Applying Choosing Wisely: Antinuclear Antibody (ANA) and Sub-Serology Testing in a Safety Net Hospital System.
ObjectiveIn 2013, the American College of Rheumatology (ACR) participated in the Choosing Wisely campaign and devised a recommendation to avoid testing antinuclear antibody (ANA) subserologies without a positive ANA and clinical suspicion of disease. The goals of our study were to describe ANA and subserology ordering practices and predictors of ordering concurrent ANA and subserologies in a safety-net hospital.MethodsWe identified ANA and subserologies (dsDNA, Sm, RNP, SSA, SSB, Scl-70 and centromere) completed at Denver Health between 1/1/2005 and 12/31/2011. Variables included demographics, primary insurance, service, and setting from which the test was ordered. We performed multivariable logistic regression to determine predictors of concurrent ordering of ANA and subserologies.ResultsDuring seven years, 3221 ANA were performed in 2771 individuals and 211 (6.6%) were performed concurrently with at least one subserology. The most common concurrent subserologies were dsDNA (21.8%), SSA (20.8%), and SSB (19.7%). In the multivariable logistic analysis, significant predictors of concurrent ANA and subserologies were the labs being ordered from subspecialty care (OR 8.12, 95% CI 5.27-12.50, p-value <0.0001) or from urgent/inpatient care (OR 3.86, 95% CI 1.78-8.38, p-value 0.001). A significant predictor of decreased odds was male gender (OR 0.32, 95% CI 0.21-0.49, p-value <0.0001). Five individuals (2.2% of the negative ANA with subserologies ordered) had a negative ANA but positive subserologies.ConclusionOf 3221 ANA, 6.6% were performed concurrently with subserologies, and subspecialists were more likely to order concurrent tests. A negative ANA predicted negative subserologies with rare exceptions, which validates the ACR's recommendations
WHERE’D YOU GET THOSE NIGHTCRAWLER HANDS? THE INFORMATION LITERACY PRACTICES OF COSPLAYERS
In a time of abundant information, misinformation, and disinformation, information literacy—the ability to find, evaluate, use, and share information—is a key competency for people of all ages. Traditional models of information literacy depict a universal set of skills that can only be learned via instruction from an information professional and must be used in a linear order. More recent models describe it as a set of fluid sociocultural practices that is unique to a particular context, shared by a group of people in that context, and able to be developed through both instruction and personal experience. One context in which information literacy has rarely been studied is that of the affinity space, an informal space—physical or digital—in which participants come together around a shared interest and learn. This qualitative research study investigates the information literacy practices of participants in the affinity space surrounding cosplay, a creative pursuit in which people dress up and roleplay as beloved characters from narratives such as television, video games, and comic books. The study used information horizon maps to ask participants to graphically depict themselves, the information sources they use for cosplay, and relationships between themselves and the resources, as well as relationships the resources have to each other. Through information horizon interviews, participants discussed specific cosplay-related information-seeking incidents and the resources and strategies they used to find the information they needed. Findings indicate that participants use a variety of resources, with all participants mentioning turning to other people for help. Participants also described using many different social media platforms, web-based resources, and events. Participants discussed the way they curate cosplay-related information and the role of trial-and-error in their information seeking process. Trial-and-error is not usually included in either traditional or recent sociocultural models of information literacy. This study suggests that future models should incorporate this part of the information-seeking process, especially when describing information-seeking and use in everyday life contexts rather than academic or professional contexts. This dissertation concludes with a set of recommendations for information literacy instructors to leverage interest-driven information literacy practices in formal educational environments.Doctor of Philosoph

Pairing fluctuations and pseudogaps in the attractive Hubbard model
The two-dimensional attractive Hubbard model is studied in the weak to
intermediate coupling regime by employing a non-perturbative approach. It is
first shown that this approach is in quantitative agreement with Monte Carlo
calculations for both single-particle and two-particle quantities. Both the
density of states and the single-particle spectral weight show a pseudogap at
the Fermi energy below some characteristic temperature T*, also in good
agreement with quantum Monte Carlo calculations. The pseudogap is caused by
critical pairing fluctuations in the low-temperature renormalized classical
regime of the two-dimensional system. With increasing temperature
the spectral weight fills in the pseudogap instead of closing it and the
pseudogap appears earlier in the density of states than in the spectral
function. Small temperature changes around T* can modify the spectral weight
over frequency scales much larger than temperature. Several qualitative results
for the s-wave case should remain true for d-wave superconductors.Comment: 20 pages, 12 figure
Application of a Mathematical Model to Describe the Effects of Chlorpyrifos on Caenorhabditis elegans Development
