    Systemic enzyme therapy in chronic venous disease: a review

    Chronic venous disease (CVD), a sequel of venous insufficiency, has great medical and socioeconomic impact. Varicose veins and venous ulcer are amongst its commonest manifestations. In CVD, incompetent valves, weakened vascular walls, venous hypertension and increased permeability of venous walls lead to the release of proinflammatory mediators like tumor necrosis factor (TNF)-α, interleukin (IL)-1β, reactive oxygen species (R.O.S.), and reactive nitrogen species (R.N.S.) in the venous milieu. Pharmacotherapy with nonsteroidal anti-inflammatory drugs (NSAIDs) is often used to relieve pain caused by venous disease. However, there is a need for therapies that target the microcirculatory disorders and act on chronic inflammatory processes. Systemic enzyme therapy (SET), with orally administered combination of proteolytic enzymes- trypsin, bromelain, and flavonoid rutoside, has been used since decades for their anti-inflammatory, analgesic, anti-edematous, antithrombotic and antioxidant properties. This review discusses the various relevant pharmacodynamic properties demonstrated by the ingredients, followed by clinical studies of SET, which have demonstrated benefit in both subjective and objective parameters. These studies indicate that SET has good efficacy, tolerability and holds great promise to improve the quality of life of a patient with CVD.  

    Results and Outcome Reporting In ClinicalTrials.gov, What Makes it Happen?

    At the end of the past century there were multiple concerns regarding lack of transparency in the conduct of clinical trials as well as some ethical and scientific issues affecting the trials' design and reporting. In 2000 ClinicalTrials.gov data repository was developed and deployed to serve public and scientific communities with valid data on clinical trials. Later in order to increase deposited data completeness and transparency of medical research a set of restrains had been imposed making the results deposition compulsory for multiple cases.We investigated efficiency of the results deposition and outcome reporting as well as what factors make positive impact on providing information of interest and what makes it more difficult, whether efficiency depends on what kind of institution was a trial sponsor. Data from the ClinicalTrials.gov repository has been classified based on what kind of institution a trial sponsor was. The odds ratio was calculated for results and outcome reporting by different sponsors' class.As of 01/01/2012 118,602 clinical trials data deposits were made to the depository. They came from 9068 different sources. 35344 (29.8%) of them are assigned as FDA regulated and 25151 (21.2%) as Section 801 controlled substances. Despite multiple regulatory requirements, only about 35% of trials had clinical study results deposited, the maximum 55.56% of trials with the results, was observed for trials completed in 2008.The most positive impact on depositing results, the imposed restrains made for hospitals and clinics. Health care companies showed much higher efficiency than other investigated classes both in higher fraction of trials with results and in providing at least one outcome for their trials. They also more often than others deposit results when it is not strictly required, particularly, in the case of non-interventional studies

    Biomass equations for forest regrowth in the eastern Amazon using randomized branch sampling.

    Florestas secundárias ocupam uma área extensa e crescente na bacia Amazônica, porém determinações acuradas do impacto dessas florestas nos ciclos de carbono e nutrientes têm sido dificultadas pelo número reduzido de equações alométricas. Neste estudo, nós desenvolvemos equações em nível de comunidade e espécies individuais para estimar a biomassa total da parte aérea de uma floresta secundária com 15 anos de idade na Amazônia oriental. O trabalho de campo utilizou amostragem aleatória de ramos, que é uma técnica rápida, porém pouco utilizada em florestas tropicais. Baseada no erro padrão da série de segmentos individuais (14%), a consistência da série de segmentos totais amostrados foi considerada elevada, sugerindo que o método pode ser eficiente em comparação com procedimentos tradicionais. Os melhores ajustes foram obtidos com a equação tradicional Y=a×DBHb, onde Y é a biomassa, DBH é o diâmetro à altura do peito, e a e b são parâmetros para cada espécie arbórea. Ajustes razoáveis também foram alcançados com equações da forma Y=a(BA×H), onde Y é a biomassa, BA é a área basal, H é a altura e a é um parâmetro específico para cada espécie arbórea. Comparações com equações disponíveis na literatura indicaram uma faixa de erro provável de -33% a +29% usando-se relações desenvolvidas para outros sítios.     Opinions on registering trial details: a survey of academic researchers

