    The walking estimated limitation stated by history (WELSH): a visual tool to self-reported walking impairment in a predominantly illiterate population

    BACKGROUND: The prevalence of cardiovascular diseases is increasing in low-income countries. Various questionnaires to estimate walking capacity in patients are available in multiple languages but they are not suitable for illiterate patients. OBJECTIVE: The walking estimated limitation stated by history (WELSH) tool aims at rating individual walking disability using only drawings and four items. METHODS: A six-month prospective study was performed on new patients referred to the Department of Cardiology at the Centre Hospitalier Universitaire Sourô Sanou in Bobo-Dioulasso, Burkina Faso. We administered the WELSH tool after a short oral presentation in the patient\u27s language or dialect. Thereafter, patients performed a six-minute walking test in the hospital corridor under the supervision of a nurse who was blinded to the results of the WELSH score. We performed a step-by-step multilinear regression analysis to determine the factors predicting maximal walking distance (MWD). RESULTS: There were 40 female and 10 male patients in this study. Their ages ranged from 54.8 ± 10.7 years. Only 32% of the patients had attended primary school. Most patients were classified as stage I to III of the New York Heart Association (NYHA) classification. The objective measurement of MWD during a six-minute walking test showed no association with the subjects\u27 educational level, body mass index, NYHA stage or gender, but a significant correlation with the WELSH scores. The Spearman r-value for the WELSH score-to-MWD relationship was 0.605 (p < 0.001). CONCLUSIONS: The WELSH tool is feasible and correlated with measured MWD in a population of predominantly illiterate patients

    Effets des graines torréfiées de Vigna unguiculata (niébé) comme source de protéines, dans l’alimentation des poules locales en ponte au Burkina Faso, sur leurs performances zootechniques et la rentabilité économique des régimes

    L’étude avait pour but d’évaluer les effets de l’incorporation des graines torréfiées de niébé dans l’alimentation de la poule locale en ponte, sur ses performances zootechniques et économique. Six coqs et soixante poules de race locale de 7,5 mois d’âge ont été répartis en six lots, par tirage aléatoire. Deux régimes alimentaires aux mêmes contraintes nutritionnelles dont un témoin et un régime expérimental comportant 8% de graines torréfiées de niébé ont été préparés. La ponte, les caractéristiques des oeufs, les consommations et les mortalités sont suivies hebdomadairement pendant 24 semaines. L’éclosion des oeufs a été suivie durant 03 semaines. La marge brute induite par chaque régime a été évaluée à la fin de l’essai. Au terme de l’étude, les taux de ponte, d’éclosions et l’indice de consommation ont été respectivement de 30,52%, 58,70% et 8,37% pour le régime expérimental, contre 29,67%, 53,81% et 8,52% pour le témoin. Les marges brutes ont été de 4807,82 FCFA pour le régime expérimental et de 4818,89 FCFA pour le témoin. Cependant, aucune différence significative n’est observée entre les paramètres mesurés (P>0,05). Ceci suggère que les graines torréfiées de niébé peuvent être incorporées dans l’alimentation des poules locales en ponte, comme source de protéines.Mots clés : Poule locale en ponte, graines de Vigna unguiculata, source de protéines, performances zootechniques, rentabilité économique

    Le phosphore limite-t-il la production intensive du riz dans la plaine de Bagré au Burkina Faso ?

    Le phosphore (P) est un élément important dans la nutrition minérale du riz et dans l’élaboration du rendement paddy. Des essais soustractifs ont été conduits en milieu paysan dans la plaine rizicole de Bagré au cours de trois campagnes successives pour évaluer l’effet de l’application de P sur le rendement paddy. La capacité nutritive du sol en phosphore a varié entre 7 et 15 kg P ha-1. L’application de P a augmenté l’efficacité physiologique interne de P qui a varié entre 211 et 290 kg paddy kg-1 P (moyenne 249 kg kg-1). L’efficacité agronomique de P a été de 33,7 kg paddy kg–1 de P appliqué. L’augmentation de rendement due à l’application de phosphore a été de 19%. En moyenne, 70% du phosphore total absorbé est contenu dans les grains. L’évolution du rendement paddy indique que P est le second facteur limitant dans la plaine de Bagré après l’azote. L’application de P_engrais à une dose moyenne de 30 kg P ha-1 était suffisante pour compenser les pertes dues aux exportations. Le phosphore limite la production de riz à Bagré mais son application comme engrais contribue à améliorer les rendements paddy et à compenser les pertes de P du système.Mots clés : Capacité nutritive des sols, rizières, efficacité d’utilisation de nutriments, bilan minéral

    Postpartum family-planning barriers and catalysts in Burkina Faso and the Democratic Republic of Congo: a multiperspective study.

