    Professional appraisal of online information about children’s footwear measurement and fit : readability, usability and quality

    Parents increasingly use the internet to seek health information, share information and for purchasing textiles and footwear. This shift in footwear purchasing habits raises concern about how (and if) parents are getting their children's feet measured, and what support strategies are in place to support the fit of footwear. In response to this, some companies and healthcare organisations have developed resources to support home measurement of foot size, and link these measures to footwear selection, measurement and fitting. The aim of this research was to undertake an appraisal of web-based resources about measurement and fit of children's footwear, focussing specifically on readability, usability and quality. Search terms relating to children's foot measurement were compiled and online searching was undertaken. Search results were saved and screened for relevance. Existing resources were categorised based on their source e.g. a footwear company or a health website. The 15 most commonly identified resources were reviewed by a professional panel for readability, content, usability and validity. One researcher also assessed the accessibility and reading ease of the resources. Online resources were predominantly from commercial footwear companies (54%). Health information sources from professional bodies made up 4.2% of the resources identified. The top 15 resources had appropriate reading ease scores for parents (SMOG Index 4.3-8.2). Accessibility scores (the product of the number of times it appeared in search results and its ranking in the results) were highest for commercial footwear companies. The panel scores for readability ranged from 2.7 to 9 out of 10, with a similar range for content, usability and validity. Information for parents seeking to purchase footwear for their children is readily available online but this was largely dominated by commercial footwear companies. The quality and usability of this information is of a moderate standard; notable improvements could be made to the validity of the task the child is asked to undertake and the measures being taken. Improvements in these resources would improve the data input to the selection of footwear and therefore have a beneficial impact on footwear fit in children.     Accelerating off-lattice kinetic Monte Carlo simulations to predict hydrogen vacancy-cluster interactions in α–Fe

    We present an enhanced off-lattice kinetic Monte Carlo (OLKMC) model, based on a new method for tolerant classification of atomistic local-environments that is invariant under Euclidean-transformations and permutations of atoms. Our method ensures that environments within a norm-based tolerance are classified as equivalent. During OLKMC simulations, our method guarantees to elide the maximum number of redundant saddle-point searches in symmetrically equivalent local-environments. Hence, we are able to study the trapping/detrapping of hydrogen from up to five-vacancy clusters and simultaneously the effect hydrogen has on the diffusivity of these clusters. These processes occur at vastly different timescales at room temperature in body-centred cubic iron. We predict the diffusion pathways of clusters/complexes without a priori assumptions of their mechanisms, not only reproducing previously reported mechanisms but also discovering new ones for larger complexes. We detail the hydrogen-induced changes in the clusters’ diffusion mechanisms and find evidence that, in contrast to mono-vacancies, the introduction of hydrogen to larger clusters can increase their diffusivity. We compare the effective hydrogen diffusivity to Oriani’s classical theory of trapping, finding general agreement and some evidence that hydrogen may not always be in equilibrium with traps, when the traps are mobile. Finally, we compute the trapping atmosphere of meta-stable states surrounding non-point traps, opening new avenues to better understand and predict hydrogen embrittlement in complex alloys

    International approaches to paediatric podiatry curricula : it's the same, but different

    Pre-registration / entry-level programmes of study provide the core knowledge, skills and abilities required for clinical practice. These programmes are where students are introduced to specialist domains of practice and begin to shape their professional interests. The aim of this research was to describe paediatric curricula within pre-registration and entry level podiatry programmes across comparable universities and offer a contemporary synthesis of international practices. An exploratory, cross-sectional, online survey was undertaken across a three-month period. Representatives from podiatry programmes delivering pre-registration or entry level podiatry degrees in which graduates are eligible for Professional and Statutory Body registration within their country (deemed at a Bachelor degree or higher), were invited to participate. The survey was administered online using Online Surveys. Descriptive statistics were used to describe the data due to the exploratory nature of the research question and design. There were responses from seven (54% of 13) universities in the United Kingdom (UK), nine (100% of nine) universities in Australia and four (50% of eight) of the invited universities external to the UK and Australia (New Zealand, Malta, Ireland, South Africa). There was some variation in curriculum content, but all universities reported to cover ontogeny and developmental milestones and general paediatric orthopaedic conditions. There was further discrepancy with the number of hours dedicated to paediatric podiatry within the curricula (ranging from  26 h). The findings from this study highlight some disparity in the delivery of training for students relating to paediatrics. The data suggests that there is a need for international coordination in establishing priorities for the paediatric curricula. This will ensure consistency in baseline knowledge, modes of training, amount and nature of curriculum delivery during undergraduate or entry level podiatry training

