    TABOTAMP® in Oral and Maxillofacial Surgery – mechanism, indications and contraindications

    Since the 1950s, oxidised regenerate cellulose (TABOTAMP ®) has been successfully applied for local haemostasis in intra- or postoperative bleeding complications. The scope of this medical product is large and it has found its place in oral and maxillofacial surgery. Although the application of TABOTAMP ® in medicine and dentistry is common, further complications next to the absence of the haemostatic effect may be encountered if used incorrectly. The aim of this article is an update on the proper use of TABOTAMP ®. Further, tips are provided for the indication-specific application of this useful agent in clinical practice

    The effects of treadmill exercise on oxidative stress in Mongolian gerbils with penicillin-induced epilepsy

    Aim: To evaluate the role of treadmill exercise on the oxidative stress in Mongolian gerbils with penicillin-induced epilepsy. Methods: This experimental study included 18 male Mongolian gerbils which were divided into three groups; sham-control group, penicillin group (500 units) and exercise + penicillin (500 units) group. Each animal group was composed of six Mongolian gerbils. The epileptiform activity was verified by electrocorticographic recordings.  Results: The latency of the penicillin+exercise group was longer than the penicillin group, but this difference was not statistically significant. Following the penicillin administration, spike wave frequencies of epileptiform activity in the 10, 30, and 35 minutes were significantly lower in the penicillin+exercise group, compared with the penicillin group. There were generally significant decreases in the spike wave amplitude medians in the penicillin+exercise groups compared with the penicillin group in all time periods between 0 and 5 minutes. The serum superoxide dismutase, catalase and glutathione peroxidase levels increased in the penicillin+exercise group compared with those in the penicillin group.  Conclusion: The results of present study indicate that regular exercise may contribute to the amelioration of epileptic activity by increasing the antioxidant effect.     Effects of Urtica dioica on hepatic ischemia‐reperfusion injury in rats

    OBJECTIVES: To evaluate the effects of Urtica dioica on hepatic ischemia-reperfusion injury. METHODS: Thirty adult male Wistar albino rats were divided into three groups: sham group (group 1), control group (group 2), and Urtica dioica group (group 3). All the rats were exposed to hepatic ischemia for 60 min, followed by 60 min of reperfusion. In group 2, a total of 2 ml/kg 0.9% saline solution was given intraperitoneally. In group 3, a total of 2 ml/kg Urtica dioica was given intraperitoneally. At the end of the procedure, liver tissue and blood samples were taken from all rats. Serum aspartate aminotransferase, alanine aminotransferase, lactate dehydrogenase, ceruloplasmin, catalase, paraoxonase, arylesterase, and lipid hydroperoxide levels were measured. Liver tissue histopathologies were also evaluated by light microscopy. RESULTS: Serum aspartate aminotransferase, alanine aminotransferase and lactate dehydrogenase levels were significantly higher in group 2 than in group 1, and significantly lower in group 3 than in group 2. Also, group 2 had higher serum lipid hydroperoxides and ceruloplasmin levels but lower catalase, paraoxonase, and arylesterase levels than group 1. In group 3, serum lipid hydroperoxides and ceruloplasmin levels were significantly lower, and catalase, paraoxonase, and arylesterase levels were higher than those in group 2. Histopathological examination showed that liver tissue damage was significantly decreased in group 3 compared with group 2. CONCLUSIONS: Urtica dioica has a protective effect on the liver in hepatic ischemia-reperfusion-injured rats

    Is radical surgery of an inverted papilloma of the maxillary sinus obsolete? a case report

    Abstract Background Sinonasal inverted papilloma is a locally aggressive tumor arising from the Schneiderian membrane which lines the nasal cavity and paranasal sinuses. Aggressive surgical approaches, such as lateral rhinotomy, were used until recently for complete removal of the inverted papilloma. Currently, endoscopic resection is the gold standard in the treatment of inverted papilloma. However, there are situations that justify an open approach. For example there are studies that report a higher postoperative recurrence rate after endonasal endoscopic resection, particularly in the treatment of recurrent diseases. While endoscopic resection performed by an experienced surgeon is definitely a minimally invasive therapy, an open approach is not necessarily associated with functional and aesthetic disadvantages. This case report describes the treatment of inverted papilloma by an open approach. This has been described before but the new gold standard of endoscopic resection has to be taken into account before any treatment decision is made nowadays. Case presentation Contrast-enhanced magnetic resonance imaging of the head and neck area was indicated in a 72-year-old white German man who presented with suspected squamous cell carcinoma of his lower lip. Magnetic resonance imaging additionally revealed a 3×2 cm2 polycyclic arranged mucosal thickening with cystic and solid contrast affine shares at the antral laterocaudal area of his right maxillary sinus, extending from his right lateral nasal wall to his maxillary sinus floor. He received antral polypectomy with medial maxillectomy via a unilateral LeFort I osteotomy approach. His pterygoid plate was preserved. A histological examination demonstrated a tumor composed of hyperplastic squamous epithelium protruding into the stroma (surface epithelial cells grew downward into the underlying supportive tissue), thus producing a grossly convoluted cerebriform appearance. Two weeks later, the patient regained a well-formed maxilla without any restrictions. He has remained disease-free for 25 months following the surgery and surveillance was continued in our tumor clinic. Conclusions Endoscopic resection of an inverted papilloma continues to be the gold standard. However, some cases require a radical approach. This does not necessarily increase patient morbidity

