Causes of prehospital misinterpretations of ST elevation myocardial infarction

Objectives: To determine the causes of software misinterpretation of ST elevation myocardial infarction (STEMI) compared to clinically identified STEMI to identify opportunities to improve prehospital STEMI identification. Methods: We compared ECGs acquired from July 2011 through June 2012 using the LIFEPAK 15 on adult patients transported by the Los Angeles Fire Department. Cases included patients ≥18 years who received a prehospital ECG. Software interpretation of the ECG (STEMI or not) was compared with data in the regional EMS registry to classify the interpretation as true positive (TP), true negative (TN), false positive (FP), or false negative (FN). For cases where classification was not possible using registry data, 3 blinded cardiologists interpreted the ECG. Each discordance was subsequently reviewed to determine the likely cause of misclassification. The cardiologists independently reviewed a sample of these discordant ECGs and the causes of misclassification were updated in an iterative fashion. Results: Of 44,611 cases, 50% were male (median age 65; inter-quartile range 52–80). Cases were classified as 482 (1.1%) TP, 711 (1.6%) FP, 43371 (97.2%) TN, and 47 (0.11%) FN. Of the 711 classified as FP, 126 (18%) were considered appropriate for, though did not undergo, emergent coronary angiography, because the ECG showed definite (52 cases) or borderline (65 cases) ischemic ST elevation, a STEMI equivalent (5 cases) or ST-elevation due to vasospasm (4 cases). The sensitivity was 92.8% [95% CI 90.6, 94.7%] and the specificity 98.7% [95% CI 98.6, 98.8%]. The leading causes of FP were ECG artifact (20%), early repolarization (16%), probable pericarditis/myocarditis (13%), indeterminate (12%), left ventricular hypertrophy (8%), and right bundle branch block (5%). There were 18 additional reasons for FP interpretation (<4% each). The leading causes of FN were borderline ST-segment elevations less than the algorithm threshold (40%) and tall T waves reducing the ST/T ratio below threshold (15%). There were 11 additional reasons for FN interpretation occurring ≤3 times each. Conclusion: The leading causes of FP automated interpretation of STEMI were ECG artifact and non-ischemic causes of ST-segment elevation. FN were rare and were related to ST-segment elevation or ST/T ratio that did not meet the software algorithm threshold

Implementasi Program Wirausaha Baru Oleh Dinas Tenaga Kerja Dan Transmigrasi Dalam Mendukung Gerdu Kempling Kota Semarang Tahun 2014

The Government of Semarang through Local Regulation Number 4 of 2008 about poverty reduction in Semarang City which is an acceleration in poverty reduction efforts. The strategy called Gerdu Kempling (Integrated Health, Economy, Education, Infrastructure, and Environment ) and one of the program that is New Entrepreneur Program by Dinas Tenaga Kerja dan Transmigrasi Kota Semarang. This research was meant to find out how the implementation of New Entrepreneur Program by Dinas Tenaga Kerja Dan Transmigrasi that supports Gerdu Kempling Kota Semarang in 2014 and knowing the influence factors of this implementation. New Entrepreneur Program has been part of Gerdu Kempling starting in 2011. There are three locations in this research: Village of Bulusan, Ngadirgo and Padangsari. This research using qualitative descriptive research methods. The subject in this study consisted of eight (8) informants. The results showed that the implementation of New Entrepreneur Program are still less effective that is seen from the precision implementation aspects. The factors that influence the implementation such as the goals and basic of policy, resource policy, communication and implementation activities, the implementing agency characteristics, external conditions as well as the disposition of the implementor are still less optimal too. Based on these conclusions, the researcher recommend to the implementation agency and target of this program need high commitment and take maximal advantages for sustainable in order to achieve the purpose of this program

1 Pengaruh Mengkonsumsi Rebusan Daun Sirsak Terhadap Penurunan Nyeri Pada Penderita Gout Artritis Di Wilayah Kerja Puskesmas Pineleng

