    Synthèse et fonctionnalisation d'aldéhydes issus de la coupure d'esters gras insaturés

    La valorisation du carbone renouvelable joue un rôle croissant dans l'industrie chimique. Ces travaux rapportent l'utilisation d'huiles végétales comme matières premières en substitution de celles d'origine fossiles pour la synthèse de monomères bio-sourcés destinés à la production de polyesters ou de polyamides.La production du 9 oxononanoate de méthyle, comme molécule plateforme, à partir d'esters méthyliques d'huile de colza a été réalisée par coupure oxydante (ozonolyse) sans solvant à température ambiante, suivie d'une réduction des intermédiaires par hydrogénation catalytique sous pression de H2 et de Pd(5)/C. Ainsi, le rendement en aldéhyde-ester est de 92%. Ce procédé a été appliqué à la synthèse de molécules plateformes avec des longueurs de chaînes de 9 à 13 atomes de carbone. Une matière première renouvelable, des conditions de réaction douces, le recyclage du catalyseur et des co-produits non toxiques et valorisables ont permis de développer un procédé durable plus respectueux de l'environnement. La réduction de la fonction aldéhyde a été menée par hydrogénation catalytique, à 50C dans le méthanol, pour former l'alcool-ester correspondant. Le nickel de Raney ainsi que le Pd(5)/C offrent des rendements en 9-hydroxynonanoate de méthyle supérieurs à 90 %, mais le premier catalyseur conduit à un temps de réaction plus court. L'amination réductrice de la fonction aldéhyde a été menée avec succès à partir de NH3 gazeux et de Pd(5)/C, à 50C dans le méthanol, pour conduire majoritairement à la synthèse de l'amino-ester primaire. La quantité de NH3(g) influence la sélectivité et au moins trois équivalents sont nécessaires pour limiter la formation d'amino-ester.A high interest has been devoted to the use of renewable carbon in the chemical industry. The goal of this work is the substitution of fossil oils by vegetable oils to synthesize bio sourced monomers for polyesters and polyamides production. The synthesis of methyl 9-oxononanoate as a platform molecule from fatty acid methyl esters of rapeseed oil was carried out in solvent free ozonolysis at room temperature. Intermediary ozonides was reduced to aldehydes by catalytic hydrogenation under H2 pressure and in the presence of Pd(5)/C catalyst giving a 92% carbonyl yield. This process was applied to a wide range of unsaturated esters with a chain length within 9 and 13 carbon atoms. This process allows the selective conversion of renewable materials to value added chemicals, in mild conditions and in the presence of a recyclable catalyst. Moreover, the co products are non toxic and valuable. Hydrogenation of aldehyde group was performed in the presence of methanol at 50C under hydrogen pressure. Raney Nickel and Pd(5)/C exhibit a yield to methyl 9 hydroxynonanoate higher than 90%. Reductive amination of the aldehyde functional group was investigated with success in the presence of NH3(g) and Pd(5)/C at 50C in methanol to produce primary amino ester. The amount of ammoniac is directly correlated to the selectivity of the reaction.     Effects of aleurone supplementation on glucose-insulin metabolism and gut microbiome in untrained healthy horses

