    Diagnostic accuracy of clinical outcome prediction using nursing data in intensive care patients: A systematic review

    Background: Nursing data consist of observations of patients' conditions and information on nurses' clinical judgment based on critically ill patients' behavior and physiological signs. Nursing data in electronic health records were recently emphasized as important predictors of patients' deterioration but have not been systematically reviewed. Objective: We conducted a systematic review of prediction models using nursing data for clinical outcomes, such as prolonged hospital stay, readmission, and mortality in intensive care patients, compared to physiological data only. In addition, the type of nursing data used in prediction model developments was investigated. Design: A systematic review. Methods: PubMed, CINAHL, Cochrane CENTRAL, EMBASE, IEEE Xplore Digital Library, Web of Science, and Scopus were searched. Clinical outcome prediction models using nursing data for intensive care patients were included. Clinical outcomes were prolonged hospital stay, readmission, and mortality. Data were extracted from selected studies such as study design, data source, outcome definition, sample size, predictors, reference test, model development, model performance, and evaluation. The risk of bias and applicability was assessed using the Prediction model Risk of Bias Assessment Tool checklist. Descriptive summaries were produced based on paired forest plots and summary receiver operating characteristic curves. Results: Sixteen studies were included in the systematic review. The data types of predictors used in prediction models were categorized as physiological data, nursing data, and clinical notes. The types of nursing data consisted of nursing notes, assessments, documentation frequency, and flowsheet comments. The studies using physiological data as a reference test showed higher predictive performance in combined data or nursing data than in physiological data. The overall risk of bias indicated that most of the included studies have a high risk. Conclusions: This study was conducted to identify and review the diagnostic accuracy of clinical outcome prediction using nursing data in intensive care patients. Most of the included studies developed models using nursing notes, and other studies used nursing assessments, documentation frequency, and flowsheet comments. Although the findings need careful interpretation due to the high risk of bias, the area under the curve scores of nursing data and combined data were higher than physiological data alone. It is necessary to establish a strategy in prediction modeling to utilize nursing data, clinical notes, and physiological data as predictors, considering the clinical context rather than physiological data alone. This is an academically valuable cases of a specific situation of internal communication within the response team during the public health emergency. Objectives: The empirical case in G District allows exploration of several aspects of the internal communication using SNS at a public health emergency response. This study provides an overview of the SNS posts communicated for quarantine management, and attempts to describe these communication method's potential for influencing quarantine management. Method: Qualitative and quantitative content analysis were conducted on 656 posts, from June 9, 2015, when the Band was created, until October 30, when the MERS-CoV epidemic was temporarily ended. In addition, social network analysis was performed on 27 posts that monitoring staffs questioned about their problems occurred during the quarantine management. Results: Closed-type SNS became common space for public health workers to internally interact and exchange information during public health emergency. In particular, it functioned as substitute internal communication channels when lack of coordination and internal information deficit to the response team were presented. As well as these roles, it was confirmed the achievement of SNS communication by efficient resolution of problems in quarantine management. Furthermore, most influential participants in communication network were identified, such as chief decision-maker and specialized functional managers (e.g., medical and livelihood management). Conclusion: Local public health workers and professionals should actively utilize closed-type SNS, for the purpose of communicating effectively within a response team and manage efficiently the quarantines in a public health emergency.1.     Background: Electronic health record (EHR) systems often face user resistance in hospitals, which results in a failure to acquire their full benefits. To implement the EHR successfully, it is crucial to reduce nurses' resistance to use the system. This study aimed to investigate the factors associated with nurses' resistance to use the EHR system. Methods: A descriptive correlational study was conducted with nurses working at four university hospitals in Korea using self-administered questionnaires to measure user resistance behavior, resistance to change, perceived usefulness, perceived ease of use, perceived value, colleagues' opinions, self-efficacy for change, and organizational support for change. Path analysis was performed to examine direct and indirect association with user resistance behavior. Results: A total of 223 nurses completed the questionnaires. All seven factors were found to be significantly associated with user resistance, either directly or indirectly. The total effect on user resistance behavior was highest in resistance to change (0.65), followed by perceived usefulness (- 0.33); both had direct but no indirect effects. Conversely, self-efficacy for change (- 0.25), perceived value (- 0.21), colleagues' opinions (- 0.16), perceived ease of use (- 0.16), and organizational support for change (- 0.05) had indirect but no direct effects. Conclusions: The study examined the factors associated with nurses' user resistance behavior after the implementation of a new EHR system. These findings could help hospitals develop better EHR implementation strategies to reduce user resistance behavior among the nursing staff.ope

