    PERCEPTIONS AND KNOWLEDGE OF A SELECTED GROUP OF ORGANIC CONSUMERS IN BLOEMFONTEIN, FREE STATE PROVINCE

    Published ArticleOrganic agriculture is a production method that upholds the well-being of soil, ecosystems and people. It relies on ecological processes, biodiversity and cycles tailored to local conditions. This descriptive study assessed the knowledge and perceptions of organic food consumers in Bloemfontein. A convenience sample of 60 consumers (male and female) was selected to participate in the study. A structured questionnaire was designed to obtain data at a local fresh-produce grocer. Most of the consumers showed positive and negative attitudes towards organic products. Most consumers had basic knowledge regarding organic products, but it also became evident that there was uncertainty surrounding facts of organic products, such as labelling standards. Only 12% of respondents interpreted the term “organic” correctly. Human health (86.67%) and environmental concerns (28.33%) were the key factors influencing consumer preference for organic food. Organic fruit and vegetables (45.37%) were the most popular purchased organic items amongst consumers. The majority of consumers (61.67%) stated that organic products are affordable. The results of the present study provided valuable insight into the knowledge, perceptions and attitudes of consumers towards organically grown foods. The general positive attitude of consumers can be attributed to consumers' basic knowledge of, and preference for, organic food. Despite the organic lifestyle being in the minority in this region, there continues to be a growing demand for organic products in Bloemfontein, and for this reason producers have ensured a continued supply of organic products

    A semi-quantitative survey of macroinvertebrates at selected sites to evaluate the ecosystem health of the Olifants River

    This study was conducted to evaluate the ecosystem health of the Olifants River by means of semi-quantitative surveys of the macroinvertebrates at 7 selected sites in the catchment. These surveys were performed during the high- and low-flow seasons for 2 consecutive years. Macroinvertebrates were collected by using a net consisting of a 30 cm square steel frame with a sturdy handle, to which a Perlon gauze net with a mesh of 1 mm was attached. Semi-quantitative surveys were done by sampling the vegetation, as well as the substratum, with the net at each site for approximately 15 min. The pH, water temperature and conductivity were measured in situ at each site during the different surveys. Samples were fixed and preserved in 90% ethanol and thereafter sorted, identified up to family level and counted. The specimens were categorised as tolerant, moderately sensitive or highly sensitive, according to the guidelines set by the South African Scoring System Version 5 (SASS5). Although a total of 95 taxa were recovered during this study, only 7 of these taxa were categorised as highly sensitive, it can be concluded that the water of the Olifants River is in a poor state of health as revealed by the macroinvertebrate assemblages.Keywords: Olifants River, macroinvertebrates, river healt

    A comparison of mollusc diversity between the relatively pristine Marico River and the impacted Crocodile River, two major tributaries of the Limpopo River, South Africa

    A study of the freshwater mollusc diversity was conducted at selected sites in the relatively pristine  Marico River and the impacted Crocodile River, the major tributaries of the Limpopo River. Four  surveys were conducted, two in an early (May 2013 and 2014) and two in a late (November 2013 and 2014) low-flow period. Semi-quantitative surveys were done by sampling the vegetation, as well as the substratum, with a standard SASS net for approximately 15 min each. Environmental parameters including water temperature, electrical conductivity (EC) and pH were measured at each site. Molluscs were identified up to species level, sorted, counted, and the presence of juvenile specimens recorded. Historical data for the 1/16th degree square grids (loci), in which each of the sampling sites of the current study was located, were extracted from the National Freshwater Snail Collection at the Unit for Environmental Sciences and Management, Potchefstroom Campus of the North-West University. During this study, 20 and 9 species were recovered from the Marico and Crocodile Rivers, respectively, as compared to 13 and 12 species on record for these loci, respectively. Juvenile specimens were present during the four surveys at most of the sites. Canonical correspondence analyses were applied which revealed that biotopes, water temperature and EC played the most significant role in the distribution and abundance of species. The relatively high mollusc diversity and the fact that juveniles were present throughout the study, demonstrated that current habitat and environmental conditions were suitable to promote recruitment and the sustainability of diverse mollusc populations in the Marico River and its tributaries. However, in contrast to this, the exploitation of and habitat transformation in the Crocodile River has resulted in the decrease of biotopes which eventually could have led to the  decrease in diversity and the establishment of P. acuta, an exotic invader species.Keywords: freshwater molluscs, biodiversity, Marico River, Crocodile River, environmental variable

