    “Digital double” is a rapidly developing technology in recent years. The term “digital double” appeared a long time ago. For more than thirty years, digital doubles, as virtual representatives of real objects, have been used, in particular, in the space sphere. However, this technology became widely known at the time of the beginning of digitalization processes in order to activate the functioning and development of business. The article explores the possibility of using the resource model “Digital twin of an organization” to describe human behavior in specific situations. The probability of choosing an action is determined from a variety of  alternatives, based on an assessment of the significance of perceived incentives and restrictions. When predicting human behavior, along with external incentives, internal motives for actions and actions should also be taken into account

    The motivation as one of the key indicators of the quality of higher education, as a powerful driver of the educational process, and one of the main catalysts of personal and professional growth of the student has been considered. The results of the diagnosis of the level of motivation and structure of the motivational complex, carried out with the help of two psychological techniques, modified by the authors of the article, have been briefly outlined and displayed. Possible reasons for the “lack” of motivation among students of management specialties have been identified. Meanwhile, it has been offered to look on the lack of motivation a little wider, than the authors were able to identify in the analysis of sources on this theme. Possible directions of optimization of motivational structure of future managers have been studied. The vector of further studies of educational motivation as an indicator of the quality of management education has been outlined

    High resolution manometry and new classification of esophageal motility disorders

    Purpose of the review. To present application of Chicago classification criteria of esophageal motility disorders defined in high resolution manometry in clinical practice. Basic provisions. High-resolution manometry is the most exact hi-tech diagnostic method for esophageal motor function disorders according to Chicago classification v3.0. Uniqueness of the method consists in capacity to define integrated quantitative and qualitative metrics of esophageal contractile function and to establish their specific disorders e.g.: change of intrabolus pressure at disorders of esophagogastric junction (EGj) outflow, hypercontractile esophagus, fragmented contractions and weak or failed peristalsis, distal esophageal spasm. Assessment of the type of achalasia subtypes has significant impact on the patients’ treatment choice. According to anatomical location of the lower esophageal sphincter and crural diaphragm several morphological types of gastro-esophageal junction are defined that determine severity of gastroesophageal reflux disease. Multiple rapid swallow responses during esophageal high-resolution manometry reflect esophageal body peristaltic reserve and is a predictor of postoperative complications. Differential diagnosis of belching type became possible at combined application of high-resolution manometry and impedance measurement. Conclusion. High-resolution manometry is a fundamental diagnostic test of esophageal motor function disorders. Clinical application of this method significantly expands diagnostic potential and allows to carry out personalized treatment that increases treatment quality

    Macrophage phenotype after human refluxate exposure, esophageal dysmotility and their correlation with gastroesophageal reflux disease

    Aim of the study. To investigate the esophageal dysmotility, changes in the esophageal mucosa and the immune response depending on the type of refluxate in gastroesophageal reflux disease (GERD) patients.Material and methods. 68 patients with GERD were recruited: 28 (14 men; mean age, 45.74 ± 2.23 years) nonerosive reflux disease (NERD), 22 (15 men; mean age, 45.0 ± 3.24 years) erosive reflux disease (EE), 18 (13 men; mean age, 47.22 ± 2.95) Barrett’s Esophagus (BE). GERD patients underwent esophageal high-resolution manometry (HRM) with a 22-channel water-perfused catheter and Solar GI system (Medical Measurements Systems, Enschede, the Netherlands), 24-hour impedance and pH monitoring using the Ohmega Ambulatory Impedance pH Recorder (Medical Measurements Systems). We analyzed receptor characteristics of monocyte-derived macrophages in all groups of patients.Results. On HRM examination, we showed that DCI (distal contractile integral) in NERD patients was higher than in EE (p = 0.088) and BE (p = 0.076), also LES RP (lower esophageal sphincter resting pressure) in NERD patients was higher than in EE (p = 0.039) and BE (p = 0.012). The analysis of reflux characteristics showed that the total reflux time with pH < 4 for BE patients was longer than that for NERD and EE patients. An analysis of receptor characteristics of monocyte-derived macrophages showed the prevalence of CD25 and CD80 expression in all groups of patients.Conclusion. An analysis of the phenotype of macrophages derived from blood monocytes of GERD patients revealed a prevalence of М1 macrophages that was typical for the Th1 type of immune response. The degree of esophageal dysmotility was correlated with GERD severity and type

