Diagnostic accuracy of clinical outcome prediction using nursing data in intensive care patients: A systematic review

Background: Nursing data consist of observations of patients' conditions and information on nurses' clinical judgment based on critically ill patients' behavior and physiological signs. Nursing data in electronic health records were recently emphasized as important predictors of patients' deterioration but have not been systematically reviewed. Objective: We conducted a systematic review of prediction models using nursing data for clinical outcomes, such as prolonged hospital stay, readmission, and mortality in intensive care patients, compared to physiological data only. In addition, the type of nursing data used in prediction model developments was investigated. Design: A systematic review. Methods: PubMed, CINAHL, Cochrane CENTRAL, EMBASE, IEEE Xplore Digital Library, Web of Science, and Scopus were searched. Clinical outcome prediction models using nursing data for intensive care patients were included. Clinical outcomes were prolonged hospital stay, readmission, and mortality. Data were extracted from selected studies such as study design, data source, outcome definition, sample size, predictors, reference test, model development, model performance, and evaluation. The risk of bias and applicability was assessed using the Prediction model Risk of Bias Assessment Tool checklist. Descriptive summaries were produced based on paired forest plots and summary receiver operating characteristic curves. Results: Sixteen studies were included in the systematic review. The data types of predictors used in prediction models were categorized as physiological data, nursing data, and clinical notes. The types of nursing data consisted of nursing notes, assessments, documentation frequency, and flowsheet comments. The studies using physiological data as a reference test showed higher predictive performance in combined data or nursing data than in physiological data. The overall risk of bias indicated that most of the included studies have a high risk. Conclusions: This study was conducted to identify and review the diagnostic accuracy of clinical outcome prediction using nursing data in intensive care patients. Most of the included studies developed models using nursing notes, and other studies used nursing assessments, documentation frequency, and flowsheet comments. Although the findings need careful interpretation due to the high risk of bias, the area under the curve scores of nursing data and combined data were higher than physiological data alone. It is necessary to establish a strategy in prediction modeling to utilize nursing data, clinical notes, and physiological data as predictors, considering the clinical context rather than physiological data alone. 공중보건 위기상황에서의 자가격리자 관리를 위한 내부소통에 관한 탐색적 연구: 2015년 중동호흡기증후군 유행 시 네이버 밴드를 활용한 서울특별시 G구 사례를 중심으로