    COVID-19 Pandemi Sırasındaki Acil Cerrahi Bakım ve Ameliyathane Uygulamalarında Perioperatif Uygulama ve Rehberlik; Deneyimlerimiz

    Aim: The novel coronavirus SARS-CoV-2 (COVID-19) can infect healthcare workers. We developed an institutional algorithm to protect operating room team members during the COVID-19 pandemic and rationally conserve personal protective equipment (PPE). We aimed to review the latest data on the COVID-19 pandemic and essential information for practice in emergency surgery and the operating room.Materials and Methods: An interventional platform (operating room, interventional suite, and endoscopy) with our committee formed with our doctors consisting of different branches, we developed our guidelines based on potential patterns of spread, risk of exposure, and conservation of PPE. We aimed to share our experiences with 128 patients who were taken into operation in a 2-month period.Anesthetic management and infection control guidelines for emergency procedures for patients with suspected 2019-nCoV were drafted and applied in Medical Faculty of Namık Kemal University.Results: A decision tree algorithm describing our institutional guidelines for precautions for operating room team members was created. This algorithm is based on the urgency of operation, anticipated viral burden at the surgical site, the opportunity for a procedure to aerosolize virus, and the likelihood a patient could be infected based on symptoms and testing.Conclusion: Despite COVID-19 being a new threat, we have shown that by developing an easy-to-follow decision algorithm for the interventional platform teams, we can ensure optimal healthcare worker safety.Amaç: Yeni koronavirüs SARS-CoV-2 (COVID-19) sağlık çalışanlarını enfekte edebilir. COVID-19 salgını sırasında ameliyathane ekip üyelerini korumak ve rasyonel olarak kişisel koruyucu ekipman (KKE) için kurumsal bir algoritma geliştirdik. Acil cerrahi ve ameliyathanede COVID-19 salgını ile ilgili en güncel bilgileri gözden geçirmeyi amaçladık. Materyal ve Metot: Farklı branşlardan oluşan doktorlarımızla oluşturduğumuz komitemiz ile girişimsel bir platform (ameliyathane, girişimsel ve endoskopi), potansiyel yayılma örüntüleri, maruz kalma riski ve KKE'nin korunmasına dayanan kılavuzlarımızı geliştirdik. Deneyimlerimizi 2 aylık bir sürede ameliyat edilen 128 hasta ile paylaşmayı amaçladık. 2019-nCoV şüphesi olan hastalar için acil durum prosedürleri için anestezi yönetimi ve enfeksiyon kontrol kılavuzları Namık Kemal Üniversitesi Tıp Fakültesi'nde hazırlanmış ve uygulanmıştır. Bulgular: Ameliyathane ekibi üyelerine yönelik önlemler için kurumsal yönergelerimizi açıklayan bir karar algoritması oluşturuldu. Bu algoritma ameliyatın aciliyetine, cerrahi bölgede beklenen viral yüke, virüsü aerosol haline getirme prosedürü fırsatına ve hastanın semptomlara ve testlere dayanarak enfekte olma olasılığına dayanır. Sonuç: COVID-19'un yeni bir tehdit olmasına rağmen, girişimsel platform ekipleri için izlemesi kolay bir karar algoritması geliştirerek, optimum sağlık çalışanı güvenliğini sağlayabildiğimizi gösterdik