    Aleurone, a layer of the bran fraction, is deemed to be responsible for the positive health effects associated with the consumption of whole-grain products. Studies on rodents, pigs, and humans report beneficial effects of aleurone in five main areas: the reduction of oxidative stress, immunomodulatory effects, modulation of energy management, digestive health, and the storage of vitamins and minerals. Our study is the first aleurone supplementation study performed in horses. The aim of this study was to investigate the effect of an increase in the dose levels of aleurone on the postprandial glucose-insulin metabolism and the gut microbiome in untrained healthy horses. Seven adult Standardbred horses were supplemented with four different dose levels of aleurone (50, 100, 200, and 400 g/day for 1 week) by using a Latin square model with a 1-week wash out in between doses. On day 7 of each supplementation week, postprandial blood glucose-insulin was measured and fecal samples were collected. 16S ribosomal RNA (rRNA) gene sequencing was performed and QIIME2 software was used for microbiome analysis. Microbial community function was assessed by using the predictive metagenome analysis tool Phylogenetic Investigation of Communities by Reconstruction of Unobserved States (PICRUSt) and using the Metacyc database of metabolic pathways. The relative abundancies of a pathway were analyzed by using analysis of composition of microbiomes (ANCOM) in R. There was a significant dose-dependent increase in the postprandial time to peak of glucose (p = 0.030), a significant delay in the time to peak of insulin (p = 0.025), and a significant decrease in both the insulin peak level (p = 0.049) and insulin area under the curve (AUC) (p = 0.019) with increasing dose levels of aleurone, with a consideration of 200 g being the lowest significant dose. Alpha diversity and beta diversity of the fecal microbiome showed no significant changes. Aleurone significantly decreased the relative abundance of the genera Roseburia, Shuttleworthia, Anaerostipes, Faecalibacter, and Succinovibrionaceae. The most pronounced changes in the relative abundance at phyla level were seen in Firmicutes and Verrucomicrobia (downregulation) and Bacteroidetes and Spirochaetes (upregulation). The PICRUSt analysis shows that aleurone induces a downregulation of the degradation of L-glutamate and taurine and an upregulation of the three consecutive pathways of the phospholipid membrane synthesis of the Archaea domain. The results of this study suggest a multimodal effect of aleurone on glucose-insulin metabolism, which is most likely to be caused by its effect on feed texture and subsequent digestive processing; and a synergistic effect of individual aleurone components on the glucose-insulin metabolism and microbiome composition and function

    Patient perception of disease burden in diffuse cutaneous systemic sclerosis

    Purpose: Systemic sclerosis is a rare multi-organ autoimmune rheumatic disease, resulting in progressive fibrosis of the skin/internal organs. This study aimed to understand the impact of diffuse cutaneous systemic sclerosis symptoms and disease burden from the patient's perspective. Methods: This was a mixed methodology, market research study involving ethnography, structured interviews, video diaries, and patient tasks. Patients had been diagnosed with diffuse cutaneous systemic sclerosis for ⩾ 6 months and were recruited via healthcare professionals or patient associations (France, Italy, the United Kingdom, and the United States). Patients filmed short (~15 min) daily video diaries about their lives over 7 days and participated in ethnographic sessions, patient tasks, and structured video interviews. In Germany and Spain, patients participated in 60-min telephone interviews. Results: Twenty-three patients (mean age: 54 years; 83% women; minimum disease duration: 6 months) participated in the study. Time to diagnosis was prolonged, as patients overlooked their symptoms and some healthcare professionals attributed symptoms to other causes. Patients rarely received additional information or support services at diagnosis. Importantly, although patients were aware of the seriousness of organ involvement, they reported that skin changes, pain, and fatigue impaired their ability to perform routine tasks. Patients had a high prescription treatment burden (mean: 10 tablets/day; up to >25 tablets/day) with additional non-prescription medication taken for other comorbidities. Treatment discontinuation was common due to side effects. Patients experienced diffuse cutaneous systemic sclerosis as a loss of independence and self-esteem. Moreover, patients tended to have small support networks, and emotional support services were not offered as standard care. Conclusion: Patients with diffuse cutaneous systemic sclerosis had high treatment and disease burdens, with skin changes, pain, and fatigue profoundly affecting their lives. There is an unmet need for patient information at the time of diagnosis and emotional support services throughout the patient's journey with diffuse cutaneous systemic sclerosis. Based on the results of this study, we provide recommendations for improving diffuse cutaneous systemic sclerosis care

    Progressive skin fibrosis is associated with a decline in lung function and worse survival in patients with diffuse cutaneous systemic sclerosis in the European Scleroderma Trials and Research (EUSTAR) cohort

