    T2 mapping outperforms normalised FLAIR in identifying hippocampal sclerosis

    Rationale Qualitatively, FLAIR MR imaging is sensitive to the detection of hippocampal sclerosis (HS). Quantitative analysis of T2 maps provides a useful objective measure and increased sensitivity over visual inspection of T2-weighted scans. We aimed to determine whether quantification of normalised FLAIR is as sensitive as T2 mapping in detection of HS. Method Dual echo T2 and FLAIR MR images were retrospectively analysed in 27 patients with histologically confirmed HS and increased T2 signal in ipsilateral hippocampus and 14 healthy controls. Regions of interest were manually segmented in all hippocampi aiming to avoid inclusion of CSF. Hippocampal T2 values and measures of normalised FLAIR Signal Intensity (nFSI) were compared in healthy and sclerotic hippocampi. Results HS was identified on T2 values with 100% sensitivity and 100% specificity. HS was identified on nFSI measures with 60% sensitivity and 93% specificity. Conclusion T2 mapping is superior to nFSI for identification of HS

    Benefit of Ultrasound Curriculum Development for Family Medicine Residents

    Introduction/Purpose: Musculoskeletal problems comprise some of the most common reasons for ambulatory care encounters in the United States, accounting for 8.3% of the 1.2 billion visits per year according to the CDC. Musculoskeletal Ultrasound use has become more common in primary care for diagnosis and therapeutics. In Family Medicine Residency (FMR) Programs there is a deficiency of a structured, competency-based musculoskeletal ultrasound (MSK US) training despite its growing popularity. Currently, there is no formalized requirement for Ultrasound education as in other residency programs in spite of its benefit. Methods: We received a positive response on our needs analysis survey for incorporating MSKUS in FMR. 96%(24/25 residents surveyed) reporting they would be interested in receiving MSK US education. Following this, Family Medicine residents and medical students currently rotating in Family Medicine were recruited. A Sports Medicine attending with expertise in MSKUS supervised curriculum-design, exam question assessment and post lecture competency assessment. A structured curriculum was developed for 2 lectures starting with “Introduction to MSKUS”, followed by a focused lecture on “MSKUS of the Shoulder”. This included educational material, reserved practice time, a pre- and post-test for each lecture, and a practical following the second lecture. Results: Twelve residents and eight medical students completed the course series and evaluation. Using SAS Enterprise Guide 7.1 for analysis, we found sufficient evidence of a significant positive change in self-reported MSKUS knowledge from pre to post for both medical students (p=.03125) and residents (p=.0039). We also found a significant positive change in self-reported MSKUS of the Shoulder knowledge from pre to post in both medical students (p=.0156) and residents (p=.0010) as well as a significant increase in MSK US comfort level from pre-curriculum to post-curriculum (

    Global mortality from dementia : application of a newmethod and results from the Global Burden of Disease Study 2019

    Introduction: Dementia is currently one of the leading causes of mortality globally, and mortality due to dementia will likely increase in the future along with corresponding increases in population growth and population aging. However, large inconsistencies in coding practices in vital registration systems over time and between countries complicate the estimation of global dementia mortality. Methods: We meta-analyzed the excess risk of death in those with dementia and multiplied these estimates by the proportion of dementia deaths occurring in those with severe, end-stage disease to calculate the total number of deaths that could be attributed to dementia. Results: We estimated that there were 1.62 million (95% uncertainty interval [UI]: 0.41–4.21) deaths globally due to dementia in 2019. More dementia deaths occurred in women (1.06 million [0.27–2.71]) than men (0.56 million [0.14–1.51]), largely but not entirely due to the higher life expectancy in women (age-standardized female-tomale ratio 1.19 [1.10–1.26]). Due to population aging, there was a large increase in allage mortality rates from dementia between 1990 and 2019 (100.1% [89.1–117.5]). In 2019, deaths due to dementia ranked seventh globally in all ages and fourth among individuals 70 and older compared to deaths from other diseases estimated in the Global Burden of Disease (GBD) study. Discussion: Mortality due to dementia represents a substantial global burden, and is expected to continue to grow into the future as an older, aging population expands globally

    Structure and function of language networks in temporal lobe epilepsy

    Individuals with temporal lobe epilepsy (TLE) may have significant language deficits. Language capabilities may further decline following temporal lobe resections. The language network, comprised of dispersed grey matter regions interconnected with white matter fibres, may be atypical in those with TLE. This review explores the structural changes to the language network and the functional reorganisation of language abilities in TLE. We discuss the importance of detailed reporting of patient's characteristics, such as, left- and right-sided focal epilepsies as well as lesional and non-lesional pathological subtypes. These factors can affect the healthy functioning of grey and/or white matter. Dysfunction of white matter and displacement of grey matter function could impact each other's ability, in turn, producing an interactive effect on typical language organisation and function. Surgical intervention can result in impairment of function if the resection includes parts of this structure-function network that are critical to language. Further, impairment may occur if language function has been reorganized and is included in a resection. Conversely, resection of an epileptogenic zone may be associated with recovery of cortical function and thus improvement in language function. We explore the abnormality of functional regions in a clinically applicable framework and highlight the differences in the underlying language network. Avoidance of language decline following surgical intervention may depend on tailored resections to avoid critical areas of grey matter and their white matter connections. Further work is required to elucidate the plasticity of the language network in TLE and to identify sub-types of language representation, both of which will be useful in planning surgery to spare language function

    Is Training Load Associated with Symptoms of Overuse Injury in Dancers? A Prospective Observational Study