    Congenital anomalies in black South African liveborn neonates at an urban academic hospital

    Study objective. The aim was to study the spectrum of clinical problems and outcomes in infants born at an urban academic hospital. In consequence, as part of the overall study, the incidence of congenital anomalies and the outcomes of affected infants were recorded.Design. This was a prospective, hospital-based study, undertaken on liveborn infants born over a 3-year period, 1 May 1986 to 30 April 1989.Setting. Kalafong Hospital, Pretoria.Main results. A total of 17 351 liveborn infants was examined and the total congenital anomalies incidence was 11 ,87 per 1 000 Iivebirths. The central nervous system was the system most frequently involved (2,30 per 1 000 livebirths), followed by the musculoskeletal system (2,13 per 1 000 livebirths). The commonest individual congenital anomaly was Down syndrome (1,33 per 1 000 Iivebirths), followed by neural tube defects (0,99 per 1 000 livebirths) and ventricular septal defects (0,69 per 1 000 livebirths). In 11 % (2,25 per 1 000 livebirths) of neonatal deaths, infant loss was attributable to congenital anomalies.Conclusions. The incidence of congenital anomalies in black South African neonates, born in an urban setting, is as high as in other First- and Third-World countries, and the incidence of some individual congenital anomalies is higher. This study indicates the need for further research and the establishment of prenatal, genetics and paediatric facilities to manage these problems

    Comparing gene expression profiles of adults with isolated spinal tuberculosis to disseminated spinal tuberculosis identified by 18^{18}FDG-PET/CT at time of diagnosis, 6- and 12-months follow-up: classifying clinical stages of spinal tuberculosis and monitoring treatment response (Spinal TB X cohort study)

    BACKGROUND: Tuberculosis (TB) is one of the top ten causes of death worldwide, with approximately 10 million cases annually. Focus has been on pulmonary TB, while extrapulmonary TB (EPTB) has received little attention. Diagnosis of EPTB remains challenging due to the invasive procedures required for sample collection. Spinal TB (STB) accounts for 10% of EPTB and often leads to lifelong debilitating disease due to devastating spinal deformation and compression of neural structures. Little is known about the extent of disease, although both isolated STB and a disseminated form of STB have been described. In our Spinal TB X cohort study, we aim to describe the clinical phenotype of STB using whole-body 18^{18}FDG-PET/CT, identify a specific gene expression profile for different stages of dissemination and compare findings to previously described gene expression signatures for latent and active pulmonary TB. METHODS: A single-centre, prospective cohort study will be established to describe the distributional pattern of STB detected by whole-body 18^{18}FDG-PET/CT and gene expression profile of patients with suspected STB on magnetic resonance imaging (MRI) at point of diagnosis, six months, and 12 months. Blood biobanking will be performed at these time points. Specimens for microbiology will be obtained from sputum/urine, from easily accessible sites of disease (e.g., lymph nodes, abscess) identified in the first 18^{18}FDG-PET/CT, from CT-guided biopsy and/or surgery. Clinical parameters and functional scores will be collected at every physical visit. Data will be entered into RedCap® database; data cleaning, validation and analysis will be performed by the study team. The University of Cape Town Ethics Committee approved the protocol (243/2022). DISCUSSION: The Spinal TB X cohort study is the first prospective cohort study using whole-body 18FDG-PET/CT scans in patients with microbiologically confirmed spinal tuberculosis. Dual imaging techniques of the spine using 18^{18}FDG-PET/CT and magnetic resonance imaging as well as tissue diagnosis (microbiology and histopathology) will allow us to develop a virtual biopsy model. If successful, a distinct gene-expression profile will aid in blood-based diagnosis (point of care testing) as well as treatment monitoring and would lead to earlier diagnosis of this devastating disease

