    Acute schistosomiasis, a diagnostic and therapeutic challenge

    AbstractIn non-endemic countries, acute (invasive) schistosomiasis (AS) is typically seen in non-immune travellers, whereas chronic schistosomiasis is more frequently diagnosed in immigrants. Travellers with AS initially present with non-specific signs such as fever, cough, headache, and urticaria. Life-threatening cardiac and neurological complications may occur. The positive diagnosis of AS relies on seroconversion, which appears together with hypereosinophilia approximately 3 weeks after the onset of symptoms. When prescribed during AS, praziquantel usually does not prevent the chronic phase of the disease and is associated with exacerbation of signs and symptoms in approximately 50% of cases. According to the published literature, corticosteroids may be recommended alone or in association with praziquantel. When associated with corticosteroids, pharmacokinetic interactions may impair the efficacy of praziquantel. We suggest that corticosteroids should be restricted to use in patients with systemic complications of AS, whereas praziquantel should be initiated only when ova are detected in either stools or urine, depending on the culprit species

    Influence of Force-Length Relationship and Task-Specific Constraints on Finger Force-Generating Capacities

    Grip strength loss in extended and flexed wrist postures has been explained by reduced force-generating capacities of extrinsic finger flexor resulting from non-optimal length, owing to the force-length relationship. Recent works suggested that other muscles, especially wrist extensors, participate in this grip strength loss. The objective of this study was to clarify the role of the force-length relationship in finger force production. 18 participants performed maximal isometric finger force production during pinch grip (Pinch) and four-finger pressing (Press) tasks in four different wrist postures (extended, flexed, neutral, spontaneous). The maximum finger force (MFF), finger and wrist joint angles, as well as activation of four muscles were determined using dynamometry, motion capture, and electromyography. The force and length of the four muscles were estimated from joint angles and muscle activation using a musculoskeletal model. MFF decreased for flexed wrist during Pinch but remained stable across wrist postures during Press. The results suggested that the loss of pinch grip force in deviated wrist posture is partially related to force-length relationship of finger extensors.     The use of herbal tea with Artemisia annua by travelers and traditional communities in Africa has increased in recent years as a supposed form of malaria prophylaxis, although its use is not recommended due to lack of efficacy. The risk of severe malaria complications that can lead to death is real regarding said behavior, and awareness needs to be raised. We report a case of severe Plasmodium falciparum malaria imported in the Amazon rainforest by a traveler returning from Cameroon who treated himself with Artemisia annua herbal tea

    Travel-Associated Zika Virus Disease Acquired in the Americas Through February 2016

    BACKGROUND: Zika virus has spread rapidly in the Americas and has been imported into many nonendemic countries by travelers. OBJECTIVE: To describe clinical manifestations and epidemiology of Zika virus disease in travelers exposed in the Americas. DESIGN: Descriptive, using GeoSentinel records. SETTING: 63 travel and tropical medicine clinics in 30 countries. PATIENTS: Ill returned travelers with a confirmed, probable, or clinically suspected diagnosis of Zika virus disease seen between January 2013 and 29 February 2016. MEASUREMENTS: Frequencies of demographic, trip, and clinical characteristics and complications. RESULTS: Starting in May 2015, 93 cases of Zika virus disease were reported. Common symptoms included exanthema (88%), fever (76%), and arthralgia (72%). Fifty-nine percent of patients were exposed in South America; 71% were diagnosed in Europe. Case status was established most commonly by polymerase chain reaction (PCR) testing of blood and less often by PCR testing of other body fluids or serology and plaque-reduction neutralization testing. Two patients developed Guillain–Barre syndrome, and 3 of 4 pregnancies had adverse outcomes (microcephaly, major fetal neurologic abnormalities, and intrauterine fetal death). LIMITATION: Surveillance data collected by specialized clinics may not be representative of all ill returned travelers, and denominator data are unavailable. CONCLUSION: These surveillance data help characterize the clinical manifestations and adverse outcomes of Zika virus disease among travelers infected in the Americas and show a need for global standardization of diagnostic testing. The serious fetal complications observed in this study highlight the importance of travel advisories and prevention measures for pregnant women and their partners. Travelers are sentinels for global Zika virus circulation and may facilitate further transmission

    Influence de la concentration de la solution nutritive sur la croissance et la nutrition minérale de la tomate

    L’influence de deux concentrations de solution nutritive a été étudiée sur la croissance et la nutrition minérale de la tomate en culture hydroponique sous serre au stade début floraison. La force ionique de la solution nutritive la plus concentrée était de 20.00 meq L–1 (SNC), valeur proche de celle qui est habituellement utilisée pour la culture de la tomate ; celle de la solution diluée (SND) était de 3.25 meq L–1. Pendant les deux semaines de l’expérimentation, la concentration du milieu nutritif n’a pas eu d’effet sur la croissance et le développement des tomates. La solution nutritive concentrée a induit une consommation de luxe en macroéléments (calcium, magnésium, soufre et phosphore) et un antagonisme entre les ions divalents (Ca-Mg) et le potassium. La solution nutritive diluée a limité la précipitation de sels dans le milieu, à la surface et dans les racines ; elle a amélioré l’efficience de l’eau consommée et la nutrition en microéléments des feuilles (Fe, Mn, Cu, Zn). Pour la culture hors sol de la tomate, il semble donc préférable d’utiliser des solutions nutritives plus diluées que celles qui sont habituellement utilisées. Cette pratique permettrait de diminuer les quantités d’éléments minéraux utilisées et surtout limiterait l’impact des effluents de solutions nutritives non recyclées sur l’environnement. Par contre, pour éviter tout risque de facteur limitant nutritionnel, il serait indispensable de contrôler régulièrement le niveau des concentrations en éléments minéraux dans la solution nutritive
