    Wound botulism in injectors of drugs: upsurge in cases in England during 2004.

    Wound infections due to Clostridium botulinum were not recognised in the UK and Republic of Ireland before 2000. C. botulinum produces a potent neurotoxin which can cause paralysis and death. In 2000 and 2001, ten cases were clinically recognised, with a further 23 in 2002, 15 in 2003 and 40 cases in 2004. All cases occurred in heroin injectors. Seventy cases occurred in England; the remainder occurred in Scotland (12 cases), Wales (2 cases) and the Republic of Ireland (4 cases). Overall, 40 (45%) of the 88 cases were laboratory confirmed by the detection of botulinum neurotoxin in serum, or by the isolation of C. botulinum from wounds. Of the 40 cases in 2004, 36 occurred in England, and of the 12 that were laboratory confirmed, 10 were due to type A. There was some geographical clustering of the cases during 2004, with most cases occurring in London and in the Yorkshire and Humberside region of northeast England

    Multiscale modeling of molecular structure and optical properties of complex supramolecular aggregates

    Supramolecular aggregates of synthetic dye molecules offer great perspectives to prepare biomimetic functional materials for light-harvesting and energy transport. The design is complicated by the fact that structure-property relationships are hard to establish, because the molecular packing results from a delicate balance of interactions and the excitonic properties that dictate the optics and excited state dynamics, in turn sensitively depend on this packing. Here we show how an iterative multiscale approach combining molecular dynamics and quantum mechanical exciton modeling can be used to obtain accurate insight into the packing of thousands of cyanine dye molecules in a complex double-walled tubular aggregate in close interaction with its solvent environment. Our approach allows us to answer open questions not only on the structure of these prototypical aggregates, but also about their molecular-scale structural and energetic heterogeneity, as well as on the microscopic origin of their photophysical properties. This opens the route to accurate predictions of energy transport and other functional properties

    A three-range approach enhances the prognostic utility of CSF biomarkers in Alzheimer's disease

    Introduction: Alzheimer's disease consensus recommends biomarker dichotomization, a practice with well-described clinical strengths and methodological limitations. Although neuroimaging studies have explored alternative biomarker interpretation strategies, a formally defined three-range approach and its prognostic impact remains under-explored for cerebrospinal fluid (CSF) biomarkers. Methods: With two-graph receiver-operating characteristics based on different reference schemes, we derived three-range cut-points for CSF Elecsys biomarkers. According to baseline CSF status, we assessed the prognostic utility of this in predicting risk of clinical progression and longitudinal trajectories of cognitive decline and amyloid–beta (Aβ) positron emission tomography (PET) accumulation in non-demented individuals (Alzheimer's Disease Neuroimaging Initiative [ADNI]; n = 1246). In all analyses, we compared herein-derived three-range CSF cut-points to previously described binary ones. Results: In our main longitudinal analyses, we highlight CSF p-tau181/Aβ1-42 three-range cut-points derived based on the cognitively normal Aβ-PET negative versus dementia Aβ-PET positive reference scheme for best depicting a prognostically relevant biomarker abnormality range. Longitudinally, our approach revealed a divergent intermediate cognitive trajectory undetected by dichotomization and a clearly abnormal group at higher risk for cognitive decline, with power analyses suggesting the latter group as potential trial enrichment candidates. Furthermore, we demonstrate that individuals with intermediate-range CSF status have similar rates of Aβ deposition to those in the clearly abnormal group. Discussion: The proposed approach can refine clinico-biological prognostic assessment and potentially enhance trial recruitment, as it captures faster biomarker-related cognitive decline in comparison to binary cut-points. Although this approach has implications for trial recruitment and observational studies, further discussion is needed regarding clinical practice applications

    Periodicities in the high-mass X-ray binary system RXJ0146.9+6121/LSI+61 235

    The high-mass X-ray binary RX J0146.9+6121, with optical counterpart LS I+61°235 (V831 Cas), is an intriguing system on the outskirts of the open cluster NGC 663. It contains the slowest Be type X-ray pulsar known with a pulse period of around 1400 s and, primarily from the study of variation in the emission line profile of Hα, it is known to have a Be decretion disc with a one-armed density wave period of approximately 1240 d. Here we present the results of an extensive photometric campaign, supplemented with optical spectroscopy, aimed at measuring short time-scale periodicities. We find three significant periodicities in the photometric data at, in order of statistical significance, 0.34, 0.67 and 0.10 d. We give arguments to support the interpretation that the 0.34 and 0.10 d periods could be due to stellar oscillations of the B-type primary star and that the 0.67 d period is the spin period of the Be star with a spin axis inclination of 23+10−8 degrees. We measured a systemic velocity of −37.0 ± 4.3 km s−1 confirming that LS I+61°235 has a high probability of membership in the young cluster NGC 663 from which the system's age can be estimated as 20–25 Myr. From archival RXTE All Sky Monitor (ASM) data we further find ‘super’ X-ray outbursts roughly every 450 d. If these super outbursts are caused by the alignment of the compact star with the one-armed decretion disc enhancement, then the orbital period is approximately 330 d

    Structural properties of disk galaxies. II. Intrinsic shape of bulges

    (Abridged) The structural parameters of a magnitude-limited sample of 148 unbarred S0-Sb galaxies were analyzed to derive the intrinsic shape of their bulges. We developed a new method to derive the intrinsic shape of bulges based on the geometrical relationships between the apparent and intrinsic shapes of bulges and disks. The equatorial ellipticity and intrinsic flattening of bulges were obtained from the length of the apparent major and minor semi-axes of the bulge, twist angle between the apparent major axis of the bulge and the galaxy line of nodes, and galaxy inclination. We found that the intrinsic shape is well constrained for a subsample of 115 bulges with favorable viewing angles. A large fraction of them is characterized by an elliptical section (B/A<0.9). This fraction is 33%, 55%, and 43% if using their maximum, mean, or median equatorial ellipticity, respectively. Most are flattened along their polar axis (C<(A+B)/2). The distribution of triaxiality is strongly bimodal. This bimodality is driven by bulges with Sersic index n>2, or equivalently, by the bulges of galaxies with a bulge-to-total ratio B/T>0.3. In particular, bulges with n\leq2 and with B/T\leq0.3 show a larger fraction of oblate axisymmetric (or nearly axisymmetric) bulges, a smaller fraction of triaxial bulges, and fewer prolate axisymmetric (or nearly axisymmetric) bulges with respect to bulges with n>2 and with B/T>0.3, respectively. According to predictions of the numerical simulations of bulge formation, bulges with n\leq2, which show a high fraction of oblate axisymmetric (or nearly axisymmetric) shapes and have B/T\leq0.3, could be the result of dissipational minor mergers. Both major dissipational and dissipationless mergers seem to be required to explain the variety of shapes found for bulges with n>2 and B/T>0.3.Comment: 16 pages, 12 figures; accepted for publication in A&
