    Effects of geographic scale on population factors in acute disease diffusion analysis

    Objective To explore socio-demographic data of the population as proxies for risk factors in disease transmission modeling at different geographic scales. Methods Patient records of confirmed H1N1 influenza were analyzed at three geographic aggregation levels together with population census statistics. Results The study confirmed that four population factors were related in different degrees to disease incidence, but the results varied according to spatial resolution. The degree of association actually decreased when data of a higher spatial resolution were used. Conclusions We concluded that variables at suitable spatial resolution may be useful in improving the predictive powers of models for disease outbreaks.published_or_final_versio

    An Early Warning System for Detecting H1N1 Disease Outbreak - A Spatio-temporal Approach

    The outbreaks of new and emerging infectious diseases in recent decades have caused widespread social and economic disruptions in the global economy. Various guidelines for pandemic influenza planning are based upon traditional infection control, best practice and evidence. This article describes the development of an early warning system for detecting disease outbreaks in the urban setting of Hong Kong, using 216 confirmed cases of H1N1 influenza from 1 May 2009 to 20 June 2009. The prediction model uses two variables ā€“ daily influenza cases and population numbers ā€“ as input to the spatio-temporal and stochastic SEIR model to forecast impending disease cases. The fairly encouraging forecast accuracy metrics for the 1- and 2-day advance prediction suggest that the number of impending cases could be estimated with some degree of certainty. Much like a weather forecast system, the procedure combines technical and scientific skills using empirical data but the interpretation requires experience and intuitive reasoning.postprin

    Lipid control and use of lipid-regulating drugs for prevention of cardiovascular events in Chinese type 2 diabetic patients: a prospective cohort study

    <p>Abstract</p> <p>Background</p> <p>Dyslipidaemia is an important but modifiable risk factor of cardiovascular disease (CVD) in type 2 diabetes. Yet, the effectiveness of lipid regulating drugs in Asians is lacking. We examined the effects of lipid control and treatment with lipid regulating drugs on new onset of CVD in Chinese type 2 diabetic patients.</p> <p>Methods</p> <p>In this prospective cohort consisting of 4521 type 2 diabetic patients without history of CVD and naĆÆve for lipid regulating treatment recruited consecutively from 1996 to 2005, 371 developed CVD after a median follow-up of 4.9 years. We used Cox proportional hazard regression to obtain the hazard ratios (HR) of lipids and use of lipid regulating drugs for risk of CVD.</p> <p>Results</p> <p>The multivariate-adjusted HR (95% confidence interval) of CVD in patients with high LDL-cholesterol (ā‰„ 3.0 mmol/L) was 1.36 (1.08 - 1.71), compared with lower values. Using the whole range value of HDL-cholesterol, the risk of CVD was reduced by 41% with every 1 mmol/L increase in HDL-cholesterol. Plasma triglyceride did not predict CVD. Statins use was associated with lower CVD risk [HR = 0.66 (0.50 - 0.88)]. In sub-cohort analysis, statins use was associated with a HR of 0.60 (0.44 - 0.82) in patients with high LDL-cholesterol (ā‰„ 3.0 mmol/L) and 0.49 (0.28 - 0.88) in patients with low HDL-cholesterol. In patients with LDL-cholesterol < 3.0 mmol/L, use of fibrate was associated with HR of 0.34 (0.12 - 1.00). Only statins were effective in reducing incident CVD in patients with metabolic syndrome [(HR = 0.58(0.42--0.80)].</p> <p>Conclusions</p> <p>In Chinese type 2 diabetic patients, high LDL-cholesterol and low HDL-cholesterol predicted incident CVD. Overall, patients treated with statins had 40-50% risk reduction in CVD compared to non-users.</p

    Multimodality minimally invasive autopsy-A feasible and accurate approach to post-mortem examination