    The article analyzes methods for the preparation of cymenes based on using petrochemical raw materials: toluene and propylene/isopropanol. It is concluded that at present advanced technologies for the production of individual cymenes are absent, and the heterogeneous catalytic processes require significant improvement.Проанализированы методы получения цимолов, базирующиеся на использовании нефтехими-ческого сырья - толуола, пропилена/изопропилового спирта.     The Large Enriched Germanium Experiment for Neutrinoless Double Beta Decay (LEGEND)

    The observation of neutrinoless double-beta decay (0νββ{\nu}{\beta}{\beta}) would show that lepton number is violated, reveal that neutrinos are Majorana particles, and provide information on neutrino mass. A discovery-capable experiment covering the inverted ordering region, with effective Majorana neutrino masses of 15 - 50 meV, will require a tonne-scale experiment with excellent energy resolution and extremely low backgrounds, at the level of \sim0.1 count /(FWHM\cdott\cdotyr) in the region of the signal. The current generation 76^{76}Ge experiments GERDA and the MAJORANA DEMONSTRATOR utilizing high purity Germanium detectors with an intrinsic energy resolution of 0.12%, have achieved the lowest backgrounds by over an order of magnitude in the 0νββ{\nu}{\beta}{\beta} signal region of all 0νββ{\nu}{\beta}{\beta} experiments. Building on this success, the LEGEND collaboration has been formed to pursue a tonne-scale 76^{76}Ge experiment. The collaboration aims to develop a phased 0νββ{\nu}{\beta}{\beta} experimental program with discovery potential at a half-life approaching or at 102810^{28} years, using existing resources as appropriate to expedite physics results.Comment: Proceedings of the MEDEX'17 meeting (Prague, May 29 - June 2, 2017

    Патоморфоз заболеваний бронхолегочной системы у работающих в контакте с аэрозолями цветных металлов

    Etiopathogenic features and diagnostic criteria of occupational diseases studying in workers engaged in colored metallurgy have been given in the paper. Polyvalent sensitization to metal allergens (nickel, chrome, beryllium, manganese) was found. A toxic effect of nickel on DNA was shown that could be used as a biomarker of exposure for biological monitoring in colored metallurgy workers. Biochemical investigations determined the main pathogenic mechanisms underlying pathomorphology of bronchopulmonary diseases caused by the exposure of colored metals, such as activation of lipid peroxidation, "proteolysis – antiproteolysis" imbalance, growing significance of infection. This is the first study demonstrating clinical and biochemical parallels between characteristics of development and course of respiratory pathology caused by the exposure of colored metals. Infectious, inflammatory, toxico-allergic, and destructive processes predominated in this pathology. Preventive and rehabilitation strategies have been developed.В статье представлены результаты изучения этиопатогенетических особенностей формирования профессиональных, производственно обусловленных заболеваний у работающих в цветной металлургии, определены критерии их диагностики. Выявлена поливалентная сенсибилизация к основным металлам-аллергенам (никель, хром, марганец, бериллий). Установлено токсическое влияние никеля на ДНК, что может служить биомаркером экспозиции и использоваться для биологического мониторинга у работающих в цветной металлургии.Результаты биохимических исследований определили основные патогенетические механизмы, лежащие в основе патоморфоза бронхолегочной патологии при воздействии аэрозолей цветных металлов: активация перекисного окисления липидов и дисбаланс в системе "протеолиз-антипротеолиз", возрастание роли инфекционного фактора. Впервые выявлены клинико-биохимические параллели между особенностями формирования и течением бронхолегочной патологии, сформировавшейся под воздействием аэрозоля цветных металлов с преобладанием инфекционно-воспалительного, токсико-аллергического, воспалительно-деструктивного компонентов, и биохимическим профилем организма.
