임상화학검사 정도관리성적의 평가 및 관련요인 분석

보건학과/박사[영문] [한글] 임상화학 검사실의 정확도 및 정밀도에 영향을 미치는 요인을 구명하기 위하여 대한임 상검사정도관리학회에 가입하여 1989년말 현재 임상화학검사 외부정도조사에 참여했던 19 1개 검사실 중 154개 검사실을 대상으로 검사실의 일반적 특성, 인력 특성, 정도조사 관 리물질 측정과정, 내부정도관리체계 그리고 외부정도조사에의 참여정도를 비교 분석하여 아래와 같은 결과를 얻었다. 첫째, 정확도 성적에 따라 검사실을 분류해 보면 임상화학검사 측정항목수가 20개 항목 이하인 검사실의 46.9%가 임상화학전담 임상병리전문의가 없는 검사실의 34.6%가, 임상 화학전담 임상병리사수가 1명인 검사실의 45.2%가 그리고 책임기사의 근무년수가 5년 이 하인 검사실의 45.6%가 '하 ' 군에 속하였다. 또한 수기법 파이펫을 사용한 검사실의 38. 05가 관리도를 작성 게시하지 않은 검사실의 40.0%가 그리고 외부정도조사 불참횟수가 3 회 이상인 검사실의 47.4%가 '하'군에 속하였다. 둘째, 이 연구에서 선정된 독립변수들 각각에 대한 평균 정확도 수준의 차이를 분 석한 결과 임상화학전담 임상별리사수, 책임기사의 근무년수, 관리도의 작성 게시 유무, 외부정도조사 불참횟수가 통계적으로 유의하였다. 또한 평균 정밀도 수준의 차이를 분석 한 결과 임상화학검사 측정항목수, 임상화학전담 임상병리전문의수, 외부정도조사 불참횟 수가 통계적으로 유의하였다. 셋째, 구축된 모형내에 포함된 다른 변수들에 의한 영향을 통제한 후 독립변수들 각각 의 정도관리성적에 대한 영향력의 크기와 방향을 검증하기 위하여 선형중회귀분석을 실시 한 결과 책임기사의 근무년수와 외부정도조사 불참횟수가 정확도성적에 유의하게 영향을 미치는 요인이었으며, 이 요인들의 회귀계수의 방향은예상했던 것과 일치하였다. 선정된 8개 변수들에 의하여 정확도 성적은 19.4%를 설명할 수 있었으며, 구축된 완전모형은 유 의하였다. 임상화학전담 임상병 리사 1인당 임상화학 검사건수와 외부정도조사 불참횟수가 정밀도 성적에 유의하게 영향 을 미치는 요인이었으며, 회귀계수의 방향은 임상화학전담 임상병리사 1인당 임상화학 검 사건수의 경우 예상했던 것과 일치 하쳤으나 외부정도조사 불참횟수의 경우에는 일치하지 않았다. 선정된 8개 변수들에 의하여 정밀도 성적은 14.2%를 설명할 수 있었으며, 구축 된 완전모형은 유의하였다. 넷째, 모형내의 나머지 독립변수들에 의한 영향을 통제한 후 정도관리성적의 향상에 기 여하는 요인을 분석하고, 비차비를 산출하기 위하여 반응범주가 세 개인 로지스틱 회귀분 석 (일반화 로짓모형)을 실시한 결과 임상화학전담 임상병리전문의수, 시약의 선정과정, 파이펫 자동화 여부, 외부정도조산 불참휫수는 정확도성적 '하'를 '중'으로 높이는데 통 계적으로 유의하게 효과적이었다. 이연구에서 구축한 일반화 로짓모형은 모수의 적합도 검정 결과 정확도 및 정밀도모형에서 각각 유의하였다. 모형내의 나머지 독립변수들에 의 한 영향을 통제 하였을 때 정확도 성적 '중'군에 대한 '하'군치 비차비를 보면 임상화학 전담 임상병리전문의수는 3.56, 시약의 선정과정은 1.85, 파이펫 자동화 여부는 3.56 그 리고 외부정도조사 불참횟수는 1.40이었다. 이상의 연구결과에 의하면 검사실 책임자의 관리감독, 책임기사및 임상병리사의 검사업 무에 대한 전문화 분석도구 및 장비의 자동화, 체계적이고 총체적인 내부정도관리의 계획 및 실시 여부 그리고 외부정도조사에의 계속적인 참여가 검사성적의 향상에 기여한다는 결론을 얻었다. 따라서 이와 같은 요인들에 대한 적절하고 효율적인 관리와 통제를 통하 여 검사실의 신뢰도 향상을 도모하고, 검사실간의 변동의 크기를 감소시킬 수 있을 것으 로 생각되어 진다. 또한 앞으로 공식적인 정도관리 프로그램에 미가입하고 있으면서, 내 부정도관리 조차도 실시하고 있지 않은 개인 병·의원급의 검사실을 대상으로 보다 실질 적인 문제를 파악하여 검사성적의 향상을 위한 기본 여건을 마련하고 제시하기 위한 노력 이 필요하겠다. Laboratory Proficiency-Test Performance in Korea : Its relationship to structural and process variables Ein Soon Shin Department of Public Health The Graduate School, Yonsei University (Directed by professor Han Joong Kim, M.D.,Ph.D.) This study is designed to analyze clincal laboratory proficiency test performance and identify determinants of perfonrmance scores. The perficiency performance was measured in terms of accuracy and percision while explanatory variables include both structural and process characteristics in clinical laboratory performance. The study included 154 laboratories participating in progiciency-testing surveys of the korean Society of Quality Control in Clinical Pathology. The results of the study are summarized below. 1. Laboratories were classified into three categories according to the distribution of accuracy levels. Laboratories classified at the low accuracy level were likely to have following characteristics; the less experienced chief technician; and the use of manual pipett. 2. Number of technicians, years of experience of the chief technician, an exhibition of quality control graph chart, and numbers of lack of participation in proficiency-testing trials were significantly different in the mean accuracy level. Number of test items, number of coctors, and numbers of lack of participation in proficiency-testion trials were sehnificanlty different in the mean precision level. 3. The multiple regression model explained 19.4% of the variation in the accuracy level, and this full-model was significant. Years of experience of the chief technician, and numbers of lack of participation in proficiency-testing trials were significant, with signs consistent with the predicted direction. Another regression model explained 14.2% of the variation in the precision level, and this model also was significant. Numbers of test per technician was significant, consistent with the predicted direction. Contrary to the expectation, numbers of lack of participation in proficiency-testion trials revealed inverse sign. 4. Generalized logit model sith three response levels revealed that a number of doctors, selection of suitable reagent, using automatic pipett and a numbers of lack of participation in proficiency-testion trials were significant enough to improve the performance from the lower group to the middle group. In summary, management and supervision by medical specialists, experience of the chief technician, automation of instruments, intralaboratory quality control system, and continued participation of laboratories in proficiency-testing program were found to be associated with higher performance. These factors, therefore, should be effectively monitored and controlled to improve laboratory performance and to diminish interlaboratory variability. For further study, it is necessary to identify factors realted to higher performance for laboratories not participating in proficiency-testing program and without an intralaboratory quality control program.restrictio