This is an academically valuable cases of a specific situation of internal communication within the response team during the public health emergency. Objectives: The empirical case in G District allows exploration of several aspects of the internal communication using SNS at a public health emergency response. This study provides an overview of the SNS posts communicated for quarantine management, and attempts to describe these communication method's potential for influencing quarantine management. Method: Qualitative and quantitative content analysis were conducted on 656 posts, from June 9, 2015, when the Band was created, until October 30, when the MERS-CoV epidemic was temporarily ended. In addition, social network analysis was performed on 27 posts that monitoring staffs questioned about their problems occurred during the quarantine management. Results: Closed-type SNS became common space for public health workers to internally interact and exchange information during public health emergency. In particular, it functioned as substitute internal communication channels when lack of coordination and internal information deficit to the response team were presented. As well as these roles, it was confirmed the achievement of SNS communication by efficient resolution of problems in quarantine management. Furthermore, most influential participants in communication network were identified, such as chief decision-maker and specialized functional managers (e.g., medical and livelihood management). Conclusion: Local public health workers and professionals should actively utilize closed-type SNS, for the purpose of communicating effectively within a response team and manage efficiently the quarantines in a public health emergency.1. Comparison of Conceptual Networks in Plant Nutrition of Middle School Science Textbook and Students Learning

Factors associated with nurses' user resistance to change of electronic health record systems

Background: Electronic health record (EHR) systems often face user resistance in hospitals, which results in a failure to acquire their full benefits. To implement the EHR successfully, it is crucial to reduce nurses' resistance to use the system. This study aimed to investigate the factors associated with nurses' resistance to use the EHR system. Methods: A descriptive correlational study was conducted with nurses working at four university hospitals in Korea using self-administered questionnaires to measure user resistance behavior, resistance to change, perceived usefulness, perceived ease of use, perceived value, colleagues' opinions, self-efficacy for change, and organizational support for change. Path analysis was performed to examine direct and indirect association with user resistance behavior. Results: A total of 223 nurses completed the questionnaires. All seven factors were found to be significantly associated with user resistance, either directly or indirectly. The total effect on user resistance behavior was highest in resistance to change (0.65), followed by perceived usefulness (- 0.33); both had direct but no indirect