An Islamic religious education teacher as a professional in the field of religion, at least has the main task of being a teacher, educator and guide for students. Moral and ethics are the main goals of learning for Islamic religious education teachers, because when they are still in school, it is easier for students to form and instill good values regarding morals and ethics. Allah SWT, the Lord of the universe, has created reason and lust in humans, so this shows that every human being is inseparable from goodness and error.     Serological Studies in Bullous Pemphigoid: A Literature Review of Antibody Titers at Presentation and in Clinical Remission

    Bullous pemphigoid is associated with antibodies to a 230 kDa and a 180 kDa protein. In a literature review we investigated the role of auto-antibodies as detected by different serological assays. Nine reports containing data on 143 patients were analyzed. Pre-treatment data showed that indirect immunofluorescence, enzyme-linked immunosorbent assay (ELISA) and immunoblotting offer an 82.2% or greater probability of being positive. At the end of the study period, all patients had clinically improved, whether or not they were on therapy. Auto-antibodies were present in 29% of patients evaluated by monkey esophagus immunofluorescence and 75% of those evaluated by human skin immunofluorescence. Positive titers were also reported in 67.6% of patients evaluated by ELISA. In 100% of patients in whom immunoblotting was performed the titers became negative. In 3 patients (5.3%) using human skin immunofluorescence and in one patient (1.4%) using ELISA the titers were increased at the end of the study period. The correlation between anti-basement membrane zone antibodies and the clinical course of bullous pemphigoid requires further and long-term studies

    BACKGROUND: Gastric leak after laparoscopic sleeve gastrectomy (LSG) is a serious complication. Currently, the literature lacks long-term outcomes in LSG and leak rates after reinforcement of the staple line. The aims are two-fold: to present leak rates from using staple line reinforcement and six year outcomes of LSG in relation to resolution of obesity-related comorbidities and long-term weight loss. MATERIALS AND METHODS: This is a single-institution, retrospectively reviewed study of 204 patient case files. Data from all patients undergoing LSG between December 2007 and May 2013 was collected. RESULTS: The total complication rate was 6.9% (14/204), with no recorded staple line leaks. The mean postoperative Body Mass Index (BMI) at 1 year, 2 years, 3 years, 4 years, 5 years, and 6 years was 39.3 ± 8, 38.7 ± 8, 40.4 ± 9, 40.5 ± 10, 43.0 ± 10, and 42.4 ± 7, respectively. The mean % excess weight loss at 1 year, 3 years, and 6 years was 48.4 ± 19, 51.7 ± 28, and 41.0 ± 21, respectively. There were no significant differences between follow-ups at year 1 and 3 (p > 0.05), and between year 3 and 6 (p > 0.05) for the mean % excess weight loss. The resolution rates for all patients were 74%, 61%, 79%, and 90% for hypertension, hypercholesterolemia, diabetes mellitus type 2 and obstructive sleep apnea, respectively. CONCLUSION: The synthetic bioabsorbable reinforcement material shows no staple line leaks making it safe to use. LSG as a procedure had a high resolution of obesity-related comorbidities as well as sustainable long-term weight loss

    Impaired bone marrow homing of cytokine-activated CD34<sup>+</sup> cells in the NOD/SCID model

    The reduced engraftment potential of hematopoietic stem/progenitor cells (HSPCs) after exposure to cytokines may be related to the impaired homing ability of actively cycling cells. We tested this hypothesis by quantifying the short-term horning of human adult CD34+ cells in nonobese diabetic/severe combined immunodeficient (NOD/SCID) animals. We show that the loss of engraftment ability of cytokine-activated CD34+ cells is associated with a reduction in homing of colony-forming cells (CFCs) to bone marrow (BM) at 24 hours after transplantation (from median 2.8% [range, 1.9%-6.1%] to 0.3% [0.0%-0.7%]; n = 3; P < .01), coincident with an increase in CFC accumulation in the lungs (P < .01). Impaired BM homing of cytokine-activated cells was not restored by using sorted cells in G 0G1 or by inducing cell cycle arrest at the G 1/S border. Blocking Fas ligation in vivo did not increase the BM homing of cultured cells. Finally, we tested cytokine combinations or culture conditions previously reported to restore the engraftment of cultured cells but did not find that any of these was able to reverse the changes in homing behavior of cytokine-exposed cells. We suggest that these changes in homing and, as a consequence, engraftment result from the increased migratory capacity of infused activated cells, leading to the loss of selectivity of the homing process. © 2004 by The American Society of Hematology

