    Why don't poor men eat fruit? Socioeconomic differences in motivations for fruit consumption.

    BACKGROUND: Those of lower socioeconomic status (SES) tend to have less healthy diets than those of higher SES. This study aimed to assess whether differences in motivations for particular foods might contribute to socioeconomic differences in consumption. METHODS: Participants (n = 732) rated their frequency of consumption and explicit liking of fruit, cake and cheese. They reported eating motivations (e.g., health, hunger, price) and related attributes of the investigated foods (healthiness, expected satiety, value for money). Participants were randomly assigned to an implicit liking task (Single Category Implicit Association Task) for one food category. Analyses were conducted separately for different SES measures (income, education, occupational group). RESULTS: Lower SES and male participants reported eating less fruit, but no SES differences were found for cheese or cake. Analyses therefore focused on fruit. In implicit liking analyses, results (for income and education) reflected patterning in consumption, with lower SES and male participants liking fruit less. In explicit liking analyses, no differences were found by SES. Higher SES participants (all indicators) were more likely to report health and weight control and less likely report price as motivators of food choices. For perceptions of fruit, no SES-based differences were found in healthiness whilst significant interactions (but not main effects) were found (for income and education) for expected satiety and value for money. Neither liking nor perceptions of fruit were found to mediate the relationship between SES and frequency of fruit consumption. CONCLUSIONS: There is evidence for social patterning in food motivation, but differences are modified by the choice of implicit or explicit measures.     OBJECTIVE: In the context of a food purchasing environment filled with advertising and promotions, and an increased desire from policy makers to guide individuals toward choosing healthier foods, this study tests whether priming methods that use healthy food adverts to increase preference for healthier food generalize to a representative population. METHODS: In two studies (Study 1 n = 143; Study 2 n = 764), participants were randomly allocated to a prime condition, where they viewed fruit and vegetable advertisements, or a control condition, with no advertisements. A subsequent forced choice task assessed preference between fruits and other sweet snacks. Additional measures included current hunger and thirst, dietary restraint, age, gender, education and self-reported weight and height. RESULTS: In Study 1, hunger reduced preferences for fruits (OR (95% CI) = 0.38 (0.26-0.56), p <0.0001), an effect countered by the prime (OR (95% CI) = 2.29 (1.33-3.96), p = 0.003). In Study 2, the effect of the prime did not generalize to a representative population. More educated participants, as used in Study 1, chose more fruit when hungry and primed (OR (95% CI) = 1.42 (1.13-1.79), p = 0.003), while less educated participants' fruit choice was unaffected by hunger or the prime. CONCLUSION: This study provides preliminary evidence that the effects of adverts on healthy eating choices depend on key individual traits (education level) and states (hunger), do not generalize to a broader population and have the potential to increase health inequalities arising from food choice.We gratefully acknowledge the participation of all NIHR Cambridge BioResource (CBR) volunteers.     A probabilistic approach for quantitative identification of multiple delaminations in laminated composite beams using guided waves

    Available online 16 September 2016In this study a probabilistic approach is proposed to identify multiple delaminations in laminated composite beams using guided waves. The proposed method is a model-based approach, which provides a quantitative identification of the delaminations. This study puts forward a practical damage identification method, and hence, it can identify multiple delaminations using guided wave signal measured at a single measurement point on the laminated composite beams. The proposed method first determines the number of delaminations using Bayesian model class selection method. The Bayesian statistical framework is then employed to not only identify the delamination locations, lengths and through-thickness locations, but also quantify the associated uncertainties, which provides valuable information for engineers in making decision on necessary remedial work. In addition the proposed method employs the time-domain spectral finite element method and Bayesian updating with Subset simulation to further improve the computational efficiency. The proposed probabilistic approach is verified and demonstrated using data obtained from numerical simulations, which consider both measurement noise and modeling error, and experimental data. The results show that the proposed method can accurately determine the number of delaminations, and the identified delamination locations, lengths and through-thickness locations are closed to the true values.Shuai He, Ching-Tai N

    Entomologic and molecular investigation into Plasmodium vivax transmission in Singapore, 2009

