Integrativer Ansatz zum Ausbau berufsrelevanter SchlĂŒsselkompetenzen in der Ingenieurausbildung
Durch den Bologna-Prozess gewann die insbesondere in den IngenieurwissenÂschafÂten existierende Forderung nach fachĂŒbergreifenden Studieninhalten eine neue Dynamik. Im Rahmen der Modifizierung und Neugestaltung der Curricula fĂŒr Bachelor- und MasterabschlĂŒsse entstanden Möglichkeiten, den Erwerb von SchlĂŒsÂÂselkompetenzen in die universitĂ€re Ausbildung zu integrieren. In diesem Zug wurden im Rahmen des Maschinenbau-Studiums am Karlsruher Institut fĂŒr TechÂnologie (KIT) AnsĂ€tze zur integrativen VermittÂlung dieser Kompetenzen entÂwickelt, die in die Konzeption einer Lehrveranstaltung mit dem Titel "ArbeitsÂtechÂniken fĂŒr den MaschiÂnenbau" mĂŒndeten, deren Konzept und Umsetzung hier erlĂ€uÂtert wird.

Brothers in arms : the value of coalitions in sanctions regimes
This paper examines the impact of coalitions on the economic costs of the 2012 Iran and 2014 Russia sanctions. By estimating and simulating a quantitative general equilibrium trade model under different coalition set-ups, we (i) dissect welfare losses for sanction-senders and target; (ii) compare prospective coalition partners and; (iii) provide bounds for the sanctions potential â the maximum welfare change attainable â when sanctions are scaled vertically, i.e. across sectors up to an embargo, or horizontally, i.e. across countries up to a global regime. To gauge the significance of simulation outcomes, we implement a Bayesian bootstrap procedure that generates confidence bands. We find that the implemented measures against Iran and Russia inflicted considerable economic harm, yielding 32 â 37% of the vertical sanctions potential. Our key finding is that coalitions lower the average welfare loss incurred from sanctions relative to unilateral implementation. They also increase the welfare loss imposed on Iran and Russia. Adding China to the coalition further amplifies the welfare loss by 79% for Iran and 22% for Russia. Finally, we quantify transfers that would equalize losses across coalition members. These hypothetical transfers can be seen as a sanctions-equivalent of NATO spending goals and provide a measure of the relative burden borne by coalition countries
Sustained Viral Suppression With Dolutegravir Monotherapy Over 192 Weeks in Patients Starting Combination Antiretroviral Therapy During Primary Human Immunodeficiency Virus Infection (EARLY-SIMPLIFIED): A Randomized, Controlled, Multi-site, Noninferiority Trial
BACKGROUND: Starting combination antiretroviral therapy (cART) during primary human immunodeficiency virus type 1 (HIV-1) infection results in a smaller HIV-1 latent reservoir, reduced immune activation, and less viral diversity compared to starting cART during chronic infection. We report results of a 4-year study designed to determine whether these properties would allow sustained virological suppression after simplification of cART to dolutegravir (DTG) monotherapy.
METHODS: EARLY-SIMPLIFIED is a randomized, open-label, noninferiority trial. People with HIV (PWH) who started cART <180 days after a documented primary HIV-1 infection with suppressed viral load were randomized (2:1) to DTG monotherapy with 50 mg daily or continuation of cART. The primary endpoints were the proportion of PWH with viral failure at 48, 96, 144, and 192 weeks; noninferiority margin was 10%. After 96 weeks, randomization was lifted and patients were permitted to switch treatment groups as desired.
RESULTS: Of 101 PWH randomized, 68 were assigned to DTG monotherapy and 33 to cART. At week 96 in the per-protocol population, 64/64 (100%) showed virological response in the DTG monotherapy group versus 30/30 (100%) in the cART group (difference, 0.00%; upper bound of 95% confidence interval 6.22%). This demonstrated noninferiority of DTG monotherapy at the prespecified level. At week 192, the study end, no virological failure occurred in either group during 13 308 and 4897 person weeks of follow-up for the DTG monotherapy (n = 80) and cART groups, respectively.
CONCLUSIONS: This trial suggests that early cART initiation during primary HIV infection allows sustained virological suppression after switching to DTG monotherapy
Detection of Viral Infection and Bacterial Coinfection and Superinfection in Coronavirus Disease 2019 Patients Presenting to the Emergency Department Using the 29-mRNA Host Response Classifier IMX-BVN-3: A Multicenter Study.
Background
Identification of bacterial coinfection in patients with coronavirus disease 2019 (COVID-19) facilitates appropriate initiation or withholding of antibiotics. The Inflammatix Bacterial Viral Noninfected (IMX-BVN) classifier determines the likelihood of bacterial and viral infections. In a multicenter study, we investigated whether IMX-BVN version 3 (IMX-BVN-3) identifies patients with COVID-19 and bacterial coinfections or superinfections.
