    META-ANALYSIS OF MEDICAL AND NON-MEDICAL TREATMENTS OF THE PRODROMAL PHASE OF PSYCHOTIC ILLNESS IN AT-RISK MENTAL STATES

    Introduction: There are now many existing studies which assess the treatments available for ‘at risk mental states’, as patients who are believed to be in the prodromal phase of psychotic illness are referred to. However, concerns regarding side effects of possible treatments remain. We here conduct a meta-analysis of the studies available up to October 2010. The aim of this study is to decide what would be the best treatment for ‘at high risk patients’. Results: All the available studies examining potential treatments during the prodromal phase of psychotic illness were collected. They all showed comparable efficacy, which reached statistical significance, excluding the one study using olanzapine, which in fact ‘tended towards significance’. Discussion: Treatments appear promising but a balance needs to be kept between adverse events and effectiveness of preventing psychosis. Conclusion: It is necessary to search further for treatments in order to identify effective treatments with fewer adverse side effects in this phase of psychotic illness

    Adaptive Critic Design in Learning to Play Game of Go

    This paper examines the performance of an HDP-type adaptive critic design (ACD) of the game Go. The game Go is an ideal problem domain for exploring machine learning; it has simple rules but requires complex strategies to play well. All current commercial Go programs are knowledge based implementations; they utilize input feature and pattern matching along with minimax type search techniques. But the extremely high branching factor puts a limit on their capabilities, and they are very weak compared to the relative strengths of other game programs like chess. In this paper, the Go-playing ACD consists of a critic network and an action network. The HDP type critic network learns to predict the cumulative utility function of the current board position from training games, and, the action network chooses a next move which maximizes critics next step cost-to-go. After about 6000 different training games against a public domain program, WALLY, the network (playing WHITE) began to win in some of the games and showed slow but steady improvements on test game

    A silver(I) triflate-catalyzed protocol for the post-Ugi synthesis of tetracyclic spiroindolines has been developed. The protocol worked best for indole-3-carbaldehyde-derived Ugi adducts obtained using anilines and 3-aryl propiolic acids. Thus, it is complementary to the previous cationic gold-catalyzed procedure that was developed for analogues Ugi substrates derived from aliphatic amines and 3-alkyl propiolic acids. Furthermore, we have demonstrated that under our new settings this domino Friedel-Crafts ipso cyclization / imine trapping process could be efficiently combined with the preceding four-component Ugi reaction into a two-step one-pot transformation

    Efficacy of fentanyl transdermal patch in the treatment of chronic soft tissue cancer pain

    Background: Cancer pain may be a major problem for health care providers worldwide. According to different studies reporting the pain severity, one-third of patients reported to have moderate to severe pain. Management of cancer pain is one of the most important goals of palliative care. Recently, different research results on the efficacy of opioid analgesics in chronic pain management have played a role to implement standards in pain control by government agencies worldwide. Objectives: This study aimed to investigate the efficacy of fentanyl transdermal patch in the treatment of chronic soft tissue cancer pain. Patients and Methods: In a prospective descriptive study, we evaluated 86 patients with soft tissue tumors with chronic pain referred to cancer institute of Imam Khomeini Hospital, Tehran, Iran, during 2006-2007. For all patients, transdermal fentanyl patch (25 μg/h) was administered. The appearance of patches was the same. Pain severity was measured by Visual Analogue Scale (VAS) initially and 24, 48 and 72 hours after the initiation of treatment. Results: Patients' characteristics and VAS score before the treatment were not significantly different (P > 0.05). According to our findings, the pain severity was significantly reduced after the treatment (P = 0.001). The incidence of adverse events in patients was significantly high (72). The most common adverse events were sleepiness, nausea and vomiting in 30.2 and 18.6, respectively. Conclusions: Transdermal fentanyl patch was an effective and safe method to reduce pain in patients with soft tissue tumors. Moreover, it could improve the quality of life in these patients, but adverse events occurred in approximately 72 of patients. © 2015, Iranian Society of Regional Anesthesia and Pain Medicine (ISRAPM)

    Prevalence and Antibacterial Susceptibility in Mastitis in Buffalo and Cattle in District Lahore-Pakistan

    A total of 450 milk samples including both 272 buffalo and 178 cattle were randomly collected in and around District Lahore to study the incidence of mastitis and antibiotic sensitivity by performing Culture and Sensitivity test. The prevalence of mastitis in buffalo was found 20.98% while in cattle 24.71%. The prevalence of both Clinical and subclincal mastitis in buffalo were 40.35%, 59.64% and in cattle 61.26%, 30.63% respectively. The milk samples mixed with bothmucus and blood in buffalo and cattle were 5.51% and 4.49% respectively. Quarters wise prevalence was 47.72%, 11.36%, 36.36% and 4.54% in the left fore, left hind, right fore and right hind quarters in cattle while In buffaloes, the prevalence was 0%, 68.96%, 11.49% and 19.54% in the left fore, left hind, right fore and right hind quarters respectively. The Ciprofloxacin antibiotic was found highly Sensitive in buffalo while Gentamicin in cattle

    The semi-arid tropics (SAT) include parts of 48 countries in the developing world: in most of India, locations in south east Asia, a swathe across sub-Saharan Africa, much of southern and eastern Africa, and a few locations in Latin America (Fig 1). Semi-arid tropical regions are characterized by unpredictable weather, long dry seasons, inconsistent rainfall, and soils that are poor in nutrients. Sorghum, millet, cowpea, chickpea, pigeonpea and groundnut are the vital crops that feed the poor people living in the SAT. Environmental stresses represent the most limiting factors for agricultural productivity. Apart from biotic stresses caused by plant pathogens, there are a number of abiotic stresses such as extremes temperatures, drought, salinity and radiation which all have detrimental effects on plant growth and yield, especially when several occur together (Mittler 2006)