Gout artritis merupakan penyakit yang ditandai dengan nyeri yang terjadi berulang-ulang yang disebabkan adanya endapan kristal monosodium urat yang tertumpuk di dalam sendi sebagai akibat tingginya kadar asam urat di dalam darah. Mengkonsumsi rebusan daun sirsak (Anonna muricata) adalah salah satu jenis terapi nonfamakologi yang bertujuan untuk menurunkan tingkat nyeri pada penderita gout artritis karena senyawa yang terkandung dalam daun sirsak berfungsi sebagai analgetik yang mempu mengurangi nyeri gout.Tujuan penelitan ini adalah untuk menganalisis pengaruh mengkonsumsi rebusan daun sirsak terhadap penurunan nyeri pada penderita gout artritis di wilayah kerja Puskesmas Pineleng.Sampel diambil dengan menggunakan total sampling yaitu 34 orang yang memenuhi kriteria inklusi.Desain penelitian yang digunakan adalah Time Series Design dan data yang dikumpulkan dari responden menggunakan lembar observasi.Hasil penelitian uji Wilcoxon sign rank test pada hasil akhir didapatkan nilai p = 0,004 < α = 0,005 sehingga dapat diambil Kesimpulan bahwa hipotesis penelitian diterima, hal ini menunjukan bahwa ada pengaruh mengkonsumsi rebusan daun sirsak terhadap penurunan nyeri pada penderita gout artritis di wilayah kerja Puskesmas Pineleng.Saran untuk penelitian selanjutnya dapat menggunakan populasi yang lebih besar untuk hasil yang lebih akurat serta dapat mengembangkan penelitian tentang pengaruh mengkonsumsi rebusan daun sirsak terhadap variabel yang lain seperti penurunan tekanan darah pada penderita hipertensi

Temporal variability and statistics of the Strehl ratio in adaptive-optics images

We have investigated the temporal variability and statistics of the "instantaneous" Strehl ratio. The observations were carried out with the 3.63-m AEOS telescope equipped with a high-order adaptive optics system. In this paper Strehl ratio is defined as the peak intensity of a single short exposure. We have also studied the behaviour of the phase variance computed on the reconstructed wavefronts. We tested the Marechal approximation and used it to explain the observed negative skewness of the Strehl ratio distribution. The estimate of the phase variance is shown to fit a three-parameter Gamma distribution model. We show that simple scaling of the reconstructed wavefronts has a large impact on the shape of the Strehl ratio distribution.Comment: submitted to PAS

ISS Operations Cost Reductions Through Automation of Real-Time Planning Tasks

In 2007 the Johnson Space Center s Mission Operations Directorate (MOD) management team challenged their organizations to find ways to reduce the cost of operations for supporting the International Space Station (ISS) in the Mission Control Center (MCC). Each MOD organization was asked to define and execute projects that would help them attain cost reductions by 2012. The MOD Operations Division Flight Planning Branch responded to this challenge by launching several software automation projects that would allow them to greatly improve console operations and reduce ISS console staffing and intern reduce operating costs. These tasks ranged from improving the management and integration mission plan changes, to automating the uploading and downloading of information to and from the ISS and the associated ground complex tasks that required multiple decision points. The software solutions leveraged several different technologies including customized web applications and implementation of industry standard web services architecture; as well as engaging a previously TRL 4-5 technology developed by Ames Research Center (ARC) that utilized an intelligent agent-based system to manage and automate file traffic flow, archive data, and generate console logs. These projects to date have allowed the MOD Operations organization to remove one full time (7 x 24 x 365) ISS console position in 2010; with the goal of eliminating a second full time ISS console support position by 2012. The team will also reduce one long range planning console position by 2014. When complete, these Flight Planning Branch projects will account for the elimination of 3 console positions and a reduction in staffing of 11 engineering personnel (EP) for ISS

Re-evaluation of IIH as the Ideal Terrestrial Analog for Sans: Is There a Better Model to Consider?

While astronauts are returning from long duration spaceflight with multiple ocular signs that mimic those seen in terrestrial patients with elevated intracranial pressure (ICP), evidence has yet to prove a clinically significant increase in ICP during space.1 Preliminary research evidence may even suggest that ICP decreases in microgravity. Idiopathic intracranial hypertension (IIH) has long been considered the ideal terrestrial analogue to Spaceflight Associated Neuro-ocular Syndrome (SANS).1 However, there are several critical features of SANS that do not complement any reported case of IIH on Earth.
