    The paradoxes of quality

    International journal of engineering and industrial management. - ISSN 1647-578X. - N. 6 (2014). - p. 11-21This research work intends to contribute to the clarification which involves the ISO 9001:2008 certification and its applicability by presenting an analysis and subsequent reflection on the impact of certification organizations, as well as compliance with the requirements of the standard. The results achieved with this work on the paradoxes of quality allowed the confirmation of what many people already suspected. These will serve as initial knowledge, once there were no other studies on this subject. Marked the paradoxes, we highlight that there are companies that claim to exist simultaneously "involvement of all in solving problems" and on the other hand claim that "there are cohesive work groups". Likewise, state that “practicing continuous improvement” and simultaneously they have "difficulty in changing the rules of the workplace"

    A multivariate randomized controlled experiment about the effects of mindfulness priming on EEG neurofeedback self-regulation serious games

    Neurofeedback training (NFT) is a technique often proposed to train brain activity SR with promising results. However, some criticism has been raised due to the lack of evaluation, reliability, and validation of its learning effects. The current work evaluates the hypothesis that SR learning may be improved by priming the subject before NFT with guided mindfulness meditation (MM). The proposed framework was tested in a two-way parallel-group randomized controlled intervention with a single session alpha NFT, in a simplistic serious game design. Sixty-two healthy naïve subjects, aged between 18 and 43 years, were divided into MM priming and no-priming groups. Although both the EG and CG successfully attained the up-regulation of alpha rhythms (F(1,59) = 20.67, p ηp2 = 0.26), the EG showed a significantly enhanced ability (t(29) = 4.38, p t(29) = 1.18, p > 0.1). Furthermore, EG superior performance on NFT seems to be explained by the subject’s lack of awareness at pre-intervention, less vigour at post-intervention, increased task engagement, and a relaxed non-judgemental attitude towards the NFT tasks.     Psychosocial adjustment to a lower limb amputation ten months after surgery

    Objective: This longitudinal multisite study examined the influence of demographic characteristics psychological reactions, functionality coping strategies, and social support on psychosocial adjustment to lower limb amputation 10 months after suigery. Method: Of an initial referral of 206 Portuguese patients, a sample of 86 patients who underwent a lower limb amputation due to Diabetes Mellitus Type II were evaluated during the hospitalization that preceded surgery (t0) and at inpatient follow-up consultations, 1 (tl) 6 (t2), and 10 months (t3) after surgery. Results: Higher levels of anxiety symptoms and functionality at presurgery were associated with lower social adjustment to amputation and with higher adjustment to the limitations (t3) respectively. Traumatic stress symptoms (tl) were negatively associated with general and social adjustment, and with the adjustment to the limitations (t3). Perceived social support (t2) mediated the relationship between traumatic stress symptoms (tl) and adjustment to the limitations (t3). Male gender was associated with a higher anxiety and depression symptoms (t0) and with a higher level of functionality (tl). Male gender was associated with functionality at presurgery and postsurgery, and with anxiety and depression symptoms of presurgery. Implications: Results support the need to improve psychological screening and early treatment of anxiety symptoms before the surgery, as well a.s depression and traumatic stress symptoms after a lower limb amputation, and the promotion of social support over time, in order to promote psychosocial adjustment to amputation.     Validation of the HPV Impact Profile in Portuguese women with human papilloma virus

