacity, we validated sample pooling (n= 5) prior to RNA extraction. The results showed a sensibility of 73% (IC 95%: [46%- 99%]) and specificity of 100% against individuals. A tailor-made soft- ware called “VIGI-COVID” was designed to properly manage data. A prospective cohort study was conducted since 15/09/20 to 15/09/21. 860 HCWs were included in the epidemiological surveillance and 1765 swabs were performed. The annual cumulative incidence was 2,30% IC95% [1,26% - 3,39%] (20/860), and 43% of the 860 HCWs were swabbed more than once. Our study demonstrated the utili- ty of comprehensive screening of asymptomatic HCWs during the COVID19 pandemic. Early identification and isolation of infected HCWs prevent the onward transmission of SARS-CoV-2, reducing the risk of healthcare-associated outbreaks.Fil: Esandi, MarĂ­a del Carmen.     A collaborative translational research framework to study the association between the alteration of the vaginal microbiota and preterm birth: Preliminary results

    Preterm birth (PTB) constitutes a major health problem in our country. There is a variety of potential etiological mechanisms for spontaneous PTB, including infectious and inflammatory pathways, stress-related influences and genetic predispositions. Uncovering the multifactorial processes and the interplay of risk factors that lead to spontaneous birth is necessary to identify effective strategies for preventing PTB. Our study analyzes the possible association between the vaginal microbiota and preterm birth. We conducted a prospective cohort study on 163 pregnant women treated in Hospital Dr. Penna Bahia Blanca (October, 2017- August, 2019), to analyze the alteration of vaginal microbiota by Nugent score (dysbacteriosis/ vaginosis), the assessment of Candida spp. and other pathogenic microorganisms at weeks 24/28 of pregnancy. Follow-up until delivery, with identification and recording of PTB or premature membrane rupture (PMR) events. The average age was 26 ± 6.1. Sixty seven % of women had a vaginal microbiota alteration: 35 % dysbacteriosis-vaginosis, 27 % candidiasis, and 41 % a sexual transmitted pathogen like Chlamydia trachomatis, Mycoplasma genitalium or Trichomonas vaginalis. Mixed infections were detected in 33 % of women and 28 % were positive for HPV. Sixty nine % concluded the follow-up until delivery and 14 % presented at least one of the outcomes of interest: 7 % PTB; 6 % PMR and 1 % PTB and PMR. The frequency of PTB was higher in the presence of some types of infections, however, a higher number of cases is still required for a statistical significance analysis. These results are of clinical and public health relevance due to high rates of vaginal microbiota alteration found in the analyzed population of pregnant women. In addition, the implementation of a translational research represented a huge challenge that impacted positively on public health by improving the systematizing care processes in and between hospital units as well as on primary health care attention.Fil: Esandi, María del Carmen.