effects. Conversely, self-efficacy for change (- 0.25), perceived value (- 0.21), colleagues' opinions (- 0.16), perceived ease of use (- 0.16), and organizational support for change (- 0.05) had indirect but no direct effects. Conclusions: The study examined the factors associated with nurses' user resistance behavior after the implementation of a new EHR system. These findings could help hospitals develop better EHR implementation strategies to reduce user resistance behavior among the nursing staff.ope

(A) study on the subjects' perception and response about national medical aid program of a health center

간호학과/석사[한글] 의료보호제도란 정부의 제4차 경제개발 5개년 계획의 주요 내용인 사회개발사업중 하나로서 건강문제가 있어도 경제적인 능력이 없어서 도움을 받지 못하는 영세민에게 의료보호를 줌으로서 건강한 생활을 영위하도록 한 사업이다. 이 사업은 1977년 1월 4일부터 시행되었으며 이를 시행하기 위하여 정부에서는 생활보호대상자와 저소득자를 대상으로 황색카드, 녹색카드의 진료증을 구분 발부하였다. 또한 진료지역을 구분하고 1차 진료기관과 2차 진료기관을 지정하여 총 2백9만5천명의 생활보호대상자 및 저소득자를 대사으로 진료사업을 벌여 왔다. 본 연구의 목적은 첫째, 일 보건소 의료보호이용자의 일반적인 특성, 의료보호에 대한 인식과 보건소 이용 후 반응을 실태조사 함으로서, 둘째, 의료보호 분야의 보다 진보된 연구활동 및 의료보호사업 수행에 기여하고자함으로 하였다. 연구대상은 1978년 10월 현재 한달간 보건소를 방문하는 대상자로 하였다. 표집대상추출은 한달을 대표할 수 있는 일주일을 표본추출하여 이 기간동안 방문한 대상자를 전수조사 하였다. 자료수집은 본 연구조사를 위하여 훈련된 면접자가 1978년 10월 16일부터 21일까지 6일 동안 질문지를 가지고 대상자를 직접 면접하여 타기식 방법으로 기록하였다. 통계적 분석방법은 대상자의 일반적 특성과 각 문항별 응답은 백분율을 사용하였고, 일반적 특성과 의료보호에 대한 지식과의 관계는 T-test와 F-test를 사용하였으며 일반적 특성과 보건소 이용 후 반응과의 관계는 x**2-test를 사용하여 유의성의 차이를 검정하였다. 연구결과를 요약하면 다음과 같다. 1. 일반적인 특성 보건소를 방문한 의료보호대상자는 남자 36.3%, 여자 63.7%로 나타났다. 교육정도는 국민학교 교육이수자 이하가 21.2%로 나타났다. 한달간 월수입은 5만원 미만이 69%, 5만원 이상이 31%로 나타났다. 지난 한달간의 의료비 지출로서 지출이 없는 경우가 54.8%, 5,000원 미만이 21.3%, 5,000원 이상이 23.9%였다. 아픈 경우 돋 보건소에 오는 경우는 30.8%, 오지 않는 경우는 69.2%로 나타났으며 곧 보건소로 오지 않는 겅우 약방이용 38.9%, 자가치료 38.9% 병원이용 15.3%, 한약방이용 6.9%로 나타났다. 2. 의료보호에 대한 인식 보건소에서 제공하는 서비스 종류에 관한 질문에서 평균은 14점 만점에 6,796으로 나타났다. 50% 이상의 응답자가 보건소에서 할 수 있다고 응답한 항목은 가슴사진, 소변검사, 대변검사, 예방접종, 고혈압치료, 위궤양 등 5항목이었으며 50%이하의 응답자가 보건소에서 할 수 있다고 응답한 항목은 건강진단, 피검사, 응급처치, 의, 간단한 수술, 매독치료, 경중의 화상, 식중독, 예뻐지는 성형수술 등 9항목이었다. 전체적으로 모르겠다라고 응답한 율은 44.5%로 정확한 지식이 없어 인식정도가 �C다는 것을 나타내고 있다. 의료보호에 대한 지식은 진료증이 있으면 아무 병원이나 갈 수 있다는 항목(63.7%)외의 다른 항목은 지식 정도가 낮았다. (의료보호 종류 14.2%, 녹색카드 환자가 입원시 정부보조 20.4%, 국가에서 보조해 준 돈 갚는 년수 10.6%, 거주지 변경확인처 16.8%.) 의료보호에 대한 인식정도는 나이가 적을수록 (F=2.2123 d.f.=112, P<0.1), 교육정도가 높을수록 (F=4.6486 d.f.=112, P<0.05) 높게 나타났다. 3. 보건소 이용 후 반응 보건소 방문 전 서비스에 대한 기대와 그 결과에서 약처방이 제일 높아 기대 85.8%, 결과 94.7%를 나타내고 있다. 기타 병의 원인을 안다(기대 15.0%, 결과 24.8%), 확실한 진단명을 안다(기대 23.9%, 결과 33.6%), 집에서 주의해야 할 점을 설명한다(기대 9.7%, 결과 33.6%)등의 항목들은 기대 및 결과가 낮았다. 치료에 만족도는 63.7%가 만족한다고 응답했다. 의료기관 이용은 진료증이 생긴 이후 전보다 더 이용한다 69.6%, 별 차이 없다가 30.4%로 나타났다. 