[์˜๋ฌธ] National Medical Aid Program is one of the government's social development enterprises of the 4th economic development plan. It is for needy people whose health problem can not be solved by themselves because of their economic conditions with the hope of their being able to lead healthy lives by means of health benefits. This program was put into operation from January 4, 1977. In order to do this, the government insued two kinds of medical treatment certificate card, yellow card for those who need life protection and green card for low income people. Health service areas were also classified as primary and secondary treatment organizations. Until now the total number of subject people is 2,095,000. This study is a descriptive study attempted in order to present some data helpful for the betterment of the health benefit enterprise by surveying the subjects' perception and response about National Medical Aid Program and examining the relationship between some characteristics and the subjects' perceptions and responses. The subjects for this study were a time sample of one week selected from one month. The total number of subjects was 113. Data was collected for 6 days from October 16th to October 23th, 1978 through personal interviews with questionaires by well trained interviewers. Statistical analysis of the data were percentage for each item, T-test and F-test for the relationship between general characteristics and subjects' perception about National Medical Aid Program, and X**2-test for the relationship between general characteristics and response. Result of this study can be summarized as follows. 1. General characteristics of subjects There were 63.7% female and 36.3% male subjects. Seventy seven point nine percent of the subjects educational level was under elementary school graduations and only 22.1% of the subjects completed middle school course. 69.9% of the heads of subjects' household were unemployed, 21.2% were laborers. Sixty nine percent of respondents' income during one mouth were under 50,000 and 31% were over 50,000. when they were sick, 30.8% of the subjects came to the Health center and the rest 69.2% didn't come. Those who didn't come to the Health center, 38.9% were using the pharmacy, 38.9% were staying at home without any treatment, 15.3% were using hospitals, 6.9% were using Chinese clinics. 2. Subjects' perception about National Medical Aid Program The average of subjects' perception about the kinds of service which the Health center provide was 6.796 (total 14) and standard deviation was 3.88a3. 44.8% of subjects responded "don't know well" chest x-ray, urinalysis, stool examination, vaccination, Hypertension treatment, ulcer treatment were the items which over 50% of subjects responded that the Health center provide, and the items which under 50% of subjects responded were physical check up, hematology, emergency care, simple surgery, syphillis treatment, mild burn care, food poisoning, plastic surgery. Subjects' perception about National Medical Aid Program was low with the exception of the item 'Subjects can utilize every health care facilities with certificate card. (63.7%) Other were; the kinds of card (14.2%), amounts of government's subsidy when one uses the green card for admission to hospital (20.4%), years one is allowed to pay back government's subsidy(10.6%), the place one should notify when they move (16.8%). In the relationship of the general characteristics of the subjects' perception; the group with younger age (F=2.2123 df=112 P<0.1) and higher education (F=4.6486 df=112 P<0.05) had a tendency for higher perception about National Medical aid Program. 3. Subjects' response after receiving service Concerning subjects' expectations for service before visiting the Health center and results after receiving service, the percents of results after receiving service were higher than expectations. Drug prescription was highest; expectation (85.8%), result (94.7%). Subjects' satisfaction with treatment were 63.7%. [์˜๋ฌธ] The object of this study was to examine the factors affecting the invasion of hemorrhoids in military servicemen. The case group 89 inpatients who have been diagnosed as hemorrhoids from Mar. 19, 2008 through Apr. 18, 2008, sampled out from those military servicemen who are receiving hospitalized medical care in military hospital. The control group was 341 soldiers who are fulfilling their military service in 00 Division and ** Division on front line. Multivariate logistic regression analysis was carried out to analyze the factors that influence upon the contraction of hemorrhoids. The results were as follows: 1. There was statistically significant difference on personal characteristics between the case and control group in history of age and educational background. 2. There was statistically significant difference on personal health-related characteristics between the case and control group in history of fatigue and family history. 3. In comparison between the case and control group (after joining military service) on personal hygiene, excretion and eating habits, there was notable difference in the number of taking site baths, past history, anal hemorrhage, presence or absence of constipation, and the number of excretion. 4. In comparison between the case and control group (after joining military service) on personal hygiene, excretion and eating habits, there was notable difference in the number of taking showers, the number of taking site baths, presence or absence of constipation, the number of excretion, and the length of time of excretion, and having water after waking up. 5. Multivariate logistic regression analysis was carried out to analyze the factors that influence upon the contraction of hemorrhoids. The ratio of sample whose age is over 25 to below 25 were 3.45, and the ratio of sample who often feel tired to those who do not were 0.26. The ratio of sample who had taken sitz baths (before joining military service) to those who had taken often were 5.73, the ratio of sample who had anal hemorrhage (before joining military service) to those who do not were 2.83, the ratio of sample who had not taken daily excrement (before joining military service) to those who had daily excrement were 0.39, the ratio of sample who had not been regular in the length of time in excrement (before joining military service) to those who were 0.41. The ratio of sample who takes shower on regular basis (after joining military service) to those who do not were 7.33, and the ratio of sample who had not been regular in the length of time in excrement (after joining military service) to those who does were 10.08. In conclusion, the result suggests that the contraction or recurrence of hemorrhoids could be distinctively reduced if military personnel pay more attention to their environment and improve the facility (bathroom, shower room, etc) to have mental comfort while having excrement.ope
