    Deformacja wzdłużna stentu uwalniającego lek Synergy w trakcie wyjmowania balonu tnącego

    Contemporary stent platforms have reduced strut thickness and fewer connectors. This improves deliverability and conformability, at the expense of reduction in strength. Recently longitudinal shortening has been reported, often associated with adjunctive devices after stent deployment. We present an unusual, impressive case of longitudinal shortening of a previously implanted Synergy stent (Boston Scientific, MA USA), within the proximal left anterior descending artery (LAD). The patient subsequently represented with restenosis of a further stent in the mid LAD. On withdrawal of a Flextome cutting balloon catheter (Boston Scientific), this device snared and dramatically shortened the Synergy. This was successfully treated with progressive dilatation using a series of enlarging balloons and subsequent implantation of another stent (Xience Xpedition, Abbott, CA USA). This case illustrates, that despite having additional connectors to enhance longitudinal strength, Synergy is not immune from compression. Furthermore, it is imperative, that stents are sized and post-dilated appropriately to ensure complete expansion.Rusztowania nowoczesnych stentów cechują się zmniejszoną grubością rozpórek i mniejszą liczbą łączników. To powoduje, że łatwiej je wprowadzić do naczynia i odpowiednio dopasować, jednak zmniejsza ich wytrzymałość. W ostatnim czasie opisywano wiele przypadków skrócenia stentu, często związanych ze stosowaniem dodatkowych narzędzi po implantacji stentu. W niniejszej pracy przedstawiono rzadki, ciekawy przypadek skrócenia wszczepionego wcześniej stenu Synergy (BostonScientific, USA), znajdującego się w proksymalnym odcinku gałęzi międzykomorowej przedniej (LAD). W następstwieu chorego wystąpiła restenoza w dalszej części stentu w środkowym odcinku LAD. W trakcie wyjmowania cewnika z balonem tnącym Flextome (Boston Scientific) urządzenie zaczepiło o stent Synergy i spowodowało jego znaczne skrócenie. Udało się poprawić drożność naczynia przez stopniowe rozszerzanie stenu za pomocą coraz większych balonów oraz wszczepienia kolejnego stentu (Xience Xpedition, Abbott, USA). Opisany przypadek ukazuje, że mimo zastosowania dodatkowych łączników służących wzmocnieniu wytrzymałości stentSynergy nie jest odporny na kompresję. Ponadto należy się zawsze upewnić, że stent został odpowiednio dobrany podwzględem rozmiaru i odpowiednio rozszerzony, aby zapewnić jego całkowite rozprężenie

    Parenchymal involvement on CT pulmonary angiography in SARS-CoV-2 Alpha variant infection and correlation of COVID-19 CT severity score with clinical disease severity and short-term prognosis in a UK cohort

    AIM: To determine if there is a difference in radiological, biochemical, or clinical severity between patients infected with Alpha-variant SARS-CoV-2 compared with those infected with pre-existing strains, and to determine if the computed tomography (CT) severity score (CTSS) for COVID-19 pneumonitis correlates with clinical severity and can prognosticate outcomes. MATERIALS AND METHODS: Blinded CTSS scoring was applied to 137 hospital patients who had undergone both CT pulmonary angiography (CTPA) and whole-genome sequencing of SARS-CoV-2 within 14 days of CTPA between 1/12/20–5/1/21. RESULTS: There was no evidence of a difference in imaging severity on CTPA, viral load, clinical parameters of severity, or outcomes between Alpha and preceding variants. CTSS on CTPA strongly correlates with clinical and biochemical severity at the time of CTPA, and with patient outcomes. Classifying CTSS into a binary value of “high” and “low”, with a cut-off score of 14, patients with a high score have a significantly increased risk of deterioration, as defined by subsequent admission to critical care or death (multivariate hazard ratio [HR] 2.76, p<0.001), and hospital length of stay (17.4 versus 7.9 days, p<0.0001). CONCLUSION: There was no evidence of a difference in radiological severity of Alpha variant infection compared with pre-existing strains. High CTSS applied to CTPA is associated with increased risk of COVID-19 severity and poorer clinical outcomes and may be of use particularly in settings where CT is not performed for diagnosis of COVID-19 but rather is used following clinical deterioration

    Reward processing in autism: a thematic series

    This thematic series presents theoretical and empirical papers focused on understanding autism from the perspective of reward processing deficits. Although the core symptoms of autism have not traditionally been conceptualized with respect to altered reward-based processes, it is clear that brain reward circuitry plays a critical role in guiding social and nonsocial learning and behavior throughout development. Additionally, brain reward circuitry may respond to social sources of information in ways that are similar to responses to primary rewards, and recent clinical data consistently suggest abnormal behavioral and neurobiologic responses to rewards in autism. This thematic series presents empirical data and review papers that highlight the utility of considering autism from the perspective of reward processing deficits. Our hope is that this novel framework may further elucidate autism pathophysiology, with the ultimate goal of yielding novel insights with potential therapeutic implications

    The evaluation of a healthcare passport to improve quality of care and communication for people living with dementia (EQuIP): a protocol paper for a qualitative, longitudinal study

    Background\ud \ud There is an urgent need for the development of simple communication tools that convey the strengths, assets, and healthcare needs of people living with dementia. A Healthcare Passport may improve communication with range of health and social support services, enhancing quality and continuity of care, and to permit a consideration of the challenges and how these might be managed effectively and compassionately. This study aims to evaluate the acceptability and use of this type of intervention for people living with dementia and their carers.\ud \ud \ud Methods/Design\ud \ud This is a qualitative longitudinal study informed by a critical realist review. The participants will be individuals identified as having mild-moderate dementia and informal carers. The in-depth interviews will occur at three points over the course of 18 months as they use the passport. This will be supplemented by analysis of the content of the passports and information from health and social care providers on the daily practicalities of using the passport in a range of healthcare settings.\ud \ud \ud Discussion\ud \ud By using a critical realist review and a qualitative, longitudinal approach, the study allows for the assessment of a complex intervention in a manner which goes beyond evaluating the basic efficacy of the passport, but looking more deeply at how it worked, for whom, and in what context. It has the potential to develop new data on how interventions improve communication across a range of service providers, while encouraging health and social care professionals to respect and encourage the development of self-management and retention of personhood throughout the progression of life-limiting illnesses

    Doing politics in the recent Arab uprisings: Towards a political discourse analysis of the Arab Spring slogans

    The present paper aims to analyse a number of those slogans collected from the sit-in quarters in Egypt, Libya and Yemen. Using political discourse analysis, it unravels various typical discourse structures and strategies that are used in slogans in the construction of a sub-genre of political discourse in the Arab world. Drawing data from several mediums, including banners, wall graffiti, audio-visual instruments, chanting, speeches and songs, this paper tries to show the extent to which the slogans serve as a medium by which political complaints and comments are dispensed and consumed. This paper draws on a rhetorical analysis to find out their persuasive effect on shaping the Arab intellect and on the change of the political atmosphere in the region. Lastly, this paper attempts to show to what extent the slogans meet the standards of political discourse and whether they can be considered as a sub-genre of political discourse or not.IS