    PURPOSE: To address the high unmet need for postpartum family planning (PPFP) in resource-limited settings, particularly in sub-Saharan Africa, this study aimed to identify PPFP-related barriers and catalysts to inform policies and programs to increase access to postpartum contraception. METHODS: Using qualitative methodology, we explored the perspectives of women, adolescent girls, men, religious and community leaders, service providers, and decision makers from three rural communities in Burkina Faso and three rural-urban communities in the Democratic Republic of Congo. Both countries have high unmet need for PPFP and are priority countries of the French Muskoka Fund for Maternal and Child Health. RESULTS: Perceived catalysts included negative traditional views on the consequences borne by closely spaced children and their mothers; a 6-week postpartum visit dedicated to PPFP (albeit poorly attended); political will and enabling policies for FP; and support from certain religious leaders and men. Main reported barriers were the lack of male engagement; out-of-pocket copayment for contraceptives; reliance on amenorrhea for pregnancy prevention without knowing its limits; misconceptions about modern contraceptives, including prerequisites for the lactational amenorrhea method; sexual abstinence supported by religious and traditional norms for up to 3-6 months, although women reported earlier resumption of sexual activity; low prioritization of scheduled postpartum visits by women; and limited availability of readily accessible methods, PPFP counseling materials, and clinic days and scheduled visits dedicated to contraceptive services. CONCLUSION: Based on results found to be most actionable, the following interventions have the potential to optimize access to PPFP services: counseling women on postpartum-pregnancy risks and options at different points in time before and after childbirth through the use of appropriate information, education, and counseling materials; integrating PPFP services into existing maternal and child health visits; making contraceptives readily available and affordable; and meaningfully engaging male partners

    Sida acuta is a shrub indigenous to pantropical regions. The plant is widely used for its various pharmacological properties. Among compounds of pharmacological interest occurring in the plant, are indoloquinoline alkaloids. The aim of the present study was to investigate the antimicrobial activity ofalkaloids of S. acuta from Burkina Faso. The alkaloids had a good antimicrobial activity against the test microorganisms. In the agar-well diffusion assay, highest inhibition zone diameters were recorded with Gram-positive bacteria. The broth microdilution assay gave minimal inhibitory concentration values ranging from 16 to 400 ìg/ml and minimal bactericidal concentration values ranging from 80 to up to 400 ìg/ml. The gas chromatography-mass spectrometry analysis of the same alkaloids led to the identification of cryptolepine and quindoline as the major components

    Acceptability of new formulations of Corn-Soy Blends and Lipid-based Nutrient Supplements in Province du Passoré, Burkina Faso

    The objective of this study was to evaluate the acceptability of new formulations of six corn-soy blended flours (CSB) and six lipid-based nutrient supplements (LNS) with different quantities of milk and qualities of soy to be used for the treatment of moderate acute malnutrition (MAM). Furthermore, we wanted to explore the acceptability of foods currently used for the prevention and treatment of malnutrition in Burkina Faso to identify possible barriers that could affect the acceptability of the new formulations of supplementary foods. The study was carried out prior to a randomized controlled trial evaluating the effectiveness of these new formulations. The study involved an observed test-meal and a three-day take-home ration of the experimental food supplements to 6-30-months-old healthy children, followed by questionnaire-based interviews about the acceptability of these supplements. Interviews and focus group discussions were carried out to explore the acceptability of foods currently used for the prevention and treatment of malnutrition. The results suggest that both LNS and CSB products with different quantities of milk and qualities of soy are equally well accepted among healthy children in rural Burkina Faso based on general appreciation of the supplements and organoleptic properties. All experimental foods received good ratings and there was no significant difference between the foods. However, after the take-home ration, 58% of participants receiving CSB reported having left-overs at the end of the day compared to 37% (n=33) of the participants receiving LNS (p=0.004), suggesting that CSB was not as readily consumed as LNS. Yet, both CSB and LNS products were perceived as easy to administer and the frequency of feeding was estimated to be adequate. The study also found that similar foods, used for the prevention and treatment of malnutrition, were well appreciated in the study location. LNS were to a higher degree associated with medicine or foods with medicinal properties, but both LNS and CSB were perceived as beneficial to child health

    From regional pulse vaccination to global disease eradication: insights from a mathematical model of Poliomyelitis

    Mass-vaccination campaigns are an important strategy in the global fight against poliomyelitis and measles. The large-scale logistics required for these mass immunisation campaigns magnifies the need for research into the effectiveness and optimal deployment of pulse vaccination. In order to better understand this control strategy, we propose a mathematical model accounting for the disease dynamics in connected regions, incorporating seasonality, environmental reservoirs and independent periodic pulse vaccination schedules in each region. The effective reproduction number, ReR_e, is defined and proved to be a global threshold for persistence of the disease. Analytical and numerical calculations show the importance of synchronising the pulse vaccinations in connected regions and the timing of the pulses with respect to the pathogen circulation seasonality. Our results indicate that it may be crucial for mass-vaccination programs, such as national immunisation days, to be synchronised across different regions. In addition, simulations show that a migration imbalance can increase ReR_e and alter how pulse vaccination should be optimally distributed among the patches, similar to results found with constant-rate vaccination.