    Takwil As-Sirāṭ Al-Mustaqīm dalam Pembacaan Tafsir Mulla Sadra

    This paper is a literature review of Mullā Ṣadrā's thoughts on takwil as-ṣirāṭ al-mustaqīm which is studied through his main work, entitled Tafsīr Al-Qur'ān Al-Karīm. By using the descriptive-analytical method, the conclusion is that takwil as-ṣirāṭ al-mustaqīm in Mullā Ṣadrā's view is studied through two approaches, namely the text analysis method and mukāshafah as a process of knowing the true meaning of the term as-ṣirāṭ al-mustaqīm. In discourse, it is known that the commentators are divided into two views, namely the historical view which examines the term as-ṣirāṭ al-mustaqīm based on textual narrations and the view of ijtihad based on rationality. The implication of these two views is to limit the meaning and essence of the term as-ṣirāṭ al-mustaqīm based on historical and intellectual references. In order to overcome these problems, this paper aims to explain the nature and meaning of as-ṣirāṭ al-mustaqīm based on Mullā Ṣadrā's takwil by mediating historical and rational views. The result of this paper is to offer a new perspective on understanding the terms of the Qur’an transcendentally so that each individual can clearly understand the meanings and nature of as-ṣirāṭ al-mustaqīm

    The departing point of this article is the objection towards tafsīr ishārī. This objection can be categorized as an ontological foundation, that tafsīr ishārī stands on the personal kashf, for which reason it cannot stand as reason and an epistemological foundation. Through a comprehensive-analytical study, this study elaborates on the theory correspondence of Qurʾānī, afāqī, and anfusī proposed by Haydar Amuli, a theory that stands as the ontological foundation of tafsīr ishārī. The deduction of the correspondence between Qurʾānī, afāqī, and anfusī or the deduction of the reference of the verse runs through kashf. Kashf is the epistemological basis proposed by Haydar Amuli. The correspondence theory is the basic assumption of the sufī interpretation. This theory is capable of briding the disjuncture of the relationship between the verses and their references in the Sufi interpretation

    Parathyroid hormone secreting metaplastic breast carcinoma: a case report of paraneoplastic syndrome

    Paraneoplastic syndrome (PNS) is a distant neurological manifestation of an underlying tumour. Humoral hypercalcaemia of malignancy is a form of paraneoplastic syndrome where there is an increased in calcium levels. In this article we report a 48 year old lady, with no known medical illness and presented with symptomatic hypercalcaemia. She presented to our institution complaining of a fungating and ulcerating mass over the right breast, measuring 11 X 15 cm and associated with gradual paraxial body weakness. Further investigations revealed the lesion to be an invasive breast carcinoma with metaplastic features. Computed tomography (CT) scan showed a locally advanced breast carcinoma with right axillary node metastasis, without evidence of distant metastasis. Serum calcium, i-PTH together with SPECT has confirmed the patient to have a humoral hypercalcaemia malignancy with elevated ectopic parathyroid hormone level. Bone scan did not reveal any evidence of metastasis. High volume intake and loop diuretics were employed in the management of hypercalcaemia, which was eventually resolved following mastectomy with axillary clearance of the ipsilateral side. Currently she’s she is under oncologist’s follow up for further management

    Evaluating the Effectiveness of Lumber Corset Wearing in Low Back Ache: A Rehabilitation Center Based Cross-sectional Study

    Patients are most commonly advised by medical practitioners even after remaining wide range of controversy regarding wearing of lumber corset in low backache (LBA). Therefore, this study aimed to determine the potential evidence of using lumber orthosis in LBA. The study adopted 50 participants (21 male and 29 female) ages ranging between 20 to 60 years as sample selecting randomly from September to December 2015. Outcome evaluated by calculating and presenting descriptive statistics at 0.05 p-value and x2 test with confidence intervals (95%), Odd Ratio (OR), and Relative Risk (RR). Age and sex were not statistically significant determinants (x2 1.172, p 0.279 and x2 0.593, p 0.441, respectively). Wearing orthosis and reduce pain in a journey found as protective (RR 0.79 and RR 0.94) also poor relationship discovered in considering OR (OR 0.242, 95% CI 0.021-2.780 and OR 0.857, 95% CI 0.164-4.467). Our data were unable to provide adequate proof that wearing lumber corset bring any clinical or therapeutic benefit in managing LBA to the patients
