Study of a novel range-dependent propogation effect with application to the axial injection of signals from the Kaneohe source
A novel range-dependent propagation effect occurs when a source is placed on the seafloor in shallow water with a downward refracting sound speed profile, and sound waves propagate down a slope into deep water. Under these conditions, small grazing-angle sound waves slide along the bottom downward and outward from the source until they reach the depth of the sound channel axis in deep water, where they are detached from the sloping bottom and continue to propagate outward near the sound channel axis. This mudslide effect is one of a few robust and predictable acoustic propagation effects that occur in range-dependent ocean environments. As a consequence of this effect, a bottom mounted source in shallow water can inject a significant amount of acoustic energy into the axis of the deep ocean sound channel that can then propagate to very long ranges. Numerical simulations with a full-wave range-dependent acoustic model show that the Kaneohe experiment had the appropriate source, bathymetry, and sound speed profiles that allows this effect to operate efficiently. This supports the interpretation that some of the near-axial acoustic signals, received near the coast of California from the bottom mounted source located in shallow water in Kaneohe Bay, Oahu, Hawaii, were injected into the sound channel of the deep Pacific Ocean by this mechanism. Effect of pegloticase on renal function in patients with chronic kidney disease: a post hoc subgroup analysis of 2 randomized, placebo-controlled, phase 3 clinical trials
BACKGROUND: Pegloticase is approved in the US for treatment of refractory chronic gout. Since chronic kidney disease (CKD) is common in these patients, we conducted a post-hoc analysis of 2 replicate phase 3 trials and the subsequent open-label extension study to determine the effects of pegloticase on renal function in patients with CKD stages 3 and 4, as well as the effects of renal dysfunction on pegloticase efficacy and safety.
FINDINGS: Patients with renal insufficiency were randomized to pegloticase 8 mg every 2 weeks (n = 42), pegloticase 8 mg every 4 weeks (n = 41), or placebo (n = 20) for 6 months as defined by the study protocols. Renal function was assessed by estimated glomerular filtration rate (eGFR). All patients completing the randomized trials could participate in an open-label extension study for a further 2.5 years. Uric acid response, the primary end point in the trials, was plasma uric acid \u3c 6.0 mg/dl for 80% of months 3 and 6.Mean eGFR in both pegloticase dosing cohorts remained constant over the randomized treatment phase and long-term open-label extension study. The number of patients achieving uric acid response was similar across CKD stages (32% stage 1, 23% stage 2, 35% stage 3, and 39% stage 4, respectively, P = 0.3). There was no difference in the pegloticase safety profile based on CKD stage.
CONCLUSIONS: Pegloticase treatment does not impact eGFR in CKD patients and response to pegloticase is independent of CKD stage.

Temperature and Emission-Measure Profiles Along Long-Lived Solar Coronal Loops Observed with TRACE
We report an initial study of temperature and emission measure distributions
along four steady loops observed with the Transition Region and Coronal
Explorer (TRACE) at the limb of the Sun. The temperature diagnostic is the
filter ratio of the extreme-ultraviolet 171-angstrom and 195-angstrom
passbands. The emission measure diagnostic is the count rate in the
171-angstrom passband. We find essentially no temperature variation along the
loops. We compare the observed loop structure with theoretical isothermal and

Transversality of Electromagnetic Waves in the Calculus-Based Introductory Physics Course
Introductory calculus-based physics textbooks state that electromagnetic
waves are transverse and list many of their properties, but most such textbooks
do not bring forth arguments why this is so. Both physical and theoretical
arguments are at a level appropriate for students of courses based on such
books, and could be readily used by instructors of such courses. Here, we
discuss two physical arguments (based on polarization experiments and on lack
of monopole electromagnetic radiation), and the full argument for the
transversality of (plane) electromagnetic waves based on the integral Maxwell
equations. We also show, at a level appropriate for the introductory course,
why the electric and magnetic fields in a wave are in phase and the relation of