Studies on the Stability of the Quality Control Material of Clinical Chemistry and its Effects on the Precision

보건학과/석사[영문] [한글] 서울시내 Y중합병원 임상화학 검사실에서 1985년부터 1986년까지 2년동안 실시했던 내 부정도관리 기록을 기초로 하여 자가제조한 보존혈청을 사용하여 내부정도관리를 실시했 던 16종의 분석물질들의 불안정성이 검사결과의 정밀도에 미치는 영향을 관찰하여 보았다 . 보존혈청을 사용했을 때 각 분석물질들의 안정성을 평가하기 위해 단순회귀분석 (simpl e regression analysis)을 실시하였으며 , 정밀도는 변이계수로 측정하였다. 불안정성이 검사결과의 정밀도에 미치는 영향은 Wilcoxon's rank-sum test와 Student t-test를 이용 하여 분석하여 다음과 같은 결과를 얻었다. 첫째, 16종의 분석물질들에 대하여 관리물질의 안정성을 평가한 결과 inorganic phosph orus 가 가장 안정하였고, AST (aspartate aminotransferase)가 가장 불안정하였다. Sodi um, urea nitrogen, calcium, T. protein 그리고 albumin은 대체로 안정한 편이었다. 둘째, 불안정성의 크기를 상대적 변화율로 측정한 결과 T.bilirubin의 불안정성의 크기 가 가장 컸고(0.008-0.042), urea nitrogen의 불안정성의 크기가 가장 작았다(0.078∼0.0 13). 셋째, 불안정성의 양상을 방향에 따라 보면 I.Phosphorus는 증가추세의 불안정성이 있 었고, carbon dioxide, albumin 그리 고 AST는 감소추세의 불안정성이 있었다. 나머지 분 석물질들은 양쪽방향의 불규칙적인 불안정성을 보여 사용기간이 경과함과 따라 농도가 유 의하게 증가하거나 감소하였다 (P<0.05). 넷째, sodium, potassium, chloride, carbon dioxide, glucose, urea nitrogen, calciu m, I. phosphorus, uric acid, T. protein. albumin 그리고 cholesterol의 평균변이계수 는 5.0%이하로 매우 높은 정밀도 수준이었고,creatinine, T. bilirubin, AST 그리고 r-GT (ganua-glutamyl transferase)의 평균변이계수는 5.7-8.0%였다. 다섯째, 측정농도와 정밀도 성적과의 상관성을 보면 sodium, potassium, carbon dioxid e, creatinine, calcium, I. phosphorus, uric acid, T. protein, albumin 그리고 AST에 서 평균측정농도가 가장 높은 보존혈청에서의 정밀도 성적이 평균측정농도가 가장 낮은 보존혈청에서의 정밀도 성적보다 우수했다. 여섯째, 안정성에 따른 정밀도 수준의 차이를 분석한 결과 유의한 차이가 없었다 (p> 0 .05). 일곱째, 불안정성에 따른 정밀도 수준의 차이를 분석한 결과 유의한 차이가 없었다 (p> 0.05). 여덟째, 불안정성이 관찰된 경우에 안정성이 유지된 기간과 불안정한 기간동안의 정밀 도 사이에는 유의한 차이가 없었다(p>0.05). 이상과 같이 임상화학 내부정도관리에 사용된 보존혈청의 정밀도는 높은 수준이었고, 관찰된 불안정성은 정밀도 수준에 영향을 미치지 않았다. 따라서 검사실 자가제조한 보존 혈청은 내부정도관리 정밀도모니터를 위한 목적으로 사용할 때 별무리가 없을 것으로 생 각된다.
