Rural School District Leadership and Governance: Eating Your Veggies to Stay on the Balcony
School board trustees and superintendents must navigate overlapping roles and intersectional identities when engaging in rural school district governance work. If these individuals are not clear about the scope of their role and do not have a common understanding about the importance of high expectations for all, lack of coherent governance can negatively impact their district. Recently, one school district in rural Idaho has experienced phenomenal success. The district is in a very different place than it was just a little over five years ago. What You Don’t Know Can Hurt You: Communicating Health Law with Vermont Physicians
Background: Physicians today need a working knowledge of pertinent medical law. With an increased focus on patients’ rights in health care, states are encouraged to set specific laws protecting patients. The additional medical legislation places a challenge on physicians to continually update their medical-legal acumen such as disease reporting, malpractice issues, and medical information access.Little research has been conducted on physicians’ knowledge of the law and medicine. Are Mandates the Answer? Improving Palliative Care and Pain Management in Vermont
Background: The Vermont legislature (bill H.435, Sec. 19) has tasked the Vermont Board of Medical Practice (VBMP) with making a formal recommendation on improving Vermont health professionals’ knowledge and practice of Palliative Care and Pain Management (PC/PM). An adaptive signaling network in melanoma inflammatory niches confers tolerance to MAPK signaling inhibition
Mitogen-activated protein kinase (MAPK) pathway antagonists induce profound clinical responses in advanced cutaneous melanoma, but complete remissions are frustrated by the development of acquired resistance. Before resistance emerges, adaptive responses establish a mutation-independent drug tolerance. Antagonizing these adaptive responses could improve drug effects, thereby thwarting the emergence of acquired resistance. In this study, we reveal that inflammatory niches consisting of tumor-associated macrophages and fibroblasts contribute to treatment tolerance through a cytokine-signaling network that involves macrophage-derived IL-1β and fibroblast-derived CXCR2 ligands. Fibroblasts require IL-1β to produce CXCR2 ligands, and loss of host IL-1R signaling in vivo reduces melanoma growth. In tumors from patients on treatment, signaling from inflammatory niches is amplified in the presence of MAPK inhibitors. Signaling from inflammatory niches counteracts combined BRAF/MEK (MAPK/extracellular signal–regulated kinase kinase) inhibitor treatment, and consequently, inhibiting IL-1R or CXCR2 signaling in vivo enhanced the efficacy of MAPK inhibitors. As such, the system has been designed with emphasis on modularity, miniaturization and ease of operator usage and expansion. The MicroDAS product line includes a series of components used as building blocks to configure systems of virtually any size. The modular design of these components allows considerable latitude to the instrumentation engineer in configuring systems for simple or complex applications. The modular concept has been extended to the design of plug-in modules for different functional requirements and system applications.