일반적 특성에 따른 보건소 이용 후 반응은 치료에 만족한 사람이 보건소를 계속 이용한다고 나타났다. [영문] National Medical Aid Program is one of the government's social development enterprises of the 4th economic development plan. It is for needy people whose health problem can not be solved by themselves because of their economic conditions with the hope of their being able to lead healthy lives by means of health benefits. This program was put into operation from January 4, 1977. In order to do this, the government insued two kinds of medical treatment certificate card, yellow card for those who need life protection and green card for low income people. Health service areas were also classified as primary and secondary treatment organizations. Until now the total number of subject people is 2,095,000. This study is a descriptive study attempted in order to present some data helpful for the betterment of the health benefit enterprise by surveying the subjects' perception and response about National Medical Aid Program and examining the relationship between some characteristics and the subjects' perceptions and responses. The subjects for this study were a time sample of one week selected from one month. The total number of subjects was 113. Data was collected for 6 days from October 16th to October 23th, 1978 through personal interviews with questionaires by well trained interviewers. Statistical analysis of the data were percentage for each item, T-test and F-test for the relationship between general characteristics and subjects' perception about National Medical Aid Program, and X**2-test for the relationship between general characteristics and response. Result of this study can be summarized as follows. 1. General characteristics of subjects There were 63.7% female and 36.3% male subjects. Seventy seven point nine percent of the subjects educational level was under elementary school graduations and only 22.1% of the subjects completed middle school course. 69.9% of the heads of subjects' household were unemployed, 21.2% were laborers. Sixty nine percent of respondents' income during one mouth were under 50,000 and 31% were over 50,000. when they were sick, 30.8% of the subjects came to the Health center and the rest 69.2% didn't come. Those who didn't come to the Health center, 38.9% were using the pharmacy, 38.9% were staying at home without any treatment, 15.3% were using hospitals, 6.9% were using Chinese clinics. 2. Subjects' perception about National Medical Aid Program The average of subjects' perception about the kinds of service which the Health center provide was 6.796 (total 14) and standard deviation was 3.88a3. 44.8% of subjects responded "don't know well" chest x-ray, urinalysis, stool examination, vaccination, Hypertension treatment, ulcer treatment were the items which over 50% of subjects responded that the Health center provide, and the items which under 50% of subjects responded were physical check up, hematology, emergency care, simple surgery, syphillis treatment, mild burn care, food poisoning, plastic surgery. Subjects' perception about National Medical Aid Program was low with the exception of the item 'Subjects can utilize every health care facilities with certificate card. (63.7%) Other were; the kinds of card (14.2%), amounts of government's subsidy when one uses the green card for admission to hospital (20.4%), years one is allowed to pay back government's subsidy(10.6%), the place one should notify when they move (16.8%). In the relationship of the general characteristics of the subjects' perception; the group with younger age (F=2.2123 df=112 P<0.1) and higher education (F=4.6486 df=112 P<0.05) had a tendency for higher perception about National Medical aid Program. 