    Objective: Human papilloma virus (HPV) is the world's most common causative agent in sexually transmitted viral infections; thus requiring clinical guidance, before and after treatment, to promote better quality of life in women diagnosed with HPV. The HPV Impact Profile (HIP) is a comprehensive instrument used to assess the psychosocial impact of HPV and related interventions in a wide range of domains. The main purpose of this study was to validate the HIP in Portuguese women, so that it can be used in this population, both in research and in clinical settings. Methods: A sample of 178 Portuguese women infected with HPV were assessed with the following instruments: Index of Sexual Satisfaction, Hospital Anxiety and Depression Scale and HPV Impact Profile. Results: The results of the confirmatory factor analysis showed some changes to the original model, which resulted in an adapted version with six domains, compared to the seven domains of the original model. The final model showed a good fit to the data. All Cronbach's alphas were higher than 0.79 indicating a high level of internal consistency of the subscales. As in the original HIP questionnaire, the exception was sexual impact, with a lower, but acceptable, Cronbach's alpha. Convergent and discriminant validities of the instrument were also assessed. Conclusions: The Portuguese version of the HIP presents good psychometric properties, allowing its use in clinical trials as well as in clinical practice in order to evaluate quality of life in women with HPV.This study was supported by the Portuguese Foundation for Science and Technology and the Portuguese Ministry of Science, Technology and Higher Education through national funds and co-financed by FEDER through COMPETE2020 under the PT2020 Partnership Agreement [POCI-01-0145-FEDER-007653]

    Psychological factors as predictors os adjustment to life of people with chronic diseases

    Doenças crónicas são de longa duração, de progressão lenta e induzem alterações na vida das pessoas, que são confrontadas com um conjunto de fatores que exercem um impacto negativo na sua qualidade de vida (QdV). A QdV é um conceito envolvendo componentes essenciais da qualidade humana: físicas, psicológicas, sociais, culturais e espirituais. Após o diagnóstico e com a doença estabilizada, os doentes procuram novas formas de lidar com esta. Este estudo teve como objetivo identificar fatores psicossociais preditivos (otimismo, afeto positivo e negativo, adesão aos tratamentos, suporte social e espiritualidade) da QdV (bem-estar geral, saúde física, saúde mental) e bem-estar subjetivo (BES) em pessoas com doenças crónicas. Amostra constituída por 774 indivíduos [30% diabetes, 27,1% cancro, 17,2% diabetes, 12% epilepsia, 11,5% esclerose múltipla e 2,2% miastenia, 70,5% do sexo feminino, idade M(DP)=42,9(11,6), educação M(DP)=9,6(4,7), anos diagnóstico M(DP)=12,8(9,7), classificação da doença M(DP)=6,6 (2,8)], recrutados nos hospitais centrais portugueses. Aplicando Modelos de Equações Estruturais e ajustando para variáveis sociodemográficas e clínicas, verificou-se que, pessoas mais otimistas, mais ativas e com uma melhor adesão aos tratamentos apresentam um melhor bem-estar geral, uma melhor saúde mental e um melhor bem-estar subjetivo; uma melhor adesão aos tratamentos contribui para uma melhor saúde física; melhor suporte social reflete-se numa melhor saúde mental; pessoas com mais espiritualidade apresentam uma melhor saúde física e uma melhor saúde mental. Estas conclusões contribuem para a definição de uma terapia que pode ajudar a uma melhor adaptação dos protocolos de tratamento para atender às necessidades dos doentes.ABSTRACT - Chronic diseases are long-lived, slow progression and induce changes in the lives of people, who are confronted with a set of factors that have a negative impact on their quality of life (QoL). The QoL is a concept involving essential components of human quality: physical, psychological, social, cultural and spiritual. After diagnosis and with stable disease, patients try to find new ways to deal with their new condition. This study aimed to identify predictive psychosocial factors (optimism, positive and negative affect, treatment adherence, social support and spirituality) QoL (general well-being, physical health, mental health) and subjective well-being (SWB) in people with chronic diseases. The sample of 774 subjects [30 % diabetes, 27.1 % cancer, 17.2 % diabetes, 12 % epilepsy, 11.5 % multiple sclerosis and 2.2 % gravis, 70.5 % female, age M(SD)=42.9(11.6), education M(SD)=9.6(4.7) years diagnosed M(SD)=12.8(9.7), disease classification M(SD)=6.6(2.8)], was recruited from the Portuguese central hospitals. Applying Structural Equation Modeling and adjusting for sociodemographic and clinical variables, it was found that more optimistic people, more active and with a better treatment adherence, have better overall well-being, better mental health and better subjective well-being; better treatment adherence contributes to better physical health, better social support reflected in better mental health, people with more spirituality have better physical health and better mental health. These findings contribute to the development of a therapy that can help to better adaptation of treatment protocols to meet the needs of patients