    <p>Abstract</p> <p>Background</p> <p>The World Health Organization (WHO) has established a set of items related to study design and administrative information that should build the minimum set of data in a study register. A more comprehensive data set for registration is currently developed by the Ottawa Group. Since nothing is known about the attitudes of academic researchers towards prospective study registration, we surveyed academic researchers about their opinion regarding the registration of study details proposed by the WHO and the Ottawa Group.</p> <p>Methods</p> <p>This was a web-based survey of academic researchers currently running an investigator-initiated clinical study which is registered with clinicaltrials.gov. In July 2006 we contacted 1299 principal investigators of clinical studies by e-mail explaining the purpose of the survey and a link to access a 52-item questionnaire based on the proposed minimum data set by the Ottawa Group. Two reminder e-mails were sent each two weeks apart. Association between willingness to disclose study details and study phase was assessed using the chi-squared test for trend. To explore the potential influence of non-response bias we used logistic regression to assess associations between factors associated with non-response and the willingness to register study details.</p> <p>Results</p> <p>Overall response was low as only 282/1299 (22%) principal investigators participated in the survey. Disclosing study documents, in particular the study protocol and financial agreements, was found to be most problematic with only 31% of respondents willing to disclose these publicly. Consequently, only 34/282 (12%) agreed to disclose all details proposed by the Ottawa Group. Logistic regression indicated no association between characteristics of non-responders and willingness to disclose details.</p> <p>Conclusion</p> <p>Principal investigators of non-industry sponsored studies are reluctant to disclose all data items proposed by the Ottawa Group. Disclosing the study protocol and financial agreements was found to be most problematic. Future discussions on trial registration should not only focus on industry but also on academic researchers.</p

    Correlation but no causation between leaf nitrogen and maximum assimilation: the role of drought and reproduction in gas exchange in an understory tropical plant Miconia ciliata (Melastomataceae).

    Alternative hypotheses were tested to explain a previously reported anomaly in the response of leaf photosynthetic capacity at light saturation (A..) in Miconia ciliata to dry-season irrigation. The anomaly is characterized by an abrupt increase in leaf A._ for nonirrigated plants at the onset of the rainy season to values that significantly exceeded corresponding measurements for plants that were irrigated during the previous dry season. Hypothesis I posits that a pulse in leaf nitrogen increases CO2 assimilation in nonirrigated plants at the onset of the wet season and is dampened for irrigated plants; this hypothesis was rejected because, although a wet-season nitrogen pulse did occur, it was identical for both irrigated and nonirrigated plants and was preceded by the increase in assimilation by nonirrigated plants. Hypothesis 2 posits that a reproduction-related, compensatory photosynthetic response occurs in nonirrigated plants following the onset of the wet season and is dampened in irrigated plants; consistent with hypothesis 2, high maximum assimilation rates for control plants in the wet season were significantly correlated with fruiting and flowering, whereas irrigation caused flowering and fruiting in the dry season, spreading M. ciliata reproductive activity in irrigated plants across the entire year

    Automated Bangla sign language translation system for alphabets by means of MobileNet

    Individuals with hearing and speaking impairment communicate using sign language. The movement of hand, body and expressions of face are the means by which the people, who are unable to hear and speak, can communicate. Bangla sign alphabets are formed with one or two hand movements. There are some features which differentiates the signs. To detect and recognize the signs, analyzing its shape and comparing its features is necessary. This paper aims to propose a model and build a computer systemthat can recognize Bangla Sign Lanugage alphabets and translate them to corresponding Bangla letters by means of deep convolutional neural network (CNN). CNN has been introduced in this model in form of a pre-trained model called “MobileNet” which produced an average accuracy of 95.71% in recognizing 36 Bangla Sign Language alphabets