The nematode Caenorhabditis elegans is being assessed as an alternative model organism as part of an interagency effort to develop better means to test potentially toxic substances. As part of this effort, assays that use the COPAS Biosort flow sorting technology to record optical measurements (time of flight (TOF) and extinction (EXT)) of individual nematodes under various chemical exposure conditions are being developed. A mathematical model has been created that uses Biosort data to quantitatively and qualitatively describe C. elegans growth, and link changes in growth rates to biological events. Chlorpyrifos, an organophosphate pesticide known to cause developmental delays and malformations in mammals, was used as a model toxicant to test the applicability of the growth model for in vivo toxicological testing.L1 larval nematodes were exposed to a range of sub-lethal chlorpyrifos concentrations (0-75 microM) and measured every 12 h. In the absence of toxicant, C. elegans matured from L1s to gravid adults by 60 h. A mathematical model was used to estimate nematode size distributions at various times. Mathematical modeling of the distributions allowed the number of measured nematodes and log(EXT) and log(TOF) growth rates to be estimated. The model revealed three distinct growth phases. The points at which estimated growth rates changed (change points) were constant across the ten chlorpyrifos concentrations. Concentration response curves with respect to several model-estimated quantities (numbers of measured nematodes, mean log(TOF) and log(EXT), growth rates, and time to reach change points) showed a significant decrease in C. elegans growth with increasing chlorpyrifos concentration.Effects of chlorpyrifos on C. elegans growth and development were mathematically modeled. Statistical tests confirmed a significant concentration effect on several model endpoints. This confirmed that chlorpyrifos affects C. elegans development in a concentration dependent manner. The most noticeable effect on growth occurred during early larval stages: L2 and L3. This study supports the utility of the C. elegans growth assay and mathematical modeling in determining the effects of potentially toxic substances in an alternative model organism using high-throughput technologies
Spectroscopic Studies of the Iron and Manganese Reconstituted Tyrosyl Radical in Bacillus Cereus Ribonucleotide Reductase R2 Protein
Ribonucleotide reductase (RNR) catalyzes the rate limiting step in DNA synthesis where ribonucleotides are reduced to the corresponding deoxyribonucleotides. Class Ib RNRs consist of two homodimeric subunits: R1E, which houses the active site; and R2F, which contains a metallo cofactor and a tyrosyl radical that initiates the ribonucleotide reduction reaction. We studied the R2F subunit of B. cereus reconstituted with iron or alternatively with manganese ions, then subsequently reacted with molecular oxygen to generate two tyrosyl-radicals. The two similar X-band EPR spectra did not change significantly over 4 to 50 K. From the 285 GHz EPR spectrum of the iron form, a g1-value of 2.0090 for the tyrosyl radical was extracted. This g1-value is similar to that observed in class Ia E. coli R2 and class Ib R2Fs with iron-oxygen cluster, suggesting the absence of hydrogen bond to the phenoxyl group. This was confirmed by resonance Raman spectroscopy, where the stretching vibration associated to the radical (C-O, ν7a = 1500 cm−1) was found to be insensitive to deuterium-oxide exchange. Additionally, the 18O-sensitive Fe-O-Fe symmetric stretching (483 cm−1) of the metallo-cofactor was also insensitive to deuterium-oxide exchange indicating no hydrogen bonding to the di-iron-oxygen cluster, and thus, different from mouse R2 with a hydrogen bonded cluster. The HF-EPR spectrum of the manganese reconstituted RNR R2F gave a g1-value of ∼2.0094. The tyrosyl radical microwave power saturation behavior of the iron-oxygen cluster form was as observed in class Ia R2, with diamagnetic di-ferric cluster ground state, while the properties of the manganese reconstituted form indicated a magnetic ground state of the manganese-cluster. The recent activity measurements (Crona et al., (2011) J Biol Chem 286: 33053–33060) indicates that both the manganese and iron reconstituted RNR R2F could be functional. Developing the Silver Economy and Related Government Resources for Seniors: A Position Paper
The precarious rights of senior citizens, especially those who are highly educated and who are expected to counsel and guide the younger generations, has stimulated the creation internationally of advocacy associations and opinion leader groups. The strength of these groups, however, varies from country to country. In some countries, they are supported and are the focus of intense interest; in others, they are practically ignored. For this is reason we believe that the creation of a network of all these associations is essential. The proposed network would act as a support for the already-existing policies of the United Nations' High Commission for Human Rights, of independent experts, and of the Global Alliance for the Rights of Older People. All three have long ago recommended the creation of a recognized instrument for uniting presently scattered efforts. The proposed network, therefore, will seek to promote the international exchange of relevant expertise, and it will reinforce the commitments and actions that single countries are currently taking to meet these objectives. For example, informative public events can be organised to promote particular support initiatives and to provide an opportunity for new members of the network to be presented. The network will promote health for senior citizens, disease prevention, senior mobility, safe free time for seniors, alimentary education, protection against new risks and dangers, as well as equity in the services necessary for seniors to adopt new information and communication technologies. In the case of retired academic members, the network will promote equality with respect to continuing use of digital technologies (particularly email), continuing access to research libraries, and the guaranteed ability for seniors to fund their own research programs and to deliver free seminars