    Funcionamento sexual e saúde mental em seis doenças crónicas: convergências e divergências

    Ainda que a saúde mental dos doentes crónicos seja cada vez mais uma preocupação dos profissionais de saúde, não é generalizada a avaliação rotineira do seu funcionamento sexual, que poderá ter impacto sobre a sua saúde mental. O objectivo do presente estudo é explorar a relação entre funcionamento sexual e saúde mental em doentes crónicos. Foram avaliados 77 adultos com diabetes tipo 1, 40 com diabetes tipo 2, 100 com esclerose múltipla, 79 com epilepsia, 205 com obesidade e 106 com cancro, recorrendo a um Questionário Sócio-demográfico e Clínico, à Escala de Função Sexual do MSQOL-54 e à Escala de Saúde Mental do SF-36. Na amostra total, verificaram-se correlações lineares estatisticamente significativas entre Funcionamento Sexual e Saúde Mental nos dois sexos (homens: r(161)=-0,35, p<0,0001; mulheres: r(387)=-0,36, p<0,0001). Entre os homens, as correlações oscilaram entre rs(33)=-0,58 (p<0,0001) e rs(34)=-0,21 (p=0,23); entre as mulheres, entre rs(161)=-0,49 (p<0,0001) e rs(19)=-0,03 (p=0,89). Mais concretamente, nos indivíduos com diabetes tipo 1 e cancro verificaram-se correlações lineares estatisticamente significativas entre Funcionamento Sexual e Saúde Mental nos dois sexos; nos indivíduos com diabetes tipo 2 e esclerose múltipla não se verificaram correlações significativas em nenhum dos sexos; nos indivíduos com obesidade só se verificaram correlações significativas nos indivíduos do sexo feminino; nos indivíduos com epilepsia só se verificaram correlações significativas nos indivíduos do sexo masculino. Os resultados sugerem que a promoção do funcionamento sexual de doentes crónicos poderá saldar-se por uma melhoria na sua saúde mental (e vice-versa), mas não em todas as doenças crónicas analisadas.Sob o apoio da bolsa da Fundação para a Ciência e a Tecnologia PTDC/PSI/71635/2006

    Estrutura da boa vida em pessoas com doença crónica

    Doenças crónicas (DC) são doenças que têm de ser geridas em vez de curadas. Giovannini, Bitti, Sarchielli e Speltini (1986) caracterizam as doenças crónicas como: a) doenças de longa duração, b) que tendem a prolongar-se por toda a vida do doente, c) que provocam invalidez em graus variáveis, d) são devidas a causas não reversíveis, e) que exigem formas particulares de reeducação, f) que obrigam o doente a seguir determinadas prescrições terapêuticas, g) que normalmente exigem a aprendizagem de um novo estilo de vida, h) que necessitam de controlo periódico, de observação e de tratamento regulares. As DC não se definem pela sua aparente ou real gravidade. As pessoas que têm DC podem fazer a vida do dia-a-dia como qualquer outro cidadão, e grande parte deles acaba por falecer de velhice ou de outras doenças, que não a DC que os acompanhou grande parte da vida. Para estas pessoas viverem uma boa vida o processo de ajustamento é decisivo. O ajustamento à DC torna-se então um objectivo fundamental para as pessoas e para a sociedade. O ajustamento pode definir-se como uma resposta a uma alteração do meio ambiente, que leva um organismo a adaptar-se a essa alteração. Esta definição, explicam as autoras, implica que ele ocorre ao longo do tempo e, também, que é um resultado desejável. Para Stanton, Revenson, e Tennen, (2007), o ajustamento engloba inúmeros componentes que cruzam domínios interpessoais, cognitivos, emocionais, físicos e comportamentais. Estes autores referem que da revisão da literatura se pode concluir que, grosso modo a DC requer, ajustamento em múltiplos domínios, que o ajustamento se desenrola ao longo do tempo, e que existe uma heterogeneidade acentuada no modo como os indivíduos se ajustam à DC. O ajustamento é um conceito do senso comum que tanto é verbo como substantivo