    Overuse injuries in dance are extremely common and often difficult to treat. High training load and dancing with pain are frequently regarded as risk factors for musculoskeletal injuries in professional dancers. The aims of this study were to assess for: 1. any association between training load (TL) and symptoms of overuse injury in professional dancers, and 2. any difference between the number of "time-loss" injuries and injuries causing significant symptoms not leading to decreased performance time. Twenty-one dancers from a professional contemporary dance company were followed for 7 weeks. They completed the dance-specific Self-Estimated Functional Inability because of Pain (SEFIP) questionnaire on a weekly basis to quantify musculoskeletal pain. Their TL was calculated by multiplying the Ratings of Perceived Exertion scale (RPE Borg CR10) by the daily training time. Associations between TL and SEFIP scores, recorded on a weekly basis, were evaluated using a mixed linear model with repeated measurements. No significant association was found between TL and severity of musculoskeletal pain. However, the TL of the dancers with no symptoms of overuse-injury, SEFIP = 0, was significantly lower compared to the dancers with symptoms, SEFIP > 0; p = 0.02. No time loss because of injury was reported during the study period. There were 251 symptoms of overuse injury reported, and 67% of the recorded time was danced with pain. It is concluded that dancers without musculoskeletal pain had lower TLs. While no time-loss injuries were found, two-third of the participants danced with pain during this 7-week period

    The Effect of Vascular Segmentation Methods on Stereotactic Trajectory Planning for Drug-Resistant Focal Epilepsy: A Retrospective Cohort Study

    Background: Stereotactic neurosurgical procedures carry a risk of intracranial hemorrhage, which may result in significant morbidity and mortality. Vascular imaging is crucial for planning stereotactic procedures to prevent conflicts with intracranial vasculature. There is a wide range of vascular imaging methods used for stereoelectroencephalography (SEEG) trajectory planning. Computer-assisted planning (CAP) improves planning time and trajectory metrics. We aimed to quantify the effect of different vascular imaging protocols on CAP trajectories for SEEG. Methods: Ten patients who had undergone SEEG (95 electrodes) following preoperative acquisition of gadolinium-enhanced magnetic resonance imaging (MR + Gad), magnetic resonance angiography and magnetic resonance angiography (MRV + MRA), and digital subtraction catheter angiography (DSA) were identified from a prospectively maintained database. SEEG implantations were planned using CAP using DSA segmentations as the gold standard. Strategies were then recreated using MRV + MRA and MR + Gad to define the “apparent” and “true” risk scores associated with each modality. Vessels of varying diameter were then iteratively removed from the DSA segmentation to identify the size at which all 3 vascular modalities returned the same safety metrics. Results: CAP performed using DSA vessel segmentations resulted in significantly lower “true” risk scores and greater minimum distances from vasculature compared with the “true” risk associated with MR + Gad and MRV + MRA. MRV + MRA and MR + Gad returned similar risk scores to DSA when vessels <2 mm and <4 mm were not considered, respectively. Conclusions: Significant variability in vascular imaging and trajectory planning practices exist for SEEG. CAP performed with MR + Gad or MRV + MRA alone returns “falsely” lower risk scores compared with DSA. It is unclear whether DSA is oversensitive and thus restricting potential trajectories

    Structural imaging biomarkers of sudden unexpected death in epilepsy.

    Sudden unexpected death in epilepsy is a major cause of premature death in people with epilepsy. We aimed to assess whether structural changes potentially attributable to sudden death pathogenesis were present on magnetic resonance imaging in people who subsequently died of sudden unexpected death in epilepsy. In a retrospective, voxel-based analysis of T1 volume scans, we compared grey matter volumes in 12 cases of sudden unexpected death in epilepsy (two definite, 10 probable; eight males), acquired 2 years [median, interquartile range (IQR) 2.8] before death [median (IQR) age at scanning 33.5 (22) years], with 34 people at high risk [age 30.5 (12); 19 males], 19 at low risk [age 30 (7.5); 12 males] of sudden death, and 15 healthy controls [age 37 (16); seven males]. At-risk subjects were defined based on risk factors of sudden unexpected death in epilepsy identified in a recent combined risk factor analysis. We identified increased grey matter volume in the right anterior hippocampus/amygdala and parahippocampus in sudden death cases and people at high risk, when compared to those at low risk and controls. Compared to controls, posterior thalamic grey matter volume, an area mediating oxygen regulation, was reduced in cases of sudden unexpected death in epilepsy and subjects at high risk. The extent of reduction correlated with disease duration in all subjects with epilepsy. Increased amygdalo-hippocampal grey matter volume with right-sided changes is consistent with histo-pathological findings reported in sudden infant death syndrome. We speculate that the right-sided predominance reflects asymmetric central influences on autonomic outflow, contributing to cardiac arrhythmia. Pulvinar damage may impair hypoxia regulation. The imaging findings in sudden unexpected death in epilepsy and people at high risk may be useful as a biomarker for risk-stratification in future studies

    Although temporal lobe epilepsy (TLE) results in widespread changes in MRI measures of tissue volume, diffusion and functional connectivity, changes in tissue composition in TLE have not been investigated with MRI. Quantitative susceptibility mapping (QSM) is sensitive to changes in tissue composition, in particular to iron and myelin. Here, we show for the first time that QSM is sensitive to gray matter abnormalities in 31patients with temporal lobe epilepsy (TLE) compared to 23 healthy controls, and showed significant susceptibility changes in the hippocampus in left TLE patients, and in the bilateral thalamus in both left and right TLE
