    Obstructive sleep apnea syndrome has been linked to cardiovascular complications. The diagnosis of effects of sleep apnea on heart may be difficult before apparent examination findings. The aim of the present study was to evaluate changes of myocardial contractile properties in patients affected by sleep apnea showing tissue Doppler imaging, in the face of quite normal myocardial contractility on standard echocardiography.     Do calcium sensitizers affect right ventricular functions in patients with chronic heart failure?

    Calcium sensitizers also improve cardiac function by increasing the contraction of the myocardium without significantly increasing intracellular calcium levels. Although right ventricular function is an important role for better cardiac global function, there is no study about effects of levosimendan on right ventricular function measured by tissue Doppler imaging. The aim of the present study was to evaluate changes of myocardial properties in patients with idiopathic dilated cardiomyopathy using tissue Doppler imaging after levosimendan infusion. This tissue Doppler study shows that levosimendan also affects myocardial especially systolic waves of right ventricle and those of left ventricle. (C) 2006 Elsevier Ireland Ltd. All rights reserved

    Neutrophilic pleocytosis in cerebrospinal fluid: Adult-onset Still's disease

    We describe a unique patient whose clinical and laboratory findings fulfill diagnostic criteria of adult onset Still's disease and at the same time, this case was complicated by aseptic meningitis with neutrophilic pleocytosis in cerebrospinal fluid, as well as sensorineural hearing loss. The symptoms of the patient improved greatly with prednisolone therapy. Some studies in the literature suggest that this disease may lead to aseptic meningitis with neutrophilic pleocytosis

    The prevalence of microalbuminuria and relevant cardiovascular risk factors in Turkish hypertensive patients

    PubMed ID: 22257801Objectives: A growing body of data illustrates the importance of microalbuminuria (MAU) as a strong predictor of cardiovascular risk in the hypertensive population. The present study was designed to define the prevalence of MAU and associated cardiovascular risk factors among Turkish hypertensive outpatients. Study design: Representing the Turkish arm of the multinational i-SEARCH study involving 1,750 sites in 26 countries around the world, a total of 1,926 hypertensive patients from different centers were included in this observational and cross-sectional survey study. Patients with reasons for a false-positive MAU test were excluded. The prevalence of MAU was assessed using a dipstick test, and patients were inquired about comorbidities, comedication, and known cardiovascular risk factors. Results: The overall prevalence of MAU was 64.7% and there was no difference between genders. Most of the patients (82.5%) had uncontrolled hypertension, 35.6% had dyslipidemia, and 35.5% had diabetes, predominantly type 2. Almost one-third of the patients (26.4%) had at least one cardiovascular-related comorbidity, with 20.3% having documented coronary artery disease (CAD). Almost all patients (96.8%) had one or more risk factors for cardiovascular disease in addition to hypertension, including family history of myocardial infarction or CAD, diabetes, dyslipidemia, lack of physical exercise, and smoking. A trend towards higher MAU values in the presence of CAD was determined. Conclusion: Microalbuminuria tests should be routinely used as a screening and monitoring tool for the assessment of subsequent cardiovascular morbidity and mortality among hypertensive patients. © 2011 Turkish Society of Cardiology

    Education professionals are concerned about the dropping levels of motivation and engagement students experience as they go through schooling. Sustained low motivation across the student population leads to increased dropout rates, which in turn leads to many detrimental consequences. The literature shows clear correlations between low self-efficacy and low motivation, alongside a lack of tools to address the problem. This thesis tested a student-centered intervention composed of a goal setting instrument, an expressive writing instrument, and several coping mechanisms. Self-efficacy was tested in eight 16-year-old Finnish students before and after the intervention using a validated self-efficacy scale. A thematic analysis was conducted to assess the importance and effects of each of the components of the intervention, and to account for external variables. All the participants showed an increase in self-efficacy after the intervention, which was attributed to be the main cause. The goal setting instrument was reported to have the strongest effect on self-efficacy, followed by the expressive writing instrument. The coping mechanisms showed null effects. The results suggest that the goal setting instrument causes new appraisals of the most influential source of self-efficacy: performance attainments. Additionally, expressive writing serves as a foundational previous step to goal setting. However, further research is required to confirm the validity of these findings. The small sample size, the strong localization, and the fact that self-efficacy is not directly observable, severely limit the results of the study. In addition, a longer study should be conducted to observe the durability of the effects of the intervention
