    Developing and testing a Korean patient classification system for general wards based on nursing intensity

    Abstract Aim To develop a new general wards patient classification tool based on the nursing intensity level that reflects patientsā€™ clinical characteristics and indirect nursing activities. Design A crossā€sectional design was adopted. This methodological study developed a patient classification system to sort general ward patients based on the intensity of their nursing needs and verified the validity and reliability of this classification system. Methods Thirteen experts verified the toolsā€™ content validity. Data collectors and head nurses classified 150 patients from two hospitals with four general wards and various nurse staffing levels. Interā€rater reliability was analysed. Staff nurses classified 846 patients following the Korean patient classification system on nursing intensity scores that reflected patientsā€™ clinical status. Content validity was verified based on the classification results. Using Kā€group cluster analysis, score ranges for four groups were identified. Results The developed tool includes 8 domains, (symptom management, infection control, nutrition and medication, personal hygiene and secretion, activity, sleep and rest, guidance in nursing/emotional support, nursing activity planning and coordination, indirect activity), 24 subdomains, 66 nursing activities and 124 criteria.     Association between local government social expenditures and mortality levels in Korea

    OBJECTIVES: We examined the association between social expenditures of the local government and the mortality level in Korea, 2004 to 2010. METHODS: We used social expenditure data of 230 local governments during 2004 to 2010 from the Social Expenditure Database prepared by the Korean Institute for Health and Social Affairs. Fixed effect panel data regression analysis was adopted to look for associations between social expenditures and age-standardized mortality and the premature death index. RESULTS: Social expenditures of local governments per capita was not significantly associated with standardized mortality but was associated with the premature death index (decline of 1.0 [for males] and 0.5 [for females] for each expenditure of 100 000 Korean won, i.e., approximately 100 US dollar). As an index of the voluntary effort of local governments, the self-managed project ratio was associated with a decline in the standardized mortality in females (decline of 0.4 for each increase of 1%). The share of health care was not significant. CONCLUSIONS: There were associations between social expenditures of the local government and the mortality level in Korea. In particular, social expenditures per capita were significantly associated with a decline in premature death. However, the voluntary efforts of local governments were not significantly related to the decline in premature death

    New Insights into the Photodegradation Mechanism of the PTB7-Th Film: Photooxidation of pi-Conjugated Backbone upon Sunlight Illumination

    Understanding the photodegradation mechanism of photoactive materials is critical for enhancing the long-term stability of organic photovoltaic cells (OPVs). However, definitive photodegradation mechanisms have not been reported yet. Here we report the comprehensive understanding of the photodegradation mechanism of the PTB7-Th polymer film. UV/vis absorption and photoluminescence spectra show that the pi-conjugated backbone and the intermolecular pi-pi interactions are simultaneously broken under sunlight in air. Raman spectra reveal that the initial photooxidation begins at the thiophene ring in the benzo[1,2-b;3,3-b]dithiophene (BDT) unit, followed by the ring-opening of the thiophene and the break of the pi-conjugated system. Infrared spectra indicate that -S-C=O and -COOH groups are formed as a result of the photooxidation. On the basis of these observations, we propose that the thiophene ring in the BDT unit reacts with oxygen to generate the BDT-O-2 adduct, which then produces thioester and carboxylic acid. The reaction sites in the BDT unit are consistent with the electrophilic attack positions of oxygen predicted by condensed Fukui functions. Furthermore, the DFT calculated spectrum of the proposed oxidation product agrees well with all the spectroscopic observations. Conclusively, the present work provides an important clue for understanding photodegradation of OPV materials

    MiRGator v3.0: A microRNA portal for deep sequencing, expression profiling and mRNA targeting

    Biogenesis and molecular function are two key subjects in the field of microRNA (miRNA) research. Deep sequencing has become the principal technique in cataloging of miRNA repertoire and generating expression profiles in an unbiased manner. Here, we describe the miRGator v3.0 update (http://mirgator.kobic.re.kr) that compiled the deep sequencing miRNA data available in public and implemented several novel tools to facilitate exploration of massive data. The miR-seq browser supports users to examine short read alignment with the secondary structure and read count information available in concurrent windows. Features such as sequence editing, sorting, ordering, import and export of user data would be of great utility for studying iso-miRs, miRNA editing and modifications. miRNA-target relation is essential for understanding miRNA function. Coexpression analysis of miRNA and target mRNAs, based on miRNA-seq and RNA-seq data from the same sample, is visualized in the heat-map and network views where users can investigate the inverse correlation of gene expression and target relations, compiled from various databases of predicted and validated targets. By keeping datasets and analytic tools up-to-date, miRGator should continue to serve as an integrated resource for biogenesis and functional investigation of miRNAs
