The current diagnosis of superficial bladder cancer must be reconsidered
The high recurrence and progression rates in superficial bladder cancer are partially related to the deficiencies of the standard conventional diagnostic modalities. Therefore, innovative noninvasive and invasive detection devices have been studied during the last decade. New diagnostic urine markers are under intensive investigation in order to exclude the presence of urothelial cancer, but the value of all these tests is still insufficiently validated in diagnosis and follow-up. With the introduction of 5-amino-levulinic acid fluorescence endoscopy, the efficacy of the detection device has been significantly improved. Flat lesions such as carcinoma in situ can be completely detected besides exophytic tumors. This is of particular importance because the fate of the patient depends to an important extent on these tumor entities. Transurethral resection and surveillance of bladder cancer supported by 5-aminolevulinic acid-induced fluorescence endoscopy
Purpose: We determined whether neoplastic disease, which was missed under white light can be found during transurethral resection of bladder cancer by 5-aminolevulinic acid-induced porphyrin fluorescence. Materials and Methods: 5-Aminolevulinic acid-induced fluorescence endoscopy was carried out in 328 cases. A 3% 5-aminolevulinic acid solution was instilled intravesically in a mean time of 2.8 h before endoscopy, The fluorescence was excited by a special incoherent light source which provided blue light in addition to white light. Results: In 82 (25%) cases additional neoplastic lesions were found only because of their red porphyrin fluorescence which was induced by 5-aminolevulinic acid. 31% of these neoplastic foci which were found in normal and nonspecific inflamed mucosa had a poorly differentiated histology. Conclusions: 5-Aminolevulinjc acid facilitates detection of neoplastic disease during transurethral resection of bladder cancer and increases the accuracy of diagnosis
Simultaneous microsurgical spermatic vein ligation and sclerotherapy - A combined procedure for the treatment of recurrent or persistent varicocele
Objectives: Microsurgical ligation as well as antegrade sclerotherapy have been established in varicocele treatment. The aim of this study was to evaluate whether a combination of microsurgery. and sclerotherapy can: provide a safe and effective treatment of varicocele recurrence or persistence. Methods. Nine patients with, recurrent or persistent varicoceles were operated by means of the combination method. Under microscopic control varix veins were ligated selectively preserving: lymphatics and arteries. Ectopic veins as a possible source for varicocele persistence or recurrence were also ligated. Finally, an intraoperative venography with subsequent sclerotherapy was, performed through one of the dissected veins. Results. Despite: difficult anatomical situations after previous surgical interventions, the operations were perform, ed successfully without any complications. Clinical controls showed varicocele disappearance without damage of the testis. No varicocele recurrence or persistence was observed. Conclusions. This method combines the advantages of both methods. Precision of the microsurgical technique is combined with velocity of sclerotherapy. Thus, it may represent an Interesting alternative to conventional operation methods especially in the treatment of recurrent or persistent varicoceles. Copyright (C) 2001 S. Karger AG, Basel
Urinary tract infection in renal transplant recipients
Introduction: Urinary tract infection (UTI) is the commonest bacterial infection occurring in renal transplant recipients, and it is associated with significant morbidity. This study aimed to assess the characteristics of all UTI episodes diagnosed in renal transplant patients who attended regularly for follow up in the nephrology department of National Heart Center, Tripoli, Libya.
Methods: Data were collected by retrospective review of patients' medical records. UTI was defined as a urine culture containing more than 105 colonies/ml and pyuria (10 leukocytes /HPF).
Results: Out of a total of 112 kidney transplant patients, UTI was diagnosed in 33 patients (29.5%). The mean age of affected patients was 43±20 years with a range of 20-63 years. Most of the episodes (72%) occurred during the first 3 months after transplantation, and 60.6% of affected patients had more than one episode of infection. A larger proportion of females were affected than males (40.8% versus 20.6% respectively, P=0.02). No significant difference was detected in the proportion of affected patients whether the donor was live-unrelated or live-related (32.3% versus 28.4% respectively, P=0.43). The commonest causative microorganism was E. coli (38.7%), followed by klebsiella (25.8%), Staphylococcus (25.8%), and others (9.7%). The commonest drug used for treatment was ciprofloxacin (51.6%), followed by amoxicillin-clavulinic acid (22.6%), meropenem (12.9%), and others (12.9%).
Conclusion: The prevalence of UTI in our cohort of patients is similar to that reported by others. The commonest causative agent was E. coli, and ciprofloxacin was the most commonly used drug.