    The ADIPS pilot national diabetes in pregnancy benchmarking programme

    Background: To test the feasibility of benchmarking the care of women with pregnancies complicated by hyperglycaemia. Methods: A retrospective audit of volunteer diabetes services in Australia and New Zealand involving singleton pregnancies resulting in live births between 2014 and 2020. Ranges are shown and compared across services. Results: The audit included 10,144 pregnancies (gestational diabetes mellitus (GDM) = 8696; type 1 diabetes (T1D) = 435; type 2 diabetes (T2D) = 1013) from 11 diabetes services. Among women with GDM, diet alone was used in 39.4% (ranging among centres from 28.8-57.3%), metformin alone in 18.8% (0.4-43.7%), and metformin and insulin in 10.1% (1.5-23.4%); when compared between sites, all p 6.5% (48 mmol/mol)), 78.4% and 54.6%, respectively (p < 0.001). Conclusion: Management of maternal hyperglycaemia and pregnancy outcomes varied significantly. The maintenance and extension of this benchmarking service provides opportunities to identify policy and clinical approaches to improve pregnancy outcomes among women with hyperglycaemia in pregnancy

    Outcomes of laboratory-confirmed SARS-CoV-2 infection during resurgence driven by Omicron lineages BA.4 and BA.5 compared with previous waves in the Western Cape Province, South Africa

    OBJECTIVE: We aimed to compare clinical severity of Omicron BA.4/BA.5 infection with BA.1 and earlier variant infections among laboratory-confirmed SARS-CoV-2 cases in the Western Cape, South Africa, using timing of infection to infer the lineage/variant causing infection. METHODS: We included public sector patients aged ≥20 years with laboratory-confirmed COVID-19 between 1-21 May 2022 (BA.4/BA.5 wave) and equivalent prior wave periods. We compared the risk between waves of (i) death and (ii) severe hospitalization/death (all within 21 days of diagnosis) using Cox regression adjusted for demographics, comorbidities, admission pressure, vaccination and prior infection. RESULTS: Among 3,793 patients from the BA.4/BA.5 wave and 190,836 patients from previous waves the risk of severe hospitalization/death was similar in the BA.4/BA.5 and BA.1 waves (adjusted hazard ratio (aHR) 1.12; 95% confidence interval (CI) 0.93; 1.34). Both Omicron waves had lower risk of severe outcomes than previous waves. Prior infection (aHR 0.29, 95% CI 0.24; 0.36) and vaccination (aHR 0.17; 95% CI 0.07; 0.40 for at least 3 doses vs. no vaccine) were protective. CONCLUSION: Disease severity was similar amongst diagnosed COVID-19 cases in the BA.4/BA.5 and BA.1 periods in the context of growing immunity against SARS-CoV-2 due to prior infection and vaccination, both of which were strongly protective

    The prevalence of nutritional anemia in pregnancy in an east Anatolian province, Turkey

    <p>Abstract</p> <p>Background</p> <p>Anemia is considered a severe public health problem by World Health Organization when anemia prevalence is equal to or greater than 40% in the population. The purpose of this study was to determine the anemia prevalence with the associated factors in pregnant women and to determine the serum iron, folate and B12 vitamin status in anaemic pregnants in Malatya province.</p> <p>Methods</p> <p>This is a cross-sectional survey. A multi-sage stratified probability-proportional-to-size cluster sampling methodology was used. A total of 823 pregnant women from sixty clusters were studied. Women were administered a questionnaire related with the subject and blood samples were drawn. Total blood count was performed within four hours and serum iron, folate and B12 vitamin were studied after storing sera at -20 C for six months.</p> <p>Results</p> <p>Anemia prevalence was 27.1% (Hb < 11.0 gr/dl). Having four or more living children (OR = 2.2), being at the third trimester (OR = 2.3) and having a low family income (OR = 1.6) were determined as the independent predictors of anemia in pregnancy. Anemia was also associated with soil eating (PICA) in the univariate analysis (p < 0.05). Of anaemic women, 50.0% had a transferrin saturation less than 10% indicating iron deficiency, 34.5% were deficient in B12 vitamin and 71.7% were deficient in folate. Most of the anemias were normocytic-normochromic (56.5%) indicating mixed anemia.</p> <p>Conclusions</p> <p>In Malatya, for pregnant women anemia was a moderate public health problem. Coexisting of iron, folate and B vitamin deficiencies was observed among anaemics. To continue anemia control strategies with reasonable care and diligence was recommended.</p