    Prolonged occupational standing : the impact of time and footwear

    Prolonged occupational standing is strongly related to musculoskeletal disorders. Despite being the main external load bearing interface between the floor and musculoskeletal system, and easy to manipulate, footwear is often ignored as an intervention for standing related disorders. This study aimed to record the effect of prolonged standing on the body and determine the impact of footwear material. Two pairs of surgical clogs were tested, varying only in material hardness. Participants (n=12) undertook 3 hours of standing in each shoe whilst they completed a repetitive series of simulated work tasks. Biomechanical and subjective data were recorded at the start, end and every 30 minutes during the three hours of standing. There was an increase in discomfort of the low back and all lower extremity regions alongside an increase in calf circumference, selected kinematic and kinetic variables, and changes in plantar pressure distribution over the three hours. Altering the footwear material impacted on low back discomfort and plantar pressures. Shoe preference varied amongst individuals, but was consistently associated with an increased medial midfoot contact area. Overall, this study has demonstrated the effect of prolonged standing on the body, has shown footwear impacts both subjective and biomechanical measures and provides information regarding individual differences and footwear preferences

    Development and evaluation of a dual density insole for people standing for long periods of time at work

    Background: Appropriate footwear is important for those who stand for prolonged periods of time at work, enabling them to remain comfortable, healthy and safe. Preferences for different footwear cushioning or hardness are often person specific and one shoe or insole will not be the choice for all. The aim of this study was to develop a range of insole options to maintain comfort during long periods of standing at work and test insole material preferences in the workplace. Methods: The study consisted of two parts. Part one evaluated 9 insoles of the same geometry that varied in hardness under 2 different plantar regions (n = 34). Insole preference, plantar pressure and selected anthropometric foot measures were taken. Three insole designs based on the most preferred options were identified from this part. In part two, these three insoles were evaluated with 22 workers immediately after trying them on (1 min) and after a working day. Foot anthropometric measures and subjective questions concerning material hardness preferences and self-reported foot characteristics were used to investigate whether either had a relationship with insole preference. Results: Part one found insole preference predominantly varied according to material hardness under the medial arch rather than the heel/forefoot. Softer material under the heel and forefoot was associated with a reduction in peak pressures in these regions (p < 0.05). The most preferred insole had lower pressures under the hallux and first metatarsal phalangeal joint, and greater pressures and contact area under the medial midfoot (p < 0.05) compared to the least preferred insole. Height and foot anthropometrics were related to insole preference. In part two, under real world conditions, insole preference changed for 65% of participants between the immediate assessment (1 min) and after a whole workday, with dorsum height related to the latter (p < 0.05). Subjective questions for self-assessed arch height and footwear feel identified 66.7% of the insole preferences after 1 day at work, compared to 36% using immediate assessment of insole preference. Conclusion: Preference for material hardness varies underneath the medial arch of the foot and is time dependent. Simple foot measures and questions about comfort can guide selection of preferred insoles

    The changing landscape of professional practice in podiatry, lessons to be learned from other professions about the barriers to change – a narrative review

