LongâTerm Outcome and Quality of Life in Patients With Stroke Presenting With Extensive Early Infarction
Background
The benefit of mechanical thrombectomy in patients with low Alberta Stroke Program Early Computed Tomography Score (ASPECTS) for shortâterm outcomes is debatable and longâterm outcomes remain unknown. This retrospective, monocentric cohort study aimed to assess the association between reperfusion grade and the longâterm functional outcome measured with modified Rankin scale as well as the longâterm healthârelated quality of life recorded at the last followâup in patients according to baseline ASPECTS (0â5 versus 6â10).
Methods
Deceased patients were identified from the Swiss population register and followâup telephone interviews were conducted with all surviving patients with stroke treated with mechanical thrombectomy between January 1, 2010, and December 31, 2018. Favorable outcome was defined as modified Rankin scale 0 to 3; healthârelated quality of life was assessed using the 3âlevel version of the EuroQol 5âdimensional questionnaire. The EuroQol 5âdimension utility index was calculated for statistical analyses. The reperfusion grade was core laboratory adjudicated using the expanded treatment in cerebral ischemia score. Adjusted odds ratios for the association between the reperfusion grade assessed by expanded treatment in cerebral ischemia and outcomes were calculated from multivariable logistic regression.
Results
Of the 1114 patients with available longâterm followâup records (median followâup, 3.67 years), 997 were included in the final analysis. Respectively, patients with low ASPECTS more often had complaints regarding mobility (67.1% versus 42.1%, P<0.001), selfâcare (53.4% versus 31.2%, P<0.001), and usual activities (65.8% versus 41.4%, P<0.001) than patients with high ASPECTS, whereas reported pain/discomfort (65.7% versus 69.9%, P=0.49) and anxiety/depression (71.2% versus 78.9%, P=0.17) did not differ. In patients with low ASPECTS, increasing reperfusion grade was associated with a higher likelihood of longâterm favorable functional outcome (adjusted odds ratio, 1.43; 95% CI, 1.09â1.88 [P=0.01]) and healthârelated quality of life (adjusted linear correlation coefficient, 0.05; 95% CI, 0.02â0.08) despite early extensive infarction.
Conclusion
Despite low baseline ASPECTS, a higher reperfusion grade results in better functional outcomes and may improve healthârelated quality of life in the long term
Long-Term Effect of Mechanical Thrombectomy in Stroke Patients According to Advanced Imaging Characteristics.
PURPOSE
Data on long-term effect of mechanical thrombectomy (MT) in patients with large ischemic cores (â„âŻ70âŻml) are scarce. Our study aimed to assess the long-term outcomes in MT-patients according to baseline advanced imaging parameters.
METHODS
We performed a single-centre retrospective cohort study of stroke patients receiving MT between January 1, 2010 and December 31, 2018. We assessed baseline imaging to determine core and mismatch volumes and hypoperfusion intensity ratio (with low ratio reflecting good collateral status) using RAPID automated post-processing software. Main outcomes were cross-sectional long-term mortality, functional outcome and quality of life by May 2020. Analysis were stratified by the final reperfusion status.
RESULTS
In total 519 patients were included of whom 288 (55.5%) have deceased at follow-up (median follow-up time 28 months, interquartile range 1-55). Successful reperfusion was associated with lower long-term mortality in patients with ischemic core volumes â„âŻ70âŻml (adjusted hazard ratio (aHR) 0.20; 95% confidence interval (95% CI) 0.10-0.44) and â„âŻ100âŻml (aHR 0.26; 95% CI 0.08-0.87). The effect of successful reperfusion on long-term mortality was significant only in the presence of relevant mismatch (aHR 0.17; 95% CI 0.01-0.44). Increasing reperfusion grade was associated with a higher rate of favorable outcomes (mRS 0-3) also in patients with ischemic core volume â„âŻ70âŻml (aOR 3.58, 95% CI 1.64-7.83).