3. Subjects' response after receiving service Concerning subjects' expectations for service before visiting the Health center and results after receiving service, the percents of results after receiving service were higher than expectations. Drug prescription was highest; expectation (85.8%), result (94.7%). Subjects' satisfaction with treatment were 63.7%. In the relationship of some characteristics of the subject's response after visiting the Health center, the group who visited the Health center more frequently were more satisfied with the service (X**2=15.47 df=4 P<0.05), the group who satisfied with the service visited the Health center more than unsatisfactory group. 간호학과/박사[한글]본 연구는 재가 편마비 환자의 건강증진행위를 설명하는 요인을 규명하고 이들요인들의 영향력을 파악함으로써,성공적인 건강증진행위 수행에 효과적인 중재전략 개발을 위한 통합적인 설명모형을 개발하고 검증하고자 시도되었다.이를 위해 재가 편마비 환자의 건강증진행위에 관한 선행연구,문헌과 Pender의건강증진 모델을 바탕으로,일상생활수행능력,종교성,가족지지는 외생변수로,건강증진행위,자기효능감,장애수용,지각된 장애성,우울은 내생변수로 구성된 가설적 모형을 구축하였다.자료수집은 2005년 12월 5일부터 2006년 2월 28일까지 G시의 1개 대학병원,1개 종합병원의 재활의학과 또는 신경내ㆍ외과 외래,1개 한방병원 신경내과 외래,2개 보건소 연계 가정방문,1개 노인복지회관 가정파견서비스,1개 뇌졸중환자 인터넷 카페 동호회의 재가 편마비 환자를 대상으로 설문조사를 실시하여 총 239부가 최종분석에 사용되었다.수집된 자료는 SPSSWin 12.0program을 이용하여 기술 통계와 상관관계 분석을,AMOS 4.0 program을 이용하여 공분산 구조분석을실시하였다.연구결과는 다음과 같다.1.가설적 모형의 전반적인 적합도를 확인한 결과,적합지수가 일부 부적절하여수정이 필요한 것으로 나타났다 (χ2=143.843, p=0.001, RMSR=0.041,RMSEA=0.045,GFI=0.939,AGFI=0.893,NFI=0.942,NNFI=0.968,PNFI=0.597, χ2/df=1.483).2.모형의 적합도를 개선하기 위하여 이론적 배경과 논리적 타당성을 고려하며4개의 경로를 삭제하고,수정지수가 높은 5개의 오차 상관관계를 추가하여 수정모형을 확정하였다.모형수정 후 가설적 모형보다 간명하면서도 전반적인 적합지수가 높게 나타났다 (χ2=116.159,p=0.090,RMSR=0.034,RMSEA=0.029,GFI=0.951,AGFI=0.914,NFI=0.953,NNFI=0.987,PNFI=0.605,χ2/df=1.198).3.18개의 가설적 경로 중 수정모형을 통해 12개의 경로가 지지되었으며,수정모형내의 설명변수들은 건강증진행위를 55% 설명하였다.가설검증의 결과는 다음과 같다.1)건강증진행위에 유의하게 직접 영향을 주는 변수는 자기효능 (β=.746,p<.01)과 가족지지 (.=.193,p<.05)이었고,특히 자기효능감은 강력한 예측변인이었다.2)자기효능감은 우울 (β=-.553,p < .01)과 장애수용 (β=.194,p < .05)에 의해직접적인 영향을 받았다.3)장애수용은 지각된 장애성 (β=-.527,p<.01),종교성 (.=.385,p<.05),가족지지 (.=.361,p<.05)에 의해 직접적인 영향을 받았다.4)지각된 장애성은 일상생활수행능력 (.=-.453,p < .01)과 가족지지 (.=-.887,p < .01)에 의해 직접적인 영향을 받았다.5)우울은 장애수용 (β=-.337,p< .01),일상생활수행능력 (.=-.404,p < .01),가족지지 (.=-.278,p<.01)에 의해 직접적인 영향을 받았다.재가 편마비 환자의 건강증진행위를 촉진시키기 위해서는 신체 및 종교적인 개인적 요인,인간 상호간 요인,정서 및 인지적 요인 등 다양한 직ㆍ간접적 영향요인들을 근거로 한 포괄적인 간호중재 프로그램이 계획되어야 할 것이다.환자들이자원을 찾아 활용하고,자기효능감을 개발할 수 있도록 도울 뿐만 아니라 정서적인 상태의 향상과 신체적 활동성의 강화,그리고 종교적 성장을 이끄는 전략을 개발하고 사용하는 것이 필요할 것이다. 공공기관의 청년채용 결정 요인에 관한 연구 : 조직 특성을 중심으로

(A) case-control study on hemorrhoids risk factors in Korean army : 대조군 연구