    <p>Abstract</p> <p>Background</p> <p>Singapore has been certified malaria free since November 1982 by the World Health Organization and despite occasional local transmission, the country has maintained the standing. In 2009, three clusters of malaria cases were reported in Singapore.</p> <p>Methods</p> <p>Epidemiological, entomological and molecular studies were carried out to investigate the three clusters, namely Mandai-Sungei Kadut, Jurong Island and Sembawang.</p> <p>Results</p> <p>A total of 29 malaria patients, with no recent travel history, were reported in the three clusters. Molecular analysis based on the <it>msp3α </it>and <it>msp1 </it>genes showed two independent local transmissions: one in Mandai-Sungei Kadut and another in Sembawang. Almost all cases within each cluster were epidemiologically linked. In Jurong Island cluster, epidemiological link remains uncertain, as almost all cases had a unique genetic profile. Only two cases shared a common profile and were found to be linked to the Mandai-Sungei Kadut cluster. Entomological investigation found <it>Anopheles sinensis </it>to be the predominant Anopheline in the two areas where local transmission of <it>P. vivax </it>was confirmed. <it>Anopheles sinensis </it>was found to be attracted to human bait and bites as early as 19:45 hrs. However, all <it>Anopheles </it>mosquitoes caught were negative for sporozoites and oocysts by dissection.</p> <p>Conclusion</p> <p>Investigation of <it>P. vivax </it>cases from the three cluster areas confirmed the occurrence of local transmission in two areas. Although <it>An. sinensis </it>was the predominant Anopheline found in areas with confirmed transmission, the vector/s responsible for the outbreaks still remains cryptic.</p

    Bridging dentistry and traditional Chinese medicine : a study on the current mutual understanding among the future practitioners

    Objectives: This project aimed to promote the mutual understanding between dental and TCM students. The objectives were to describe the current teaching of dentistry in the TCM schools and that of TCM in the dental school, to describe the TCM students’ knowledge on dentistry and the dental students’ knowledge on TCM, and to produce promotional materials for enhancing the mutual understanding between dental and TCM students. Methods: Information on the current teaching of dentistry in the TCM schools and that of TCM in the dental school was collected through a search on the undergraduate programme syllabus and conducting interviews with the leaders of the schools. Questionnaire surveys were conducted among the final year students of the TCM and the dental schools to collect information on their knowledge of the counter profession. Two leaflets and a short video were produced with the aid of computer software and used in the promotional activities. Results: It was found that very little information on dentistry was covered in the undergraduate programmes of the three TCM schools, and vice versa in the dental school. A total of 39 TCM students and 55 dental students completed the questionnaire survey. Most of the students did not know the basic theories and practice of the counter profession. Less than half of the student knew what types of patient cases may be suitable for cross-referral to the other profession. Conclusions: The TCM and dental schools in Hong Kong do not have scheduled teaching on the counter profession. The TCM students have little knowledge on dentistry and how to collaborate with dental practitioners on patients presented with oral problems. Conversely, it is the same for the dental students. Lastly, informative printed and audio-visual promotional materials can be produced with limited resources and used to facilitate the mutual understanding between TCM and dental practitioners.published_or_final_versio

    Towards a global partnership model in interprofessional education for cross-sector problem-solving

    Objectives A partnership model in interprofessional education (IPE) is important in promoting a sense of global citizenship while preparing students for cross-sector problem-solving. However, the literature remains scant in providing useful guidance for the development of an IPE programme co-implemented by external partners. In this pioneering study, we describe the processes of forging global partnerships in co-implementing IPE and evaluate the programme in light of the preliminary data available. Methods This study is generally quantitative. We collected data from a total of 747 health and social care students from four higher education institutions. We utilized a descriptive narrative format and a quantitative design to present our experiences of running IPE with external partners and performed independent t-tests and analysis of variance to examine pretest and posttest mean differences in students’ data. Results We identified factors in establishing a cross-institutional IPE programme. These factors include complementarity of expertise, mutual benefits, internet connectivity, interactivity of design, and time difference. We found significant pretest–posttest differences in students’ readiness for interprofessional learning (teamwork and collaboration, positive professional identity, roles, and responsibilities). We also found a significant decrease in students’ social interaction anxiety after the IPE simulation. Conclusions The narrative of our experiences described in this manuscript could be considered by higher education institutions seeking to forge meaningful external partnerships in their effort to establish interprofessional global health education

    Coronavirus-positive Nasopharyngeal Aspirate as Predictor for Severe Acute Respiratory Syndrome Mortality

    Severe acute respiratory syndrome (SARS) has caused a major epidemic worldwide. A novel coronavirus is deemed to be the causative agent. Early diagnosis can be made with reverse transcriptase-polymerase chain reaction (RT-PCR) of nasopharyngeal aspirate samples. We compared symptoms of 156 SARS-positive and 62 SARS-negative patients in Hong Kong; SARS was confirmed by RT-PCR. The RT-PCR–positive patients had significantly more shortness of breath, a lower lymphocyte count, and a lower lactate dehydrogenase level; they were also more likely to have bilateral and multifocal chest radiograph involvement, to be admitted to intensive care, to need mechanical ventilation, and to have higher mortality rates. By multivariate analysis, positive RT-PCR on nasopharyngeal aspirate samples was an independent predictor of death within 30 days
