HIV-1 drug resistance and molecular epidemiology in Honduras
The high genetic diversity and rapid evolution of HIV-1 poses a challenge to the worldwide prevention and treatment programs. Effective antiretroviral treatment has significantly improved the quality of life for HIV-infected patients. However, it came with a cost because resistant viruses emerge and sometimes are transmitted, which can reduce the efficacy of first-line antiretroviral therapy. Sequencing of the HIV-1 genome can provide information on both viral diversity and antiretroviral resistance profiles. This thesis work investigated HIV-1 resistance and molecular epidemiology in Honduras.
In paper I the prevalence of antiretroviral drug resistance was investigated in 138 HIV-positive Honduras patients with signs of treatment failure by partial sequencing of the HIV-1 pol gene. The prevalence of antiretroviral resistance was high and resistance mutations were detected in 112 patients (81%). Virologic failure was the strongest predictor of treatment failure and poor access to viral load testing in Honduras was identified as an important problem.
Paper II investigated transmitted drug resistance in a representative sample of 200 treatment-naïve, newly diagnosed Honduran HIV-1 patients. The prevalence of transmitted drug resistance was 7%: 5% for NNRTI, 3% for NRTI and 0.5% for PI. Recent infection, as determined by the serological BED assay, was observed in 12% of the patients and was associated with a higher prevalence of transmitted drug resistance.
Little is known about how HIV-1 has entered and spread in Honduras and Central America. In paper III the molecular epidemiology of HIV-1 in Honduras was investigated using pol gene sequences from a representative sample of 257 Honduran patients. The Honduran HIV-1 epidemic was found to be dominated by six subtype B clades that were introduced into Honduras between 1966 and 1984. One HIV-1 clade has been particularly successful and accounts for 64% of the current HIV-1 cases in the country. The analyses suggested that HIV-1 was introduced into Honduras from the United States of America.
In paper IV phylogenetic analyses were also used to understand the spread of HIV-1 in Central America using 625 HIV-1 pol gene sequences collected between 2002 and 2010 in Belize, Costa Rica, El Salvador, Honduras, Nicaragua and Panama. Published sequences from neighboring countries (n=57) and the rest of the world (n=740) were included as controls. Maximum- likelihood analyses showed that almost all (98.9%) sequences were of subtype B and that 436 (70%) sequences formed five significantly supported, monophyletic clades, which almost exclusively contained Central American sequences. One clade contained 386 (62%) sequences from all six countries; the other four clades were more country- specific, suggesting a compartmentalized epidemic. Bayesian coalescent-based methods were used to time the HIV-1 introductions and showed that the most recent common ancestor of the main subtype B introductions into Central America dated back to 1960-1970’s.
In conclusion, this thesis highlights the importance of drug resistance surveillance in treated and untreated patients, and points to a need for increased access and use of HIV testing, CD4 counts, viral load and resistance testing in Honduras. Understanding the factors responsible for the HIV-1 epidemic in Honduras and Central America has important implications in terms of intervention and control strategies
Substance Abuse via Legally Prescribed Drugs: The Case of Vicodin in the United States
Vicodin is the most commonly prescribed pain reliever in the United States.
Research indicates that there are two million people who are currently abusing
Vicodin, and the majority of those who abuse Vicodin were initially exposed to
it via prescription. Our goal is to determine the most effective strategies for
reducing the overall population of Vicodin abusers. More specifically, we focus
on whether prevention methods aimed at educating doctors and patients on the
potential for drug abuse or treatment methods implemented after a person abuses
Vicodin will have a greater overall impact. We consider one linear and two
non-linear compartmental models in which medical users of Vicodin can
transition into the abuser compartment or leave the population by no longer
taking the drug. Once Vicodin abusers, people can transition into a treatment
compartment, with the possibility of leaving the population through successful
completion of treatment or of relapsing and re-entering the abusive
compartment. The linear model assumes no social interaction, while both
non-linear models consider interaction. One considers interaction with abusers
affecting the relapse rate, while the other assumes both this and an additional
interaction between the number of abusers and the number of new prescriptions.
Sensitivity analyses are conducted varying the rates of success of these
intervention methods measured by the parameters to determine which strategy has
the greatest impact on controlling the population of Vicodin abusers. From
these models and analyses, we determine that manipulating parameters tied to
prevention measures has a greater impact on reducing the population of abusers
than manipulating parameters associated with treatment. We also note that
increasing the rate at which abusers seek treatment affects the population of
abusers more than the success rate of treatment itself

