    Effect of combined use of L carnitine and hemodialysis on clinical efficacy and quality of life of uremic patients

    Purpose: To investigate the clinical efficacy of combined use of L-carnitine and hemodialysis in the treatment of uremic patients, and its effect on their quality of life. Methods: A total of 160 uremic patients who were admitted to Lujiang County People's Hospital from November 2018 to August 2020 were selected and divided equally into dialysis group (hemodialysis), and combined group (L-carnitine + hemodialysis). Some clinical indices and parameters, including safety profile, National Institutes of Health Stroke Scale (NIHSS) score, CD4+ and CD4+/CD8+, and plasma protein levels were evaluated for both study groups. Results: Patients treated with L-carnitine + hemodialysis in the combined group resulted in significantly higher clinical treatment effectiveness than those in the dialysis group (p < 0.05). However, safety profiles were comparable in the two groups (p > 0.05). No significant difference in NIHSS score between the two groups before treatment, but L-carnitine + hemodialysis led to a better NIHSS score than in the dialysis group after treatment (p < 0.05). However, there were better levels of CD4+ and CD4+/CD8+ in the combined group than in the dialysis group (p < 0.05). Similarly, although pre-treatment plasma protein levels in the two groups were comparable, there were significantly lower plasma protein levels in the combined group than in the dialysis group post-treatment (p < 0.05). Conclusion: The combination of L-carnitine and hemodialysis in the treatment of uremia patients improves the clinical management of the patients, enhances their quality of life, and shows good safety profile. However, further clinical trials should be carried out prior to the application of the combined treatment in clinical practice

    The discharge muffler of the rotary compressor is used to reduce the discharge noise produced by pressure pulsations. In order to obtain the transmission loss of discharge muffler, four microphone experimental devices used to measure the transmission loss of the muffler is established. The device is based on the transfer function method on the assumption that there is only plane wave in the tube, thus the measurement range of the device is discussed at the same time. Then the transmission loss of a contraction chamber is measured and calculated theoretically and the validity of the experimental device is verified through comparing the measurement and theoretical result. At last, a few various mufflers are measured using the experimental device and simulated by finite element method (FEM). The transmission loss of mufflers in the air is obtained and the experimental and simulation results are verified by each other

    With the popularization of the inverter-driven air-conditioner and the requirement of comfort and energy saving, the operating frequency of inverter-driven air-conditioner is lower and lower, even to 1Hz. But when the air-conditioner operates under the frequency of 10Hz, a periodic sound will appear in the air-conditioner outdoor unit, which should be researched and solved. According to the experiment test and theoretical analysis, it is found that the periodic sound is the excited resonance when the valve opens. The resonator is composed by the discharge port of top-flange and discharge cavity which is made up by cylinder, roller, top-flange and bottom-flange. In one cycle the characteristic of the periodic sound is that the sound will be only taken place when the valve opens and the frequency of the sound will be higher and higher with the crank angle increasing. According to the experimental testing about different type compressor and air-conditioner outdoor unit, it is found that this sound exists in every kinds of rolling piston compressor. Using proper muffler and increasing the thickness of the valve could both reduce this periodic sound

    Neuropsychiatric Lupus Erythematosus: Future Directions and Challenges; a Systematic Review and Survey

    This study aimed to systematically review neuropsychiatric lupus erythematosus (NPSLE) and establish a simplified diagnostic criterion for NPSLE. Publications from 1994 to 2018 in the database (Wanfang data (http://www.wanfangdata.com.cn/index.html) and China National Knowledge Internet (http://www.cnki.net)) were included. In total, 284 original case reports and 24 unpublished cases were collected, and clinical parameters were analyzed. An attempt was made to develop a set of simplified diagnostic criteria for NPSLE based on cases described in the survey and literature; moreover, and pathophysiology and management guidelines were studied. The incidence rate of NPSLE was estimated to be 12.4% of SLE patients in China. A total of 408 NPSLE patients had 652 NP events, of which 91.2% affected the central nervous system and 8.8% affected the peripheral nervous system. Five signs (manifestations, disease activity, antibodies, thrombosis, and skin lesions) showed that negative and positive predictive values were more than 70%, included in the diagnostic criteria. The specificity, accuracy, and positive predictive value (PPV) of the revised diagnostic criteria were significantly higher than those of the American College of Rheumatology (ACR) criteria (w2=13.642, 15.591, 65.010, po0.001). The area under the curve (AUC) for revised diagnostic criteria was 0.962 (standard error=0.015, 95% confidence intervals [CI] =0.933–0.990), while the AUC for the ACR criteria was 0.900 (standard error=0.024, 95% CI=0.853–0.946). The AUC for the revised diagnostic criteria was different from that for the ACR criteria (Z=2.19, po0.05). Understanding the pathophysiologic mechanisms leading to NPSLE is essential for the evaluation and design of effective interventions. The set of diagnostic criteria proposed here represents a simplified, reliable, and costeffective approach used to diagnose NPSLE. The revised diagnostic criteria may improve the accuracy rate for diagnosing NPSLE compared to the ACR criteria
