Long-term outcome in 324 polytrauma patients: what factors are associated with posttraumatic stress disorder and depressive disorder symptoms?
Background: Physical impairment is well-known to last for many years after a severe injury, and there is a high impact on the quality of the survivor's life. The purpose of this study was to examine if this is also true for psychological impairment with symptoms of posttraumatic stress disorder or depression after polytrauma. Design: Retrospective cohort outcome study. Setting: Level I trauma centre. Population: 637 polytrauma trauma patients who were treated at our Level I trauma centre between 1973 and 1990. Minimum follow-up was 10 years after the injury. Methods: Patients were asked to fill in a questionnaire, including parts of the Posttraumatic Stress Diagnostic Scale, the Impact of Event Scale-Revised and the German Hospital Anxiety and Depression Scale, to evaluate mental health. Clinical outcome was assessed before by standardised scores. Results: Three hundred and twenty-four questionnaires were evaluated. One hundred and forty-nine (45.9%) patients presented with symptoms of mental impairment. Quality of life was significantly higher in the mentally healthy group, while the impaired group achieved a lower rehabilitation status. Conclusions: Mental impairment can be found in multiple trauma victims, even after 10 years or more. Treating physicians should not only focus on early physical rehabilitation but also focus on early mental rehabilitation to prevent long-term problems in both physical and mental disability
Fexinidazole â A New Oral Nitroimidazole Drug Candidate Entering Clinical Development for the Treatment of Sleeping Sickness
This article describes the preclinical profile of fexinidazole, a new drug candidate with the potential to become a novel, oral, safe and effective short-course treatment for curing both stage 1 and 2 human African trypanosomiasis and replace the old and highly problematic treatment modalities available today. Fexinidazole is orally available and rapidly metabolized in two metabolites having equivalent biological activity to the parent and contributing significantly to the in vivo efficacy in animal models of both stage 1 and 2 HAT. Animal toxicology studies indicate that fexinidazole has an excellent safety profile, with no particular issues identified. Fexinidazole is a 5-nitroimidazole and, whilst it is Ames-positive, it is devoid of any genetic toxicity in mammalian cells and therefore does not pose a genotoxic risk for use in man. Fexinidazole, which was rediscovered through a process of compound mining, is the first new drug candidate for stage 2 HAT having entered clinical trials in thirty years, and has the potential to revolutionize therapy of this fatal disease at a cost that is acceptable in the endemic regions
Assessment of microRNA-related SNP effects in the 3âČ untranslated region of the IL22RA2 risk locus in multiple sclerosis
Abstract Recent large-scale association studies have identified over 100 MS risk loci. One of these MS risk variants is single-nucleotide polymorphism (SNP) rs17066096, located 14 kb downstream of IL22RA2. IL22RA2 represents a compelling MS candidate gene due to the role of IL-22 in autoimmunity; however, rs17066096 does not map into any known functional element. We assessed whether rs17066096 or a nearby proxy SNP may exert pathogenic effects by affecting microRNA-to-mRNA binding and thus IL22RA2 expression using comprehensive in silico predictions, in vitro reporter assays, and genotyping experiments in 6,722 individuals. In silico screening identified two predicted microRNA binding sites in the 3âČUTR of IL22RA2 (for hsa-miR-2278 and hsamiR-411-5p) encompassing a SNP (rs28366) in moderate linkage disequilibrium with rs17066096 (r 2 =0.4). The binding of both microRNAs to the IL22RA2 3âČUTR was confirmed in vitro, but their binding affinities were not significantly affected by rs28366. Mit dem vorgestellten Verfahren, das am Berliner Zentrum fĂŒr Mechatronische
Medizintechnik (BZMM) entwickelt wurde, ist eine prÀoperative