Los sistemas hacen parte de un papel fundamental en el cumplimiento de metas y
objetivos trazados a corto o largo plazo por las organizaciones, ya que son un conjunto de
elementos relacionados entre sí y que interaccionan con el entorno dando lugar al nuevo
pensamiento sistemático, el cual permite satisfacer las necesidades de la información en una
estructura empresarial, ya que se encarga de almacenar controlar y procesar todo tipo de datos
con el fin de facilitar procesos (Silva Murillo, 2009).
Por otro lado, los sistemas de información tienen como objetivo administrar, recolectar,
procesar y almacenar datos (Laudon & Laudon 2004). Consecuentemente, dichos sistemas
ayudan a distribuir la información para apoyar la toma de decisiones, minimizar problemas y
garantizar el control. Gracias a su eficiencia y eficacia los sistemas de información permiten
generar datos confiables para dicha toma de decisiones.
Así mismo, los sistemas de información contable se encargan de recolectar, manipular,
almacenar datos y preparar documentos por medio de las aplicaciones de contabilidad de una
organización, con el propósito de producir información de manera oportuna para la toma de
decisiones (Bernal, 2004). Estos se caracterizan por ser una base de las actividades
empresariales, debido a su capacidad de informar el incremento del patrimonio, la productividad
y el posicionamiento de las organizaciones.Systems play a fundamental role in meeting goals and
objectives set in the short or long term by organizations, since they are a set of
elements related to each other and that interact with the environment giving rise to the new
systematic thinking, which allows to satisfy the needs of the information in a
business structure, since it is responsible for storing, controlling and processing all types of data
in order to facilitate processes (Silva Murillo, 2009).
On the other hand, information systems aim to manage, collect,
process and store data (Laudon & Laudon 2004). Consequently, these systems
help distribute information to support decision making, minimize problems, and
guarantee control. Thanks to their efficiency and effectiveness, information systems allow
generate reliable data for such decision making.
Likewise, accounting information systems are responsible for collecting, manipulating,
store data and prepare documents through the accounting applications of a
organization, with the purpose of producing information in a timely manner for decision-making.

CANOES: detecting rare copy number variants from whole exome sequencing data
We present CANOES, an algorithm for the detection of rare copy number variants from exome sequencing data. CANOES models read counts using a negative binomial distribution and estimates variance of the read counts using a regression-based approach based on selected reference samples in a given dataset. We test CANOES on a family-based exome sequencing dataset, and show that its sensitivity and specificity is comparable to that of XHMM. Moreover, the method is complementary to Gaussian approximation-based methods (e.g. XHMM or CoNIFER). When CANOES is used in combination with these methods, it will be possible to produce high accuracy calls, as demonstrated by a much reduced and more realistic de novo rate in results from trio data

Aplicación del modelo Misión –Visión en una empresa familiar Agro-industrial: The application of the Vision Mision model to an agro industry family business
La presente investigación tiene como objetivo general, aplicar el modelo de Dirección Estratégica Misión-Visión, y adaptarlo a las necesidades de una empresa familiar agroproductiva dedicada a la elaboración de panela. Con el fin crear ventajas competitivas y planes de acción que permitan superar las dificultades propias de este sector, se utilizó del método de investigación descriptivo, para el levantamiento de información se aplicó una ficha de observación y entrevista al personal para obtener una perspectiva real de entorno. Adicionalmente se usó, el método inductivo con la aplicación modelo de Dirección Estratégica Misión- Visión. En la trayectoria de la investigación se concluye que en la empresa están ausente elementos de la dirección estratégica, como los objetivos estratégicos y mapa de valores, que se tornan indispensables para potencializar la cadena operativa y administrativa de una empresa ya sea pequeña mediana o grande.

ABSTRACT
The general objective of this research is to apply the Mission-Vision Strategic Management model, and adapt it to the needs of an agroproductive family business dedicated to the production of panela. In order to create competitive advantages and action plans to overcome the difficulties of this sector, we used the descriptive research method, to collect information, we applied an observation sheet and interview the staff to obtain a real perspective of the environment . Additionally, the inductive method was used with the model application of Mission-Vision Strategic Management. In the trajectory of the investigation it is concluded that elements of the Strategic Management are absent from the company, such as the strategic objectives and the map of values, which become indispensable to enhance the operational and administrative chain of a company, whether small or medium size.

EVALUACIÓN CLÍNICO EPIDEMIOLÓGICA DE LOS TUMORES ABDOMINALES MALIGNOS EN EL NIÑO
Este trabajo de investigación responde a un resultado de un proyecto científico en el cual se evalúa el comportamiento de los pacientes con tumores abdominales en el servicio de Cirugía del Hospital Pediátrico de Holguín, Cuba, en el período de noviembre de 2010 hasta noviembre de 2021. Se realizó un estudio ambispectivo observacional a una serie de pacientes con tumores abdominales. La población o universo estuvo constituida por todos los pacientes que ingresaron en el servicio de Cirugía Pediátrica con tumores abdominales. Se utilizó un muestreo aleatorio simple y se escogió una muestra de 52 pacientes a los que se les diagnosticó los tres tumores más frecuentes: neuroblastoma , tumor de Wilms y linfoma no Hodgkin abdominal. Al concluir el estudio se constató que, por lo general, los pacientes asisten a nuestra institución en estadios avanzados de la enfermedad con un diagnóstico tardío y, a pesar de las terapéuticas existentes, tienen mal pronóstico.