    Background: The delivery of healthcare is changing and aligned with this, the podiatry profession continues to change with evidence informed practice and extending roles. As change is now a constant, this gives clinicians the opportunity to take ownership to drive that change forward. In some cases, practitioners and their teams have done so, where others have been reluctant to embrace change. It is not clear to what extent good practice is being shared, whether interventions to bring about change have been successful, or what barriers exist that have prevented change from occurring. The aim of this article is to explore the barriers to changing professional practice and what lessons podiatry can learn from other health care professions. Main body: A literature search was carried out which informed a narrative review of the findings. Eligible papers had to (1) examine the barriers to change strategies, (2) explore knowledge, attitudes and roles during change interventions, (3) explore how the patients/service users contribute to the change process (4) include studies from predominantly primary care in developed countries. Ninety-two papers were included in the final review. Four papers included change interventions involving podiatrists. The barriers influencing change were synthesised into three themes (1) the organisational context, (2) the awareness, knowledge and attitudes of the professional, (3) the patient as a service user and consumer. Conclusions: Minimal evidence exists about the barriers to changing professional practice in podiatry. However, there is substantial literature on barriers and implementation strategies aimed at changing professional practices in other health professions. Change in practice is often resisted at an organisational, professional or service user level. The limited literature about change in podiatry, a rapidly changing healthcare workforce and the wide range of contexts that podiatrists work, highlights the need to improve the ways in which podiatrists can share successful attempts to change practice

    Seasonal patterns of metals and nutrients in Phragmites australis (Cav.) Trin. ex Steudel in a constructed wetland in the west of Ireland

An understanding of the seasonal variation in the standing stock of metals and nutrients in emergent vegetation of constructed wetlands (CWs), as well as the amounts present in aboveground (AG) and belowground (BG) biomass, is crucial to their design and management. Given that biomass harvesting is a labour and time consuming operation, a paucity of information currently exists on accumulation and standing stocks in biomass in CWs, in particular in North Western European countries. To address this knowledge gap, this paper examined the seasonal variations of metals and nutrients in Phragmites australis (Cav.) Trin. ex Steudel in a CW treating municipal wastewater, with a view to identifying an optimal time for biomass harvesting of the AG vegetation. Although the AG biomass was greatest in August (1636 ± 507 g m −2 ), the maximum concentrations and accumulations of metals and nutrients occurred at different times throughout the duration of the study (April to November). Furthermore, with the exception of zinc and nitrogen, metals and nutrients measured in BG biomass ranged from 66% (phosphorus) to greater than 80% (nickel and chromium) of the AG biomass. This indicates that analysis of only the emergent shoots may significantly underestimate the metal and nutrient uptake and capacity of the plant. In order to effectively target the bulk of metals and nutrients, an AG harvest in late August or September is suggested

    The social amoeba Dictyostelium discoideum provides an excellent model for research across a broad range of disciplines within biology. The organism diverged from the plant, yeast, fungi and animal kingdoms around 1 billion years ago but retains common aspects found in these kingdoms. Dictyostelium has a low level of genetic complexity and provides a range of molecular, cellular, biochemical and developmental biology experimental techniques, enabling multidisciplinary studies to be carried out in a wide range of areas, leading to research breakthroughs. Numerous laboratories within the United Kingdom employ Dictyostelium as their core research model. This review introduces Dictyostelium and then highlights research from several leading British research laboratories, covering their distinct areas of research, the benefits of using the model, and the breakthroughs that have arisen due to the use of Dictyostelium as a tractable model system

    Midday measurements of leaf water potential and stomatal conductance are highly correlated with daily water use of Thompson Seedless grapevines

    A study was conducted to determine the relationship between midday measurements of vine water status and daily water use of grapevines measured with a weighing lysimeter. Water applications to the vines were terminated on August 24th for 9 days and again on September 14th for 22 days. Daily water use of the vines in the lysimeter (ETLYS) was approximately 40 L vine−1 (5.3 mm) prior to turning the pump off, and it decreased to 22.3 L vine−1 by September 2nd. Pre-dawn leaf water potential (ΨPD) and midday Ψl on August 24th were −0.075 and −0.76 MPa, respectively, with midday Ψl decreasing to −1.28 MPa on September 2nd. Leaf g s decreased from ~500 to ~200 mmol m−2 s−1 during the two dry-down periods. Midday measurements of g s and Ψl were significantly correlated with one another (r = 0.96) and both with ETLYS/ETo (r = ~0.9). The decreases in Ψl, g s, and ETLYS/ETo in this study were also a linear function of the decrease in volumetric soil water content. The results indicate that even modest water stress can greatly reduce grapevine water use and that short-term measures of vine water status taken at midday are a reflection of daily grapevine water us