국제보건학과/석사[한글] 본 연구는 군내 제반 환경의 열악함과 개인위생을 소홀히 할 수 있는 낯선 환경속에서 발생할 수 있는 치핵에 관하여 군내 치핵의 위험요인을 찾고자 실시한 환자-대조군 연구이다. 환자군은 군내 후방병원 2곳, 군단지원병원 1곳에서 “치핵” 진단받고 입원중인 환자로 군에 와서 치핵이 생겼거나 그 증상이 더욱 심해진 경우를 택하였으며 2008년 3월 19일부터 2008년 4월 18일까지 입원중인 환자 89명이며, 대조군은 2008년 3월 20일 현재 경기권 00사단과 **사단에 소속된 치핵 병력이 없는 병사 341명을 연구 대상으로 하였다. 연구대상자의 특성, 개인건강관련 특성, 군 입대 전 / 직후(입대일～100일)의 위생, 배변, 식습관 관련특성이 치핵 발생과 관련이 있는지를 알아보고자 X2 검정을 하였으며, 치핵발생에 영향을 주는 요인을 파악하고자 다변량 로지스틱 회귀분석을 하였다. 본 연구의 결과를 요약하면 다음과 같다. 1. 연구대상자의 특성에 따른 환자군과 대조군 비교에서 연령, 학력이 통계적으로 유의한 차이가 있었다. 2. 개인 건강관련 특성에 따른 환자군와 대조군의 비교에서 가족력, 피로가 통계적으로 유의한 차이가 있었다. 3. 군 입대 전 위생, 배변, 식습관 관련 특성과 환자군과 대조군의 비교에서 좌욕빈도, 치핵 과거력, 항문 출혈, 만성변비, 배변횟수가 통계적으로 유의한 차이가 있었다. 4. 군 입대 직후(입대일~100일) 위생, 배변, 식습관 관련 특성과 환자군과 대조군의 비교에서 샤워/목욕 빈도, 좌욕 빈도, 만성변비, 배변횟수, 배변시간, 기상 후 수분섭취여부가 통계적으로 유의한 차이가 있었다. 5. 치핵에 통계적으로 유의하게 영향을 미치는 요인을 알아보고자 다변량 로지스틱 회귀분석을 한 결과 연령이 25세이상인 병사가 25세미만인 병사에 비해 비차비는 3.45, 피로를 자주 느끼는 병사가 피로를 거의 느끼지 않는 병사에 비해 비차비는 0.26배이었다. 군 입대 전 좌욕을 하지 않는 병사가 좌욕을 하는 병사에 비해 비차비는 5.73, 군 입대 전 항문 출혈이 있는 병사가 없는 병사에 비해 비차비는 2.83, 군 입대 전 1일 1회미만 배변하는 병사가 1일 1회이상 배변하는 병사에 비해 비차비는 0.39, 군 입대 전 배변소요시간이 10분이상인 병사가 10분미만 소요되는 병사에 비해 비차비는 0.41이었다. 군 입대 직후(입대일~100일) 샤워/목욕을 자주하는 병사가 그렇지 않은 병사에 비해 비차비가 7.33, 군 입대 직후 배변소요시간이 10분이상인 병사가 10분미만 소요되는 병사에 비해 비차비는 10.06이었다. 이상의 결과를 종합해 볼 때 통계적으로 유의한 변수 중 병사 개인들이 관심을 갖고 관리하고 병사개인의 사생활이 보장되는 샤워시설 확충하며 화장실 환경을 개선하여 배변 시 심리적으로 편안함을 느낄 수 있도록 하고 병사 개개인이 규칙적인 배변습관을 가질 수 있도록 배려한다면 치핵의 발생 및 재발을 감소시킬 가능성이 있다. [영문] The object of this study was to examine the factors affecting the invasion of hemorrhoids in military servicemen. The case group 89 inpatients who have been diagnosed as hemorrhoids from Mar. 19, 2008 through Apr. 18, 2008, sampled out from those military servicemen who are receiving hospitalized medical care in military hospital. The control group was 341 soldiers who are fulfilling their military service in 00 Division and ** Division on front line. Multivariate logistic regression analysis was carried out to analyze the factors that influence upon the contraction of hemorrhoids. The results were as follows: 1. There was statistically significant difference on personal characteristics between the case and control group in history of age and educational background. 2. There was statistically significant difference on personal health-related characteristics between the case and control group in history of fatigue and family history. 3. In comparison between the case and control group (after joining military service) on personal hygiene, excretion and eating habits, there was notable difference in the number of taking site baths, past history, anal hemorrhage, presence or absence of constipation, and the number of excretion. 4. In comparison between the case and control group (after joining military service) on personal hygiene, excretion and eating habits, there was notable difference in the number of taking showers, the number of taking site baths, presence or absence of constipation, the number of excretion, and the length of time of excretion, and having water after waking up. 5. Multivariate logistic regression analysis was carried out to analyze the factors that influence upon the contraction of hemorrhoids. The ratio of sample whose age is