Methods
Patients with polymerase chain reaction-confirmed COVID-19 were enrolled in Berlin, Germany; Basel, Switzerland; and Cleveland, Ohio upon emergency department or hospital admission. PAXgene Blood RNA was extracted and 29 host mRNAs were quantified. IMX-BVN-3 categorized patients into very unlikely, unlikely, possible, and very likely bacterial and viral interpretation bands. IMX-BVN-3 results were compared with clinically adjudicated infection status.
Results
IMX-BVN-3 categorized 102 of 111 (91.9%) COVID-19 patients into very likely or possible, 7 (6.3%) into unlikely, and 2 (1.8%) into very unlikely viral bands. Approximately 94% of patients had IMX-BVN-3 unlikely or very unlikely bacterial results. Among 7 (6.3%) patients with possible (n = 4) or very likely (n = 3) bacterial results, 6 (85.7%) had clinically adjudicated bacterial coinfection or superinfection. Overall, 19 of 111 subjects for whom adjudication was performed had a bacterial infection; 7 of these showed a very likely or likely bacterial result in IMX-BVN-3.
Conclusions
IMX-BVN-3 identified COVID-19 patients as virally infected and identified bacterial coinfections and superinfections. Future studies will determine whether a point-of-care version of the classifier may improve the management of COVID-19 patients, including appropriate antibiotic use
The 10m AEI prototype facility A brief overview
The AEI 10 m prototype interferometer facility is currently being constructed
at the Albert Einstein Institute in Hannover, Germany. It aims to perform
experiments for future gravitational wave detectors using advanced techniques.
Seismically isolated benches are planned to be interferometrically
interconnected and stabilized, forming a low-noise testbed inside a 100 m^3
ultra-high vacuum system. A well-stabilized high power laser will perform
differential position readout of 100 g test masses in a 10 m suspended
arm-cavity enhanced Michelson interferometer at the crossover of measurement
(shot) noise and backaction (quantum radiation pressure) noise, the so-called
Standard Quantum Limit (SQL). Such a sensitivity enables experiments in the
highly topical field of macroscopic quantum mechanics. In this article we

Meta-analysis uncovers genome-wide significant variants for rapid kidney function decline
Rapid decline of glomerular filtration rate estimated from creatinine (eGFRcrea) is associated with severe clinical endpoints. In contrast to cross-sectionally assessed eGFRcrea, the genetic basis for rapid eGFRcrea decline is largely unknown. To help define this, we meta-analyzed 42 genome-wide association studies from the Chronic Kidney Diseases Genetics Consortium and United Kingdom Biobank to identify genetic loci for rapid eGFRcrea decline. Two definitions of eGFRcrea decline were used: 3 mL/min/1.73m(2)/year or more ("Rapid3"; encompassing 34,874 cases, 107,090 controls) and eGFRcrea decline 25% or more and eGFRcrea under 60 mL/min/1.73m(2) at follow-up among those with eGFRcrea 60 mL/min/1.73m(2) or more at baseline ("CKDi25"; encompassing 19,901 cases, 175,244 controls). Seven independent variants were identified across six loci for Rapid3 and/or CKDi25: consisting of five variants at four loci with genome-wide significance (near UMOD-PDILT (2), PRKAG2, WDR72, OR2S2) and two variants among 265 known eGFRcrea variants (near GATM, LARP4B). All these loci were novel for Rapid3 and/or CKDi25 and our bioinformatic follow-up prioritized variants and genes underneath these loci. The OR2S2 locus is novel for any eGFRcrea trait including interesting candidates. For the five genome-wide significant lead variants, we found supporting effects for annual change in blood urea nitrogen or cystatin-based eGFR, but not for GATM or (LARP4B). Individuals at high compared to those at low genetic risk (8-14 vs. 0-5 adverse alleles) had a 1.20-fold increased risk of acute kidney injury (95% confidence interval 1.08-1.33). Thus, our identified loci for rapid kidney function decline may help prioritize therapeutic targets and identify mechanisms and individuals at risk for sustained deterioration of kidney function
Associations between depressive symptoms and disease progression in older patients with chronic kidney disease: results of the EQUAL study