    Objectives: The aim of our study is the comparison of the results of conventional smear (CC) technique and liquidbased cytology (LBC) technique used as cervical cancer screening methods. Material and methods: The results of 47954 patients submitted to smear screening in our gynecology clinic between January 2008 and December 2014 have been studied. The smear results have been divided into two groups CC and LBC according to the technique used. Results: When considering the distribution within CC group, the results were as follows: intraepithelial cell abnormalities 2,0% (n=619), insufficient sample for analysis 2,1% (n=660), Atypical squamous cells of undetermined significance (ASC-US) 1.8% (n=554), Low grade squamous intraepithelial lesion (LGSIL) 0.1% (n=35), High grade squamous intraepithelial lesion (HGSIL) 0.1% (n=16), Atypical squamous cells – cannot exclude HGSIL (ASC-H) 0.029% (n=9), Atypical glandular cells- not other wise specified (AGC-NOS) 0.012% (n=4), squamous carcinoma 0.003% (n=1). When considering the distribution in LBC group, the results were as follows: intraepithelial cell abnormalities2.1% (n=357), insufficient sample for analysis 0.9% (n=144), ASC-US 1.8% (n=296), LGSIL 0.2% (n=38), HGSIL 0.1% (n=8), ASC-H 0.1% (n=10), AGC-NOS 0.017% (n=3), squamous carcinoma 0.011% (n=2). Conclusions: Although the rates of epithelial cell abnormalities are similar for both tests, LSIL results are more frequently observed in LBC technique. In LBC technique, the number of insufficient sample for analysis is quite low compared to CC group and thus constitutes an advantage.  Cel pracy: Celem badania było porównanie wyników konwencjonalnej (CC) i płynnej cytologii (LBC) stosowanej w skriningu raka szyjki macicy. Materiał i metoda: Przeanalizowano wyniki od 47954 pacjentek objętych cytologicznym badaniem skriningowym w naszym oddziale ginekologicznym w okresie od stycznia 2008 do grudnia 2014. Wyniki cytologiczne podzielono na dwie grupy CC i LBC w zależności od techniki pobierania. Wyniki: W grupie CC wyniki przedstawiały się nastepująco: nieprawidłowości komórek śródnabłonkowych 2,0% (n=619), nieodpowiednia próbka do analizy 2,1% (n=660), ASC-US 1.8% (n=554), LGSIL 0.1% (n=35), HGSIL 0.1% (n=16), ASC-H 0.029% (n=9), AGC-NOS 0.012% (n=4), rak płaskonabłonkowy 0.003% (n=1). W grupie LBC wyniki przedstawiały się następująco: nieprawidłowości komórek śródnabłonkowych 2.1% (n=357), nieodpowiednia próbka do analizy 0.9% (n=144), ASC-US 1.8% (n=296), LGSIL 0.2% (n=38), HGSIL 0.1% (n=8), ASC-H 0.1% (n=10), AGC-NOS 0.017% (n=3), rak płaskonabłonkowy 0.011% (n=2). Wnioski: Chociaż odsetek nieprawidłowości komórek śródnabłonkowych jest podobny dla obu testów, wyniki LSIL są częściej obserwowane w technice LBC. W metodzie LBC liczba próbek nieodpowiednich do analizy jest dość niska w porównaniu do grupy CC, stąd jest to jej niewątpliwa zaleta.

    The Effect of Pulsatile and Non-Pulsatile Extra Corporeal Perfusion on Cerebral Oxygen Saturation in Cardiopulmonary Bypass Patients (Flow Type On Cerebral Oxygenatıon)

    Aim:The flow type generated by a heart-lung machine is important in cardiopulmonary bypass. The use of pulsatile flow versus non-pulsatile flow during cardiopulmonary bypass has been a controversy among clinicians. We compared the effect of non-pulsatile and pulsatile flow during cardiopulmonary bypass on cerebral oxygenation.Materials and Methods:We conducted a retrospective study of 50 adult patients who underwent coronary artery bypass graft surgery at our university hospital, with near infrared spectroscopy used to compare differences in cerebral oxygenation between the pulsatile and non-pulsatile flow type.Results:There was no difference between the effect of pulsatile and non-pulsatile flow on the saturation of hemoglobin (SpO2), nor on the partial pressure of oxygen (pO2) and carbon dioxide (pCO2). The near infrared spectroscopy results were not different between the two flow types.Conclusion:There was no effect of the flow type generated by a heart-lung machine (pulsatile or non-pulsatile) on cerebral oxygenation in adult patients

    Plant Root Enhancement by Plant Growth Promoting Rhizobacteria

    Soil microorganisms perform a variety of functions, some of which are extremely helpful to the maintenance of ecological sustainability. Bacteria thriving in the plant rhizosphere drive plant development through a variety of ways, which are referred to as PGPRs (plant growth-promoting rhizobacteria). Despite the fact that there are many different types of PGPRs, their significance and applications in sustainable agriculture are still debated and limited. The performance of PGPRs vary, which might be related to a variety of environmental conditions that impact their development and proliferation in plants. PGPR is a nonpathogenic, friendly bacterium that stimulates plant development by altering hormone concentrations and nutritional needs, as well as mitigating stress-related damage. PGPRs colonize root hairs and lateral roots in plants, where they may exhibit their beneficial characteristics. Rhizobacteria that promote plant development have the ability to control root system architecture (RSA), as well as the vegetative growth and physiology of the entire plant. The generation of hormones like Indole acetic acid (IAA) by PGPR has long been linked to RSA effects.