CONCLUSION
Our study demonstrated a sustainable benefit of better reperfusion status in patients with large ischemic core volumes. Our results suggest that patient deselection based on large ischemic cores alone is not advisable
Status of the RFQ linac installation and conditioning of the Linear IFMIF Prototype Accelerator
Abstract The Radio Frequency Quadrupole (RFQ) linac and 1.6 MW RF power system of the Linear IFMIF Prototype Accelerator (LIPAc) facility in the International Fusion Energy Research Center (IFERC) in Rokkasho (Japan) has been installed and conditioned. During the assembly and tuning process, the RFQ cavity was protected with a temporary tent from the potential deterioration of performance caused by dust. The vacuum in the cavity was improved through the 100 °C baking process of the cavity. The high power test of the 175âŻMHz RF systems up to 200âŻkW in CW for each of the eight RF chains was performed for checking its stable output reproducibility in Japan, before connecting 9â3/16 inch coaxial transmission lines from the RF chains to the RF input couplers of the cavity. It was confirmed that the eight RF chains provided the balanced RF power to the single RFQ cavity in-phase using a feedback loop and a synchronization system. The peak power in the cavity achieved 150âŻkW in the pulsed mode, which corresponds approximately to the required electric field to accelerate proton beam. Such RF conditioning process is ongoing to achieve 600âŻkW approximately required for deuteron beam commissioning planned in 2018
The political ontology of collaborative water governance
This article examines the various definitions of, and analytical approaches to, collaborative water governance (CWG). While the conceptâs usage has increased over the past decade, there lacks any deep engagement with the concept of the political at the heart of CWG. This article argues that contemporary approaches to CWG risk emptying the concept of its utility and coherence. Correcting this deficiency requires a focus on the social and ideational constructions of water. This will strengthen future collaborative water arrangements and enable deeper appreciation of the ways the political makes and remakes what is possible in water governance
Identification of six new susceptibility loci for invasive epithelial ovarian cancer.
Genome-wide association studies (GWAS) have identified 12 epithelial ovarian cancer (EOC) susceptibility alleles. The pattern of association at these loci is consistent in BRCA1 and BRCA2 mutation carriers who are at high risk of EOC. After imputation to 1000 Genomes Project data, we assessed associations of 11 million genetic variants with EOC risk from 15,437 cases unselected for family history and 30,845 controls and from 15,252 BRCA1 mutation carriers and 8,211 BRCA2 mutation carriers (3,096 with ovarian cancer), and we combined the results in a meta-analysis. This new study design yielded increased statistical power, leading to the discovery of six new EOC susceptibility loci. Variants at 1p36 (nearest gene, WNT4), 4q26 (SYNPO2), 9q34.2 (ABO) and 17q11.2 (ATAD5) were associated with EOC risk, and at 1p34.3 (RSPO1) and 6p22.1 (GPX6) variants were specifically associated with the serous EOC subtype, all with P < 5 Ă 10(-8). ECMO for COVID-19 patients in Europe and Israel
Since March 15th, 2020, 177 centres from Europe and Israel have joined the study, routinely reporting on the ECMO support they provide to COVID-19 patients. The mean annual number of cases treated with ECMO in the participating centres before the pandemic (2019) was 55. The number of COVID-19 patients has increased rapidly each week reaching 1531 treated patients as of September 14th. The greatest number of cases has been reported from France (n = 385), UK (n = 193), Germany (n = 176), Spain (n = 166), and Italy (n = 136) .The mean age of treated patients was 52.6 years (range 16â80), 79% were male. The ECMO configuration used was VV in 91% of cases, VA in 5% and other in 4%. The mean PaO2 before ECMO implantation was 65 mmHg. The mean duration of ECMO support thus far has been 18 days and the mean ICU length of stay of these patients was 33 days. As of the 14th September, overall 841 patients have been weaned from ECMO
support, 601 died during ECMO support, 71 died after withdrawal of ECMO, 79 are still receiving ECMO support and for 10 patients status n.a. . Our preliminary data suggest that patients placed
on ECMO with severe refractory respiratory or cardiac failure secondary to COVID-19 have a reasonable (55%) chance of survival. Further extensive data analysis is expected to provide invaluable information on the demographics, severity of illness, indications and different ECMO management strategies in these patients
Traceable Asynchronous Workflows in Video Retrieval with vitrivr-VR
Virtual reality (VR) interfaces allow for entirely new modes of user interaction with systems and interfaces. Much like in physical workspaces, documents, tools, and interfaces can be used, put aside, and used again later. Such asynchronous workflows are a great advantage of virtual environments, as they enable users to perform multiple tasks in an interleaved manner. However, VR interfaces also face new challenges, such as text input without physical keyboards, and the analysis of such asynchronous workflows. In this paper we present the version of vitrivr-VR participating in the Video Browser Showdown (VBS) 2023. We describe the current state of our system, with a focus on improvements in text input methods and logging of asynchronous workflows