    Background: Autopsy examination has been the bedrock of western medicine. With the decline in the autopsy rate secondary to the negative psychological impact to the deceased's relatives, the benefits of autopsy have been undermined. Minimally invasive autopsy has been introduced but has not been widely adopted as an alternative to the 'traditional' open approach. This technique not only provides information on the cause of death abut also minimizes the disfigurement induced to the deceased. Our study aims to explore the feasibility and evaluate the accuracy of this technique. Methods: A series of coroner cases ordered for autopsy were examined by a group including an experienced forensic pathologist and two experienced laparoscopic surgeons using thoracoscopic, laparoscopic, endoluminal or endovascular approaches. The procedure was video-recorded and the provisional diagnoses and causes of death were made based on the findings. These findings were subsequently correlated with the full autopsy examination. A few limited clinical post-mortem examinations were also performed with consent from relatives. Results: A total of 22 cases of minimally invasive autopsies were performed from November 2007 to March 2008. The median duration of the procedures was 78.3 Ā± 20.7 min. Thoracoscopies and laparoscopies were performed in 18 patients while additional arterioscopic examination with endoscope was performed in two patients with suspected aortic diseases. Four consented limited clinical autopsies were also performed: two of them involved thoracoscopic biopsies of lung tissues, one was a para-mortem upper endoscopy for the investigation of pathology of the stomach and the other one was laparoscopy for a patient, who died of unexplained acidosis. Comparison with full autopsies showed that the accuracy of the diagnosis was 94.4%, the sensitivity was 90%, the specificity was 100%, the positive predictive value was 100% and the negative predictive value was 88.9%. Conclusion: Minimally invasive autopsy is a feasible approach, yielding accurate findings when compared with conventional autopsies. The former can be a valuable tool for obtaining more valuable information in situations when the next-of-kin of the deceased does not consent to a conventional autopsy. Ā© 2009 Elsevier Ireland Ltd.     Developmental changes in the role of different metalinguistic awareness skills in Chinese reading acquisition from preschool to third grade

This is an open-access article distributed under the terms of the Creative Commons Attribution License, which permits unrestricted use, distribution, and reproduction in any medium, provided the original author and source are credited.The present study investigated the relationship between Chinese reading skills and metalinguistic awareness skills such as phonological, morphological, and orthographic awareness for 101 Preschool, 94 Grade-1, 98 Grade-2, and 98 Grade-3 children from two primary schools in Mainland China. The aim of the study was to examine how each of these metalinguistic awareness skills would exert their influence on the success of reading in Chinese with age. The results showed that all three metalinguistic awareness skills significantly predicted reading success. It further revealed that orthographic awareness played a dominant role in the early stages of reading acquisition, and its influence decreased with age, while the opposite was true for the contribution of morphological awareness. The results were in stark contrast with studies in English, where phonological awareness is typically shown as the single most potent metalinguistic awareness factor in literacy acquisition.     CR2Cancer: a database for chromatin regulators in human cancer

    Identification of Master Regulators (MRs) during cancer development contributes greatly to the design of novel targeted therapy. However, it is challenging to trace the MR that regulate the gene signature (e.g. differentially expressed genes) derived from a limited number of transcriptome profiles. The main reason is that the targets (collectively termed as regulon) of MRs in cancer type-specific context is not available for enrichment analysis in the signature. To this aim, we generated the regulon of MRs using high throughput omics data from The Cancer Genome Atlas (TCGA), and developed a web server MR4Cancer, which can prioritize MRs driving phenotypic differentiation of interest in 26 cancer types. Based on the input gene list or expression profiles, it outputs six groups of ranked MRs, including transcription regulators, miRNAs, recurrently mutated genes, kinases, phosphatases, and hubs from protein-protein interaction network. Simultaneously, Gene Ontology and canonical pathway analyses are also conducted to further elucidate the function of MR candidates. Moreover, our tool presents dynamic network visualization for MR-target relations, and users can interactively customize it as high quality figures for their publications. We expect this user-friendly and powerful web application will provide researchers new insights into carcinogenesis and therapeutic intervention.published_or_final_versio
