Construction and characterization of an improved recombinant baculovirus producing polyhedra that contain Bacillus thuringiensis Cry1Ac crystal protein

Thesis (master`s)--서울대학교 대학원 :농생명공학부,2004.For the enhancement of insecticidal activity in wt AcNPV and the reduction of side-effect on environment in recombinant baculovirus, an improved recombinant baculovirus, Autographa californica nucleopolyhedrovirus (AcNPV) producing polyhedra with Bacillus thuringiensis (Bt) Cry1Ac crystal protein and having a potential of returning to wt AcNPV in several passage, has been constructed and characterized. The toxic domain of Bt cry1Ac gene was inserted between two polyhedrin genes which were cloned in tandem into pBacPAK8 under the control of polyhedrin promoter and the resulted construct, pBactrus was used as transfer vector. By the cotransfection of parental virus, bAcGOZA and pBactrus, the recombinant baculovirus named as Bactrus was constructed. SDS-PAGE and immunoblot analysis of polyhedra produced by Bactrus in insect cells showed that it expressed a 130 kDa of fusion protein and what is more, 30 kDa of polyhedrin resulted from a homologous recombinantion. The Bactrus produced recombinant polyhedra which were nearly similar to those of wt AcNPV. The Insecticidal activity of recombinant polyhedra against fall armyworm (Spodoptera exigua) larvae was similar to that of wt AcNPV, but for the diamondback moth (Plutella xylostella) larvae, it showed a significantly higher pathogenicity in comparison with those of wt AcNPV. In SDS-PAGE, expression level of fusion protein was decreased along serial passage. But the decrease of fusion protein expression was smaller in Sf-9 cells infected with Bactrus passaged at 10 multiplicity of infection (MOI) than that passaged at 1 MOI. Also insecticidal activity of recombinant polyhedra produced by the Bactrus passaged at 1 and 10 MOI against P. xylostella larvae was decreased along serial passage with similar trend to the result in SDS-PAGE. The production of recombinant polyhedra by the Bactrus was examined with Sf-9, Sf-21, Tn5, Se301 and High-Five cells with monolayer and suspension culture system. In monolayer culture, the largest amount of fusion protein was expressed in Se301 cells. However the highest expression of fusion protein in suspension culture was observed in High-Five cells when infected with 5 MOI and the optimal cell density on time of infection was 3.0×105 cells per ml.Maste

(The) identification of hospital administrative problems in the health insurance system in Korea

보건학과/석사[한글] 의료보험은 일반 물보험과는 달리 보험자와 피보험자 및 현물 급여(진료)를 하는 요양 기관등 3자 관계로 성립된다. 의료보험은 또한 사회 개발 및 의료보장의 일환으로 다수의 경제체(의료보험 가입 능력자)가 결합 할 수 있도록 소득 수준이 형평있는 향상이 첫째 성립 요건이며, 현물급여(진료)를 수행할 의료자원의 균형있는 발전이 둘째 성립 요건이 라 할 수 있다. 2년여에 전체 국민의 21%를 포괄하게 된 우리나라의 의료보험은 위의 2개 성립요건 중 피보험자의 관리 및 보험자(공단)의 설립, 운영등은 별 문제가 없었으나 요양기관의 현물 급여는 의료자원의 빈곤과 균형있는 분포 및 진료수가의 저렴, 그리고 업무량의 폭주로 상당한 어려움을 겪고 있다. 수진료 청구서작성 업무 과다로 공무원 및 사립학교 교직원 의료보험이 실시 된 1979년 1월 이후 대개의 종합병원의 청구서 작성 제출은 평균 2개월 이 연체되고 있어 병원 운영의 문제점이 되고 있다. 본 연구는 의료보험 사무 업무의 제반 문제점을 분석하며 의료보험 도입으로 인해 종합 병원이 어떤 운영상의 문제가 있는가를 분석하려는데 그 목적이 있다. 본 연구를 위하여 W종합병원을 연구 대상으로 하였다. 의료보험 환자가 내원하여 그에 관한 기록이 작성되고 청구서가 완료되기까지의 과정에 문제점이 어떤 것이 있는 가를 조 사 연구하였다. 이 연구는 실무자와의 면접 및 실무에 직접 참여하고 경험하고 관찰한 결 과와 의료보험과 관련된 제반 통계 자료를 정리 분석하여 의료보험 도입이 행정상에 어떠 한 문제를 야기시키는가를 연구하였다. 연구되어진 결과는 대략 다음과 같다. 1. 의료보험 청구서 작성의 1건당 최소 평균 소요 시간은 49분이다. 이는 일반환자 기 록 정리에 드는 시간외에 과외로 소요되는 시간이다. 청구서 서식의 복잡성 및 기재량 과다로 요양기관 사무량 과다의 문제점을 야기하여 나 아가서는 이는 병원 운영면에도 영향을 미치고 있음이 발견되었다. 즉 의료보험의 도입으로 인해 직원을 별도로 채용하여 의료보험 업무를 전담하게해야 한다는 과외의 문제를 야기시켰다. 2. 의료보험 도입과 관련하여 병원 운영비의 중심이 되는 인건비 지출을 분석한 결과, 의료보험 담당 사무직원의 인건비가 의료보험 수입 총액의 15% 정도 높은 구성비를 나타 냈다. 3. 의료보험 수진자의 25%가 본인 부담금 600원 미만이다. 이들은 종합병원에 오지않아 도 치료 가능한 질병이다. 즉 이들의 질병명은 95%가 “관찰”이라는 질병명을 갖고 있었다. 4. 즉시불 환자는 줄어들고(47%), 의료보험, 의료보호, 산재보험, 자동차보험등 후시불 환자는 늘어나고 있다(53%). 이러한 점으로 미루어 볼 때 요양기관의 운영 체제상의 중요한 변혁과 전환점이 이룩되 어야 할 것으로 사료된다. [영문] The health insurance, unlike other insurances, is composed of three parties: the insurer, the subscribers and the health care providers. The health card insurance as a segment of the entire national social welfare requires two major criteria: the equitable development of the economic standards where insurees are capable of organizing a body for their purposes, and the availability of the health care resources. The Korean health insurance, only two years old, has subscribers covering 21 per cent of entire population, Considering the short history of the programs, it satisfactorily achieved the first requirement mentioned above. However, the second requirement needs a much improvements and readjustment. Some of the problems indicated by the study are: lack of health care facilities and its geographical maldistribution, low rate of reimbursement costs, and the claims process in both workload and paper-work flows. The delayed reimbursement caused additional manpower of clerical personnel and the financial burden to the health care institutions. The purpose of this study is to find and analyze the hospital administrative problem areas of the current health insurance system, and to seek its solutions. The objectives were to identify problems and the causes of such problem in the process from the patients registration to the mailing out the bills for the reimbursement. The research was based on the actual experience in the "W" hospital, a 200 bed general hospital. The process of the study included interviews with all related clerical personnel in all levels, analysis of the paperwork process, and the personal observations throughout the entire process. The necessary data were retrieved, analyzed, and concluded with the following findings. 1. Minimum time to complete on billing required 49 minutes. 2. The cost of the personnel hired exclusively for the insurance billing showed a 15 per cent of total amount billed. 3. A 25 per cent of the beneficiaries visited the outpatient clinic paid less than 600 won the minimum deductable amount. 4. The proportion of cash-payment and billing patients showed 47 and 53 per cent respectively. The author suggests a readjustment and simplification of the paperwork process are absolutely necessary for the efficiency and the effectiveness for the more successful insurance programs in the future.restrictio