over 25 to below 25 were 3.45, and the ratio of sample who often feel tired to those who do not were 0.26. The ratio of sample who had taken sitz baths (before joining military service) to those who had taken often were 5.73, the ratio of sample who had anal hemorrhage (before joining military service) to those who do not were 2.83, the ratio of sample who had not taken daily excrement (before joining military service) to those who had daily excrement were 0.39, the ratio of sample who had not been regular in the length of time in excrement (before joining military service) to those who were 0.41. The ratio of sample who takes shower on regular basis (after joining military service) to those who do not were 7.33, and the ratio of sample who had not been regular in the length of time in excrement (after joining military service) to those who does were 10.08. In conclusion, the result suggests that the contraction or recurrence of hemorrhoids could be distinctively reduced if military personnel pay more attention to their environment and improve the facility (bathroom, shower room, etc) to have mental comfort while having excrement.ope

Level of knowledge and performance on delirium care among clinical care nurses

간호학과/석사본 연구는 중환자실 간호사의 섬망에 대한 지식수준 및 간호수행정도를 파악하고 이들의 관계를 분석함으로써 , 향후 섬망 간호수행도를 높일 수 있는 기초자료로 제공하고자 시도된 서술적 상관관계연구이다. 자료수집 기간은 2010년 10월 18일부터 11월 5일까지 이루어졌으며 서울 소재 2개 종합병원 중환자실에 근무하는 간호사 225명을 대상으로 자기기입식 설문지법을 통해 이루어졌다. 연구 도구는 섬망에 대한 지식수준을 조사하기 위해 장기은(2008)의 도구를 수정․보완하였고, 간호수행정도를 조사하기 위해 양영애(2006)의 도구를 수정․보완하여 사용하였다. 수집된 자료는 PASW 18.0 프로그램을 이용하여 실수와 백분율, 평균과 표준편차, t-test, ANOVA, Pearson''s correlation coefficient로 분석하였으며, 연구의 주요 결과는 다음과 같다.1. 중환자실 간호사의 섬망에 대한 지식 점수는 평균 28.56(±5.39)점(범위 : 9～38점)으로 대상자의 91.1%가 섬망에 대한 교육을 받은 적이 없다고 응답했음에도 불구하고 높은 수준의 지식을 갖고 있는 것으로 나타났다. 영역별 점수는 발생기전 13점 만점에 7.41(±2.01)점, 환경적 원인 6점 만점에 5.40(±0.99)점, 의학적 원인 7점 만점에 5.30(±1.87)점, 증상 14점 만점에 10.45(±2.51)점으로 발생기전에 대한 정답률이 다른 영역에 비해 상대적으로 낮은 것으로 파악되었다.2. 중환자실 간호사의 섬망에 대한 간호수행정도는 평균 111.21(±17.89)점(범위 : 53～160점)으로 사정영역은 15점 만점에 9.52(±2.47)점, 진단영역은 10점 만점에 6.02(±1.80)점, 계획영역은 15점 만점에 9.98(±2.74)점, 중재영역은 110점 만점에 75.93(±11.55)점, 평가영역은 15점 만점에 9.76(±2.83)점이었다. 중재 부분의 하위 영역은 일반적지지 15점 만점에 8.23(±2.51)점, 생리적지지 45점 만점에 35.37(±5.26)점, 정서적지지 15점 만점에 11.23(±2.17)점, 환경적지지 30점 만점에 21.10(±4.33)점이었다. 섬망의 영역 중 가장 높은 수행도를 보인 영역은 중재영역이고, 가장 낮은수행도를 보인 영역은 사정과 진단 영역으로 나타났다.3. 대상자의 일반적 특성에 따른 섬망에 대한 지식점수는 연령이 높을수록(F=4.212, p=.016), 교육정도가 높을수록(F=4.665,p=.010), 근무부서(F=2.750,p=.044)에 따라, 임상경력이 높을수록(F=3.311, p=.048), 중환자실 경력이 높을수록(F=3.775, p=.024) 통계적으로 유의하게 높게 나타났다.4. 대상자의 일반적 특성에 따른 섬망에 대한 간호수행정도는 섬망 교육을 경험한 대상자가 경험하지 않은 대상자에 비해 유의하게 높게 나타났고(t=4.115, p=.001), 근무부서별로는 외과계 중환자실 간호사가 신경계 중환자실, 내과계 중환자실 간호사에 비해 간호수행 정도가 유의하게 높게 나타났다(F=6.058, p=.001).5. 섬망에 대한 지식수준과 간호수행 간에는 유의한 상관관계가 없었다(r=.095, p=.155).이상의 연구 결과, 중환자실 간호사의 섬망에 대한 지식수준 및 간호수행도를 높이기 위해서는 섬망에 대한 기전과 사정도구 및 중재에 관한 교육 프로그램의 개발 및 제공이 필요하며, 또한 중환자실 간호사들이 이를 바탕으로 임상에서 섬망을 사정하고, 중재를 적용할 수 있는 섬망간호 가이드라인을 개발 및 적용하는 연구가 지속적으로 이루어져야 함을 시사한다.restrictio
