Disseminated mycosis in a patient with yellow fever

    Get PDF
Disseminated mycosis (DM)—with cardiac involvement and shock—is an unexpected and severe opportunistic infection in patients with yellow fever. DM can mimic bacterial sepsis and should be considered in the differential diagnosis of causes of systemic inflammatory response syndrome in this group of patients, especially in areas where an outbreak of yellow fever is ongoing. We report the case of a 53-year-old male patient who presented to the emergency department with fever, myalgia, headache, and low back pain. The laboratory investigation revealed a positive molecular test for yellow fever, hepatic injury, and renal failure. During hospitalization, the patient developed hepatic encephalopathy, ascending leukocytosis, and ascites, with signs consistent with peritonitis. On the 11th day of hospitalization, the patient developed atrioventricular block, shock and died. At autopsy, angioinvasive mycosis was evidenced mainly in the heart, lungs, kidneys, and adrenals

The occurrence of metachronous adrenocortical carcinoma has rarely been described. We report a case of a child with virilizing adrenocortical metachronous tumors that, despite several metastases, presented long-term survival (15 years). We analyzed in this tumor IGF2, IGF1R and FGFR4 gene expression, and evaluated the presence of p.R337H germline p53 mutation and somatic CTNNB1 mutation. IGF2 gene was over-expressed in both left (Weiss score 5) and right (Weiss 7) adrenocortical tumors. IGF1R expression levels were higher in the right adrenocortical tumor. FGFR4 over-expression was also detected in the right adrenocortical tumor. In addition, this patient harbors the germline p.R337H p53 mutation and loss of heterozygosity (LOH) was detected in the tumors. No somatic CTNNB1 mutations were found in both tumors. In conclusion, we demonstrated in this unusual case the over-expression of growth signaling pathways, which are molecular mechanisms previously related to adrenocortical tumorigenesis. Furthermore, the absence of somatic CTNNB1 mutations, which is a molecular marker of poor prognosis in adults, might be related to the long-term survival of this patient.

We measured visual performance in achromatic and chromatic spatial tasks of mercury-exposed subjects and compared the results with norms obtained from healthy individuals of similar age. Data were obtained for a group of 28 mercury-exposed subjects, comprising 20 Amazonian gold miners, 2 inhabitants of Amazonian riverside communities, and 6 laboratory technicians, who asked for medical care. Statistical norms were generated by testing healthy control subjects divided into three age groups. The performance of a substantial proportion of the mercury-exposed subjects was below the norms in all of these tasks. Eleven of 20 subjects (55%) performed below the norms in the achromatic contrast sensitivity task. The mercury-exposed subjects also had lower red-green contrast sensitivity deficits at all tested spatial frequencies (9/11 subjects; 81%). Three gold miners and 1 riverine (4/19 subjects, 21%) performed worse than normal subjects making more mistakes in the color arrangement test. Five of 10 subjects tested (50%), comprising 2 gold miners, 2 technicians, and 1 riverine, performed worse than normal in the color discrimination test, having areas of one or more MacAdam ellipse larger than normal subjects and high color discrimination thresholds at least in one color locus. These data indicate that psychophysical assessment can be used to quantify the degree of visual impairment of mercury-exposed subjects. They also suggest that some spatial tests such as the measurement of red-green chromatic contrast are sufficiently sensitive to detect visual dysfunction caused by mercury toxicity

Soybeans sowing in different plants’ spatial distribution can influence the phytosanitary management of this crop and, consequently, impact on grains yield. This study was carried out to evaluate the effect of plants arrangements on infestation and control of caterpillars, the deposition of spray syrup as well as assess the agronomic performance of soybean cultivated in the Brazilian Cerrado. The assay was performed during two consecutive seasons in a randomized complete block design with four replications. Soybean cultivation was implemented in 0.50 m spacing between rows, crossed (0.50 m x 0.50 m), twin rows (0.25 m / 0.75 m), and narrow (0.25 m). In the reproductive stage of plants, both crossed and narrow arrangements showed higher caterpillars’ incidence. There was a more evident risk of caterpillar incidence in arrangements that promoted better equidistance among plants. This risk was mitigated when taking into account both control and overlap of syrup, which could be incremented into inferior canopy with the enhancement of application rate. The increase in application rate from 75 to 150 L ha-1 promoted superior spray deposition volumes. Increases in grain yield was noted in the narrow arrangement

