An evaluation of pipelines for DNA variant detection can guide a reanalysis protocol to increase the diagnostic ratio of genetic diseases
Clinical exome (CE) sequencing has become a first-tier diagnostic test for hereditary diseases; however, its diagnostic rate is around 30–50%. In this study, we aimed to increase the diagnostic yield of CE using a custom reanalysis algorithm. Sequencing data were available for three cohorts using two commercial protocols applied as part of the diagnostic process. Using these cohorts, we compared the performance of general and clinically relevant variant calling and the efficacy of an in-house bioinformatic protocol (FJD-pipeline) in detecting causal variants as compared to commercial protocols. On the whole, the FJD-pipeline detected 99.74% of the causal variants identified by the commercial protocol in previously solved cases. In the unsolved cases, FJD-pipeline detects more INDELs and non-exonic variants, and is able to increase the diagnostic yield in 2.5% and 3.2% in the re-analysis of 78 cancer and 62 cardiovascular cases. These results were considered to design a reanalysis, filtering and prioritization algorithm that was tested by reassessing 68 inconclusive cases of monoallelic autosomal recessive retinal dystrophies increasing the diagnosis by 4.4%. In conclusion, a guided NGS reanalysis of unsolved cases increases the diagnostic yield in genetic disorders, making it a useful diagnostic tool in medical geneticsWe want to thank the participants for consenting to the use of their data for the study. Impact of late presentation of HIV infection on short-, mid- and long-term mortality and causes of death in a multicenter national cohort: 2004–2013
SummaryObjectivesTo analyze the impact of late presentation (LP) on overall mortality and causes of death and describe LP trends and risk factors (2004–2013).MethodsCox models and logistic regression were used to analyze data from a nation-wide cohort in Spain. LP is defined as being diagnosed when CD4 < 350 cells/ml or AIDS.ResultsOf 7165 new HIV diagnoses, 46.9% (CI95%:45.7–48.0) were LP, 240 patients died.First-year mortality was the highest (aHRLP.vs.nLP = 10.3[CI95%:5.5–19.3]); between 1 and 4 years post-diagnosis, aHRLP.vs.nLP = 1.9(1.2–3.0); and >4 years, aHRLP.vs.nLP = 1.5(0.7–3.1).First-year's main cause of death was HIV/AIDS (73%); and malignancies among those surviving >4 years (32%). HIV/AIDS-related deaths were more likely in LP (59.2% vs. 25.0%; p < 0.001). LP declined from 55.9% (2004–05) to 39.4% (2012–13), and reduced in 46.1% in men who have sex with men (MSM) and 37.6% in heterosexual men, but increased in 22.6% in heterosexual women.Factors associated with LP: sex (ORMEN.vs.WOMEN = 1.4[1.2–1.7]); age (OR31–40.vs.<30 = 1.6[1.4–1.8], OR41–50.vs.<30 = 2.2[1.8–2.6], OR>50.vs.<30 = 3.6[2.9–4.4]); behavior (ORInjectedDrugUse.vs.MSM = 2.8[2.0–3.8]; ORHeterosexual.vs.MSM = 2.2[1.7–3.0]); education (ORPrimaryEducation.vs.University = 1.5[1.1–2.0], ORLowerSecondary.vs.University = 1.3[1.1–1.5]); and geographical origin (ORSub-Saharan.vs.Spain = 1.6[1.3–2.0], ORLatin-American.vs.Spain = 1.4[1.2–1.8]).ConclusionsLP is associated with higher mortality, especially short-term- and HIV/AIDS-related mortality. Mid-term-, but not long-term mortality, remained also higher in LP than nLP. LP decreased in MSM and heterosexual men, not in heterosexual women. The groups most affected by LP are low educated, non-Spanish and heterosexual women

Pruebas externas de evaluación de química preuniversitaria: repercusión en la enseñanza-aprendizaje de la química y análisis de las pruebas de España, Reino Unido e Irlanda

de enseñanza-aprendizaje, especialmente cuando sus resultados tienen consecuencias
directas en el alumnado. La presente tesis estudia, en primer lugar, el impacto que ejercen
las pruebas de acceso a la universidad (PAU) de química en España, dilucidando los
aspectos del proceso educativo que se ven más afectados y los factores de los que
dependen; para recabar los datos pertinentes, se difundió un cuestionario entre el
profesorado de química de bachillerato de todo el territorio nacional obteniéndose una
muestra representativa de la mayoría de las comunidades autónomas, lo que permitió
analizar los efectos de las PAU en las aulas de bachillerato de los distintos distritos
universitarios. A continuación, la tesis aborda un estudio comparativo de las pruebas
externas que realiza el alumnado de España, Reino Unido e Irlanda al finalizar la
