Atypical presentation of acute pancreatitis in a man with pancreatic insufficiency and cystic fibrosis: a case report

Abstract

Introduction

Whether acute pancreatitis can occur in pancreatically insufficient individuals with cystic fibrosis remains a matter of debate.

Case presentation

We describe a case of acute pancreatitis occurring in a 52-year-old Caucasian Australian man with moderately severe cystic fibrosis lung disease and pancreatic insufficiency. An inflammatory mass within the head of his pancreas was confirmed using computed tomography, magnetic resonance imaging and pancreatic biopsy, but serum amylase and lipase remained normal throughout the acute phase of his illness. His symptoms and the pancreatic mass resolved following the insertion of a biliary stent and the introduction of ursodeoxycholic acid.

Conclusion

Our case report highlights the potential for acute pancreatitis to occur in patients with pancreatic insufficiency and cystic fibrosis. We further demonstrate that conventional biochemical markers that are normally assessed to confirm the diagnosis may not be of particular use. As patients with cystic fibrosis survive into their fourth and fifth decades of life, atypical presentations of acute pancreatitis may become more common.</p

Tri6 Is a Global Transcription Regulator in the Phytopathogen Fusarium graminearum

In F. graminearum, the transcriptional regulator Tri6 is encoded within the trichothecene gene cluster and regulates genes involved in the biosynthesis of the secondary metabolite deoxynivalenol (DON). The Tri6 protein with its Cys2His2 zinc-finger may also conform to the class of global transcription regulators. This class of global transcriptional regulators mediate various environmental cues and generally responds to the demands of cellular metabolism. To address this issue directly, we sought to find gene targets of Tri6 in F. graminearum grown in optimal nutrient conditions. Chromatin immunoprecipitation followed by Illumina sequencing (ChIP-Seq) revealed that in addition to identifying six genes within the trichothecene gene cluster, Tri1, Tri3, Tri6, Tri7, Tri12 and Tri14, the ChIP-Seq also identified 192 additional targets potentially regulated by Tri6. Functional classification revealed that, among the annotated genes, ∟40% are associated with cellular metabolism and transport and the rest of the target genes fall into the category of signal transduction and gene expression regulation. ChIP-Seq data also revealed Tri6 has the highest affinity toward its own promoter, suggesting that this gene could be subject to self-regulation. Electro mobility shift assays (EMSA) performed on the promoter of Tri6 with purified Tri6 protein identified a minimum binding motif of GTGA repeats as a consensus sequence. Finally, expression profiling of F. graminearum grown under nitrogen-limiting conditions revealed that 49 out of 198 target genes are differentially regulated by Tri6. The identification of potential new targets together with deciphering novel binding sites for Tri6, casts new light into the role of this transcriptional regulator in the overall growth and development of F. graminearum

Subjective self-assessment of physical activity is negatively affected by monitoring awareness in subjects with mild cognitive impairment : A crossover randomised controlled trial

Publisher Copyright: Š 2021 Verduci Editore s.r.l. All rights reserved.OBJECTIVE: Physical activity plays an important role in maintaining mental and physical health. This study assessed the effect of physical activity monitoring awareness on the physical activity level and subjective self-assessment of physical activity in middle-aged subjects with normal cognitive function (NCF) and mild cognitive impairment (MCI). PATIENTS AND METHODS: Thirty-five subjects aged 50-65 years with NCF and MCI were randomised into two experimental groups, each taking part in two one-week intervention periods. Subjects in group A were not aware that their physical activity was monitored in the first week (phase I) and were aware of the monitoring in the second week (phase II), whereas it was the opposite order for group B. Physical activity was assessed using the ActiGraph GT9X accelerometer and International Physical Activity Questionnaire (IPAQ). RESULTS: A total of 32 subjects (MCI: n = 12, NCF: n = 20) completed both intervention periods, with MCI subjects having significantly lower objectively assessed physical activity than NCF participants. Moreover, subjectively assessed physical activity in the MCI group was significantly higher when the participants were unaware of physical activity monitoring. A significant phase-group interaction was found in total (MET-min/d: P = 0.0072; min/d: P = 0.0194) and moderate (MET-min/d: P = 0.0015; min/d: P = 0.0020) physical activity as well as energy expenditure (p = 0.0366) assessed by the IPAQ and in the percentage of sedentary behaviour (p = 0.0330) and the average number of steps (p = 0.0342) assessed by ActiGraph. CONCLUSIONS: The awareness of physical activity assessment might decrease the ability to subjectively assess physical activity in subjects with MCI.Peer reviewe

A Regenerable Filter for Liquid Argon Purification

A filter system for removing electronegative impurities from liquid argon is described. The active components of the filter are adsorbing molecular sieve and activated-copper-coated alumina granules. The system is capable of purifying liquid argon to an oxygen-equivalent impurity concentration of better than 30 parts per trillion, corresponding to an electron drift lifetime of at least 10 ms. Reduction reactions that occur at about 250 degrees Celsius allow the filter material to be regenerated in-situ through a simple procedure. In the following work we describe the filter design, performance, and regeneration process.Comment: 12 pages with 9 figure

Radio emission of highly inclined cosmic ray air showers measured with LOPES

LOPES-10 (the first phase of LOPES, consisting of 10 antennas) detected a significant number of cosmic ray air showers with a zenith angle larger than 50∘^{\circ}, and many of these have very high radio field strengths. The most inclined event that has been detected with LOPES-10 has a zenith angle of almost 80∘^{\circ}. This is proof that the new technique is also applicable for cosmic ray air showers with high inclinations, which in the case that they are initiated close to the ground, can be a signature of neutrino events. Our results indicate that arrays of simple radio antennas can be used for the detection of highly inclined air showers, which might be triggered by neutrinos. In addition, we found that the radio pulse height (normalized with the muon number) for highly inclined events increases with the geomagnetic angle, which confirms the geomagnetic origin of radio emission in cosmic ray air showers.Comment: A&A accepte

A new method to measure the attenuation of hadrons in extensive air showers

Extensive air showers are generated through interactions of high-energy cosmic rays impinging the Earth's atmosphere. A new method is described to infer the attenuation of hadrons in air showers. The numbers of electrons and muons, registered with the scintillator array of the KASCADE experiment are used to estimate the energy of the shower inducing primary particle. A large hadron calorimeter is used to measure the hadronic energy reaching observation level. The ratio of energy reaching ground level to the energy of the primary particle is used to derive an attenuation length of hadrons in air showers. In the energy range from 10610^6 GeV to 3⋅1073\cdot10^7 GeV the attenuation length obtained increases from 170 \gcm2 to 210 \gcm2. The experimental results are compared to predictions of simulations based on contemporary high energy interaction models.Comment: accepted for publication in Physical Review