A estratificação do Estado do Paraná em regiões não trouxe vantagens, nos anos analisados, para os testes de linhagens; apenas os locais da região Sul mostraram algum grau de similaridade entre si.The objectives of this research were: to compare productivity and adaptation of conventional and glyphosate-resistant genetically engineered soybean genotypes, from different maturity groups, developed by the breeding program at Embrapa Soja for Paraná State, Brazil; to study the relative importance of the effects of location, year, cultivar and their respective interactions; and to verify the possibility of stratifying the State in more homogeneous regions, in order to reduce the number of locations for line competition experiments. Paraná State yield data from regional experiments carried out in randomized complete block design from 2001 to 2005 were used. The possibility of stratifying the State in homogeneous regions and discarding similar locations was checked by the significance of genotype x environment interaction among locations. Yield was not significantly different between genetically engineered and conventional soybean, regardless of the maturity group. The effect of location was more important than the effect of year in the environment composition. The stratification of Paraná State in regions for yield testing soybean lines brought neither significant nor consistent advantages for the evaluated years. Only locations within the South region showed some degree of similarity

Capitalizing on opportunities provided by pasture sudden death to enhance livestock sustainable management in Brazilian Amazonia

Brazil has the largest commercial beef cattle stock on Earth, and most of the cattle produced in the country is bred and finished on pastures. The cattle ranching sector represents a significant source of the country's greenhouse gas (GHG) emissions. Agricultural intensification has been highlighted as one of the main strategies in reaching global food security and reducing deforestation. The Sudden Death Disease (SDD) of pastures, which affects the most planted cultivar of Urochloa brizantha, is degrading pastures in the Amazon, contributing to low production yields and high emission rates. This paper discusses the intensification of pasture production systems and SDD, to examine the potential for pasture renovation to address livestock productivity and GHG balance, emissions and potential sinks. Does SDD represent a blessing or a curse to climate change mitigation in the Brazilian Amazon? A collection of pasture samples were assessed to measure wet and dry weight in areas with and without SDD, which were related to remote sensing data to provide an overall estimate of the total area affected by the SDD in Alta Floresta, a municipal county of southern Brazilian Amazonia. We found that 77.1% of all pastures had been committed to the syndrome, which has forced farmers to renew their pastures. This also has great potential in increasing soil carbon stocks, effectively reducing the CO2 footprint of meat production in those areas. Therefore, we firmly believe that SDD management has provided an opportunity to rebalance the emissions/sequestration equation associated with meat production by the cattle ranching sector in this Amazonin frontier

Background. Phrynops geoffroanus is a small turtle that inhabits lakes, rivers, and streams throughout South America. The body fat of this animal is used as a folk medicine in Brazil for treating illnesses such as sore throats, ear aches, mumps, rheumatism, and arthritis. The present study evaluated the antimicrobial activity of oil extracted from Phrynops geoffroanus (OPG), determined its chemical composition, and discussed the implications of its use in traditional medicine. The OPG was obtained from the ventral region of this turtle using hexane as a solvent. The antimicrobial activity of OPG was tested against standard and multiresistance strains of bacteria and fungi and its composition was determined indirectly by analyzing the methyl esters of the component fatty acids. The OPG presented a clinically relevant antifungal activity against Candida krusei ATCC 6258 (MIC 128 µg/mL). When the OPG was associated with the antibacterial and antifungal drugs, was observed a synergistic effect when associated the OPG with the gentamicin against the strain Pseudomonas aeruginosa 22. Our results indicated that OPG has clinically relevant antifungal activity against C. krusei, and demonstrated synergetic antibacterial activity in combination with commercial antibiotics against Pseudomonas aeruginosa