educación secundaria posobligatoria, con el fin de averiguar si las citadas pruebas cubren
los aspectos fundamentales de la competencia científica presentes en los currículos
oficiales para la enseñanza de la química; para alcanzar este objetivo se analizaron las
PAU de química propuestas en el periodo 2010-2016 en diez comunidades autónomas
junto con las evaluaciones externas de química realizadas en el mismo periodo en Reino
Unido e Irlanda. Los resultados obtenidos en el primer estudio, ponen de manifiesto la
repercusión de las PAU en el currículo impartido en las aulas, en la metodología empleada
por el profesorado, en los recursos didácticos empleados y en los procedimientos de
evaluación. Por su parte, el segundo estudio revela las fortalezas y debilidades de las
pruebas de evaluación externa analizadas y sus previsibles consecuencias en el proceso
de enseñanza-aprendizaje. A partir de estos resultados, se formulan propuestas para la
mejora de las PAU de química, de forma que favorezcan la formación integral del
alumnado; también se apuntan posibles líneas de investigación que permitirán ampliar el
estudio realizad
Effects of Fasting on Intermediary Metabolism Enzymes in the Liver and Muscle of Rainbow Trout
Fish can go through periods of feed deprivation, either due to natural causes influenced by temperature or management techniques in animal production, affecting their energy metabolism differently, depending on the duration of fasting in days and water temperature. This study showed the effect of different days of fasting before slaughter in rainbow trout by analyzing the intermediary metabolic enzymes in the muscle and liver. For this purpose, a non-fasting group was compared with trout that fasted for 5, 10, and 20 days (55, 107, and 200 degree days (°C d), respectively). A first phase of increased activity of enzymes involved in glycolysis was observed, increasing hexokinase enzyme activity in muscle and pyruvate kinase enzyme activity in muscle and liver. As the fasting days progressed, enzymes involved in the gluconeogenic and glycogenolytic pathways in the liver such as lactate dehydrogenase and glycogen phosphorylase increased their activity regarding the no fasted group. After 20 days of fasting (200 °C d), lipogenic and protein related enzyme activity depletion was observed, leading to significant changes in energy metabolism. Therefore, prolonged fasting with high degree days duration may compromise the energy supply for the vital development of rainbow trout
Análisis de resultados de investigación sobre la ciencia presente en la sociedad y su incorporación al aula
Este Proyecto de Innovación Docente es continuación de los proyectos anteriores PIE 43/2017-18, PIE 39/2018-19 y PIE 179/2019-20; y está en relación directa con el Proyecto I+D+i Identificación de contextos científicos en la sociedad. Herramientas para docentes y ciudadanos (RTI2018-094303-A-I00 del Ministerio de Economía y Competitividad, correspondiente al Programa Estatal de I+D+i Orientada a los Retos de la Sociedad, en el marco del Plan Estatal de Investigación Científica y Técnica y de Innovación 2019-21). También con el Proyecto FFI2015-64529, «La frontera entre ciencia y política y la ciencia en la frontera: la ciencia española, 1907-1975» del MINECO. Todos estos proyectos han venido realizándose con participación de por varios de los profesores del Grupo de Investigación UCM-970762 NEURODIDÁCTICA, CIENCIA Y SOCIEDAD.
La finalidad última de la propuesta que se presenta ahora, es analizar los datos obtenidos en estos proyectos anteriores. Además, existen otros cinco de objetivos. En suma, buscamos:
(1) analizar los datos obtenidos en los proyectos anteriores;
(2) revisar las herramientas metodológicas utilizadas para la obtención de estos datos;
(3) tomar en consideración otros modos paramétricos para medir la percepción sobre la ciencia;
(4) fomentar la participación de nuestros estudiantes, y futuros docentes, en los procesos de análisis de resultados de investigación; y
(5) incorporar a nuestros estudiantes y alguna de sus propuestas como una más de las actividades realizadas por el Museo Nacional de Ciencias Naturales. Este último objetivo se interrumpió en el proyecto anterior, en marzo del 2020, debido a las circunstancias causadas por la el covid-19, lo que nos invita a plantearnos, también,
(6) fundamentar en los conocimientos y datos científicos la toma de decisiones personales y comunitarias frente a bulos y desinformaciones
