Perspectives on supporting fathers affected by postnatal depression and a history of violence

Intimate partner violence in the perinatal period is a significant problem that remains underscreened, underdiagnosed and undertreated. The establishment of evidence-based guidelines to enable health visitors to identify couples experiencing violence and offer appropriate support has been hampered by the complex interplay between maternal and paternal mental health problems and violence. This study explored the experiences of UK fathers who voluntarily engaged with services designed to eliminate their ideation to violence. The findings indicate that the tendency to violence is increased by stresses associated with the transition to parenthood. Men felt pressured by concerns for their partner's mental health, changes in the relationship, sleep disturbances and the burden of infant care they assumed when the mother was unable to cope. Health visitors are ideally placed to assess for factors linked to the emergence of violence and put in place interventions to minimise occurrence

An Al plus Y coating process for improvement of the high-temperature oxidation resistance of a TiAl alloy

    No full text
An Al + Y codeposition by a single EB-PVD process has been developed to improve the high-temperature oxidation resistance of a TiAl alloy. The Al + Y codeposited TiAl alloy with various ratios of Al and Y evaporation sources was evaluated by isothermal and cyclic-oxidation tests. The coating layer has a composition gradient because of the difference in vapor pressure between Y and Al. The oxidation resistance can be extensively improved by the formation of an Al + Y codeposition layer and it depends on the ratio of the source material of Al and Y. The best oxidation resistance was obtained from the Al + Y codeposited TiAl alloy with a ratio of Al:2Y. With the proper ratio of Al. Y, the Al + Y codeposition coating forms Two layers of the oxides during high-temperature oxidation + the outer (Y, Al) O layer and the inner Al2O3 layer, containing small amounts of Y oxide. These oxide layers, which have a fine grain size, act as a diffusion barrier, thus suppressing the rate of inward diffusion of O and may readily relieve thermal stresses.X1114sciescopu

Real-world data for health technology assessment for reimbursement decisions in Asia: current landscape and a way forward.

There is growing interest globally in using real-world data (RWD) and real-world evidence (RWE) for health technology assessment (HTA). Optimal collection, analysis, and use of RWD/RWE to inform HTA requires a conceptual framework to standardize processes and ensure consistency. However, such framework is currently lacking in Asia, a region that is likely to benefit from RWD/RWE for at least two reasons. First, there is often limited Asian representation in clinical trials unless specifically conducted in Asian populations, and RWD may help to fill the evidence gap. Second, in a few Asian health systems, reimbursement decisions are not made at market entry; thus, allowing RWD/RWE to be collected to give more certainty about the effectiveness of technologies in the local setting and inform their appropriate use. Furthermore, an alignment of RWD/RWE policies across Asia would equip decision makers with context-relevant evidence, and improve timely patient access to new technologies. Using data collected from eleven health systems in Asia, this paper provides a review of the current landscape of RWD/RWE in Asia to inform HTA and explores a way forward to align policies within the region. This paper concludes with a proposal to establish an international collaboration among academics and HTA agencies in the region: the REAL World Data In ASia for HEalth Technology Assessment in Reimbursement (REALISE) working group, which seeks to develop a non-binding guidance document on the use of RWD/RWE to inform HTA for decision making in Asia