Background Depressive symptoms are associated with adverse clinical outcomes in patients with end-stage kidney disease; however, few small studies have examined this association in patients with earlier phases of chronic kidney disease (CKD). We studied associations between baseline depressive symptoms and clinical outcomes in older patients with advanced CKD and examined whether these associations differed depending on sex. Methods CKD patients (>= 65 years; estimated glomerular filtration rate <= 20 mL/min/1.73 m(2)) were included from a European multicentre prospective cohort between 2012 and 2019. Depressive symptoms were measured by the five-item Mental Health Inventory (cut-off <= 70; 0-100 scale). Cox proportional hazard analysis was used to study associations between depressive symptoms and time to dialysis initiation, all-cause mortality and these outcomes combined. A joint model was used to study the association between depressive symptoms and kidney function over time. Analyses were adjusted for potential baseline confounders. Results Overall kidney function decline in 1326 patients was -0.12 mL/min/1.73 m(2)/month. A total of 515 patients showed depressive symptoms. No significant association was found between depressive symptoms and kidney function over time (P = 0.08). Unlike women, men with depressive symptoms had an increased mortality rate compared with those without symptoms [adjusted hazard ratio 1.41 (95% confidence interval 1.03-1.93)]. Depressive symptoms were not significantly associated with a higher hazard of dialysis initiation, or with the combined outcome (i.e. dialysis initiation and all-cause mortality). Conclusions There was no significant association between depressive symptoms at baseline and decline in kidney function over time in older patients with advanced CKD. MTOR (mammalian target of rapamycin) inhibition with RAD001 induces cell cycle arrest in diffuse large B cell lymphoma (DLBCL) cells and sensitizes DLBCL cells to Rituximab
Bei den DLBCL lassen sich mit Hilfe von Genexpressionsanalysen zwei grosse Gruppen unterscheiden: Der GCB- und der ABC-Ă€hnliche DLBCL-Typ. In der vorliegenden Arbeit konnten die drei Modellzelllinien DB, SUDHL-4 und DOHH-2 aufgrund fehlender Zyklin D2 Expression und fehlender konstitutiver NF-ÎșB AktivitĂ€t dem GCB-Typ zugeordnet werden.
Nachgewiesen wurde der antiproliferative Effekt der beiden Immunsuppressiva Rapamycin und RAD001 auf die drei Zelllinien. Das Makrolidantibiotikum Rapamycin und sein oral verfĂŒgbares Derivat RAD001 hemmen mTOR, das bedeutende Signalwege der Zellaktivierung und Zellproliferation reguliert. Es konnte gezeigt werden, dass die beiden mTOR-Inhibitoren ĂŒber diesen Wirkmechanismus hocheffektiv in die Zellzyklus- und Apoptoseregulation einzugreifen vermögen. Sie induzieren Zellzyklusarrest, indem sie die Hochregulierung der wichtigen Zellzyklus-MolekĂŒle Zyklin D3, Zyklin A und z.T. auch Zyklin E inhibieren und das Retinoblastomaprotein dephosphorylieren und damit inaktivieren. Dagegen bleibt die Expression von CDK2 und CDK4 unbeeinfluĂt.
Desweiteren konnte gezeigt werden, dass sich der zytostatische Effekt des mTOR- Inhibitors RAD001 und der zytotoxische Effekt des CD20 Antikörpers Rituximab in ihrer EffektivitÀt verstÀrken.
Die vielversprechenden Ergebnisse nach Behandlung der drei Modelzelllinie des GC-Typs der DLBCL mit Rapamycin und RAD001 liefern ein ĂŒberzeugendes Argument fĂŒr die weitere Evaluierung der m-TOR-Inhibitoren in der Therapie dieser Subgruppe. Die Kombination mit Rituximab ist möglicherweise eine attraktive Option fĂŒr Patienten mit DLBCL.Diffuse large B-cell lymphoma (DLBCL) is a common lymphoma entity. Although a significant amount of DLBCL patients can be cured with modern chemotherapeutic regimens, a substantial proportion of patients die because of progressive disease. Therefore, new therapeutic strategies are clearly needed. Inhibitors of mTOR [mammalian target of rapamycin (Rap)] represent a new class of antiproliferative drugs with applications as immunosuppressive and anticancer agents. Extensive safety data exist on the mTOR inhibitor RAD001, which is already approved as an immunosuppressant in organ transplant recipients. Rap and RAD001 inhibited cell cycle progression in DLBCL cells by inducing a G1 arrest without inducing apoptosis. Phosphorylation of the main targets of mTOR, p70 s6 kinase and 4-EBP-1 was reduced in cells cultured in the presence of RAD001. Cell cycle arrest was accompanied by reduced phosphorylation of the retinoblastoma protein (RB) as well as reduced expression of cyclin D3 and A in all cell lines. Although the effect of the chemotherapeutic agent vincristine (vin) was not enhanced by RAD001, rituximab-induced cytotoxicity was augmented in the rituximab-sensitive cell lines. mTOR inhibition is a promising therapeutic strategy in DLBCL by inducing a G1 arrest and augments rituximab-induced cytotoxicity. Therefore, combination of these drugs might be an interesting new therapeutic approach in DLBCL patient