Nesta segunda edição do Caderno de Propriedade Intelectual e Transferência de Tecnologia, a revista finalmente começa a exercer a dimensão de territorialidade nacional a que se propunha desde a fundação.
Assim, além de artigos cujo objeto principal são novas técnicas, novos processos produtivos, novos produtos e serviços, oriundos de pesquisas cientÃficas das mais diversas áreas do conhecimento, os editores se comprazem ao verificar que a revista traz a participação de instituições localizadas em cidades e regiões tão diversas quanto o Centro-Oeste, o Sudeste e o Sul do Brasil.
Tal riqueza de conteúdo, para além de demonstrar o grande acerto dos vultuosos investimentos realizados entre 2003 e 2014 na interiorização da Rede Federal de Educação Profissional e Tecnológica, de centenas de bilhões de reais, é demonstração viva da criatividade e do caráter questionador e crÃtico do povo brasileiro que, quando alçado à condição de cientista através das polÃticas de iniciação cientÃfica e tecnológica, sempre surpreende, descobrindo alternativas, criando novos caminhos, subvertendo os métodos tradicionais, sempre em busca de soluções para a realidade objetiva e para as dificuldades da sociedade.

¿Estimula el maestro a sus estudiantes?: estudio preliminário acerca de la motivación a estudiar, de los determinantes del desempeño escolar, y de la influencia social
Este trabajo investiga, dentro la literatura acerca de los determinantes psicosociales del desempeño en aprendizaje escolar, las interrelaciones entre las capacidades motivacionales del maestro frente a sus alumnos y la influencia de la motivación sobre el rendimiento escolar, además de las influencias de aspectos relacionados a la escuela y la familia sobre el desempeño. Los conceptos son aquà abordados en búsqueda de construirse un modelo explicativo del desempeño centrado en la motivación del proprio alumno a estudiar, un modelo que será utilizado en investigación empÃrica acerca de los determinantes del desempeño que se supone sea complementaria a los muchos estudios actuales cerca del proceso de enseñanza / aprendizaje y de los determinantes psicosociales del desempeño escolar, contribuyendo con el actual estado de las investigaciones sobre este tema.Palabras-llave: Motivación a estudiar. Motivación del alumno como manifestación de la dimensión espiritual del ser humano: la acción de estudiar y la vita activa
After the 1960s, with the release of the Coleman report, the interest of governments, researchers and universities over the influence of socio-economic variables on academic performance of students grew, to the point where most of the studies about the determinants of school performance correlates income, schooling, quality of employment, and other characteristics of parents, with performance of students. Still, the most recent research shows that some characteristics of students, such as their motivation, greatly influence their reading and study activities at school and at home and, consequently, ultimately influence their performance. This preliminary study proposes a simplified model of human being, regarding its capacity for learning and creativity, which can be used later for an analysis of the influences of the teacher's performance on the Motivation of the Students and, consequently, on their school performance. The method used here is eminently the review of literature, on propositions over the human being, typical of the 20th century, carried out on the works of Arendt (2007) and Scheler (2003), from which it is composed a simplified model. It is preliminarily concluded on the validity of the model here presented, from which a complex investigation about the influence of the teacher on motivation will be constructed later.Después de los años 1960, con la divulgación del informe Coleman, se intensificó el interés de gobiernos, investigadores y universidades acerca de la influencia de las variables socio-económicas sobre el rendimiento académico de los alumnos, al punto en que la mayorÃa de los estudios acerca de los determinantes del desempeño escolar correlaciona renta, escolaridad, calidad del empleo, y otras caracterÃsticas de los padres, con el rendimiento. TodavÃa, la investigación más reciente demuestra que algunas caracterÃsticas propias del alumno, como su motivación, influyen grandemente en sus actividades de lectura y estudio en la escuela y en su casa y, por consecuencia, influyen finalmente sobre su desempeño. Este estudio preliminar propone un modelo simplificado de ser humano, en lo que dice respecto a su capacidad de aprendizaje y creatividad, que pueda ser utilizado después para un análisis de las influencias de la actuación del profesor sobre la Motivación de los Alumnos y, de esta manera, sobre rendimiento escolar. El método aquà utilizado es eminentemente el de la revisión de literatura sobre proposiciones cerca del ser humano tÃpicas del Siglo XXI, efectuada sobre los trabajos de Arendt (2007) y Scheler (2003), a partir de los cuales se trata de componer un modelo simplificado. Se concluye preliminarmente sobre la validad del modelo, a partir de lo cual se construirá una investigación compleja acerca de la influencia del maestro sobre la motivación
