    Ex-vivo porcine organs with a circulation pump are effective for teaching hemostatic skills

    Surgical residents have insufficient opportunites to learn basic hemostatic skills from clinical experience alone. We designed an ex-vivo training system using porcine organs and a circulation pump to teach hemostatic skills. Residents were surveyed before and after the training and showed significant improvement in their self-confidence (1.83 ± 1.05 vs 3.33 ± 0.87, P < 0.01) on a 5 point Likert scale. This training may be effective to educate residents in basic hemostatic skills

    Ephedra resource in Sichuan and Yunnan Provinces 2007

    金沢大学医薬保健研究域薬学系Wild Ephedra plants growing near the Tibetan border of Yunnan and Sichuan Provinces and north-central Sichuan were surveyed and their DNA and ephedrine alkaloids content were analyzed. By analysis of internal transcribed spacer 1 (ITS) 1 DNA, E. likiangensis was found to be the dominant species in these regions, which clustered into 2 major groups in the phylogenic tree. Most Ephedra plants in these regions of ordinal size contained ephedrine and pseudoephedrine of more than 0.7%, the requirement for Japanese Pharmacopoeia 15th edition, suggesting that they have potential for crude drug production of Ephedra herbs. © 2009 Pharmaceutical Society of Japan

    Takotsubo Cardiomyopathy as a Delayed Complication with a Herbicide Containing Glufosinate Ammonium in a Suicide Attempt: A Case Report

    Background. Glufosinate ammonium has a famous delayed complication as respiratory failure, however, delayed cardiogenic complication is not well known. Objectives. The aim of this study is to report a takotsubo cardiomyopathy as a delayed complication of glufosinate ammonium for suicide attempt. Case Report. A 75-year-old woman ingested about 90 mL of Basta, herbicide for suicide attempt at arousal during sleep. She came to our hospital at twelve hours after ingesting. She was admitted to our hospital for fear of delayed respiratory failure. Actually, she felt down to respiratory failure, needing a ventilator with intubation at 20 hours after ingesting. Procedure around respiratory management had smoothly done with no delay. Her vital status had been stable, however, she felt down to circulatory failure and diagnosed as Takotsubo cardiomyopathy at about 41 hours after ingestion. There was no trigger activities or events to evoke mental and physical stresses. Conclusion. We could successfully manage takotsubo cardiomyopathy resulted in circulatory failure in a patient with glufosinate poisoning for suicide attempt. Takotsubo cardiomyopathy should be taken into consideration if circulatory failure is observed for unexplained reasons

    Epidemiology of Infectious Hepatitis 3rd Report. On the Mass Examination Taken for Shakurenzi Hamlet

    The mass examination was carried out in Shakurenji, Kama- Village, Akaiwa Connty, Okayama Pref., where great many deathes have been taken place by infectious hepatitis; results were as follows: 1. 127 persons received the examination; it has covered 94.1% of the entire people. 2. Those who have had jaundice previously were enumerated as 5 cases; as it was just the season when infectious hepatitis was in vogue in other parts of the Prefecture, though slight in degree, it was imagined that probably in the past there might have occurred a few sporadic cases. Among those, 3 were taken record of as new victims of disease, 3. In 6 cases, the patients complained of certain pain, each having some sort of liver disturbance more or less. 4. There were 24 cases in which liver enlargement could be detected; its rate, 18.9%, while splenomegaly been discovered in only one case. 5. Two women in pregnancy were recognized, one sickened case, the other inapparent infection case. 6. As for wide-range judgment taken after the examinaton, must-be-treated case, 5, must-be taken-care-of case 5. inapparent infection 60, 70 in total. percentage of contraction, 55.1%, adding 3 cases starting disease as well as 3 deaths; thus, great total, 76 cases; its rate, 57.1%. 7. Percentage of contraction has proved high in thirties and above 50. By sex, females superseded, but as to age, no definite conclusion could be established. 8. In case those patients exposed would be divided by respective family and investigated, it has been revealed thus: (a) with families which have used a well in common with the family in which death case or at least patient was found, Percentage of contraction has proved high and almost in like degree, since many were adviced to receive treatment, or, be taken care of. (b) Families which employed a well in common, were recognized to have proved about the same ratio in infection, containing not a few ought to be treated or taken care of, which suggests us an infection due to a well. (c) Among the group of families, whose relatives died of this disease, master of the house who had presented himasdlf over the funeral diet has been found to have taken infection alone

    Finite frequency whole mantle P wave tomography: Improvement of subducted slab images

    We present a new whole mantle P wave tomographic model GAP_P4. We used two data groups; short-period data of more than 10 million picked-up onset times and long-period data of more than 20 thousand differential travel times measured by waveform cross-correlation. Finite frequency kernels were calculated at the corresponding frequency bands for both long- and short- period data. With respect to an earlier model GAP_P2, we find important improvements especially in the transition zone and uppermost lower mantle beneath the South China Sea and the southern Philippine Sea owing to broadband ocean bottom seismometers (BBOBS) deployed in the western Pacific Ocean where station coverage is poor. This new model is different from a model in which the full data set is interpreted with classical ray theory. BBOBS observations should be more useful to sharpen images of subducted slabs than expected from simple ray path coverage arguments

    Multifocal Motor Neuropathy

    Objective: Our objective was to do an epidemiologic survey of patients with multifocal motor neuropathy (MMN) in comparison with those with amyotrophic lateral sclerosis (ALS) in Japan. Methods: In this retrospective study, we examined 46 patients with MMN and 1,051 patients with ALS from major neuromuscular centers in Japan from 2005 to 2009. Diagnosis was based on the European Federation of Neurological Societies/Peripheral Nerve Society (EFNS/PNS) and the revised El Escorial criteria. The efficacy of intravenous immunoglobulin (IVIg) was also taken into consideration in the diagnosis of MMN. Results: The ratio of MMN to ALS patients (0-0.10) varied among the centers, but mostly converged to 0.05. The prevalence was estimated to be 0.29 MMN patients and 6.63 ALS patients per 100,000 population. Conclusions: The frequency of MMN patients was around 1 out of 20 ALS patients, and MMN was possibly underdiagnosed in some centers

    Design and baseline characteristics of the finerenone in reducing cardiovascular mortality and morbidity in diabetic kidney disease trial

    Background: Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. Patients and Methods: The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate >= 25 mL/min/1.73 m(2) and albuminuria (urinary albumin-to-creatinine ratio >= 30 to <= 5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level alpha = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. Conclusions: FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.