Relevance of Interleukin-6 and D-Dimer for Serious Non-AIDS Morbidity and Death among HIV-Positive Adults on Suppressive Antiretroviral Therapy
Background:
Despite effective antiretroviral treatment (ART), HIV-positive individuals are at increased risk of serious non-AIDS conditions (cardiovascular, liver and renal disease, and cancers), perhaps due in part to ongoing inflammation and/or coagulation. To estimate the potential risk reduction in serious non-AIDS conditions or death from any cause that might be achieved with treatments that reduce inflammation and/or coagulation, we examined associations of interleukin-6 (IL-6), D-dimer, and high-sensitivity C-reactive protein (hsCRP) levels with serious non-AIDS conditions or death in 3 large cohorts.
Methods:
In HIV-positive adults on suppressive ART, associations of IL-6, D-dimer, and hsCRP levels at study entry with serious non-AIDS conditions or death were studied using Cox regression. Hazard ratios (HR) adjusted for age, gender, study, and regression dilution bias (due to within-person biomarker variability) were used to predict risk reductions in serious non-AIDS conditions or death associated with lower “usual” levels of IL-6 and D-dimer.
Results:
Over 4.9 years of mean follow-up, 260 of the 3766 participants experienced serious non-AIDS conditions or death. IL-6, D-dimer and hsCRP were each individually associated with risk of serious non-AIDS conditions or death, HR = 1.45 (95% CI: 1.30 to 1.63), 1.28 (95% CI: 1.14 to 1.44), and 1.17 (95% CI: 1.09 to 1.26) per 2x higher biomarker levels, respectively. In joint models, IL-6 and D-dimer were independently associated with serious non-AIDS conditions or death, with consistent results across the 3 cohorts and across serious non-AIDS event types. The association of IL-6 and D-dimer with serious non-AIDS conditions or death was graded and persisted throughout follow-up. For 25% lower “usual” IL-6 and D-dimer levels, the joint biomarker model estimates a 37% reduction (95% CI: 28 to 46%) in the risk of serious non-AIDS conditions or death if the relationship is causal.
Conclusions:
Both IL-6 and D-dimer are independently associated with serious non-AIDS conditions or death among HIV-positive adults with suppressed virus. This suggests that treatments that reduce IL-6 and D-dimer levels might substantially decrease morbidity and mortality in patients on suppressive ART. Clinical trials are needed to test this hypothesis
Screening and treatment of hypertension in older adults: Less is more?
Screening and treatment of hypertension is a cornerstone of cardiovascular disease (CVD) prevention. Hypertension causes a large proportion of cases of stroke, coronary heart disease, heart failure, and associated disability and is highly prevalent especially among older adults. On the one hand, there is robust evidence that screening and treatment of hypertension prevents CVD and decreases mortality in the middle-aged population. On the other hand, among older adults, observational studies have shown either positive, negative, or no correlation between blood pressure (BP) and cardiovascular outcomes. Furthermore, there is a lack of high quality evidence for a favorable harm-benefit balance of antihypertensive treatment among older adults, especially among the oldest-old (i.e., above the age of 80 years), because very few trials have been conducted in this population. The optimal target BP may be higher among older treated hypertensive patients than among middle-aged. In addition, among frail or multimorbid older individuals, a relatively low BP may be associated with worse outcomes, and antihypertensive treatment may cause more harm than benefit. To guide hypertension screening and treatment recommendations among older patients, additional studies are needed to determine the most efficient screening strategies, to evaluate the effect of lowering BP on CVD risk and on mortality, to determine the optimal target BP, and to better understand the relationship between BP, frailty, multimorbidity, and health outcomes
Force-free twisted magnetospheres of neutron stars
The X-ray spectra observed in the persistent emission of magnetars are
evidence for the existence of a magnetosphere. The high-energy part of the
spectra is explained by resonant cyclotron upscattering of soft thermal photons
in a twisted magnetosphere, which has motivated an increasing number of efforts
to improve and generalize existing magnetosphere models. We want to build more
general configurations of twisted, force-free magnetospheres as a first step to
understanding the role played by the magnetic field geometry in the observed
spectra. First we reviewed and extended previous analytical works to assess the
viability and limitations of semi-analytical approaches. Second, we built a
numerical code able to relax an initial configuration of a nonrotating
magnetosphere to a force-free geometry, provided any arbitrary form of the
magnetic field at the star surface. The numerical code is based on a
finite-difference time-domain, divergence-free, and conservative scheme, based
of the magneto-frictional method used in other scenarios. We obtain new
numerical configurations of twisted magnetospheres, with distributions of twist
and currents that differ from previous analytical solutions. The range of
global twist of the new family of solutions is similar to the existing
semi-analytical models (up to some radians), but the achieved geometry may be
quite different. The geometry of twisted, force-free magnetospheres shows a
wider variety of possibilities than previously considered. This has
implications for the observed spectra and opens the possibility of implementing
alternative models in simulations of radiative transfer aiming at providing