Implantatherstellung anhand von Vorgaben des Chirurgen möglich. Eine
gleichzeitig erstellte passende Osteotomielinie wird intraoperativ navigiert
ĂŒbertragen, sodass die Defektdeckung mittels Implantat bereits im Rahmen der
Tumorentfernung erfolgt. Ziel dieser Arbeit ist es, ein Verfahren zu
untersuchen, dass die BerĂŒcksichtigung des zu erwartenden Gesamtfehlers der
Navigation bereits in der Planung der Osteotomie ermöglicht. Dazu wurde eine
geplante Osteotomielinie um verschiedene SicherheitsabstÀnde (0 mm = Linie 0,5
= originale Osteotomielinie; 1 mm = Linie 1,0; 2 mm = Linie 2,0; 3 mm = Linie
3,0; 4 mm = Linie 4,0) erweitert. Die entstandenen 5 Schnittlinien wurden
mittels des implementierten Pointers des optoelektronischen Navigationssystems
auf je 5 gleichartige Modelle ĂŒbertragen. Die Registrierung wurde mit 3
knocheninvasiven Markern nach dem Prinzip der referenzbasierten
Punktpaartransformation durchgefĂŒhrt. Die Osteotomie erfolgte mit einer
LindemannfrÀse unter Duraschutz. Auf 25 Modelle wurden insgesamt 1395 Punkte,
durchschnittlich 56 je Modell, ĂŒbertragen. Der mittlere Abstand zur
verwendeten Osteotomielinie betrÀgt 1,07 ± 0,86 mm. Nach Resektion wurden auf
den ResektionsrÀndern insgesamt 1539 Punkte erfasst, durchschnittlich 64 je
Modell. Der mittlere Abstand zur verwendeten Osteotomielinie betrÀgt 1 ± 0,91
mm. Der mittlere Abstand zum Implantatrand betrÀgt bei Linie 0,5 0,86 ± 0,77
mm, bei Linie 1,0 1,47 ± 1,19 mm, bei Linie 2,0 1,78 ± 1,13 mm, bei Linie 3,0
2,86 ± 1,49 mm und bei Linie 4,0 3,97 ± 1,58 mm. Das Optimum ist eine Linie,
die ein korrekturfreies Einpassen des Implantats mit möglichst ge-ringem
Abstand zum Implantatrand ermöglicht. Die besten Resultate zeigte
dementsprechend Linie 2,0. Der Abstand zwischen Resektions- und Implantatrand
liegt im geforderten Bereich (< 3 mm) und erlaubt eine gute knöcherne
Einheilung des Implantats. Der zusÀtzliche Sicherheitsabstand von 2 mm
kompensiert den Gesamtfehler am effektivsten und ermöglicht eine
korrekturfreie Implantatinsertion. Durch die hohe Ăbertragungsgenauigkeit
konnte in dieser Arbeit weiterhin die Eignung des vorgestellten Verfahrens zur
Implantatgenerierung und OsteotomielinienÂŹĂŒbertragung gezeigt werden.With the presented method, developed at the Berliner Zentrum fĂŒr
Mechatronische Medizintechnik (BZMM), a preoperative custom implant production
is possible. A corresponding osteotomy line can be transferred intraoperative
with the help of the navigation system, so that a single step tumor resection
and implantation is feasible. The aim of the following paper is to analyse a
method, that considers the expected inaccuracy of a navigation system
preoperatively. A planed osteotomy line has been modified with several safety
gaps (0 mm = line 0,5 = original osteotomy line; 1 mm = line 1,0; 2 mm = line
2,0; 3 mm = line 3,0; 4 mm = line 4,0). Each of the 5 different lines have
been transferred to 5 equal models with the implemented pointer of an
optoelectronic navigation system. Registration has been done with 3 invasive
markers. The osteotomy has been done with a Lindemann fraise with dura
protection. 1395 points have been transferred to 25 models, 56 on average. The
mean distance to the used osteotomy line is 1,07 ± 0,86 mm. After resection
1539 point have been captured, 64 on average. The mean distance to the used
osteotomy line is 1 ± 0,91 mm The mean distance to the implant edge is for
line 0,5 0,86 ± 0,77 mm, for line 1,0 1,47 ± 1,19 mm, for line 2,0 1,78 ± 1,13
mm, for line 3,0 2,86 ± 1,49 mm and for line 4,0 3,97 ± 1,58 mm. The optimum
is a line, that allows an implantation free of corrections with a distance
between implant edge and resection as small as possible. The best results
showed the line 2,0. The distance between implant edge and resection is in the
requested range (< 3 mm) and allows a good bony healing of the implant. The
additional safety gap of 2 mm compensates the aggregated error best and allows
an implant insertion free of corrections. Because of the high transfer
accuracy the suitability of the presented method for implant modeling and
osteotomy line transfer could be shown