    OBJECTIVES To determine whether progressive skin fibrosis is associated with visceral organ progression and mortality during follow-up in patients with diffuse cutaneous systemic sclerosis (dcSSc). METHODS We evaluated patients from the European Scleroderma Trials and Research database with dcSSc, baseline modified Rodnan skin score (mRSS) ≥7, valid mRSS at 12±3 months after baseline and ≥1 annual follow-up visit. Progressive skin fibrosis was defined as an increase in mRSS >5 and ≥25% from baseline to 12±3 months. Outcomes were pulmonary, cardiovascular and renal progression, and all-cause death. Associations between skin progression and outcomes were evaluated by Kaplan-Meier survival analysis and multivariable Cox regression. RESULTS Of 1021 included patients, 78 (7.6%) had progressive skin fibrosis (skin progressors). Median follow-up was 3.4 years. Survival analyses indicated that skin progressors had a significantly higher probability of FVC decline ≥10% (53.6% vs 34.4%; p<0.001) and all-cause death (15.4% vs 7.3%; p=0.003) than non-progressors. These significant associations were also found in subgroup analyses of patients with either low baseline mRSS (≤22/51) or short disease duration (≤15 months). In multivariable analyses, skin progression within 1 year was independently associated with FVC decline ≥10% (HR 1.79, 95% CI 1.20 to 2.65) and all-cause death (HR 2.58, 95% CI 1.31 to 5.09). CONCLUSIONS Progressive skin fibrosis within 1 year is associated with decline in lung function and worse survival in dcSSc during follow-up. These results confirm mRSS as a surrogate marker in dcSSc, which will be helpful for cohort enrichment in future trials and risk stratification in clinical practice

    Does eHealth Literacy Impact Patients' Opinion on the EHR?

    An electronic health record (EHR) will be established nationwide in Switzerland in 2020. Patients can decide on their own whether they open an EHR. It is still unclear what might influence the patient decision. For this reason, we performed an online survey among the Swiss population to study whether there is a lack of knowledge on the EHR which impacts the willingness to open a personal health record and agree to health data sharing. A questionnaire with 13 question was distributed in a period of 4 weeks. More than 1200 participants replied to the survey. The results were analyzed with statistical methods. There are correlations between some of the questions in our survey. We conclude that the willingness to open a personal health record directly depends on the trust into the enabling technology

    White light and multicolor emission tuning in triply doped Yb3+/Tm3+/Er3+ novel fluoro-phosphate transparent glass-ceramics

    New Yb3+, Er3+ and Tm3+ doped fluoro-phosphate glasses belonging to the system NaPO3-YF3-BaF2-CaF2 and containing up to 10 wt% of rare-earth ion fluorides were prepared and characterized by differential scanning calorimetry, absorption spectroscopy and up-conversion emission spectroscopy under excitation with a 975 nm laser diode. Transparent and homogeneous glass-ceramics have been reproducibly obtained with a view to manage the red, green and blue emission bands and generate white light. X-ray diffraction as well as electron microscopy techniques have confirmed the formation of fluorite-type cubic nanocrystals at the beginning of the crystallization process while complex nanocrystalline phases are formed after a longer heat-treatment. The prepared glass-ceramics exhibit high optical transparency even after 170 h of thermal treatment. An improvement of up-conversion emission intensity - from 10 to 160 times larger - was measured in the glass-ceramics when compared to the parent glass, suggesting an important incorporation of the rare-earth ions into the crystalline phase(s). The involved mechanisms and lifetime were described in detail as a function of heat-treatment time.     Set up of a methodology for participatory plant breeding in bread wheat in France

    In Organic Agriculture, cultivation environments and agronomic practices are very diverse. This diversity can be handled with decentralized selection based on the knowledge of farmers and scientists. A collaborative work between associations from Réseau Semences Paysannes and the DEAP team from INRA du Moulon set up an innovative breeding approach on farm based on decentralization and participation of farmers. This approach makes it possible to (i) create new population varieties of bread wheat locally adapted (genetic innovation) (ii) set up an organizational scheme based on decentralization and co construction between actors (societal innovation) and (iii) develop experimental designs, create statistical and data management tools which stimulate these genetic and societal innovations

    Association of kidney function with effectiveness of procalcitonin-guided antibiotic treatment:A patient-level meta-analysis from randomized controlled trials

    Patients with impaired kidney function have a significantly slower decrease of procalcitonin (PCT) levels during infection. Our aim was to study PCT-guided antibiotic stewardship and clinical outcomes in patients with impairments of kidney function as assessed by creatinine levels measured upon hospital admission. We pooled and analyzed individual data from 15 randomized controlled trials who were randomly assigned to receive antibiotic therapy based on a PCT-algorithms or based on standard of care. We stratified patients on the initial glomerular filtration rate (GFR, ml/min/1.73 m2) in three groups (GFR >90 [chronic kidney disease; CKD 1], GFR 15-89 [CKD 2-4] and GFR0.05). This individual patient data meta-analysis confirms that the use of PCT in patients with impaired kidney function, as assessed by admission creatinine levels, is associated with shorter antibiotic courses and lower mortality rates
